Extension for Community Health Outcomes-hepatitis C: Small steps carve big footprints in the allocation of scarce resources for hepatitis C virus treatment to remote developing areas.

Hepatitis C virus (HCV) infection is still a major health problem throughout the world. HCV patients living in rural areas are less fortunate than their counterparts residing in populous urbanized regions. The lack of medical resources and properly trained medical personnel in rural regions make it especially burdensome for HCV patients seeking treatment. Dr. Sanjeev Arora at the University of New Mexico Health Sciences Center took initiative to resolve the issue at hand by developing a model named Project Extension for Community Health Outcomes (ECHO). ECHO connects primary care providers (PC

Research and community engagement during the COVID- 19 pandemic in a resource-limited setting: a mixed methods study with epidemic preparedness implications.

BACKGROUND: When WHO declared the SARS-CoV- 2 respiratory virus pandemic in 2020, Uganda was unprepared to prevent and control its spread or manage the COVID- 19 and non-COVID patients. A community trial to assess the efficacy of an intensive community engagement and risk communication (CERC) was contemplated. However, a baseline assessment of the existing CERC status and the challenges of conducting community research in the early months of the pandemic was necessary. METHODS: A community baseline qualitative research (in-depth interviews, key informant interviews, focus group discussions, a

Understanding the unique role of community-based para-professionals delivering early childhood development in low-resource contexts: a Delphi study.

BACKGROUND: Community-based para-professionals are trained or untrained professionals who assist in the delivery of health-related care in communities where they live. The role of community-based para-professionals in supporting early childhood development (ECD) supports has attracted increased attention recently, particularly in the context of severe constraints in the global health workforce. However, these practitioners face challenges associated with low status and poor working conditions. In response, the study reported here aimed to gather expert views on essential knowledge, skills and

Embedding an illustrator in the process of co-producing resources to enhance communication and shared decision-making for patients prescribed high-risk medication.

OBJECTIVES: To examine how embedding an illustrator in the process of co-producing resources to support shared decision-making (SDM) can enhance communication between patients and healthcare professionals (HCPs). METHODS: Three case studies were identified involving high-risk medications where the utility of effective visual communication resources is key to SDM due to the risk of information being misinterpreted leading to potential serious adverse effects. An iterative approach was adopted to the phases of co-production, where the Illustrator acted as a conduit between patients and speciali

Do facilitators and barriers to community ambulation differ among stroke survivors in low resource settings? A cross-sectional study in Nigeria.

PURPOSE: Stroke survivors (SSV) in many low- and middle-income countries experience frustrating participation restriction in community ambulation (CA), which impedes community life. This study assessed facilitators and barriers to CA among community-dwelling SSV in a southwest state of Nigeria. METHODS: This cross-sectional study involved 66 community-dwelling ambulating SSV, purposively recruited from physiotherapy out-patient clinics of selected hospitals in southwest Nigeria. Semi-structured questionnaire containing physical and social environment elements of the ICF domains was administer

Community-integrated self-collected HPV-based cervix screening in a low-resource rural setting: a pragmatic, cluster-randomized trial.

Effective approaches to improve coverage of self-collected human papillomavirus (HPV)-based cervix screening (SCS) as well as attendance at treatment for HPV-positive participants are needed to inform policy on optimal integration of cervical cancer screening programs within existing infrastructure in low-resource settings. ASPIRE Mayuge was a pragmatic cluster-randomized trial in rural Mayuge district, Uganda, comparing the superiority of two recruitment implementation strategies for SCS: Door-to-Door versus Community Health Day. Villages were randomized (unblinded) to a strategy, and partici

A Community-Oriented Approach to Breast Cancer in a Low-Resource Setting: Improving Awareness, Early Detection and Treatment of Breast Cancer in Tajikistan.

Breast cancer is one of the most common cancers and causes of death in females in Tajikistan; yet less than half of the adult women in Tajikistan have heard of breast cancer. Limited access to health care contributes to late stage presentation. We developed a public-private partnership to implement a breast cancer awareness intervention in a low-resource community in Khorog, Tajikistan. We trained local health professionals in clinical breast care and conducted a breast cancer screening and treatment program. The partnership involved visiting USA-based health professionals working alongside lo

Effects of flowering plant's patch size on species composition of pollinator communities, foraging strategies, and resource partitioning in bumblebees (Hymenoptera: Apidae).

