Modified ultrafiltration and postoperative course in patients undergoing repair of tetralogy of fallot.

BACKGROUND: Expected benefits of modified ultrafiltration (MUF) include increased hematocrit, reduction of total body water and inflammatory mediators, improved left ventricular systolic function, and improved systolic blood pressure and cardiac index (CI) following cardiopulmonary bypass (CPB). This prospective randomized trial tested this hypothesis. METHODS: Seventy-nine patients undergoing intracardiac repair of tetralogy of fallot were randomized to conventional ultrafiltration (CUF) + MUF (n = 39) or only CUF group (n = 40). The primary outcome was a change in hematocrit. Secondary outc

Vascular refilling is independent of volume overload in hemodialysis with moderate ultrafiltration requirements.

Introduction Blood volume changes and vascular refilling during hemodialysis (HD) and ultrafiltration (UF) have been assumed to depend on volume overload (Vo ). It was the aim to study the magnitude of vascular refilling in stable HD patients with moderate volume expansion in everyday dialysis using novel technical approaches. Methods Patients were studied during routine dialysis and UF based on clinical dry weight assessment. Pre-dialysis Vo was independently measured by bioimpedance spectroscopy. Vascular refilling volume (Vref ) was calculated as: Vref  = Vuf  - ΔV, where ΔV is the absolute

Is there a competition between urine volume and peritoneal ultrafiltration in peritoneal dialysis patients?

Many peritoneal dialysis patients are overhydrated. Overhydration may lead to hypertension and left ventricular hypertrophy, and may be related to inflammation and malnutrition. The presence of overhydration is not always detected by clinical examination. Especially patients with peritoneal ultrafiltration failure and/or negligible residual renal function are prone to overhydration. Treatment consists of a combination of sodium and fluid restriction, in combination with increased peritoneal ultrafiltration and loop diuretics in patients with residual diuresis. Peritoneal ultrafiltration can be

Studies on the effect of humic acids and phenol on adsorption-ultrafiltration process performance.

Application of pressure-driven membrane processes, such as ultrafiltration (UF) and microfiltration (MF) for surface water treatment have become very popular during last decades. Membrane fouling by humic substances (HS) is one of the major limiting factors in these processes. In order to alleviate the unfavorable effects of the presence of HS in the feed on the process performance UF and MF are often combined with adsorption on powdered activated carbon (PAC). The main goal of the present study was to evaluate the effect of humic acid (HA) on membrane fouling during UF. Moreover, the effect o

Treatment of heart failure and ascites with ultrafiltration in patients with intractable alcoholic cardiomyopathy.

BACKGROUND: In Western countries the excess of alcohol intake causes, secondary, non ischaemic cardiomyopathy and cirrhosis. Frequently, therapy is not effective so ultrafiltration was tried on patients affected, with positive effects on life quality. We tried to verify utility and tolerance to peritoneal ultrafiltration in a group of subjects affected by heart failure secondary to alcoholic cardiomyopathy, refractory to conventional therapy. METHODS: Sixteen patients (14 males, 2 females) with heart failure and ascites affected by alcoholic cardiomyopathy were studied. All subjects were in I

Ascorbate is the principal reductant of chromium (VI) in rat liver and kidney ultrafiltrates.

Chromium (VI) reductase activity was measured in ultrafiltrates of rat liver and kidney after various pretreatments in vitro at 37 degrees C and pH 7.0. Preincubation of ultrafiltrates with L-ascorbate oxidase (EC 1.10.3.3), which specifically eliminated ascorbate, blocked approximately 80% of the Cr(VI) reductase activity. Heat-denatured ascorbate oxidase had no effect on Cr(VI) reductase activity in ultrafiltrates. Preincubation of ultrafiltrates with N-ethylmaleimide, which non-specifically blocked sulfhydryls, including reduced glutathione, decreased Cr(VI) reductase activity by only 20%.

