Symptoms of Asthma Extracted Through Natural Language Processing and Their Associations With Acute Asthma Exacerbation in Adults With Mild Asthma.

BACKGROUND: Individuals with mild asthma account for 30% to 40% of asthma exacerbations requiring emergency consultation, and nearly 30% had not-well controlled or poorly controlled asthma symptoms over the previous 4 weeks. OBJECTIVE: We sought to estimate the prevalence of various asthma symptoms and assess their association with future acute asthma exacerbations (AAEs) in patients with mild asthma. METHODS: This was a retrospective cohort study. Using administrative data, we identified 198,873 adults aged 18 to 85 years, who met criteria for mild asthma between 2013 and 2018. The presence

Non-T2 asthma.

PURPOSE OF REVIEW: This review provides a comprehensive overview of the non-T asthma phenotypes. Asthma is an umbrella term that defines a complex group of heterogenous airway disorders, which are broadly categorized into predominantly T2 or non-T2 phenotypes depending on the presence and levels of airway and systemic biomarkers associated with a T2 inflammatory response. Individuals with predominant T2 asthma have greater numbers of peripheral blood eosinophils, exhaled nitric oxide and IgE. These patients have more atopy and earlier onset asthma. In contrast, the absence or low levels of the

[Adult asthma management].

ADULT ASTHMA MANAGEMENT. Adult asthma management is based on pharmacological treatments that most often include a controller treatment based on daily inhaled corticosteroids, the dosage which determines the therapeutic step, that can be associated with β2 mimetic bronchodilators, and a reliever treatment, to use during episodes of increased asthma symptoms. These treatments need to be combined with non-pharmacological measures: control of co-morbidities and factors that contribute to poor asthma control, as well as therapeutic education. Regular monitoring is adapted to asthma control, can be

[Cough variant asthma].

Cough variant asthma (CVA), a phenotype of asthma solely presenting with coughing, is the most common cause of chronic cough in Japan, which lasts for 8 weeks or longer. It shares some clinical and pathophysiological features with classic asthma with wheezing such as seasonal or nocturnal coughing, airway hyperresponsiveness, eosinophilic airway inflammation and airway remodeling. The key diagnostic feature of CVA is the responsiveness of coughing to bronchodilators (beta-agonists), but the mainstay treatment after the established diagnosis is the long-term treatment with inhaled corticosteroi

Asthma risk factors.

BACKGROUND: Bronchial asthma is one of the most common chronic diseases in childhood, with a current prevalence of 6% to 9%, but a prevalence that is increasing at an alarming rate. Asthma is a complex genetic disorder with strong environmental influence. It imposes a growing burden on our society in terms of morbidity, quality of life, and healthcare costs. Despite large-scale efforts, only a few asthma genes have been confirmed, suggesting that the genetic underpinning of asthma is highly complex. METHODS: A review of the literature was performed regarding atopic and nonatopic asthma risk f

Holy Saturday asthma.

A 61-year-old man complained of cough and dyspnoea after exposure to colophony-containing solder fumes at work. A histamine challenge test confirmed airway hyper-responsiveness, and colophony-challenge demonstrated a 16.7% drop in peak expiratory flow rate (PEFR), supporting a diagnosis of colophony-induced occupational asthma. At review, the patient presented with cough, dyspnoea and wheeze that occurred acutely when exposed to the fumes from burning incense during Easter Saturday services, necessitating his departure from the church. Inhalation challenge tests using two blends of incense use

Other asthma considerations.

Asthma is a heterogeneous syndrome of cough, wheeze, dyspnea, and chest tightness. However, in a subset of patients, these symptoms may indicate a different underlying disease process with variable responsiveness to classic asthma therapies. Disease may progress while practitioners attempt conventional asthma therapy. Additionally, some types of asthma may require alternative approaches to relieve symptoms successfully. This article describes the differential diagnosis of asthma and discusses some of the more common asthma variants and asthma mimickers.

Defecation-related asthma.

