Emergency Diagnosis of Giant Cell Tumour (GCT) of Spine by Image Guided Fine Needle Aspiration Cytology (FNAC).

Giant cell tumour (GCT) of spine is an extremely rare neoplasm accounting 0.5% to 1.5% of all cases. The patient usually presents with weakness of lower limbs. We describe a case of 25-year-old male who presented with sudden onset of paraplegia. On plain radiograph there was an osteolytic lesion in T9 vertebra. Computed tomography (CT) scan revealed expansile lytic lesion in T9 vertebral body with involvement of posterior elements on right side with associated soft tissue mass in the extradural location extending into the spinal cord. Further Magnetic Resonance Imaging (MRI) scan (T1 contrast)

Myeloid sarcoma of the thoracic spine: A case report.

BACKGROUND: Myeloid sarcoma is an uncommon malignant neoplasm that typically arises at extramedullary sites and is associated with a diagnosis of acute myeloid leukemia. While myeloid sarcoma can involve any organ, central nervous system involvement is rare, particularly in the adult population. CASE DESCRIPTION: An 87-year-old female presented with progressive paraparesis of 5 days' duration. The magnetic resonance imaging (MRI) revealed an epidural tumor from T4 to T7 with cord compression. When she underwent a laminectomy for tumor resection, the pathology revealed a myeloid sarcoma with m

Canine Snake-Eye Myelopathy: Clinical, Magnetic Resonance Imaging, and Pathologic Findings in Four Cases.

Intramedullary signal change (ISC) is a non-specific finding that is frequently observed on magnetic resonance imaging (MRI) examinations of the canine spinal cord. ISC can represent a variety of primary pathological processes such as neoplasms or myelitides or secondary changes such as edema, cysts, gliosis, or myelomalacia. An unusual phenotype of ISC is the "snake-eye" myelopathy (SEM), which refers to bilaterally symmetric T2 hyperintensities preferentially affecting the ventral horn gray matter on transverse MR images, which resemble a pair of snake's eyes. The pathophysiology of SEM is p

Microsurgical removal of a C1-C5 laterally located intramedullary subependymoma through a postero-lateral sulcus approach: case report, surgical video and critical review of diagnostic and surgical challenges.

INTRODUCTION: Spinal subependymomas are rare, benign, and slow-growing intramedullary tumors, often misdiagnosed preoperatively due to their radiological resemblance to more common spinal cord neoplasms such as ependymomas and astrocytomas. Given their rarity, the optimal surgical approach and oncological strategy remains debated. While these tumors are histologically benign, obtaining a definitive diagnosis is crucial due to the diagnostic overlap with more aggressive lesions. In this case, a postero-lateral sulcus approach was selected based on the tumor's location and spinal cord rotation,

Surgical treatment of intramedullary spinal cord neoplasia in two dogs.

An area of intramedullary spinal cord enlargement at the thoracolumbar junction was identified by myelography in each of two dogs. Exploratory laminectomy followed by durotomy and myelotomy permitted removal of an intramedullary neoplasm in each dog. In one animal the tumour was diagnosed to be an ependymoma, in the other the tumour had the characteristics of a nephroblastoma. Following surgery each dog recovered quickly to pre-operative status or better. In one dog there was long term resolution of neurological deficits, while the other was euthanased because of persistent faecal incontinence

Imaging review of the atypical spinal epidural space pathologies.

Pathologies affecting the spinal epidural space (SES) comprise various abnormalities. However, they all have the potential to cause thecal sac narrowing or spinal cord compression. In this review, we group these pathologies into degenerative, infective, neoplastic, vascular, traumatic, and others, focusing on their imaging features. Degenerative pathologies of the SES range from disc to facet disease, with a particular emphasis on the less common degenerative pathologies in this review. Infective pathologies affecting the epidural space include spondylodiscitis and associated epidural phlegmon

Multifocal lumbar myxopapillary ependymoma presenting with drop metastasis: a case report and review of the literature.

