Neglected femoral neck fractures in young adults pose a great challenge to orthopedic surgeons because of the high risks of nonunion and osteonecrosis. We attempted to determine whether free vascularised fibular grafting through an anterior approach could enhance bone union, improve hip function and reduce complications in the treatment of such fractures. Thirteen patients with neglected femoral neck fractures were treated with a free vascularised fibular graft through an anterior approach between 2004 and 2008. The mean age was 30.9 years (range, 17-47 years). The average delay between injury

INTRODUCTION: Femoral neck fractures in children and adolescents are rare. However, their complications are frequent - avascular necrosis, femoral neck pseudoarthrosis, premature physeal closure with consequent growth disturbance and coxa vara deformity.
CASE OUTLINE: A 9.5-year-old boy was injured in a car accident, and femoral neck fracture was diagnosed. Prior to admission at our hospital he was surgically treated several times. He was admitted at our hospital eight months following the accident. On the X-ray transcervical pseudoarthrosis of the femoral neck was found, as well as coxa vara

Reconstruction using a free vascularized fibula was performed on a long bony defect after osteomyelitis of the proximal femur. For this type of bone defect, a double-barrel fibular graft is recommended. The authors used this procedure for a 58-year-old male who had osteomyelitis of the left proximal femur. As osteotomy through a transplanted fibula can be safely performed, abduction osteotomy should be considered when coxa vara remains after reconstruction with a free vascularized fibula transfer.

Camptodactyly-arthropathy-coxa vara-pericarditis syndrome (CACP) syndrome is a clearly underdiagnosed cause of constrictive pericarditis and degenerative hip arthropathy. We reviewed all cases of CACP syndrome described in the PubMed database. Founded on this review, two simple clinical maneuvers are proposed to diagnose CACP syndrome with high degree of certainty. This review also proposes that the clear pathogenetic mechanisms discovered on rare monogenic condition as CACP may aid in understanding common heterogeneous diseases such as osteoarthritis.

This is a long-term study of 53 children with fracture of the proximal femur. The age of these children at the time of injury ranged from 3 to 16 years (average, 10.2 years). The follow-up period averaged 9.4 years (range, 5-20 years). There was a single transepiphyseal fracture, 28 transcervical fractures, 21 basicervical fractures and three intertrochanteric fractures. The overall results were good in 49, fair in 25 and poor in 26% of patients. Better results were obtained in undisplaced fractures and in those displaced fractures where anatomical reduction was achieved and maintained through

The authors report their results in 17 hips treated by distal transposition of the great trochanter in patients suffering from coxa vara resulting either from congenital dislocation of the hip or Perthes disease. The results, evaluated both clinically and radiologically, were excellent. We would emphasize that distal transposition inevitably implies lateralization, which restores the normal musculo-skeletal relationship. The "hyperlordosis" sign is described and its mechanism explained. This simple physical sign is a useful test in evaluating the indications for this type of operation.

To investigate the correlation of radiological and pathohistological changes with radionuclide (99mTc-methylene diphosphonate) uptake, avascular necrosis of the femoral capital epiphysis of puppies was produced after the modified method of Henard and Calandruccio, by casting the hip joint in the position of hyperextention, full internal rotation and abduction for eighteen to twenty hours. Seventy-six puppies were divided into three groups according to frequency and time intervals of casting; in group A the animals were cast once, in group B cast twice with one week interval between the 1st and

We report a case of osteopetrosis with bilateral nonunited femoral neck and coxa vara in a 7-year-old boy. There was a history of traumatic transcervical left femoral neck fracture unsuccessfully treated by internal fixation. We performed a bilateral subtrochanteric valgus osteotomy. K-Wire fixation failed on the right side due to hardness of the bone. Finally, internal fixation with an angular plate after predrilling of the femoral neck offered stability. Bone union was achieved on both sides, resulting in full recovery of normal physical activity.

Schmid-type metaphyseal chondrodysplasia (MIM 156500) is an uncommon autosomal dominant skeletal dysplasia caused by heterozygous mutations in the COL10A1 gene (MIM 120110) encoding the α1(X) chains of type X collagen. We report an 8-year-old girl with waddling gait, short stature, mild dorsal scoliosis, coxa vara, short lower limbs, bowing of the femurs, genu varum, and metaphyseal fraying and splaying, who is a carrier of a novel heterozygous 2-bp (c.1894_1895dupTA; p.Leu633Thrfs*45) duplication in exon 3 of the COL10A1 gene.