We herein report the case of a 39-year-old man with recurrent asthma exacerbations preceded by abdominal cramps with the urge to defecate. The patient had a history of near-fatal asthma associated with these gastrointestinal symptoms starting five years before his admission. He stated that, even when his daily asthma symptoms were under control, he suffered from attacks, especially when he had a strong urge to defecate. Although the contribution of increased parasympathetic tone to the onset of bronchospasms was likely, anticholinergics were not effective. Instead, the patient's symptoms succe

Irritant-induced asthma.

PURPOSE OF REVIEW: To describe the recent insights into the definition, causes, natural outcome, and key elements of irritant-induced asthma (IIA) management. RECENT FINDINGS: IIA is a subtype of occupational asthma without immunologic sensitization and includes the typical reactive airway dysfunction syndrome (RADS) and a more gradual form called not-so-sudden IIA, when onset of asthma follows repeated low-dose exposure to irritants. The World Trade Center tragedy brought new insight in the understanding of IIA, suggesting that it can exhibit a prolonged interval between exposure and recogni

Pediatric asthma phenotypes.

PURPOSE OF REVIEW: There is currently limited ability to identify which infants and young children with recurrent wheezing will ultimately develop persistent asthma. In addition, it is not clear how risk factors influence the development of asthma in later childhood and adulthood. This review will discuss efforts to categorize these children with recurrent wheezing and develop asthma-predictive tools. RECENT FINDINGS: Transient and persistent wheezing phenotypes have been identified with atopy, reduced lung function, and viral and bacterial respiratory infection as major risk factors for pers

Exercise-induced asthma.

EIB is a common manifestation in diagnosed asthmatics and is a sensitive warning sign of worsening asthma. EIB may occur as an isolated entity in the absence of signs and symptoms of chronic asthma. Careful diagnosis is crucial in determining if this condition represents a preasthmatic state or simply a transient condition resulting from individual sensitivity to hyperventilation. The pathophysiology of EIB is hypothesized to result from rapid airway rewarming after hyperventilation. Effective treatment modalities include behavioral techniques and pharmacologic agents. Appropriate treatment re

[Anti-asthma drugs].

With respect to their main mechanism of action, anti-asthma drugs are classified as bronchodilators or anti-inflammatory drugs. Inhaled beta 2-agonists are the most effective bronchodilators. The shorts acting beta 2-agonists are used for the relief of acute symptoms whereas long acting beta 2-agonists are used on a regular basis, concomitantly with inhaled corticoids, for long-term control of symptoms. The others bronchodilators (methylxanthines or anticholinergics) may be used in addition to the inhaled beta 2-agonists. The treatment of bronchial inflammation is required in mild to severe pe

Exercise-induced asthma.

Exercise-induced asthma is the phenomenon of transient airflow obstruction, typically 5 to 15 minutes after physical exertion. The increased airway resistance produces a 15% or greater decrease in the forced expiratory volume in 1 second, or in peak expiratory flow rate. Exercise-induced asthma occurs in 90% of individuals with asthma, representing 12% to 15% of the population world-wide. The prevalence of exercise-induced asthma among athletes ranges between 3% and 11%. Several theories of the etiology exist: respiratory heat or water loss (or both), hyperventilation causing discharge of bron

Environmentally induced asthma.

Asthma is characterized by inflammation, reversible airway obstruction, and increased airway responsiveness to various stimuli. Despite advances in understanding of the pathophysiology and in developing new treatments, asthma prevalence and mortality have been rising over the last decade, after a steady decline in the 1970s. Risk factors for environmentally induced asthma include air pollutants, tobacco smoke, wood smoke, and excessive allergen exposure. In controlled human chamber studies, asthmatics demonstrate increased susceptibility to outdoor pollutants such as sulfur dioxide, nitrogen d

Asthma. Tracheobronchial vasculature.

The tracheobronchial vasculature consists of a subepithelial capillary network and a deeper system of blood sinuses or capacitance vessels. There seem to be no arteriovenous anastomoses. Sympathetic nerves constrict the vasculature by the transmitters noradrenaline and neuropeptide-Y, parasympathetic nerves dilate it by acetylcholine and vasoactive intestinal polypeptide, and sensory nerves release neuropeptides including substance P that are dilator. Most inflammatory mediators are also vasodilator. In asthma there is mucosal vasodilation due to the direct action of mediators on vascular smoo
