Challenges in genomic analysis of model systems and primary tumors of pancreatic ductal adenocarcinoma.

Pancreatic ductal adenocarcinoma (PDAC) is characterized by aggressive tumor behavior and poor prognosis. Recent next-generation sequencing (NGS)-based genomic studies have provided novel treatment modes for pancreatic cancer via the identification of cancer driver variants and molecular subtypes in PDAC. Genome-wide approaches have been extended to model systems such as patient-derived xenografts (PDXs), organoids, and cell lines for pre-clinical purposes. However, the genomic characteristics vary in the model systems, which is mainly attributed to the clonal evolution of cancer cells during

Navigating the information technology highway: computer solutions to reduce errors and enhance patient safety.

Standardized, seamless, integrated information technology in the health-care environment used with other industry tools can markedly decrease preventable errors or adverse events and increase patient safety. According to an Institute of Medicine (IOM) report released in 1999, preventable errors have caused between 44,000 and 98,000 deaths per year. Following the report, President Bill Clinton requested that the Agency of Healthcare Research and Quality, a government agency, look into the issue and fund, at the local or state level, processes that can reduce errors. Funding subsequently was mad

Network biology to artificial intelligence: building the next generation of predictive drug discovery.

Drug discovery remains constrained by high attrition rates, prolonged timelines, and limited ability to translate molecular insights into effective therapies. Although reductionist approaches targeting individual molecules have delivered transformative medicines, successful patient-centric treatment remains a major challenge. Furthermore, increasing evidence demonstrates that therapeutic outcomes emerge from complex, dynamic, and context-dependent biological networks. Systems biology has provided a framework to understand these interactions, yet its predictive capability has been limited by bi

Amyloidosis. Part II. Implications for neuroscience nurses: Alzheimer's disease.

UNLABELLED: Amyloidosis describes a group of diseases characterized by deposition of abnormal fibrous proteins in body organs and soft tissues. This disease complex is classified under the broad headings primary and secondary based on whether or not chronic infections or inflammatory conditions coexist. In addition, localized versus systemic involvement and heredofamilial considerations are used to subclassify amyloidosis. Many primary and secondary amyloid diseases involve the central, peripheral and autonomic nervous systems. This paper will focus on the latest information concerning systemi

Leveraging a QSP Platform for Prediction of Clinical Efficacy and Safety Biomarkers in Immuno-Oncology Combination Therapy.

IO Simulator is a Quantitative Systems Pharmacology platform implementing a mechanistic model of solid tumor-immune system interactions and the action of standard immuno-oncology drugs. Using a tarlatamab case study, we demonstrate how IO Simulator predicts combination therapy efficacy and cytokine biomarkers, which can be statistically related to the risk of cytokine release syndrome (CRS). This allows patient stratification and identification of doses that maximize efficacy without a substantial increase in cytokine release.

The Medication Recommendation Tracking Form: a novel tool for tracking changes in prescribed medication, clinical decision making, and use in comparative effectiveness research.

This paper describes the development and use of the Medication Recommendation Tracking Form (MRTF), a novel method for capturing physician prescribing behavior and clinical decision making. The Bipolar Trials Network developed and implemented the MRTF in a comparative effectiveness study for bipolar disorder (LiTMUS). The MRTF was used to assess the frequency, types, and reasons for medication adjustments. Changes in treatment were operationalized by the metric Necessary Clinical Adjustments (NCA), defined as medication adjustments to reduce symptoms, optimize treatment response and functionin

How to assess the severity of heart failure?

PURPOSE OF REVIEW: Mortality rates for acute decompensated heart failure and cardiogenic shock remain unacceptably high despite advances in medical therapy and mechanical circulatory support. Systems designed to quickly and accurately identify and risk stratify these patients are needed in order to improve survival. RECENT FINDINGS: The Society for Cardiovascular Angiography and Interventions developed an expert consensus statement aimed at early identification and assessment of patients with advanced heart failure and cardiogenic shock. Recent studies have validated this novel classification

Evaluation of rapid identification of gram-positive cocci in positive blood cultures by use of the AutoMicrobic system Gram-Positive Identification Card.