In 4 common Middle-European mainly bumblebee-pollinated plant species (Impatiens glandulifera, Echium vulgare, Aconitum napellus, Symphytum officinale) the influence of patch size on species composition of the pollinator community was studied. Short-tongued species were most dominant in large patches, while small patches were frequented by middle- and long-tongued bumblebees. This phenomenon was extremely obvious in Symphytum officinale and Aconitum napellus, where short-tongued species had bitten a hole in nearly every flower of large patches. Long-tongued species were forced to small patches

The Influence of Resource Constraints on Risk-Related Conversations With Specialists Among a Community-Based Sample of Women at High Risk for Breast Cancer.

BACKGROUND: Clinical guidelines recommend risk-management care for women with ≥ 20% lifetime risk of breast cancer (BC). Yet, guideline-concordant care-including conversations about BC risk with specialists-is low, and driving factors are not well understood. This study examined (a) associations between the material, psychosocial, and behavioral domains of financial constraint and BC risk conversations with genetics specialists and cancer/breast care specialists, and (b) whether competing resource demands are associated with the specialist BC conversations when controlling for insurance status

Using mobile health technology to assess childhood autism in low-resource community settings in India: An innovation to address the detection gap.

Autism is diagnosed by highly trained professionals- but most autistic people live in parts of the world that harbour few or no such autism specialists and little autism awareness. So many autistic people go undiagnosed, misdiagnosed, and misunderstood. We designed an app (START) to identify autism and related conditions in such places, in an attempt to address this global gap in access to specialists. START uses computerised games and activities for children and a questionnaire for parents to measure social, sensory, and motor skills. To check whether START can flag undiagnosed children likel

Maternal risk factors for underweight among children under-five in a resource limited setting: A community based case control study.

Previous studies conducted in Nepal have not identified the potential maternal risk for underweight among children under-five years of age in resource-poor settings. Therefore, to identify these risk factors for being underweight among children under-five years old, a community-based case-control study was conducted in a rural village in the Chitwan District in Nepal. Cases were defined as children who were diagnosed as underweight based on low weight per age, whereas controls were the children with normal weight for their age. Mothers of 93 cases and 186 controls were invited for an interview

Community-, facility-, and individual-level outcomes of a district mental healthcare plan in a low-resource setting in Nepal: A population-based evaluation.

BACKGROUND: In low-income countries, care for people with mental, neurological, and substance use (MNS) disorders is largely absent, especially in rural settings. To increase treatment coverage, integration of mental health services into community and primary healthcare settings is recommended. While this strategy is being rolled out globally, rigorous evaluation of outcomes at each stage of the service delivery pathway from detection to treatment initiation to individual outcomes of care has been missing. METHODS AND FINDINGS: A combination of methods were employed to evaluate the impact of

Single piece craniotomy in the management of a patient with large vertex epidural haematoma and deteriorating brain function in a limited resources community. Case report from Southwestern Uganda.

INTRODUCTION AND IMPORTANCE: Vertex epidural haematoma is a rare form of epidural haemorrhage and a complication of head trauma that is both a diagnostic and therapeutic challenge for the trauma surgeon. It's usually as result of laceration in the superior sagittal sinus. Whereas most patients with small vertex epidural hematomas improve with conservative treatment, large or rapidly expanding lesions are fatal and require emergency craniotomy. CASE PRESENTATION: Ours was a 28-year-old male patient who arrived at our emergency department in rural Southwestern Uganda with Glasgow coma scale of

Shared and targeted barriers to care for skin-related neglected tropical diseases in two endemic communes of Benin: Toward a framework for resource allocation.

PURPOSE: Skin-related neglected tropical diseases (skin-NTDs) persist across sub-Saharan Africa despite sustained control. Programme managers allocating scarce resources must decide whether barriers to care call for shared measures applied uniformly across districts or targeted responses tailored to each setting. We characterized the social, economic and cultural barriers to skin-NTD care in two contrasting endemic communes of Benin, tested for each barrier whether it is uniform or locally differentiated, and set out a preliminary framework for resource allocation that follows from this distin
