Envenomation-Induced Hemophagocytic Lymphohistiocytosis (HLH): First Reported Adult Case Following A Spider Bite.

INTRODUCTION: Hemophagocytic lymphohistiocytosis (HLH) is a life-threatening hyperinflammatory syndrome driven by dysregulated activation of cytotoxic T lymphocytes and macrophages. While infections and malignancies represent the most frequently identified triggers, HLH precipitated by arthropod envenomation is exceptionally rare, with prior reports confined predominantly to paediatric populations. CASE DESCRIPTION: A 65-year-old male with multiple comorbidities developed progressive fever, cytopenias, extreme hyperferritinaemia (>8,000 ng/ml), and multiorgan dysfunction following a suspected

Brown Recluse Spider Bite Superimposed With Methicillin-Resistant Staphylococcus aureus Infection: A Case Report.

Brown recluse spider (BRS) bites are known for their potential to cause localized dermonecrosis and, in severe cases, systemic complications such as loxoscelism. However, secondary infections following envenomation are rare and can dramatically alter the clinical course. We present the case of a 52-year-old male who developed progressive cellulitis, tissue necrosis, and abscess formation after a confirmed BRS bite to the left leg. Imaging revealed extensive subcutaneous fluid collections, and surgical drainage confirmed superimposed methicillin-resistant Staphylococcus aureus (MRSA) infection.

A potential life-threatening Asian funnel-web spider bite (Macrothele gigas) in central Taiwan.

Five funnel-web spiders in the genus Macrothele are widely distributed to Taiwan. We herein reported the severe case of a woman bitten by a male Macrothele gigas who present with autonomic (i.e., profuse sweating and piloerection), cardiovascular (hypertension and tachycardia), and neurologic effects (perioral numbness) in addition to local tissue swelling and regional limb pain. Morphine and ampicillin/sulbactam were administered. Her cardiovascular, neurologic, and local symptoms gradually improved, and thus was discharged 24 h post-bite. However, persistent diaphoresis and piloerection last

Acute kidney injury with pigment nephropathy following spider bite: a rarely reported entity in India.

Acute kidney injury (AKI) can be seen in tropical regions following bites of various venomous animals and insects. Renal failure is seen most commonly following the bite of spiders of the Loxosceles spp. Dermonecrosis, systemic inflammatory response, hemolysis, rhabdomyolysis, and direct venom-related effects are postulated as causes of AKI. We report a documented case of AKI with pigment nephropathy following the bite of a brown spider from a tropical region which is known to have many venomous animals but has no previous reports of AKI following spider bite. Whether this is due to absence of

Verified bites by yellow sac spiders (genus Cheiracanthium) in the United States and Australia: where is the necrosis?

Spiders of the genus Cheiracanthium are frequently reported in review articles and medical references to be a definitive cause of dermonecrosis or necrotic arachnidism in humans. We provide 20 cases of verified bites by Cheiracanthium spiders from the United States and Australia, none with necrosis. A review of the international literature on 39 verified Cheiracanthium bites found only one case of mild necrosis in the European species C. punctorium. The basis for the suggestion that this spider genus causes dermonecrosis seems to be mostly inference from venom experiments in rabbits and guinea

Reversible Myocarditis Following Black Widow Spider (Latrodectus spp.) Bite in Egypt: A case report.

Black widow spiders (BWSs) are poisonous spiders of the Arthropoda phylum that live in the Mediterranean region. The effects of BWS bites ranges from local damage to systemic manifestations including paresthesia, stiffness, abdominal cramps, nausea, vomiting, headache, anxiety, hypertension and tachycardia. However, cardiac involvement following a BWS bite is uncommon. We report a 35-year-old male patient who presented to a tertiary hospital in Menoufia, Egypt, in 2019 and developed acute pulmonary oedema with electrocardiogram (ECG) changes that showed ST elevation in leads I and aVL with rec

A case of pediatric northern black widow spider (Latrodectus variolus) bite in New York, USA.

Black widow spiders (Latrodectus spp.) are generally perceived as inhabiting southern or western United States. However, some species, such as the northern black widow, Latrodectus variolus, are also found in more temperate regions of USA and adjacent Canada. This species also appears to expand its range northward with the climate change increasing the risk of human encounters. To our knowledge, this is the first case report of a pediatric envenomation from a northern black widow bite in northeastern US. Medical practitioners often fail to recognize the signs and symptoms of latrodectism delay

Necrotic arachnidism and intractable pain from recluse spider bites treated with lumbar sympathetic block: a case report and review of literature.

Brown recluse (Loxosceles reclusa) spider bites mainly occur in the southern and Midwestern United States. The clinical manifestation of brown recluse spider bites varies from skin irritation, a small area of tissue damage to neuropathic pain, necrotic arachnidism and severe systemic reactions such as acute renal failure and even death. Treatment is controversial and nonspecific. We describe a case of extensive right lower extremity tissue necrosis and intractable neuropathic pain treated with lumbar sympathetic block in a patient with a documented brown recluse spider bite. Both his pain and

Coombs-positive hemolytic anemia secondary to brown recluse spider bite: a review of the literature and discussion of treatment.

