Multivariate Risk Adjustment of Primary Care Patient Panels in a Public Health Setting: A Comparison of Statistical Models.

We compared prospective risk adjustment models for adjusting patient panels at the San Francisco Department of Public Health. We used 4 statistical models (linear regression, two-part model, zero-inflated Poisson, and zero-inflated negative binomial) and 4 subsets of predictor variables (age/gender categories, chronic diagnoses, homelessness, and a loss to follow-up indicator) to predict primary care visit frequency. Predicted visit frequency was then used to calculate patient weights and adjusted panel sizes. The two-part model using all predictor variables performed best (R = 0.20). This mod

Outcomes of Colostomy Reversal in a Public Safety Net Hospital: The End or Beginning of a New Problem?

Colostomy reversals can be technically challenging and linked to significant morbidity. There is sparse evidence that evaluates outcomes after colostomy reversals performed by acute care surgeons. We performed a review of 61 colostomy reversals from January 2011 to January 2014. Colostomies for acute diverticulitis were predominate, n = 32 (52%). Traumatic colorectal injuries were n = 15, 25 per cent. Colorectal cancer was n = 8, 13 per cent. Sigmoid volvulus accounted for n = 2 cases. Abdominal sepsis from adhesions was n = 3. A rectal foreign body was for n = 1 case. The time to reversal was

Same-Day Total Thyroidectomy Is Safe in a High-Volume Safety-Net Hospital.

BACKGROUND: Same-day total thyroidectomy is supported by earlier studies, yet adoption remains limited, particularly in safety-net hospitals where socioeconomic barriers may complicate outpatient care. We prospectively evaluated implementation of a protocolized same-day total thyroidectomy program in a high-volume, safety-net academic medical center. STUDY DESIGN: We conducted a prospective quality improvement study of all patients undergoing total thyroidectomy from January 2023 to December 2024. Patients meeting predefined criteria were offered same-day discharge after 4-hour observation wi

Implementation of Ambulatory Kidney Supportive Care in a Safety Net Hospital.

CONTEXT: Chronic kidney disease (CKD) disproportionately impacts lower socioeconomic groups and is associated with many symptoms and complex decisions. Integration of Kidney Supportive Care (KSC) with CKD care can address these needs. To our knowledge, this approach has not been described in an underserved population. OBJECTIVES: We describe our adaptation of an ambulatory integrated KSC and CKD clinic for implementation in a safety net hospital. We report our utilization metrics; characteristics of the population served; and visit activities. METHODS: We considered modifications from the pe

Improving Colorectal Cancer Screening in a Regional Safety-Net Health System over a 10-Year Period: Lessons for Population Health.

BACKGROUND: Despite national policy efforts to increase colorectal cancer (CRC) screening, rates in vulnerable populations remain suboptimal. Many types of interventions have been employed, but their impact on improving population-level rates of CRC screening over time is uncertain. OBJECTIVE: Assess the impact of 10 years of different in-reach and outreach strategies to improve CRC screening and identify factors associated with being screen up-to-date (SUTD). DESIGN: Observational cohort study. PARTICIPANTS: Patients aged 50-74 years from 12 community-based primary care clinics in an integ

Dramatic Reduction in Opioid Prescribing for Common Outpatient Orthopaedic Lower Extremity Surgical Procedures at a Louisiana University-Based Safety-Net Hospital After Enactment of Opioid-Limiting State Legislation.

INTRODUCTION: The opioid epidemic in the United States has been fueled by overprescribing opioids, which is particularly apparent in orthopaedics. Stakeholders, including state legislatures, have attempted to curb opioid overprescribing to reduce related abuse/dependence, overdose deaths, and diversion. OBJECTIVE: To examine trends in opioid prescribing for common outpatient lower extremity orthopaedic surgeries in a population vulnerable to prolonged opioid use before and after the enactment of a 2017 Louisiana state law limiting opioid prescribing for acute pain. METHODS: Eligible patients

Addressing the quality and cost of cholecystectomy at a safety net hospital.

