Projecting excess mortality due to infectious diseases during a crisis: methods and application to the Gaza Strip, 2023-2024.

Excess mortality is a key consequence of crises due to armed conflicts and natural disasters. Israel's campaign in the Gaza Strip has exposed people to multiple risk factors for increased infectious disease burden. We developed a novel method for forward-projecting mortality due to both endemic and epidemic-prone infections in Gaza between 7 October 2023 and 6 August 2024. After defining alternative crisis scenarios, we used expert elicitation to quantify the probability of specific epidemics and increases in transmissibility and case fatality. We analysed past vaccination coverage and natural

Challenges and risk factors for infectious diseases in Gaza due to the current conflict.

BACKGROUND: The current conflict in Gaza has been particularly catastrophic, resulting in over 40 000 deaths, 90 000 injuries and extensive damage to infrastructure. Displaced residents now live in crowded shelters. AIM: To highlight how the current conditions contribute to the spread of infectious diseases among the displaced. METHODS: This is a narrative review of the situation in Gaza during the October 2023 war. It was based on observations in the Gaza Strip and published reports. RESULTS: Due to the war, Gaza is faced with contaminated water sources, damaged sewage systems, severe over

[Military medical doctrine--the scientific basis for medical support of the Armed Forces].

Basing upon the experience of soviet medicine during the Great Patriotic War, organization of medical security of the Troops and Navy personnel in the post-war period, especially during the war in Afghanistan and local military conflicts and analysis of modern tendencies in military medicine development in the armies of foreign countries the authors formulate the principles of military-medical doctrine of the Armed Forces. The authors consider the following organizing principles of military-medical doctrine to be the main ones: the use of medical service mobile forces relying on the territoria

Double jeopardy: conflict and natural disasters-driven cholera outbreaks in Sudan.

Cholera continues to be a global threat, particularly in regions affected by conflict and environmental shocks, such as Sudan. Such humanitarian crises disrupt sanitation, water, and health systems, exacerbating cholera transmission. Overcrowded camps from displacement, poor hygiene, and weak surveillance further contribute to the outbreaks even beyond national borders. This paper explores armed conflict-driven crises and population movement as epidemiological drivers of cholera. It highlights the critical need for integrated, multisectoral strategies combining vaccination, water, sanitation,

Civil society and perilous medicine in Africa: a systematic review.

BACKGROUND: Conflict has not been a public health priority for governmental organisations, and evidence on civil society organisation (CSO) contributions to the issue is limited. The present study synthesised the available evidence on the role of CSOs in conflict and health in Africa. METHODS: We employed a systematic review using a systematic search of MEDLINE, PubMed, CINHAL, Web of Science and Scopus for English written articles between 1 January 2000 to 31 December 2023. The four concepts for the search included CSO or non-governmental organisation (NGO), conflict, health and Africa. RES

Tracking the big picture during wartime: A national trauma registry perspective.

BACKGROUND: Trauma registries are a core component of modern trauma systems for surveillance, quality improvement, and research. During armed conflicts, the need for timely, population-level data increases; however, the functioning of national trauma registries during active war has been sparsely described. This study describes the performance of Israel's National Trauma Registry (INTR) during the 2023-2025 Swords of Iron War, and reports key wartime injury epidemiology alongside the central operational processes implemented to support both civilian and military health care systems. METHODS:

Short and longer-term psychological consequences of Operation Cast Lead: documentation from a mental health program in the Gaza Strip.

BACKGROUND: There is growing recognition of the psychological impact of adversity associated with armed conflict on exposed civilian populations. Yet there is a paucity of evidence on the value of mental health programs in these contexts, and of the chronology of psychological sequelae, especially in prolonged conflicts with repeated cycles of extreme violence. Here, we describe changes in the psychological profile of new patients in a mental health program after the military offensive Cast Lead, in the context of the prolonged armed conflict involving the Gaza Strip. METHODS: This study anal

Multiple sclerosis during an emergency or crisis: A narrative review focused on the effect of war and COVID-19 pandemic.

BACKGROUND: Armed conflict fractures the systems that multiple sclerosis (MS) care depends on, yet MS-specific guidance for war is scarce. The aim of this study is to synthesize evidence on MS care during armed conflict and other crises, and to develop practical, crisis-ready recommendations to maintain treatment continuity, reduce inequities, and guide clinicians and policymakers. METHODS: We reviewed peer-reviewed literature (2000-2025) on MS during conflicts; when MS-war data were scarce, we utilized transferable evidence from COVID-19 MS studies. RESULTS AND CONCLUSION: War disrupts dise

[Limb traumatic arteriovenous fistula: experience of 26 cases].