Polymer Microfiltration and Ultrafiltration Membranes Containing Inorganic Modifiers: Functional Properties and Performance-Review.

Modifying MF and UF polymer membranes with hydrophilic compounds is one of the well-known antifouling strategies, which also allows one to achieve a compromise between fluid permeability and the retention of one or another species. Hydrophilizing inorganic substances are widely used for modifying due to their enhanced chemical stability and multifunctionality. In this review, the inorganic modifiers are categorized according to their effect on organic contaminants and bacteria: (i) non-destructive (natural and synthetic aluminosilicates, phosphates and oxides of multivalent elements, particula

Investigation into Clearance of Organic Compounds from Biomanufacturing Process Streams during Ultrafiltration/Diafiltration.

OBJECTIVE: Ultrafiltration and diafiltration (UF/DF) operations have been demonstrated to clear leachables from drug substance, however there is limited data available. Consequently, comprehensive and systematic characterization of leachables clearance during UF/DF is required and essential. METHODS: To achieve this, the reduction capacity for 28 selected organic compounds spiked into 3 different proteins during UF/DF processes was investigated using liquid chromatography high-resolution mass spectrometry. Selection of compounds was based on their presence in representative biomanufacturing p

Ceramic Membrane Ultrafiltration for Biogenic Elemental Sulfur Recovery: Performance Optimization and Fouling Analysis.

Biological sulfate removal in mining wastewater generates recoverable elemental sulfur particles that require efficient downstream separation. This study evaluated ceramic membrane ultrafiltration for separating biogenic sulfur from membrane biofilm reactor effluent's supernatant using 50-, 150-, and 300-kDa membranes under varying transmembrane pressure and crossflow velocity. All conditions achieved high sulfur rejection (95.9%-98.1%), whereas normalized flux declined to 0.06-0.26, showing that hydraulic performance was controlled by fouling rather than particle retention. Response-surface a

Tannic acid: an underexplored fraction of natural organic matter in ultrafiltration membrane processes.

Ultrafiltration (UF) membranes are effective for removing natural organic matter (NOM), a known precursor of carcinogenic disinfection byproducts. Most previous studies have used humic acid (HA) as a representative NOM in UF processes, whereas research on tannic acid (TA), a naturally occurring polyphenolic compound, remains limited. In this study, the performance (i.e., water flux and NOM removal) of UF membranes treating TA-containing water was evaluated and compared with that of UF membranes treating HA-containing water. Under different pH conditions, both HA and TA showed the highest remov

Conventional ultrafiltration increases viscoelastic clot firmness during cardiopulmonary bypass: a prospective observational study.

PURPOSE: Ultrafiltration (UF) reduces dilution of all blood components during cardiopulmonary bypass (CPB), but its impact on coagulation performance remains unclear. We examined whether conventional UF mitigates the CPB-induced decline in extrinsic-pathway clot firmness (MCF-EXTEM; reference range 50-72 mm) measured by rotational thromboelastometry (ROTEM), a marker of coagulopathic bleeding risk in cardiac surgery. METHODS: In a prospective observational study, we recruited adult cardiac surgery patients who received UF near the end of CPB (n=39) and included patients with Pre-UF MCF EXTEM

Physiologically-Guided Ultrafiltration: A Proposed Ultrasound Framework for Individualized Volume Removal During Hemodialysis.

Ultrafiltration during hemodialysis is the most hemodynamically stressful intervention dialysis patients endure, yet its prescription relies on interdialytic weight gain and clinical gestalt. Rates exceeding 13 mL/kg/h carry substantially higher cardiovascular mortality, and recurrent intradialytic hypotension drives myocardial stunning, accelerated loss of residual kidney function, and progressive cardiac dysfunction. Existing volume management tools, including bioimpedance spectroscopy, relative blood volume monitoring, and even lung ultrasound, address fragments of this problem but none int

Pre-bypass ultrafiltration reduces cytokine burden of blood prime in pediatric cardiac surgery.

