Sedation and anesthesia in gastrointestinal endoscopy in 2025: how, who, and why.

Endoscopic sedation is a relevant and challenging component of the endoscopic procedures. Its goals are to relieve patient anxiety and discomfort, improve the outcome of the examination, make the procedure tolerable, and reduce the patient's memory of the event. Unfortunately, sedation may cause a delay in patient recovery and discharge, add to the cost of endoscopic procedures, and increase the risk of adverse events (eg, cardiopulmonary adverse events). Many patients can tolerate endoscopic procedures with moderate sedation using a combination of sedatives and analgesics; however, some patie

Functional near-infrared spectroscopy assessment of the brain functional connectivity of sedated patients in the emergency intensive care unit.

Sedation during mechanical ventilation is common in intensive care units (ICUs). Functional near-infrared spectroscopy (fNIRS) has shown potential in monitoring brain function. This study aimed to evaluate and compare the brain functional connectivity (FC) characteristics in different sedated patients using fNIRS. Thirty sedated patients in the ICU and 16 conscious, non-sedated patients from the emergency ward were enrolled. Sedated patients were assigned to insufficient sedation (+ 1 to + 4), light sedation (-2 to 0), or deep sedation (-5 to -3) groups based on the Richmond Agitation Sedation

Modelling of the Sedative Effects of Propofol in Patients undergoing Spinal Anaesthesia: A Pharmacodynamic Analysis.

Sedation can increase patient comfort during spinal anaesthesia. Understanding the relationship between the propofol effect-site concentration (Ce) and patient sedation level could help clinicians achieve the desired sedation level with minimal side effects. We aimed to model the relationship between the propofol Ce and adequate and deep sedation and also incorporate covariates. Thirty patients scheduled for orthopaedic surgery received spinal anaesthesia with 0.5% bupivacaine. Propofol was administered via an effect-site target-controlled infusion device using the Schnider pharmacokinetic mod

The Profile of Early Sedation Depth and Clinical Outcomes of Mechanically Ventilated Patients in Korea.

BACKGROUND: Current international guidelines recommend against deep sedation as it is associated with worse outcomes in the intensive care unit (ICU). However, in Korea the prevalence of deep sedation and its impact on patients in the ICU are not well known. METHODS: From April 2020 to July 2021, a multicenter, prospective, longitudinal, noninterventional cohort study was performed in 20 Korean ICUs. Sedation depth extent was divided into light and deep using a mean Richmond Agitation-Sedation Scale value within the first 48 hours. Propensity score matching was used to balance covariables; th

Monitoring Sedation Depth in Critical Illness, State-of-the-Art Practice.

Optimal depth of sedation in critically ill patients is essential to prevent complications related to oversedation or undersedation. Current best practices aim for a calm, responsive state unless deep sedation is clinically indicated. Tools like the Richmond Agitation-Sedation Scale and others provide structured frameworks for assessing sedation and agitation levels. RASS is the most widely used due to its simplicity, reliability, and broad validation. Combining sedation scoring systems with feedback devices such as the Bispectral Index offers a multimodal strategy for sedation management. Thi

[Ethical and clinical recommendations concerning palliative sedation].

The utility of wild-type outer membrane vesicle (wtOMV) vaccines against serogroup B (MenB) meningococcal disease has been explored since the 1970s. Public health interventions in Cuba, Norway and New Zealand have demonstrated that these protein-based vaccines can prevent MenB disease. Data from large clinical studies and retrospective statistical analyses in New Zealand give effectiveness estimates of at least 70%. A consistent pattern of moderately reactogenic and safe vaccines has been seen with the use of approximately 60 million doses of three different wtOMV vaccine formulations. The key

Comparison of the Different Anesthesia Strategies for Atrial Fibrillation Catheter Ablation: A Systematic Review and Meta-Analysis.

BACKGROUND: Catheter ablation has become a widely applied intervention for treating symptomatic atrial fibrillation (AF), which can be performed under general anesthesia (GA), deep sedation, or conscious sedation (CS). But the strategy of anesthesia remains controversial. OBJECTIVES: This systematic review and meta-analysis aims to compare the advantages of GA/deep sedation and CS in AF catheter ablation, including procedural parameters and clinical outcomes. METHODS: PubMed, Embase, and the Cochrane Library were searched up to November 2021 for randomized controlled trials and observational

[Evolution and progress of personalized sedation strategies].

Sedation and analgesia strategies are integral components of treatment for critically ill patients. They help to reduce discomfort and anxiety, minimize adverse medical events, enable safe and effective interventions, and ultimately improve patient outcomes. In recent years, with advancing research, the concepts and strategies guiding clinical sedation and analgesia have progressively evolved. Significant shifts have occurred, moving from continuous deep sedation to a model prioritizing analgesia and light sedation, and further to recommendations favoring the use of propofol and dexmedetomidin

The Feasibility of Implementing Targeted SEDation in Mechanically Ventilated Emergency Department Patients: The ED-SED Pilot Trial.

OBJECTIVES: Deep sedation in the emergency department (ED) is common, increases deep sedation in the ICU, and is negatively associated with outcome. Limiting ED deep sedation may, therefore, be a high-yield intervention to improve outcome. However, the feasibility of conducting an adequately powered ED-based clinical sedation trial is unknown. Our objectives were to assess trial feasibility in terms of: 1) recruitment, 2) protocol implementation and practice change, and 3) safety. Patient-centered clinical outcomes were assessed to better plan for a future large-scale clinical trial. DESIGN:

Sedation scales: Do they capture the concept of opioid-induced sedation?

AIM: The purpose of this study was to explore the concept of opioid-induced sedation and how nurses define and measure sedation in the hospital setting. BACKGROUND: Opioid medications are the primary treatment for acute pain in the postoperative setting. One of the most serious side effects of opioid therapy is excessive sedation and respiratory depression. Nurses have the responsibility of providing effective pain management, while keeping the patient safe from adverse sedation and respiratory depression. Thus, the assessment of sedation becomes an integral part of the nurses' responsibiliti
