Tracheal intubation is a commonly performed procedure on critically ill patients in the emergency department. It is associated with many serious complications, one of the most dangerous being unrecognized esophageal intubation, which can result in anoxic brain injury, cardiac arrest, or death. It is the responsibility of the emergency physician to do everything possible to avoid this devastating complication. Preventing unrecognized esophageal intubation requires a two-pronged approach. First, the inadvertent placement of intended tracheal tubes into the esophagus must be reduced as much as is

BACKGROUND: Pediatric tracheal intubation requires considerable expertise and can represent a challenge to many anesthesiologists. Confirmation of correct tracheal tube position relies on direct visualization or indirect measures, such as auscultation and capnography. These methods have varying sensitivity and specificity, especially in the infant and young child. Ultrasonography is noninvasive and is becoming more readily available to the anesthesiologist. In this study, we investigated the characteristic real-time ultrasonographic findings of the normal pediatric airway during tracheal intub

OBJECTIVE: Fiberoptic intubation for the management of the difficult airway is usually achieved with the patient under light sedation. The goal of the present study was to evaluate the combination of propofol and remifentanil for sedation during fiberoptic intubation.
METHODS: Plans were made to use fiberoptic intubation in 40 patients with predictably difficult airways. After topical anaesthesia of the pharynx with lidocaine, oxygen was administered via nasal cannula. A capnograph was attached to determine the adequacy of ventilation and an infusion of remifentanil (0.05 microgram/kg/min) an

OBJECTIVE: Sepsis is a leading cause of critical illness in Australia and New Zealand, with over 75% of admissions occurring through emergency departments. This study aimed to characterise the demographics, process of care, pharmacology, and peri-intubation adverse events of sepsis intubations using a prospective Australasian registry.
METHODS: A retrospective analysis of prospectively collected data from the Australian and New Zealand Emergency Department Airway Registry (ANZEDAR) was performed across 79 hospitals between January 2010 and March 2026. Intubations performed for a primary indic

Effective airway management is essential in emergency medicine, where rapid patient deterioration and limited preparation time increase the risk of hypoxia, aspiration and cardiac arrest. Traditional bedside assessments and confirmation techniques such as Mallampati scoring, auscultation and capnography have notable limitations, particularly in patients with trauma, obesity, cardiac arrest or distorted anatomy. Point-of-care ultrasound (POCUS) has emerged as a reliable adjunct that provides real-time, non-invasive visualisation of airway structures, prediction of difficult laryngoscopy and rap

Hypercapnia occurs in ventilated infants even if tidal volume (VT) and minute ventilation (VE) are maintained. We hypothesised that increased physiological dead space (Vd,phys) caused decreased minute alveolar ventilation (VA; alveolar ventilation (VA) × respiratory rate) in well-ventilated infants with hypercapnia. We investigated the relationship between dead space and partial pressure of carbon dioxide (PaCO2) and assessed VA. Intubated infants (n = 33; mean birth weight, 2257 ± 641 g; mean gestational age, 35.0 ± 3.3 weeks) were enrolled. We performed volumetric capnography (Vcap), and cal

Airway management is a critical component of resuscitation but also carries the potential to disrupt perfusion, oxygenation, and ventilation as a consequence of airway insertion efforts, the use of medications, and the conversion to positive-pressure ventilation. NAEMSP recommends:Airway management should be approached as an organized system of care, incorporating principles of teamwork and operational awareness.EMS clinicians should prevent or correct hypoxemia and hypotension prior to advanced airway insertion attempts.Continuous physiological monitoring must be used during airway management

INTRODUCTION: Current guidelines continue to lead to under- and over-triage of injured patients in the pre-hospital setting. End-tidal carbon dioxide (ETCO2) has been correlated with mortality and hemorrhagic shock in trauma patients. This study examines the correlation between ETCO2 and in-hospital outcomes among non-intubated patients in the pre-hospital setting.
METHODS: We retrospectively studied a cohort of non-intubated adult trauma patients with initial pre-hospital side-stream capnography-obtained ETCO2 presenting via ground transport from a single North Carolina EMS agency to a level

The widespread prevalence of obesity continues to rise. Obesity and dental disease share common risk factors and so the demand for dental care for obese patients is escalating. For some of these patients, there is a corresponding need to be able to provide intravenous sedation safely when it is necessary and appropriate to do so. However, obesity often presents with multiple comorbidities and airway complexities, leading to more challenging management and potentially increased risk. The risk assessment process as well as patient monitoring and management strategies will be explored in this art

BACKGROUND: In this work we describe a breath emulator system, used to simulate temporal characteristics of exhaled carbon dioxide (CO2) concentration waveform versus time simulating how much CO2 is present at each phase of the human lung respiratory process. The system provides a method for testing capnometers incorporating fast response non-dispersive infrared (NDIR) CO2 gas sensing devices - in a clinical setting, capnography devices assess ventilation which is the CO2 movement in and out of the lungs. A mathematical model describing the waveform of the expired CO2 characteristic and influe