INTRODUCTION: Spinal myxopapillary ependymomas (SME) are rare WHO grade II neoplasms of the spinal cord. Despite their good prognosis, they have a high propensity for metastasis and recurrence, although the presentation of SME as multifocal is uncommon. CASE PRESENTATION: Here we describe a rare case of a 34-year-old man who presented with painful bilateral radiculopathy with sexual dysfunction and altered sensation with defecation. The patient also reported worsening weakness of bilateral lower extremities when climbing stairs. Biopsy results revealed multifocal SME in the lumbar and sacral

Recurrent solitary fibrous tumor of the spinal cord: A case report and literature review.

Solitary fibrous tumor (SFT) is a benign mesenchymal neoplasm occurring anywhere in the body, such as the visceral pleura, while it is extremely rare in the central nervous system, especially within the spinal cord. Here, we present a case of recurrent spinal SFT in a 44-year-old woman who had the tumor resected 5 years before. This time, her magnetic resonance imaging revealed an intradural tumor at the level of C6-7. A secondary resection was performed completely, and the patient's neurological conditions recovered fully after resection. Histological and immunohistochemical findings revealed

Acute paraparesis from hemorrhagic spinal ependymoma: diagnostic dilemma and surgical management. Report of two cases and review of the literature.

Spinal ependymomas are a common type of primary spinal cord neoplasm that frequently occurs in the lumbar spine. The authors report on two patients who presented with acute neurological decline after hemorrhage into ependymomas of the filum terminale. Both were transferred to the authors' institution because of diagnostic uncertainty and a concern about possible intradural vascular abnormalities. Both patients underwent lumbar laminectomies for tumor resection. The pathological finding in each case was myxopapillary ependymoma. Both patients made a significant recovery and were ambulatory and

Value of magnetic resonance imaging in the evaluation of patients with complete and high degree block due to intracanal neoplasm.

Magnetic resonance imaging (MRI) of 26 patients with high degree or complete epidural and/or intradural myelographic block, due to primary or secondary neoplasm were analysed. The ratio of false negative MRI was 11.5 per cent. MRI findings were considered to be indicative of lesser block than indicated by myelography in almost 50 per cent of patients. Correlation between the two modalities was highest in cases associated with significant vertebral collapse, malalignment and angulation of the spine and angulation of the spinal cord.

Ependymomas: a clinicopathologic study.

Since 1924, when ependymomas were first classified as a distinctive glial neoplasm by Bailey, much has been published concerning these tumors, but there are important points of interest that are still not clear. In order to study more fully the clinical and pathologic characteristics of the ependymoma, we identified 62 patients with histologically proven neoplasms. Twenty-two were supratentorial, 21 were infratentorial, and 19 were intramedullary spinal cord tumors. These groups had mean ages of 17, 7, and 41 years, respectively, at the time of first symptoms. The presenting and accompanying s

Neurocytoma-like neoplasm of the thoracic spine in a 15-month-old child presenting with diffuse leptomeningeal dissemination and communicating hydrocephalus. Case report.

This unusual mixed glioneuronal neoplasm of the spine resembling central neurocytoma is only the second reported example of a neoplasm of this type involving the spinal cord and is, seemingly, the first to present with diffuse leptomeningeal dissemination and communicating hydrocephalus. This 15-month-old boy presented with somnolence, bilateral sixth nerve palsy, and lower-extremity weakness and was found to harbor a primary neoplasm within the thoracic spine and associated syrinx, widespread leptomeningeal dissemination, and communicating hydrocephalus. The patient underwent cerebrospinal fl

Gliomatosis of the brain and spinal cord masquerading as infective lesions.

BACKGROUND: Gliomatosis of the brain or spinal cord is an infiltrating glial neoplasm that shows widespread invasion of the central nervous system with relative sparing of the underlying cytoarchitecture. Acceptance of the idea that the condition represents a distinct entity remains controversial in the absence of conclusive pathogenetic data. The clinico-pathological problems and difficulties in the ante-mortem diagnosis as well as the clinical and pathological similarities to infective lesions are evaluated. METHODS AND RESULTS: Three cases of cerebral and spinal gliomatosis are presented t

Canine glioma in the first year of life: 5 cases.

