Characterization of Ketamine-Induced Neurotoxicity in PND7 Rats as a Model of Pediatric Anesthesia.

Ketamine, a dissociative anesthetic commonly used in pediatric medicine, may induce neurotoxic effects when administered during early neonatal life. Although ketamine-induced neuronal apoptosis has been previously documented, less is known about glial cell responses or temporal evolution of injury. This study investigated the neurotoxic effects of ketamine (20 mg/kg, 5 intraperitoneal injections administered 90 minutes apart) in postnatal day 7 Sprague-Dawley rats of both sexes, compared with age-matched controls. Brains were collected 2 and 16 hours after the last injection and evaluated by h

Pediatric anesthesia as a navigator: current practice and hot topics in preoperative assessment.

PURPOSE OF REVIEW: Preoperative assessment is a central but increasingly complex component of pediatric anesthetic care. While perioperative safety in children has improved substantially, preventable complications remain prevalent. This review examines current concepts of pediatric preoperative evaluation, highlights emerging trends and controversies, and proposes the concept of anesthesia as a perioperative "navigator", whose guiding function begins with preoperative assessment. RECENT FINDINGS: Contemporary practice of preoperative assessment increasingly includes structured risk stratifica

Regional anesthesia and muscle-wasting diseases in pediatrics: A focused educational review.

The muscular dystrophies or muscle-wasting diseases include a diverse group of genetic disorders, which result in progressive degeneration of skeletal muscles, progressive muscle weakness, and comorbid multi-system involvement. Duchenne muscle dystrophy is the most common type of muscular dystrophy with a reported incidence of 1 in every 3500-6000 male live births in the United States. Given the progressive nature of these disorders, skeletal muscle weakness frequently progresses to loss of the ability to ambulate and perform functions of daily life. In addition to affecting the skeletal muscu

Optimizing the Recovery of Pediatric Tonsillectomy: Application of Opioid-Free Anesthesia and Analgesia.

PURPOSE: Opioids are considered the gold standard for controlling postoperative acute pain. However, side effects such as nausea, vomiting, respiratory depression, and pain sensitization have gained significant attention. The risks are higher in children with enlarged tonsils and sleep apnea, making effective pain management while minimizing analgesia-related side effects a pressing concern. DESIGN: This was a randomized (1:1), single-blind, parallel-group superiority trial. METHODS: A prospective single-blind study was conducted with 44 children aged 3 to 7 years who planned to undergo tons

Back to Basics: A Curriculum to Address the Pediatric Cardiac Anesthesia Workforce Crisis.

The field of pediatric cardiac anesthesia faces a critical workforce shortage. Survival of children with congenital heart disease (CHD) has improved dramatically, increasing both lifetime procedural demand and case complexity. At the same time, the supply of fellowship-trained pediatric cardiac anesthesiologists is shrinking due to an aging workforce, declining fellowship recruitment, financial disincentives, and concerns about work-life balance and professional culture. The resulting mismatch between demand and supply threatens access to care and risks moral injury and attrition within the sp

Frequency and Indications for Anesthesia Exposure in a Pediatric Traumatic Brain Injury Cohort.

BACKGROUND: Pediatric traumatic brain injury (pTBI) is a leading cause of morbidity in the pediatric population. These patients often undergo multiple anesthetic exposures (AE) during hospitalization for their injuries. In this retrospective study, we sought to analyze the hospital course and outcomes in pTBI patients, hypothesizing that these patients required multiple AEs and that repeated AEs increased the risk for morbidity. METHODS: A chart review of pTBI patients from the Westchester Medical Center Pediatric Trauma Registry was conducted from 2012 to 2021. Demographics, injury data, and

Anesthesia management in a pediatric patient with complicatedly difficult airway: A case report.

BACKGROUND: Reports on perioperative anesthesia management in pediatric patients with difficult airways are scarce. In addition to relatively more difficulties in the technique of endotracheal intubation, the time for manipulation is restricted compared to adults. Securing the airways safely and avoiding the occurrence of hypoxemia in these patients are of significance. CASE SUMMARY: A 9-year-old boy with spastic cerebral palsy, severe malnutrition, thoracic scoliosis, thoracic and airway malformation, laryngomalacia, pneumonia, and epilepsy faced the risk of anesthesia during palliative surg

A Survey of the Society for Pediatric Anesthesia on the Use, Monitoring, and Antagonism of Neuromuscular Blockade.

BACKGROUND: Although the package insert clearly states that "the safety and efficacy of sugammadex in pediatric patients have not been established," we hypothesized that sugammadex is used widely in pediatric anesthetic practice supplanting neostigmine as the primary drug for antagonizing neuromuscular blockade (NMB). Additionally, we sought to identify the determinants by which pediatric anesthesiologists choose reversal agents and if and how they assess NMB in their practice. Finally, because of sugammadex's effects on hormonal contraception, we sought to determine whether pediatric anesthes

Influence of topical anesthesia on the sedation of pediatric emergency department patients with lacerations.

Local anesthetic infiltration of wounds causes pain which distresses children. A painless topical anesthetic solution containing tetracaine, adrenaline, and cocaine (TAC) may reduce this distress. We hypothesized that the use of TAC for anesthesia may reduce the utilization of sedation for laceration repair. We performed a retrospective review of all pediatric emergency department (PED) patients receiving parenteral meperidine, promethazine, and chlorpromazine (DPT) during the period after TAC became routinely available. For comparison, data from a recent retrospective study of DPT (preTAC) us

Baby steps to mastery: building blocks for novices in pediatric regional anesthesia.

Pediatric regional anesthesia offers significant benefits, yet its adoption faces barriers, including perceived overcomplexity. This study aimed to identify and establish a set of core, high-value, low-complexity nerve blocks to improve perioperative pain management in pediatric patients. A four-round modified Delphi consensus study was conducted with an international panel of pediatric and regional anesthesia experts. An initial long list of regional techniques was compiled by the Steering Committee and refined through iterative input. Panelists rated each technique on a 10-point Likert scale