Because rapid identification of gram-positive organisms from blood cultures may provide valuable information for patient care and because the AutoMicrobic system Gram-Positive Identification (AMS-GPI) Card (Vitek Systems, Inc., Hazelwood, Mo.) is designed for the identification of these organisms in 4 to 13 h, we designed this study to evaluate the performance of the AMS-GPI Card in the direct identification of gram-positive organisms upon detection of growth in blood culture bottles. We compared direct identification by the AMS-GPI Card with the final AMS-GPI Card identification and with our

First report of bacteremia caused by CO2-dependent methicillin-resistant Staphylococcus epidermidis in a patient with symptomatic multiple myeloma.

We report the first bacteremia case caused by CO₂-dependent methicillin-resistant Staphylococcus epidermidis (MRSE). Blood cultures from a symptomatic multiple myeloma patient yielded Gram-positive cocci. Molecular testing (BioFire FilmArray Blood Culture Identification 2 Panel) identified S. epidermidis with positive mecA/C. However, subculturing failed under ambient air, yielding colonies only under 5% CO₂. Using the MicroScan WalkAway 40 plus system (Pos Combo 1 J panel) in ambient air misidentified the isolate as Micrococcus sp. and produced indeterminate antimicrobial susceptibility testi

Benchmarking transformer-based models for medical record de-identification in a single center multi-specialty evaluation.

Protecting patient confidentiality is central to enabling research using electronic health records. Automated text de-identification offers a scalable alternative to manual redaction. However, different approaches vary in accuracy and adaptability. We evaluated four transformer-based, task-specific models and five large language models on 3,650 clinical records spanning general and specialty datasets from a UK hospital group. Records were dual-annotated by clinicians, allowing precise comparison of performance. The Microsoft Azure de-identification service achieved the highest F1 score, approa

NAMING CONVENTIONS FOR UNIDENTIFIED PATIENTS IN EMERGENCY AND TRAUMA SETTINGS: A NARRATIVE REVIEW.

Unidentified patients pose unique challenges in emergency and trauma care, where rapid registration and treatment initiation are essential despite the absence of formal identification. Naming these patients is critical for safe clinical care, communication, and legal documentation. This narrative review synthesizes the literature on who unidentified patients are, the issues associated with naming them, and the naming systems currently used. Studies consistently describe these patients as predominantly young males presenting with trauma, substance misuse, or altered consciousness, often from so

Failure to rescue in the surgical patient: a review.

PURPOSE OF REVIEW: Evaluating patient outcomes is essential in a healthcare environment focused on quality. Mortality after surgery has been considered a useful quality metric. More important than mortality rate, failure to rescue (FTR) has emerged as a metric that is important and may be improveable. The purpose of this review is to define FTR, describe patient and hospital level factors that lead to FTR, and highlight possible solutions to this problem. RECENT FINDINGS: FTR is defined as a death following a complication. Depending on the patient population, FTR rates vary from less than 1%

Identification by 16S rRNA gene sequencing of Negativicoccus succinicivorans recovered from the blood of a patient with hemochromatosis and pancreatitis.

We describe a case of Negativicoccus succinicivorans bacteremia in an adult man with hemochromatosis and acute pancreatitis. Conventional phenotypic tests and commercial identification systems failed to definitively identify the tiny anaerobic Gram-negative coccus isolated from two sets of blood cultures. The bacterium was identified by 16S rRNA gene sequencing and analysis using the SmartGene Integrated Database Network System software. This is the first published report of the recovery of this organism from a patient with invasive infection.

The Involvement of the Endocannabinoid, Glutamatergic, and GABAergic Systems in PTSD.

Post-traumatic stress disorder (PTSD) is a debilitating mental health condition that develops in response to traumatic events. The endocannabinoid, glutamatergic, and GABAergic systems play crucial roles in the neurobiological mechanisms of PTSD. Both the endocannabinoid, glutamatergic, and GABAergic systems are involved in synaptic remodeling and neuronal differentiation, ensuring efficient information transmission in the brain. Their interplay influences motivation, behavior, sensory perception, pain regulation, and visual processing. Additionally, these systems regulate processes such as ce

A scalable approach for critical care data extraction and analysis in an academic medical center.

BACKGROUND: Electronic health records are a valuable asset for research, but their use is challenging due to inconsistencies of records, heterogeneous formats and the distribution over multiple, non-integrated information systems. Hence, specialized health data engineering and data science expertise are required to enable research. To facilitate secondary use of clinical routine data collected in our intensive care wards, we developed a scalable approach, consisting of cohort generation, variable filtering and data extraction steps. OBJECTIVE: With this report we share our workflow of data re
