Anatomical and Surgical Evaluation of the Common Marmoset as an Animal Model in Hearing Research.

Recent studies have indicated that direct administration of viral vectors or small compounds to the inner ear may aid in the treatment of Sensorineural hearing loss (SNHL). However, due to species differences between humans and rodents, translating experimental results into clinical applications remains challenging. The common marmoset (Callithrix jacchus), a New World monkey, is considered a pre-clinical animal model. In the present study, we describe morphometric data acquired from the temporal bone of the common marmoset in order to define the routes of topical drug administration to the in

Auditory effects of autologous fat graft for TORP stabilization in the middle ear: a cadaveric study.

BACKGROUND: Total ossicular replacement prostheses (TORP) are often used to re-establish ossicular coupling of sound in an ear lacking a stapes supra-structure. The use of TORPs, however, is associated with a 2/3 five year failure rate due to their anatomic instability over time in the middle ear. The use of autologous fat to try and stabilize TORPs may improve long-term results with this challenging ossicular reconstruction technique. METHODS: A cadaveric temporal bone model was developed and laser Doppler vibrometry was used to measure and record round window membrane vibration in response

Finite element analysis of the osseous spiral lamina's influence on inner ear fluid flow during bone conduction stimulation.

Recent studies have investigated the anatomy and motion of the human cochlear partition, revealing insights into the flexible nature of the osseous spiral lamina (OSL). These investigations have primarily focused on air-conducted stimulation, leaving the impact of the OSL's flexibility during bone-conducted (BC) stimulation largely unexplored. By considering the OSL as either flexible or rigid in a finite element model of the human inner ear, we examined the effect of the OSL's flexibility on the fluid flow in the inner ear during BC stimulation, which was divided into contributors entering vi

Occlusion of the round window: a novel way to treat hyperacusis symptoms in superior semicircular canal dehiscence syndrome.

BACKGROUND: Conductive hyperacusis in superior semicircular canal dehiscence syndrome occurs due to the presence of a 'third window' created by the dehiscence. Reversible blocking of the round window can, in theory, cause a reduction in the compression-related volume displacement, and thereby minimise symptoms of conductive hyperacusis. This study describes a technique of permeatal blocking of the round window. METHOD: The tympanomeatal flap is elevated and the round window niche is identified. The round window membrane is subsequently identified and occluded with bone wax, muscle and fascia,

Non-syndromal round window atresia: an autosomal dominant genetic disorder with variable penetrance?

Round window atresia can be seen in association with syndromal anomalies such as mandibulofacial dysostosis, Mondini type anomalies or cretinism, or with extensive otosclerosis. Non-syndromal round window atresia is extremely rare and until today there is no proof whether non-syndromal round window atresia can be inherited. We present two family members with non-syndromal round window atresia. Based on this case and on an extensive literature review, we discuss the possible ways of sound transmission with round window occlusion as well as the possibility of an autosomal dominant inheritance pa

Temperature rises in the round window caused by various light sources during insertion of rigid endoscopes: an experimental animal study.

OBJECTIVES: The instruments used in endoscopic surgery include rigid endoscopes of different diameters and angles, sources of light and monitors. In this study, we explored temperature rises in the round window caused by insertion of rigid endoscopes of different diameters into the middle ear; the endoscopes were fitted with different light sources. DESIGN: An experimental animal study. SETTING: We subjected seven guinea pigs to simulate otological surgery at room temperature. We monitored body temperatures, along with temperature rises caused by the use of 0° rigid endoscopes of diameters 3

[Perilymph fistulas--symptoms and indications for tympanoscopy].

In the last two years thirteen cases of perilymph fistula were treated by tympanoscopy and covering with connective tissue. Recovery of hearing was not observed in 4 totally deaf ears, but improvement was recorded in each of 9 cases with impaired hearing. Vertigo disappeared in 10 of 11 cases without any correlation with the degree of hearing loss. We perform tympanoscopy in suspected perilymph fistula, because prognosis is uncertain, spontaneous recovery with remaining defects may induce ménièriform syndrome, and pathway of infection should be closed. The diagnosis of a perilymph fistula must

Active middle ear implants in patients undergoing subtotal petrosectomy: long-term follow-up.

