Up and moving: safety of early ambulation after minor renal trauma.

OBJECTIVE: To evaluate the safety and clinical outcomes of early ambulation versus traditional bedrest in stable adults with low-grade renal trauma. MATERIALS AND METHODS: This retrospective cohort study included adult patients (≥ 18 years) with AAST Grade 1-3 renal injuries and ISS < 15 admitted at a single institution between January 2020 and December 2024. Before November 2023, protocol was bedrest until stable hematocrit for 24 h prior to initiating ambulation, followed by repeat hematocrit to ensure stability. After institutional change, eligible participants were not placed on bedrest a

Ambulance responses to older adults who have fallen: a systematic review.

BACKGROUND: Approximately 10% of emergency ambulance calls are for adults, 65 years and older, who have fallen. Structured management of this group could improve outcomes and cost-effectiveness. This review sought to synthesise evidence for ambulance-based care of older adults who had fallen and the associated impact on patient outcomes. METHODS: Eligibility. Peer-reviewed primary evidence, assessing older adults (aged ≥ 65) who had fallen and received an ambulance response. Information sources. CINAHL (EBSCO), MEDLINE (Ovid), Embase (Ovid), HMIC (Ovid), Web of Science and AMBER were searched

Neonatal transport ground ambulance: the Italian perspective toward a European consensus.

UNLABELLED: The European Union (EU) has established EN 1789:2020 + A1:2023 standards for ground ambulances, categorizing them into types A1, A2, B, and C. Type C ambulances are designated for adult intensive care. However, there are no comparable specifications for neonatal ambulances, despite existing standards for neonatal transport incubators (EN 13976-1 and EN 13976-2). This regulatory gap causes confusion and inconsistency because neonatal emergency transport services (NETS) must rely on adult ambulances that are only partially adapted for newborns. These vehicles often lack essential neo

ALGORITHM FOR EARLY AMBULATION IN INDIVIDUALS UNDERGOING NON-CONVENTIONAL HIP ENDOPROSTHESIS.

INTRODUCTION: The incidence of bone metastases has increased in recent years, driven by population aging and the efficacy of treatments for visceral-origin cancers. Bone metastasis ranks as the third most common type, with approximately one-third located in the proximal femur. Prosthetic hip replacement is a key treatment option, enabling immediate weight-bearing and resumption of patients' normal activities. OBJECTIVE: This study aims to develop an algorithm to encourage early ambulation in patients undergoing hip arthroplasty with non-conventional endoprostheses, targeting improved recovery

Bleeding resuscitation in the ambulance service, an observational study of standard care in Sweden.

BACKGROUND: The ambulance service in Sweden has most often only crystalloids as resuscitation, even though restrictive use of clear fluids in bleeding patients is recommended. The aim of this study was to describe the treatment and outcome of bleeding patients treated by the ambulance service. METHODS: This was a prospective observational multi-center study. Ambulance organizations in six different regions were invited to participate, each of them for a period of six months. Adult bleeding patients where fluid resuscitation with crystalloids was initiated by the ambulance service was consecut

Analysis of human ambulation as a chaotic time-series: with nonlinear dynamics tools.

The aim of the present study is to investigate the complexity and stability of human ambulation and the implications on robotic prostheses control systems. Fourteen healthy individuals participate in two experiments, the first group run at three different speeds. The second group ascended and descended stairs of a five-level building block at a self-selected speed. All participants completed the experiment with seven inertial measurement units wrapped around the lower body segments and waist. The data were analyzed to determine the fractal dimension, spectral entropy, and the Lyapunov exponent

Progressive Decrease in Function and Ambulation Potential: A Case of Subacute Combined Degeneration.

Subacute combined degeneration (SCD) is a reversible cause of posterior and lateral spinal cord degeneration. Prolonged vitamin B12 deficiency is a common cause of SCD as it leads to inhibition of proper myelin synthesis and reduces myelin integrity. When left untreated, SCD causes progressive debility that can lead to irreversible damage. We describe the case of a 49-year-old male patient who presented with one year of worsening weakness, back pain, paresthesias, and gait abnormalities. Laboratory values revealed vitamin B12 deficiency, elevated homocysteine and methylmalonic acid, and megalo

Biomechanical Evaluation of Stair Ambulation Using Impedance Control on an Active Prosthesis.