BACKGROUND: The aim of this study was to evaluate the effects of safety-net burden on outcomes of a common, urgent operation like cholecystectomy. METHODS: We identified all cholecystectomies performed from 2005 to 2011 in the California State Inpatient Database and separated them into three cohorts based on the performing hospital's safety-net burden. Hierarchical multivariable regression analyses were performed with outcomes including laparoscopy, advanced disease, morbidity, length of hospitalization, and cost. RESULTS: Safety-net hospitals had similar rates of laparoscopy, overall advanc

Factors Associated With Evidence of Self-Measured Blood Pressure Adoption in an Urban Safety-Net Health System.

Uncontrolled hypertension is a leading cause of preventable cardiovascular mortality. Although self-measured blood pressure (SMBP) monitoring is recommended by evidence-based guidelines, adoption remains uneven in safety-net health systems. We conducted a cross-sectional analysis in an urban safety-net health system to assess factors associated with documented SMBP adoption. Using electronic health record (EHR) data from patients included in a safety-net health system's hypertension registry, we defined documented SMBP adoption as documentation of at least one patient-reported value in the EHR

The Role of Routine Staging Laparoscopy for Pancreatic Adenocarcinoma in Underserved Populations: A 14-Year Experience at a Safety-Net Hospital.

BACKGROUND: There are no consensus guidelines for staging laparoscopy (SL) in pancreatic ductal adenocarcinoma (PDAC) and existing data largely reflect insured referral populations. Routine SL has not been studied in underserved patient populations who face disparities in cancer diagnosis and treatment. We evaluated the utility of SL for potentially operable PDAC at an urban safety-net hospital and assessed its association with survival. METHODS: A single-institution retrospective review was performed of all patients undergoing SL for potentially resectable PDAC (May 2011-May 2025). The prima

Time to Cancer Screening Completion Following an SMS Reminder Intervention in a Large Federally Qualified Health Center Network: Secondary Data Analysis.

BACKGROUND: Delays in completing cancer screening diminish the preventive benefits of early detection, particularly among women receiving care in Federally Qualified Health Centers (FQHCs). Although many patients receive SMS reminders and complete screening, less is known about how quickly they complete testing or which patient-level and structural factors are associated with delays. OBJECTIVE: This study examined factors associated with time to cancer screening completion among women aged 50 years or older who received SMS reminders and completed screening across a large FQHC network in Texa

Association Between Hospital Safety-Net Status and Delivery of Rehabilitation to Older Adults With Acute Respiratory Failure.

BACKGROUND: Older adults with socioeconomic disadvantage experience greater decline in function and cognition after critical illness, an adverse outcome potentially preventable through mobilization. Whether safety-net hospitals (SNHs) that serve the highest proportions of patients with socioeconomic disadvantage are less likely to deliver rehabilitation during hospitalization with stay in the ICU is unknown. RESEARCH QUESTION: Are SNHs less likely to provide rehabilitation (physical or occupational therapy or both) services to older adults hospitalized with acute respiratory failure (ARF) who

Integrating a trauma recovery center into an urban hospital setting serving multiply marginalized patients in the Southeastern United States.

BACKGROUND: Few interpersonal violence survivors receive psychosocial services, and those who are multiply marginalized are among the least likely to receive needed care. Trauma recovery centers (TRCs) aim to reduce health disparities by increasing access to trauma-focused care. This study describes the initial adoption, implementation, and reach of the first TRC in the southeastern USA. METHODS: Funding was awarded to support the adoption of the Grady TRC within an urban safety net hospital in Atlanta, Georgia, and interdisciplinary collaboration was leveraged to support implementation. Seroprevalence of Helicobacter pylori Infection Among Asymptomatic Patients Served by a Safety-Net Healthcare System.

BACKGROUND & AIMS: Helicobacter pylori screening and eradication is a cost-effective strategy for primary gastric cancer prevention in high-risk populations outside the United States. There is a paucity of population-based H pylori seroprevalence data available in high-risk U.S. groups, particularly data for low-income, uninsured patient populations. METHODS: We used data from a cross-sectional study of a largely underserved, immigrant, and minority patient population. Inclusion criteria included asymptomatic individuals 30-65 years of age without prior H pylori testing or treatment. Consente
