STATEMENT OF PROBLEM: In clinical situations where implant placement in the maxilla is prohibited due to the lack of available bone, more invasive techniques such as autogenous bone grafting and sinus augmentation are often off-putting to patients due to the added expense, risk and morbidity. Purpose of treatment: The intention of this treatment was to dentally restore a patient with a severely resorbed edentulous maxilla, without the use of any grafting techniques.
METHODS: Under local anaesthetic, five implants were placed in the patient's maxilla using the combined techniques of alveolar r

Several systematic reviews have shown that maxillary sinus augmentation is a predictable and effective procedure for augmentation of an atrophic posterior maxilla. However, we know of no reviews that have covered all the clinical aspects. We searched the PubMed, EMBASE, Cinhal, and Cochrane databases up to January 2015 to select relevant studies that cover the different objectives of this review, including the anatomy of the maxillary sinus, surgical techniques, biomaterials used in the sinus augmentation, and potential complications.

BACKGROUND: Dental implants are used for replacing missing teeth. Placing dental implants is limited by the presence of adequate bone volume permitting their anchorage. Several bone augmentation procedures have been developed to solve this problem. Zygomatic implants are long screw-shaped implants developed as a partial or complete alternative to bone augmentation procedures for the severely atrophic maxilla. One to three zygomatic implants can be inserted through the posterior alveolar crest and maxillary sinus to engage the body of the zygomatic bone. A couple of conventional dental implants

The rehabilitation of edentulous maxillae is a complex procedure due to the involvement of esthetic and functional requirements. A trial maxillary denture can be used to identify the need for adequate upper lip support when replacing removable complete dentures by implant-fixed dental prostheses. This clinical report describes the outcome of the rehabilitation of an edentulous atrophic maxilla with unfavorable maxillomandibular relationship and deficient upper lip support. A trial denture was fabricated and used to diagnose the need for a prosthesis capable of restoring the upper lip support.

Rehabilitation of the severely resorbed maxilla poses a difficult challenge, for both restorative and surgical colleagues, as the typically large maxillary sinuses in this group of patients require extensive bone grafting if root form dental implant placement is considered. Acceptance is low, due to disadvantages associated with: i) an additional surgical procedure for an iliac bone graft harvesting and, ii) an overall delay in delivery of the final implant-supported prosthesis. The zygomatic implant placement procedure does not require any adjunctive procedures. Furthermore, the ability to im

Sinus augmentation with a lateral window technique and delayed implant placement are the traditional treatment schedules when the residual alveolar bone height in atrophic posterior maxilla is less than 5 mm. This article presents a transcrestal window approach (TWA) to replace a missing first molar in an atrophic posterior maxillary ridge with 1 to 2 mm of residual bone height (RBH). Transcrestal sinus augmentation and delayed implant placement were performed. The antral pseudocyst that was located in the nasal wall of the maxillary sinus disappeared in the cone-beam computer tomography image

BACKGROUND: Alveolar ridge deficiencies following trauma, tooth loss, or endodontic pathology may compromise implant placement in the anterior maxilla. While autogenous bone grafting remains the benchmark, a growth factor bioavailability-enhanced allograft (GFBA) has been developed as an alternative.
CASE PRESENTATION: A 44-year-old man presented for implant rehabilitation in the anterior maxilla. Tooth No. 8 was missing due to trauma that occurred approximately 20 years prior. Cone-beam computed tomography findings for site No. 8 showed an atrophic alveolar ridge with intact cortical plates

PURPOSE: A recently introduced narrow osseointegrated wedge-shaped implant has been proposed for the treatment of thin alveolar ridges that are not suitable for placement of root form screw-type implants without bone augmentation procedures. The wedge implant was designed with the aim of preserving the buccal bone plate while maintaining adequate mechanical and prosthetic performance. This multiple split-mouth case report in the posterior maxilla compares wedge-shaped and screw-type implants.
MATERIALS AND METHODS: Wedge implants (1.8 mm thickness) were placed in a narrow ridge (mean width 4.

The lateral sinus lift procedure has been extensively investigated and described as a reliable surgical solution aimed at facilitating implant placement and rehabilitation when the posterior upper maxilla is atrophic. The standard technique consists in a lateral antrostomy, the careful raising of the sinus membrane, and following apposition of a bone substitute between the membrane and the sinus floor. The present technical note illustrates a new conservative technique enabling lateral sinus lift without the use of bone substitutes and with predictable outcomes.

Maxillary alveolar atrophy often limits the placement of dental implants. This article reports on a refinement of a technique for widening the atrophic ridge by splitting the alveolar bone longitudinally. Treatment of a patient with a severely resorbed edentulous maxilla is described. Six 4-mm wide by 13-mm long threaded Osseotite implants were placed immediately within the split ridge and surrounded with a mixture of autogenous tuberosity and bovine bone. The advantages of this technique for patients include less surgical trauma and condensed treatment time.

BACKGROUND: Since 1977, osseointegrated implants have played a strong role in oral rehabilitation. Despite the large success of this technique for the edentulous maxilla and mandible with sufficient bone height, patients with atrophic jaws continue to be the challenging cases for an adequate reconstruction of aesthetics and functionality. Orthognathic surgical procedures have been developed to reposition the jaws and have been traditionally used in the dentate patient to correct a skeletal malocclusion; these procedures can be used on the edentulous patient to correct the discrepancies between

Insufficient crestal bone is a common feature encountered in the edentulous posterior maxilla due to atrophy of the alveolar ridge and maxillary sinus pneumatization. Numerous surgical techniques, grafting materials, and timing protocols have been proposed for implant-supported rehabilitation of posterior maxillae with limited bone height. In the majority of potential implant sites, residual bone height is less than 8 mm and the clinician has to select either a lateral or transcrestal sinus-elevation technique or placing short implants as the correct surgical option. Nevertheless, guidelines f