The bite of the brown recluse spider (Loxosceles reclusa) typically results in local, dermonecrotic skin lesions. Rarely, these bites may precipitate systemic disturbances of varying severity collectively known as systemic loxoscelism. The more severe systemic alterations attributed to the venom of this arachnid include hemolytic anemia, multiorgan failure, disseminated intravascular coagulation, or even death. Coombs-positive hemolysis associated with brown recluse spider bites has rarely been documented in the literature. We report 2 cases of systemic loxoscelism in young women associated wi

First record of a case of cutaneous loxoscelism caused by violin spider (Loxosceles yucatana) bite in Yucatan, Mexico.

Loxoscelism occurs when the dermonecrotic venom produced by spiders of the genus Loxosceles, known as "violin spiders," enters a person's organism through their bite. In Mexico there is an underreporting of loxoscelism cases due to the absence of laboratory tests for its diagnosis and the complexity of the clinical picture. The aim of this paper is to describe a case of cutaneous loxoscelism caused by the bite of Loxosceles yucatana in a resident of Yucatan, Mexico. Cutaneous loxoscelism is the most frequent and less severe type. This case was diagnosed by means of the symptomatology registere

A rare cause of severe periorbital edema and dermonecrotic ulcer of the eyelid in a child: brown recluse spider bite.

Spider bites are a worldwide problem. Brown recluse spider bites can lead to severe local or systemic clinical effects, such as edema, necrotic ulcer, rashes, fever, chills, nausea, vomiting, malaise, arthralgia, myalgia, hemolysis, leukocytosis, disseminated intravascular coagulation, renal failure, and death. Eyelid bites from brown recluse spiders are rare. We report a child with severe facial edema and a dermonecrotic ulcer on the eyelid. Upon laboratory examination, leukocytosis with a significant left shift was detected. The patient was treated with antibiotics, systemic corticosteroid a

Verified envenomations by crevice weaver spiders (genus Kukulcania): Bites are of minor expression but the spiders are commonly misidentified as medically important brown recluses (genus Loxosceles) in North America.

From southern North America, five verified bites by crevice weaver spiders, Kukulcania spp. (Filistatidae), are presented here, three of which are pediatric cases. Although the envenomation manifestations were of minimal expression, the salient aspect of this report is that Kukulcania spiders are frequently misidentified as brown recluse spiders (genus Loxosceles) which are infamous for causing serious dermonecrosis and rarely, life-threatening systemic effects. Misidentification of this relatively harmless spider as a medically important recluse when presented to a physician in an envenomatio

Massive dermal ulcerative lesions because of brown recluse spider bite: a rare case report and review of literature.

A brown recluse spider (BRS) bite is challenging to confirm, but can be clinically diagnosed by considering the location, the season of the year and the clinical manifestations. We described a 26-year-old male who presented after a BRS bite with a skin lesion, bruising, severe swelling and diffuse blisters on the right lower extremity after 3 days. This case should be considered in the differential diagnosis of necrotizing fasciitis. Although spider bite poisoning is rare, proper diagnosis and management are important because, in some cases, it can have devastating outcomes.

Medical grand rounds. West Virginia University Health Sciences Center. Bites and stings. Part 1. Spiders.

Most animals can bite or sting. In narrowing the kingdom down to those that harm humans, the field still is vast. It would be interesting to explore the rich variety of pathology produced in us by moray eels, lionfish, sea urchins, jellyfish, sting rays, fire ants, kissing bugs, flies, lice, mosquitoes, ticks, mites, fleas, puss caterpillars, centipedes, snakes, dogs and cats, camels, and myriad other creatures including homo sapiens (not a trivial biter)--but for this grand rounds, the topic will simply be spiders (Part 1), bees and vespid (Part 2). Vespids are the wasps, yellow jackets and h

Management of skin necrosis and wound dehiscence following brown recluse spider bite in the course of breast reconstruction: A case report.

BACKGROUND: Breast reconstruction following mastectomy is a critical component of breast cancer treatment, aimed at improving patient quality of life. However, the management is fraught with potential complications, including skin necrosis and wound dehiscence, which can significantly impact clinical outcomes. CASE PRESENTATION: We report a unique case of a patient, 5 years post-breast reconstruction following mastectomy and radiation therapy, who developed severe skin necrosis and wound dehiscence due to a brown recluse spider bite on the reconstructed breast. The complication necessitated t

Holistic Therapy in a Patient with Necrotic Ulcer Caused by the Bite of Brazilian Wandering Spider: A Case Report of Challenging Treatment with Combined Therapies.

Hard-to-heal wounds remain a significant challenge for healthcare professionals, particularly in aging populations. Although most chronic wounds are associated with diabetes or chronic venous insufficiency, rare etiologies should also be considered. One such cause is envenomation by Phoneutria spp. (native to South America, rare in Europe). Their venom contains potent neurotoxins. While systemic manifestations are more commonly reported, localized necrotic skin lesions may also occur. This case report presents a rare chronic wound following a suspected Phoneutria spider bite and highlights the

Nephrotic syndrome due to minimal change disease secondary to spider bite: clinico-pathological case of a non-described complication of latrodectism.

The patient was an 18-year-old man who developed nephrotic syndrome after a 'wheat spider' bite (Latrodectus mactans). Due to this atypical manifestation of latrodectism, a renal biopsy was performed showing minimal change disease. The nephrotic syndrome subsided after 1 week without specific treatment. This self-limited evolution suggests that the mechanism of podocyte damage was temporary and potentially mediated by a secondary mechanism of hypersensitivity or direct effect of the α-latrotoxin. The patient did not show signs of relapse in subsequent checkup. This is the first reported case o