Allogeneic red blood cells (RBCs) are commonly used for cardiopulmonary bypass (CPB) circuit priming in congenital heart surgery. While convection-based pre-bypass ultrafiltration (PBUF) corrects acid-base, electrolyte, and metabolite imbalances, its efficacy in removing RBC cytokines/chemokines remains unclear. In a prospective observational study, 22 children (median age: 4.1 months) undergoing congenital heart surgery were enrolled. PBUF of RBC-primed CPB circuits was conducted using bicarbonate-buffered hemofiltration solution. Cytokines/chemokines were quantified in RBC supernatants, CPB

Affinity ultrafiltration based metabolomic profiling directed discovery novel butyrylcholinesterase inhibitors from Uncaria sessilifructus.

The butyrylcholinesterase (BChE) is an attractive target for treating Alzheimer's disease. In this study, we report the discovery of five new monoterpene indole alkaloids (MIAs) along with three known analogues from Uncaria sessilifructus Roxb. as BChE inhibitors using affinity ultrafiltration based metabolomic profiling directed isolation strategy. Their structures were well identified through comprehensive spectroscopic and chiroptical analyses. Compounds 1-2 featured unique glycosidic linkages with 1,3-dioxane structure. All the compounds exhibited BChE inhibitory bioactivity without any cy

Blockade of sodium-glucose co-transporters improves peritoneal ultrafiltration in uraemic rodent models.

BACKGROUND: The most used PD fluids contain glucose as a primary osmotic agent. Glucose peritoneal absorption during dwell decreases the osmotic gradient of peritoneal fluids and causes undesirable metabolic consequences. Inhibitors of sodium-glucose co-transporter (SGLT) type 2 are wildly used for the treatment of diabetes, heart and kidney failure. Previous attempts to use SGLT2 blockers in experimental peritoneal dialysis yielded contrasting results. We studied whether peritoneal SGLTs blockade may improve ultrafiltration (UF) via partial inhibition of glucose uptake from dialysis fluids.

Measurement of peritoneal fluid absorption and ultrafiltration during peritoneal dialysis using segmental bioimpedance.

The aim of this study was to measure intraperitoneal volume (IPV) and ultrafiltration volume (UFV) by monitoring the abdominal resistance using segmental bioimpedance analysis (SBIA, Hydra 4200). Twenty peritoneal dialysis (PD) patients were studied during a fill with 2 L of 2.5% glucose peritoneal dialysate solution. UFVDrain (g) was measured as weight difference between fill and drain dialysate volumes. Ultrafiltration volume (UFVSBIA; ml) and absorption volume were calculated from the IPV curve derived by SBIA. UFVSBIA correlated with UFVDrain ( ). This study may provide actionable clinical

Novel housing designs for nanofiltration and ultrafiltration gravity-driven recycled membrane-based systems.

Ultra-low pressure gravity-driven membrane (GDM) systems have the potential to be significantly less costly and complex than conventional membranes for water treatment applications. To build upon this inherent advantage, this study assesses the reuse of recycled membranes in GDM systems for producing drinking water. Two reverse osmosis spiral-wound modules were recycled into nanofiltration (NF)-like and ultrafiltration (UF)-like membranes via controlled exposure to free chlorine. To operate the recycled membranes, two housing devices, based on a simple fitting and an advanced end-caps design,

Bioimpedance-based rather than weight-based ultrafiltration prescription for children on maintenance hemodialysis
.

The determination of dry weight (DW) in young children on hemodialysis (HD) remains challenging. Bioimpedance analysis (BIA) is a potentially helpful means of estimating the need for ultrafiltration and monitoring body fluids in patients on renal replacement therapy, but its role has not yet been clearly defined. The aim of this paper is to share our experience of prescribing ultrafiltration on the basis of BIA parameters alone. The body weight (BW), resistance (Rx), and reactance (Xc) of a 3-year-old girl on chronic HD were recorded pre- and post-HD over a period of 16 months. The BIA paramet