Active prostheses can provide net positive work to individuals with amputation, offering more versatility across locomotion tasks than passive prostheses. However, the effect of powered joints on bilateral biomechanics has not been widely explored for ambulation modes different than level ground and treadmill walking. In this study, we present the bilateral biomechanics of stair ascent and descent with a powered knee-ankle prosthesis compared to the biomechanical profiles of able-bodied subjects at different configurations of stair height between 102 mm and 178 mm. In addition, we include refe

Hip resection arthroplasty for acute femoral neck fractures in the non-ambulator.

OBJECTIVES: Hemiarthroplasty (HA) is the current standard of care for displaced femoral neck fractures (FNFs) in non-ambulators. Despite excellent outcomes, arthroplasty-specific risks remain, including dislocation, implant failure, periprosthetic fracture and infection, and fat embolization syndrome. To eliminate the possibility of these complications, should non-ambulatory patients with acute, native hip FNFs be treated with simple hip resection arthroplasty (HRA) instead of HA? DESIGN: Retrospective case series. SETTING: Large, urban level-1 trauma center. PATIENTS/PARTICIPANTS: Five non

Nursing Method of Patients with Severe Traumatic Brain Injury and Fracture in the Ambulance.

Источник пометил публикацию как отозванную

Craniocerebral injury accounts for 9%-21% of the injuries in all parts of the body, and the incidence rate ranks first in all types of trauma or only lower than limb fractures. This study mainly discusses the nursing methods of patients with severe traumatic brain injury and fractures in ambulances. This study is based on risk factors with moderate-to-higher association strength from retrospective studies. According to the regression coefficients of the logistic regression model, the risk factors were assigned, and the early warning score table of craniocerebral injury complicated with hyponat

Beyond ambulation: Measuring physical activity in youth with Duchenne muscular dystrophy.

Patients with Duchenne muscular dystrophy (DMD) develop skeletal muscle weakness and cardiomyopathy. Validated skeletal muscle outcome measures are limited to ambulatory patients, but most DMD patients in cardiac trials are non-ambulatory. New objective functional assessments are needed. This study's objective was to assess the correlation and longitudinal change of two measures: quantitative muscle testing (QMT) and accelerometry. Patients with DMD were prospectively enrolled and underwent QMT and wore wrist and ankle accelerometers for seven days at baseline, 1-, and 2-years. QMT measures we

Early Ambulation After Injury Is Associated With Increased Muscle Size and Strength.

BACKGROUND: Trauma and management of injuries can result in reduction or loss of mobility, which can lead to skeletal muscle deconditioning and sustained disability. Prior investigators have examined changes in skeletal muscle due to injury and immobility separately. The muscular consequences of combined immobility and trauma have not been systematically investigated. OBJECTIVE: The purpose of this study was to explore the association of time to first ambulation with skeletal muscle size and strength in patients after major trauma. METHODS: Adults (N = 19) admitted for major trauma (Injury S

[Prehospital management of acute stroke : Using the example of Saarland ambulance services].

BACKGROUND: The preclinical care strategy was changed after reevaluation of endovascular thrombectomy in the S2k guideline of the German Neurological Society (DGN). Severe strokes should be directly transferred to neurovascular centers (model "mothership"). The severity of a stroke should be determined using the LAMS (Los Angeles Motor Scale) score. MATERIALS AND METHODS: The general conditions of preclinical care of patients with stroke in the Saarland are presented. The key figures and statistical data of clinic assignments in the adapted care strategy are evaluated. RESULTS: The 2018 data

Outcomes in children evaluated but not transported by ambulance personnel: retrospective cohort study.

BACKGROUND: Not all children with an out-of-hospital emergency medical contact are transported by ambulance to the emergency department (ED). Non-transport means that after on-scene evaluation and possible treatment, ambulance personnel may advise the patient to monitor the situation at home or may refer the patient to seek medical attention by other means of transport. As selecting the right patients for ambulance transport is critical for optimising patient safety and resource use, we studied outcomes in non-transported children to identify possible risk groups that could benefit from ambula