This article outlines essential guidelines and protocols for safe outpatient sedation practice. It emphasizes the clinician's responsibility to prevent and detect life-threatening complications by adhering to recommended monitoring standards. All patients, regardless of sedation depth, must be monitored for oxygenation, ventilation, and hemodynamics. Core requirements include the use of non-invasive devices such as a blood pressure cuff, pulse oximeter, and stethoscope, with capnography mandated for moderate to deep sedation. The article also highlights advanced wireless monitoring tools for e

There is no single resuscitation strategy that will uniformly improve cardiac arrest outcomes. Traditional vital signs cannot be relied on in cardiac arrest, and the use of continuous capnography, regional cerebral tissue oxygenation, and continuous arterial monitoring are options for use early defibrillation are critical elements of resuscitation. Cardio-cerebral perfusion may be improved with the use of active compression-decompression CPR, an impedance threshold device, and head-up CPR. In refractory shockable arrest, if ECPR is not an option, consider changing defibrillator pad placement a

Oesophageal malposition of an endotracheal tube is among the leading causes of anaesthesia incidents. While clinical manoeuvres for detection of tube malposition are unreliable, monitoring (i.e. capnography) can prevent such incidents. The problem is particularly important in prehospital care, where capnography is not (yet) widely available. We tested three devices used for differentiating oesophageal from endotracheal intubation: 1. Non-CO2-dependent Oesophageal Detector Device (ODD) as described by Pollard and Wee, 2. Semi-quantitative chemical disposable capnometer EasyCAP (Nellcor), 3. Non

BACKGROUND: Unanticipated respiratory compromise that lead to unplanned intubations is a known phenomenon in hospitalized patients. Most events occur in patients at high risk in well-monitored units; less is known about the incidence, risk factors, and trajectory of patients thought at low risk on lightly monitored general care wards. The aims of our study were to quantify demographic and clinical characteristics associated with unplanned intubations on general care floors and to analyze the medications administered, monitoring strategies, and vital-sign trajectories before the event.

Tachypnea is a sentinel sign of physiologic distress and a critical, early marker of illness in emergency department (ED) patients. Despite its clinical significance, it is often overlooked or inaccurately measured. Tachypnea frequently precedes dyspnea and may be the first clue to life-threatening conditions such as sepsis, pulmonary embolism, heart failure, diabetic ketoacidosis, or tamponade. This review provides emergency physicians with a practical framework to assess tachypnea at the bedside using physical examination, point-of-care ultrasound, capnography, and noninvasive ventilation. We describe the management of a pregnant patient with osteogenesis imperfecta with a history of numerous fractures, severe scoliosis, and anticipated difficult airway. Her pregnancy was complicated by progressive shortness of breath and a fetal diagnosis of osteogenesis imperfecta. Spine anatomy precluded neuraxial anesthesia. Cesarean delivery was performed under general anesthesia at 34 weeks. Immediately after awake fiberoptic intubation and induction of general anesthesia, capnography waveform was lost with rapid profound oxygen desaturation. A supraglottic airway device was placed, oxygen

BACKGROUND: Acid-base disorders are frequently encountered in the emergency department and critical care unit and require prompt assessment for timely management. Arterial blood gas analysis remains the gold standard for evaluating acid-base status; however, it is invasive, painful, time-consuming, and costly. End-tidal carbon dioxide (ETCO2) monitoring using capnography is a noninvasive technique that reflects ventilation and metabolic status and may serve as a surrogate marker for certain arterial blood gas parameters.
AIM: To determine the relationship between ETCO2 and arterial blood gas

BACKGROUND: We sought to describe ventilation rates during out-of-hospital cardiac arrest (OHCA) resuscitation and their associations with airway management strategy and outcomes.
METHODS: We analyzed continuous end-tidal carbon dioxide capnography data from adult OHCA enrolled in the Pragmatic Airway Resuscitation Trial (PART). Using automated signal processing techniques, we determined continuous ventilation rates for consecutive 10-second epochs after airway insertion. We defined hypoventilation as a ventilation rate < 6 breaths/min. We defined hyperventilation as a ventilation rate > 12 b

Effective ventilation during cardiac arrest is vital for maintaining oxygenation and gas exchange, yet both hyperventilation and hypoventilation pose significant risks, including lung injury and neurologic damage. Current guidelines recommend specific ventilation volumes and rates, but evidence supporting these targets is limited, and actual practices often deviate, potentially impacting outcomes. Research challenges, such as the chaotic out-of-hospital setting, measurement inaccuracies, and patient heterogeneity, complicate the development of optimal strategies. Emerging technologies like rea