OBJECTIVE: In 2009, we had introduced the active middle ear implant (aMEI) round window coupling in patients undergoing a subtotal petrosectomy and reported our first results. In the current study, we evaluated the long-term firmness of the vibranting floating mass transducer (FMT) within the round window niche, the long-term audiologic results and the patient's perspective of wearing the speech processor over time. PATIENTS AND INTERVENTION: Of 10 patients, 6 female and 1 male patients (age range from 30 to 71 yr) had undergone subtotal petrosectomy with aMEI round window vibroplasty and wer

When is a conductive hearing loss not a conductive hearing loss? Causes of a mismatch in air-bone threshold measurements or a "pseudoconductive" hearing loss.

The otolaryngologist is sometimes faced with a patient with an apparent conductive hearing loss on audiometric testing who appears to have normal and mobile middle ear structures on ear exploration. Some causes of these "pseudoconductive" hearing losses can be diagnosed without resorting to surgery. This article reviews several possible mechanisms for this condition with diagnostic procedures to distinguish etiologic diseases. Conditions discussed include poor audiometry, functional hearing losses, "third window" causes, obliterated round window, abnormal inner ear connection with the cerebros

Perilymph fistula resulting from microfissures.

Perilymphatic fistulas are well documented in the otolaryngologic literature as a cause for both auditory and vestibular symptoms. The anatomic location of fistulas has been clinically confirmed in both the oval and round windows. Furthermore, membranous tears within the cochlea have been described post mortem and may be the cause of the remaining fistulas which are suspected but not confirmed surgically. A fourth source of perilymph fistula has previously been suggested by Okano and Harada. During the past several years, surgical confirmation of perilymph leakage from two microfissure areas h

Single-unit response at the round window of the guinea pig.

In guinea pigs the unit contribution (unit action potential, ap) to the response of the round window was computed following the method used by Kiang and co-workers (1976), i.e., fibre discharges registered by a microelectrode in the nerve were used as trigger pulses for the averaging process of the corresponding ap registered with a gross electrode at the round window. Normally the ap was independent of the fibre-CF, had a diphasic waveform, and its amplitude was about 0.1 microV. Small inter-animal differences were found in waveform and amplitude of the ap. In a pilot experiment exploring pat

A lumped-element model of the chinchilla middle ear.

An air-conduction circuit model was developed for the chinchilla middle ear and cochlea. The lumped-element model is based on the classic Zwislocki model of the same structures in human. Model parameters were fit to various measurements of chinchilla middle-ear transfer functions and impedances, using a combination of error-minimization-driven computer-automated and manual fitting methods. The measurements used to fit the model comprise a newer, more-extensive data set than previously used, and include measurements of stapes velocity and inner-ear sound pressure within the vestibule and the sc

Effect of leukotriene inhibitor on cochlear blood flow in salicylate ototoxicity.

Our previous studies showed that salicylate ototoxicity is associated with decreased levels of vasodilating prostaglandins (PGs) and increased vasoconstricting leukotrienes (LTs) in the perilymph and reduced cochlear blood flow (CoBF). The purpose of this study was to test the hypothesis that leukotriene inhibitor prevents salicylate ototoxicity by preventing abnormal elevation of LT levels in the inner ear, thus averting a decrease in CoBF resulting from abnormal levels of arachidonic acid metabolites in the inner ear. Ototoxicity was induced in chinchillas by either local round window membra

Bacterial tympanogenic labyrinthitis, meningitis, and sensorineural damage.

Pathologic changes (sensorineural hearing loss, labyrinthitis, meningitis) can follow otitis media. Various macromolecular substances demonstrably enter the inner ear via the round window membrane, but its permeability to bacteria is less known. We inoculated Streptococcus pneumoniae type 7F bilaterally into the middle ears of two groups of chinchillas, with and without grafted round window membranes. Inner ears of inoculated animals were observed by light and electron microscopy. None with continuous grafts had labyrinthitis. Bacteria penetrated all three layers of nongrafted round window mem

Effects of combined rupture of Reissner's membrane and the round window on hearing in the guinea pig.

Hearing impairment caused by sequential rupture of Reissner's membrane and the round window was investigated in guinea pigs to determine if the double-membrane break can cause acute profound sensorineural hearing loss. Reissner's membrane was lacerated through the stria vascularis at one of the turns of the cochlea, followed by rupture of the round window to create leakage of the perilymph. Action potentials (APs) to tone pip stimuli were recorded from the Fallopian canal electrode before and 90 min after these procedures. The results showed that the ears with the double-membrane break at the