Most canine gliomas occur in adult and aged dogs, and reports in puppies < 12-mo-old are exceedingly rare. Here we describe the occurrence of gliomas in 5 dogs ≤ 12-mo-old. The affected patients (4 males, 1 female) were 3-12-mo-old (x̄ = 6.6-mo-old). None of the dogs were brachycephalic. Clinical signs consisted of dullness (2 cases), seizures (2 cases), vestibular signs, and deafness (1 case each). All patients were euthanized. Grossly, neoplasms were pale-tan or red, soft masses in the telencephalon (4 cases) or gelatinous leptomeningeal thickening in the brain and spinal cord (1 case). Neop

Cervical disc herniation producing acute brown-sequard syndrome.

Brown-Sequard syndrome may be the result of penetrating injury to the spine, but many other etiologies have been described. This syndrome is most commonly seen with spinal trauma and extramedullary spinal neoplasm. A herniated cervical disc has been rarely reported as a cause of this syndrome. We present a case of a 28-year-old male patient diagnosed as large C3-C4 disc herniation with spinal cord compression. He presented with left hemiparesis and diminished sensation to pain and temperature in the right side below the C4 dermatome. Microdiscectomy and anterior cervical fusion with carbon fib

[Primary diffuse leptomeningeal leiomyomatosis].

A female patient aged 28 fell ill with manifestations of sensory aphasia, headache and signs of cerebral compression. Except for a bilateral papilledema there were no further pathological clinical and paraclinical findings. Later on, amaurosis, deafness, anosmia and generalized muscular hypotonia developed. The nuclear magnetic resonance image revealed a major accumulation of contrast medium in the leptomeninx. Biopsy demonstrated a mesenchymal neoplasm in the leptomeninx. After a strong rise in intracranial pressure, the patient died from a bulbar brain syndrome. Microscopy revealed a diffuse

Deciphering Novel Molecular Targets in Neuro-Oncology: An Update.

Neuro-oncology is the study of brain and spinal cord neoplasms. Molecular targets and signaling pathways are pivotal in advancing modern healthcare, particularly in personalized medicine. Signaling pathways, which regulate cellular processes such as growth, division, and survival, are frequently dysregulated in cancer. Targeting these pathways has enabled the development of personalized therapies that improve efficacy while minimizing side effects. This approach has led to significant improvements in patient outcomes, reduced treatment toxicity, and a shift toward precision medicine, driving i

Acute neurological syndrome complicating secondary hyperparathyroidism.

Longstanding severe hyperparathyroidism can lead to the formation of "brown tumors," benign but locally aggressive neoplasms. We present the case of a young woman with end-stage renal disease on hemodialysis with uncontrolled secondary hyperparathyroidism. The patient presented with progressive lower extremity weakness and back pain. Imaging showed multiple lytic bone lesions, the most impressive being a T3 spinal lesion causing cord compression. She underwent urgent neurosurgical resection of the tumor at T3 followed by subtotal parathyroidectomy. Our case touches upon the pathophysiology, co

Atypical rhabdoid tumor of lateral ventricle: Report of an unusual tumor.

Supratentorial atypical teratoid rhabdoid tumors (AT/RTs) of infancy and childhood are rare, highly malignant neoplasms, most common in the first 2 years of life. In spite of multiple treatment regimens consisting of surgical resection, radiation therapy, and multi-agent chemotherapy, the prognosis is very poor. The majority of these tumors are located in the cerebellum, cerebellopontine angle, pineal gland, spinal cord, and the suprasellar region; supratentorial location is relatively uncommon, and the intraventricular location is extremely rare. We report a rare case of AT/RT arising in the

[Tumor-related radiographic changes of the vertebral column in childhood (author's transl)].

Benign tumors and malignant primary or metastatic neoplasms of the vertebral column are rare in childhood. Radiographic changes are sometimes detected by routine investigations since no clinical symptoms are present. The type of tumor can only rarely be specified from the radiographic changes. Usually a specific diagnosis requires histologic studies. If a tumor the vertebral column is suspected detailed radiographic studies are mandatory to secure an early diagnosis. The impending danger of spinal cord compression makes an early diagnosis and rapid surgical intervention even more important. In