Jacobs syndrome is a rare trisomy (47, XYY) found in ~1 in 1000 male children associated with infertility, autism spectrum disorders, macrocephaly, hypertelorism, tall stature, and macroorchidism. Diagnosis is often delayed due to relatively subtle phenotypic changes. Craniosynostosis, a fusion of the cranial sutures, has been described in ~1 in 2000 live births, of which 25% are related to a diagnosed syndrome with the most common being Apert and Crouzon. Craniosynostosis does not have a known association with Jacobs syndrome and no prior cases have been reported. This case report seeks to de

BACKGROUND: Spinal cord injury is a rare complication after lower extremity surgery in children with skeletal dysplasia and thoracic kyphosis. We encountered two patients who had this complication, from among 51 (39 from Nemours/Alfred I. duPont Hospital for Children and 12 from Seattle Children's Hospital) who underwent lower extremity surgery during an 8.5-year period (June 2004 to December 2012). Because spinal cord injury is a devastating complication likely not known to most physicians treating patients with skeletal dysplasias, we sought to examine factors that may contribute to this rar

BACKGROUND: Skeletal disorders in primary hyperparathyroidism (PHPT) classically manifest with osteoporosis or brown tumors. While skeletal deformities are documented in resource-limited areas, coxa vara combined with genu valgum remains unreported.
CASE PRESENTATION: A 14-year-old Tibetan female from rural China presented with progressive bilateral coxa vara (left: 102.45°, right: 109.10°) and genu valgus (left: 24.20°, right: 18.80°). Staged osteotomies for lower limb deformities were planned. After the stage 1 osteotomy, severe hypercalcemia (peak: 3.79 mmol/L; reference: 2.11-2.52 mmol/L)

BACKGROUND: Proximal femoral physeal fractures are rare in abused children. Recognition may be hampered due to their rarity and lack of an ossified femoral head. Prompt diagnosis and treatment are essential to preventing coxa vara.
OBJECTIVE: To demonstrate the radiographic features of proximal femoral physeal fractures both with unossified and ossified femoral heads.
MATERIALS AND METHODS: We reviewed our Institutional Review Board-approved 21-year radiology database of 2,206 children who had a skeletal survey as part of their medical evaluation for possible abuse. Cases of proximal femoral

BACKGROUND: Neglected femoral neck fracture in young adults is an intriguing problem. This retrospective study tried to solve that challenge through open reduction, cannulated screw internal fixation, autogenous iliac bone and bone marrow grafting.
METHODS: Thirty-six cases were studied; they were classified according to Sandhu et al.'s classification. Twenty cases were type I and 16 cases were type II fractures; the mean age was 26.8 years; fracture neglect averaged 44.6 days. Twenty cases had posterior comminution and 16 cases had anterior comminution. All cases had open reduction, cannulat

OBJECTIVE: To report a unique complication of the surgical treatment of femoral neck fractures.
CLINICAL PRESENTATION AND INTERVENTION: A young adult presented with lower abdominal pain and bleeding per rectum caused by rectal transfixion by Kirschner wires (K wires). The wires were used to fix a fracture on the neck of the femur during the Iraqi occupation of Kuwait. A plain X-ray showed an old united fracture of the neck of the left femur with coxa magna, mild coxa vara and mild secondary osteoarthritis of the left hip joint. Limited barium enema disclosed transfixion of the rectum by two r

Analysis of 20 cases of metaphyseal chondrodysplasia, Schmid type as well as a review of the world literature reveals a specific autosomal dominant disorder that was often over-diagnosed in the past, sometimes resulting in incorrect genetic counselling. Significant radiologic features include an enlarged capital femoral epiphysis in early childhood, coxa vara, greater involvement of the distal femoral metaphysis than the proximal, anterior rib changes and a normal spine. Chondroosseous morphology is not specific. Presentation in nonfamilial cases is no earlier than the second year of life.

Apparatus correction of deformations of proximal segment of femur becomes one of the most effective method of treatment of hip joint diseases in children. The authors propose new arrangements of Ilizarov's apparatus to treat the most common diseases of locomotor apparatus: thigh subluxation, aseptic necrosis of the head of femur and coxa vara. The tactics of Ilizarov's apparatus application, its arrangements and indications for its utilization are based on the results of treatment of 40 patients with the above-mentioned diseases. There are presented the apparatus application outlines and clini

In cases of congenital varus deformity of the proximal femur, the screening of hip dysplasia can lead to misinterpretations. The false-positive ultrasound result is caused by the superiorly displaced greater trochanter in hips with coxa vara, which narrows the scan window. In this paper three typical patients are presented to demonstrate that the use of additional non-standard views cannot exclude hip dysplasia in all cases. Hence, such patients with femoral abnormalities and doubtful sonographic findings should be further evaluated by arthrography or magnetic resonance imaging to rule out add

A newborn female was found to have a deletion of the terminal portion of 17q. Prominent manifestations included microcephaly, apparent hypertelorism, epicanthic folds, a broad nasal bridge with anteverted nostrils, posteriorly angulated ears, micrognathia, widely spaced nipples, arachnodactyly with proximal thumbs, and a coxa vara deformity. The unbalanced translocation was inherited from the mother, who had a reciprocal translocation involving the terminal portions of 2p and 17q. To the best of our knowledge, this is the first report of a liveborn infant with deletion of the distal portion of
