Reduction of incarcerated inguinal hernia in infants using caudal epidural anaesthesia.

Incarcerated inguinal hernias in infants are commonly encountered. Reduction with intravenous sedation using morphine and related drugs carries risks of apnoea and/or respiratory arrest, especially in small, ex-premature babies who have an increased incidence of incarceration and thus opiate use is best avoided. Caudal epidural anaesthesia is widely used as pre- and post-operative analgesia in elective inguinal herniotomy in infants. In this study we sought to determine if caudal epidural anaesthesia would allow reduction of an incarcerated inguinal hernia in the acute setting, thereby obviati

Epidural anaesthesia through caudal catheters for inguinal herniotomies in awake ex-premature babies.

Ex-premature babies are at risk of apnoea after surgery. Regional anaesthesia has been used as an alternative to general anaesthesia for some surgical procedures in the belief that it may be safer. However, single dose caudal epidural and subarachnoid anaesthetics have a duration of action which may be insufficient for some operations. The level and duration of anaesthesia can be extended if local anaesthetic is given through an epidural catheter. In addition, the dose needed to provide adequate anaesthesia may be lower because the local anaesthetic is given at an appropriate segmental level.

Ultrasound guided transversus abdominis plane block. Postoperative analgesia in children with spinal dysraphism.

Pediatric regional anesthesia is widely used to relieve postoperative pain after abdominal surgery. Commonly used techniques of regional anesthesia include lumbar epidural and caudal block. However, the use of central neuraxial blockade has limitations. It is contraindicated in patients with clotting abnormalities, spinal dysraphism with tethered cord syndrome, meningomyelocele, and following spinal surgery with instrumentation. Ultrasound guided transversus abdominis plane block is a new method of regional anesthesia that can be used in settings where central neuraxial blockade is contraindic

Failed caudal block due to physiologic changes associated with a cerebrospinal fluid leak: a case report.

PURPOSE: The sum of the volumes of brain tissue, cerebrospinal fluid (CSF), and intracranial blood remain constant. This tenet of the Monroe-Kellie hypothesis is most often considered in the setting of intracranial hypertension, but it can also be applied in the setting of CSF volume depletion. We used this hypothesis to explain a case of failed caudal block in a patient with an iatrogenic CSF leak. CLINICAL FEATURES: A one-month-old baby (3 kg) born at 35 weeks' gestation presented for right inguinal hernia repair. His past medical history was significant for arthrogryposis (congenital joint

[Propofol anesthesia for a patient with congenital myotonic dystrophy].

We report the anesthetic management for a five year old boy with congenital myotonic dystrophy. The patient was scheduled for bilateral orchiopexy under general anesthesia. Anesthesia was induced with fentanyl 50 micrograms, vecuronium 0.6 mg and propofol 40 mg intravenously to facilitate tracheal intubation. During operation, we monitored train of four ratio (TOF) to confirm effect of muscle relaxation. Anesthesia was maintained with propofol (2 mg.kg-1.hr-1), nitrous oxide and caudal block. At the end of the operation, the patient recovered smoothly from anesthesia and post-operative course

[Reoperations in the treatment of caudal teratomas].

The article analyses the experience in the treatment of 49 patients with caudal teratomas for which reoperations had to be conducted. The operations were repeated because of diagnostic errors, inadequate surgical approach, and the development of intraoperative complications. Prolonged epidural anaesthesia with concurrent application of trimecaine hydrochloride and morphine was considered the most adequate type of anesthesia. In cases of fistulas draining the caudal teratomas into the rectum, the surgical tactics is determined by the location and size of the fistulas openings. To improve the re

Influence of caudal traction of ipsilateral arm on ultrasound image for supraclavicular central venous catheterization.

BACKGROUND: The first step for successful ultrasound (US)-guided subclavian vein (SCV) catheterization using a supraclavicular approach is to obtain a good longitudinal image of SCV for in-plane needle placement. We evaluated the efficacy of caudal traction of ipsilateral arm on the exposure of the SCV. METHODS: We enrolled 20 infants, 20 children, and 20 adults undergoing general anesthesia. After tracheal intubation, US probe was applied as the supraclavicular approach, and the longitudinal US image of SCV was obtained in 3 different ipsilateral arm positions: neutral, caudal traction, and

Extended Postoperative Analgesia via Caudal Catheters for Major Surgery in Neonates-A 6-Year Retrospective Study.

Background: Caudal anesthesia is an important regional anesthetic technique in neonates. The placement of a catheter can provide excellent analgesia for a prolonged period; the role of adjuvants, in particular morphine, however, remains unclear. We aimed to describe our experience with caudal catheters for major surgery in neonates. Methods: We included all neonates who had a caudal catheter placed for major abdominal and thoracic surgery and explored postoperative pain management and catheter complications. This retrospective case series included neonates with caudal catheter placement from O

Intraoperative Mapping and Monitoring for Rootlets of the Lower Cranial Nerves Related to Vocal Cord Movement.

BACKGROUND: Damage to the motor division of the lower cranial nerves that run into the jugular foramen leads to hoarseness, dysphagia, and the risk of aspiration pneumonia; therefore, its functional preservation during surgical procedures is important. Intraoperative mapping and monitoring of the motor rootlets at the cerebellomedullary cistern using endotracheal tube electrodes is a safe and effective procedure to prevent its injury. OBJECTIVE: To study the location of the somatic and autonomic motor fibers of the lower cranial nerves related to vocal cord movement. METHODS: Twenty-four pat

Comparison of the Analgesic Effects of Ultrasound-Guided Caudal Versus Ilioinguinal/Iliohypogastric Nerve Block Techniques for Pediatric Inguinal Surgeries: An Exploratory Randomized Controlled Study.

BACKGROUND: Caudal nerve blocks are commonly used for inguinal surgeries in children but can lead to complications, such as prolonged weakness. The ilioinguinal/iliohypogastric (ILIH) nerve block is an alternative that is used less frequently. This study aimed to compare the effectiveness of these techniques in pediatric patients (ages six months to 12 years) undergoing inguinal surgery. METHODS: A double-blinded, randomized controlled trial was conducted with 20 pediatric patients undergoing inguinal surgery under general anesthesia. Group 1 (n = 8) received a caudal block, while Group 2 (n

Systemic, hemodynamic and neurological effects of caudal epidural administration of Neosaxitoxin in horses.

The present study was conducted to evaluate the safety and the long-lasting anesthetic properties of Neosaxitoxin (NeoSTX) when injected into the intercoccygeal space in standing horses. A group of eighteen (n = 18) healthy client-owned mature crossbreed horses were enrolled in this study. Caudal epidural injection was ascertained by two methods, the "hanging drop" technique and radiographs. Horses were allocated into three groups (n = 6) each group receiving different dosages of NeoSTX. To assess the onset of action and duration of anesthesia, six horses received 20 μg in 2 mL, six 20 μg in 4

Blocks and other techniques pediatric surgeons can employ to reduce postoperative pain in pediatric patients.

Painful experiences, such as circumcision without the benefit of anesthesia, during the neonatal period may induce lifelong behavior changes in infants and children. By inference, this is probably true for other painful experiences encountered during childhood. It is much easier to prevent the establishment of pain than it is to eradicate it once it has become firmly established. As "captain of the ship," there are three very basic things a pediatric surgeon can do to reduce postoperative discomfort in your young patients. First, whenever possible, allow caudal blocks or other forms of regiona