INTRODUCTION: Posttraumatic arteriovenous fistulas are not a rare event in military facilities during periods of armed conflicts, but are seldom seen in the civilian health care system. PATIENTS AND METHOD: We report our approach to the management of 26 cases of posttraumatic limb arteriovenous fistulas colligated from 1996 to 2006. RESULTS: The main cause of our posttraumatic arteriovenous fistulas is the penetrating traumatism. Arteriography was the mostly used imaging exploration. The treatment was surgical for 24 patients. One case of thrombosis was reported. CONCLUSION: Our series of p

Injuries of the lower limbs caused by antipersonnel mines: the experience of the International Committee of the Red Cross.

Antipersonnel mines are used frequently in armed conflicts. Victims do not sustain their injuries only during the conflict, but sometimes years after it has ended. The object of this paper is to describe the treatment of patients with mine injuries in terms of mortality and the drain on hospital resources. A group of patients with fresh mine injuries who were treated at hospitals of the International Committee of the Red Cross was analysed retrospectively. About 25 per cent of patients need an amputation. The most common surgical amputation is below the knee. The study shows that the level of

Delivering health interventions to women, children, and adolescents in conflict settings: what have we learned from ten country case studies?

Armed conflict disproportionately affects the morbidity, mortality, and wellbeing of women, newborns, children, and adolescents. Our study presents insights from a collection of ten country case studies aiming to assess the provision of sexual, reproductive, maternal, newborn, child, and adolescent health and nutrition interventions in ten conflict-affected settings in Afghanistan, Colombia, Democratic Republic of the Congo, Mali, Nigeria, Pakistan, Somalia, South Sudan, Syria, and Yemen. We found that despite large variations in contexts and decision making processes, antenatal care, basic em

Editorial: Improving Evidence-Based Mental Health Care for Refugee and Internally Displaced Children: What Works and Why?

There has been a rapid increase in the number of displaced children over the past decades due to armed conflicts, violence, and natural disasters. In 2024 alone, the United Nations Children's Fund (UNICEF) estimated that 53.2 million children were forced to flee their homes, which is about triple the estimate of 2010.1 These children experience a multitude of stressors both before and during their displacement, including poor living conditions and serious risk of emotional, sexual, and physical abuse. Unsurprisingly, sustained exposure to such stressors places these children at heightened risk

Accounting for uncertainty in conflict mortality estimation: an application to the Gaza War in 2023-2024.

The ongoing Gaza War has resulted in significant loss of life and intensified an existing humanitarian crisis. Despite increasing demand for accurate data, mortality estimates remain challenging due to the inherent 'statistical fog of war'. Accurate quantification is hindered by incomplete reporting and uncertain age-sex distributions of casualties. Official death tolls are likely influenced by damaged infrastructure, security disruptions, and political motivations, complicating detailed demographic verification. Our study introduces a novel methodological approach-a Bayesian model incorporati

War crimes, sexual assault and medical confidentiality in Israel.

Hamas militants have raped and mutilated the bodies of dozens of men and women in Israel during their attack and captivity in Gaza. The exact extent of these atrocities, however, is unknown. For reasons of this sort and others, prosecuting sexual abusers during armed conflicts is notoriously difficult. In an attempt to make a legal case against Hamas militants, the Israeli authorities have recently ordered civilian hospitals to breach medical confidentiality and report unidentified data of patients who have suffered bodily mutilation and sexual abuse during the Hamas attack or during captivity

Chronic widespread pain and psychiatric disorders in veterans of the first Gulf War.

More than 10% of the 700,000 American troops who served during the first Gulf War (GW) are receiving treatment for a constellation of diffuse and frequently poorly defined medical and psychiatric symptoms that have been designated the GW syndrome by both clinicians and the popular media. The current clinical consensus is that the symptoms reported in GW veterans are the sequela of combat and other stressful events that have been identified in the veterans of other wars and armed conflicts. Chronic diffuse pain is one constellation of symptoms commonly reported in GW veterans. Research has conf

Psychological Interventions Targeting Depressive Symptoms in Children and Adolescents Living in War or Conflict Affected Zones: A Scoping Review.

PURPOSE OF REVIEW: This review maps psychological and psychosocial interventions aimed at reducing depressive symptoms in children and adolescents who continue to live within war- or armed conflict–affected zones. We sought to identify which approaches are specifically designed for depressive symptoms, how they are delivered in humanitarian contexts, and where robust evidence is still lacking. RECENT FINDINGS: Twenty-one studies published between 2005 and 2025 were identified across diverse settings including Palestine, Ukraine, Afghanistan, Indonesia, Nepal, and Northern Uganda.
