Predictors of volumetric growth and auditory deterioration in vestibular schwannomas followed in a wait and scan policy.

OBJECTIVE: The purpose of this study was to identify factors predicting growth and audiologic deterioration during follow-up (FU) in a wait and scan (W&S) policy of vestibular schwannomas (VSs) using a novel volumetric measuring tool. So far, only consecutive magnetic resonance imaging (MRI) is able to show growth objectively, and growth, combined with hearing function, generally dictates further intervention. Other factors predicting growth or hearing deterioration would be invaluable and might ease clinical decision making. STUDY DESIGN: Retrospective case study. SETTING: Tertiary referral

Ossiculoplasty with a cartilage-connecting hydroxyapatite prosthesis for tympanosclerotic stapes fixation.

Many cases of tympanosclerotic stapes fixation are accompanied by fixation or erosion of malleus and/or incus. This status of the ossicular chain is one of the reasons that ossiculoplasty for tympanosclerotic stapes fixation is more difficult than that for otosclerosis. We conducted a retrospective review of seven patients who were operated on for tympanosclerotic stapes fixation between 2002 and 2006. All of the patients had abnormal conditions of the malleus and/or incus and underwent stapedectomy and total ossiculoplasty with hydroxyapatite prosthesis (Apaceram T-7 type), which has a planar

Speech perception in noise using directional microphones in open-canal hearing aids.

BACKGROUND: Individuals with impaired hearing find it difficult to understand speech in the presence of background noise--a problem addressed effectively by directional microphones. As open-canal fittings have become increasingly popular in the recent past, so has the debate about the effective directional benefit available from these devices. PURPOSE: This study investigates the benefit of directional microphones in two commercially available open-canal behind-the-ear hearing aids using the Hearing in Noise Test (HINT). STUDY SAMPLE: Sixteen individuals, between 50 and 85 year of age, with

Otologic and audiologic lesions due to blast injury.

AIM: To evaluate the effect of blast injury on the otologic and hearing state over time. SETTING: Otology unit of a tertiary referral center. METHODS: Seventy-three patients aged 16 to 73 years who sustained physical trauma from an explosion underwent otologic and audiologic examination 3-4 months and one year later. RESULTS: At the first examination, high-frequency sensorineural hearing loss was detected in 57 patients (78%), mixed hearing loss in 13 (19%), and low-tone conductive hearing loss in two (3%). Conductive hearing loss had improved by one year, while the cochlear hearing loss, i

Fully implantable hearing aids in patients with congenital auricular atresia.

INTRODUCTION: Patients with congenital auricular atresia suffer from a conductive hearing loss (HL) with an air-bone gap of 50 to 60 dB. Conventional bone conducting or bone anchored hearing aids are treatment options with several disadvantages and a biophysical limitation of almost no sound attenuation in the skull bone. Surgical construction of the sound conducting apparatus has been performed by others and modified by us into a three-step procedure with in vivo prefabrication of the external ear canal and the tympanic membrane. Although the results improved after inauguration of our modific

Audiological evaluation and self-assessed hearing problems in subjects with single-sided congenital external ear malformations and associated conductive hearing loss.

Previously, unilateral hearing impairment (UHI) has been considered of little consequence. However, a recent meta-analysis of children with UHI displayed educational and behavioural problems and possible delays of speech and language development. Further, patients with UHI consequently report hearing difficulties. Our study investigated hearing function, possible inner ear protection, and self-assessed hearing problems in 57 subjects aged between 3-80 years with single-sided congenital ear malformations and conductive UHI. Pure-tone thresholds and speech recognition (quiet, noise) were measure

Audiometric results of bilateral bone-anchored hearing aid application in patients with bilateral congenital aural atresia.

The effect of bilateral application of bone-anchored hearing aids (BAHAs) was examined in terms of directional hearing and speech recognition in quiet and in noise in four patients with bilateral congenital atresia who, out of pure necessity, had been using a unilateral bone-conduction hearing aid since early life. This study comprised a prospective clinical evaluation in a single subject design; four patients with bilateral congenital atresia originating from the Nijmegen BAHA series participated. Three patients had Treacher Collins syndrome. All four patients had conductive, most probably, s

Audiometric findings in children with chronic suppurative otitis media without cholesteatoma.

Chronic suppurative otitis media without cholesteatoma (CSOMWC) is usually associated with an increase in air conduction thresholds. However, only a few investigations reported on loss of cochlear function in the pediatric population. We undertook a prospective study in order to further delineate air and bone conduction levels in children with CSOMWC and the possible relationship between sensorineural hearing loss (SNHL) and other clinical characteristics that might reflect the extent of disease. Eighty-seven children were enrolled, 40 of which had bilateral disease. Hence, the total number of

Similar phenotypes caused by mutations in OTOG and OTOGL.

OBJECTIVES: Recently, OTOG and OTOGL were identified as human deafness genes. Currently, only four families are known to have autosomal recessive hearing loss based on mutations in these genes. Because the two genes code for proteins (otogelin and otogelin-like) that are strikingly similar in structure and localization in the inner ear, this study is focused on characterizing and comparing the hearing loss caused by mutations in these genes. DESIGN: To evaluate this type of hearing, an extensive set of audiometric and vestibular examinations was performed in the 13 patients from four families

Acclimatization to hearing aids.

OBJECTIVE: Evidence for a clinically significant effect of acclimatization to hearing aids is mixed. The aim of this study was to test for auditory acclimatization effects in new unilateral and bilateral adult hearing aid users. Hypotheses were i) there would be improvements in aided speech recognition in new hearing aid users, compared with unaided listening and with a control group of experienced hearing aid users, and ii) improvements would correlate with severity of hearing loss, hearing aid use, and cognitive capacity. DESIGN: Speech recognition in noise was measured for a 65 and a 75 dB

Preoperative characteristics of auditory brainstem response in acoustic neuroma with useful hearing: importance as a preliminary investigation for intraoperative monitoring.

We classified the results of preoperative auditory brainstem response (ABR) in 121 patients with useful hearing and considered the utility of preoperative ABR as a preliminary assessment for intraoperative monitoring. Wave V was confirmed in 113 patients and was not confirmed in 8 patients. Intraoperative ABR could not detect wave V in these 8 patients. The 8 patients without wave V were classified into two groups (flat and wave I only), and the reason why wave V could not be detected may have differed between the groups. Because high-frequency hearing was impaired in flat patients, an alterna

Aging, hearing acuity, and the attentional costs of effortful listening.

A dual-task interference paradigm was used to investigate the effect of perceptual effort on recall of spoken word lists by young and older adults with good hearing and with mild-to-moderate hearing loss. In addition to poorer recall accuracy, listeners with hearing loss, especially older adults, showed larger secondary task costs while recalling the word lists even though the stimuli were presented at a sound intensity that allowed correct word identification. Findings support the hypothesis that extra effort at the sensory-perceptual level attendant to hearing loss has negative consequences

Editorial: 'auditory neuropathy' and cochlear implantation - myths and facts.

A review of current opinion concerning 'auditory neuropathy' is presented. It is suggested that electrophysiological tests, including electrocochleography, auditory brainstem responses and electrically evoked auditory brainstem responses, together with imaging, can provide information regarding the site of the underlying pathological conditions that may produce the combination of otoacoustic emissions in the absence of auditory brainstem responses in children with hearing loss. It is suggested that in 75% of cases auditory neuropathy can merely be a result of surviving outer hair cells when in
