The dental management of a patient with a cocaine-induced maxillofacial defect: a case report.

There are several dental complications associated with cocaine abuse, including adverse reactions to dental anesthetics, post-operative bleeding, and cellulitis, which can lead to necrosis of orbital, nasal, and palatal bones. Following is a report of the initial treatment rendered to a patient who had destroyed most of her hard palate over a ten-year period of cocaine abuse. There are no classic socio-economic or educational profiles for abusers of cocaine. Drug abuse victims may present as patients in any dental office. Though there are certain classic physiological and psychological symptom

Patient's denture assessment cutoff values for classifying denture-treatment needs in removable complete denture wearers.

PURPOSE: To determine the patient's denture assessment (PDA) cutoff values for classifying treatment needs in patients wearing conventional complete dentures (CCDs) and implant-retained overdentures (IODs). MATERIALS AND METHODS: Ninety-six edentulous patients wearing mandibular CCDs or IODs (with opposing maxillary CCDs) for ≥3 months were clinically categorized into three treatment-need groups: no treatment required (G1), chairside maintenance required (G2), and laboratory modification required (G3) for the denture. The participants then completed the 19-item PDA using a 100-mm visual analo

The impact of kiddy dentures on maxillary arch growth.

BACKGROUND: Kiddy dentures are used to restore the self-confidence, chewing function, and pronunciation of children or when a care provider is concerned about the loss of incisor teeth. However, most studies to date have investigated normal dental arch development, and only a few have explored the effect of kiddy dentures on upper arch development. METHODS: Fourteen Taiwanese children (7 male and 7 female) who were wearing kiddy dentures due to the loss of their upper anterior teeth were enrolled in this study. Inclusion criteria were the premature loss of maxillary incisors, the patient wear

Zygoma implant-supported prosthetic rehabilitation of a patient with a maxillary defect.

This clinical report describes the successful management of a patient who underwent extensive resection of a maxillary cancer, by introduction of a maxillary obturator prosthesis using zygoma implants. The patient was a 57-year-old man with cancer of the upper anterior gingiva. The maxillary bone in the affected region had been extensively excised by radical surgery. Owing to loss of teeth retaining the denture, the existing prosthesis was unstable, and the patient experienced severe speech and mastication disorders. Four zygoma implants (two on each side), and two conventional dental implants

Prosthetic rehabilitation of acquired maxillary defects secondary to mucormycosis: clinical cases.

Maxillary necrosis can occur due to bacterial infections such as osteomyelitis, viral infections, such as herpes zoster or fungal infections, such as mucormycosis, aspergillosis etc. Mucormycosis is an opportunistic fungal infection, which mainly infects immunocompromised patients. Once the maxilla is involved, surgical resection and debridement of the necrosed areas can result in extensive maxillary defects. The clinician is to face many a challenge in order to replace not only the missing teeth, but also the lost soft tissues and bone, including hard palate and alveolar ridges. The prosthesi

Head and Neck Cancer Treatment with Mandibular Overdenture on Implants.

Treatments with prostheses on implants must be very well planned and executed, in order to have adequate maintenance and longevity. Thus, the present work demonstrates, through a clinical case report, the making of an overdenture prosthesis on two implants previously installed in the mandible and a conventional complete denture in the upper arch, considering the biopsychosocial principles of the patient. Male patient, 74-year-old, who underwent radiotherapy treatment, attended the Oral Oncology Center - FOA UNESP, for oral rehabilitation, after treatment of squamous cell carcinoma, complaining

Multidisciplinary treatment of epulis fissuratum: A case report.

BACKGROUND: Epulis fissuratum is an inflammatory fibrous hyperplasia characterized by pseudotumoral growth in the soft tissues of the oral mucosa. It is a multifactorial condition caused by factors such as poor oral hygiene, prolonged use of ill-fitting dentures, continuous use of dentures without intervals of rest, and underlying systemic conditions such as diabetes mellitus and coagulation disorders. Although initially asymptomatic if left untreated, the lesion may result in complications such as ulcerations, secondary infections, and interference with speech, masticatory function, and oral

Effect of maxillary edentulousness on mandibular residual ridges.

Bone mineral density (BMD) was measured in five different regions of the mandible of 77 postmenopausal women. These women were also classified according to the remaining, clinically determined alveolar height in the edentulous distal regions of the mandible. The duration of edentulousness in the maxilla and in the mandible was determined anamnestically. All subjects were edentulous in the maxilla, and 42 were also edentulous in the mandible. The other 35 women had teeth only in region d35-d45. This study shows that after extraction of the last teeth in the mandible, the BMD of the cortical bon

Implant-supported palatal lift prosthesis in a patient with velopharyngeal incompetence: a case report.

OBJECTIVE: To describe the use of dental implants in the treatment of velopharyngeal incompetence. BACKGROUND: Velopharyngeal incompetence is characterized by the inability to contract the tissues of the soft palate. The most common causes are neuromuscular problems resulting from concussion and degenerative conditions of the central nervous system. MATERIALS AND METHODS: The treatment using palatal lift prosthesis is well established in the literature; however, reports on the use of osseointegrated implants are scarce. Rehabilitation using only this type of fibromucosal support is difficult

Dental implant-assisted prosthetic rehabilitation of a patient with a bilateral maxillectomy defect secondary to mucormycosis.

Prosthodontic rehabilitation of patients with bimaxillary resection involving the maxillae, hard and soft palates, and paranasal sinuses presents a significant challenge in restoring speech, deglutition, mastication, and respiration. This clinical report describes the prosthodontic management of a young girl treated for leukemia who required a bilateral maxillectomy secondary to mucormycosis. Distraction osteogenesis, bone grafts, osseointegrated implants, and magnet attachments were used to provide retention, support, and stability of a large definitive obturator.

The reverse zygomatic implant: a new implant for maxillofacial reconstruction.

This case report describes the rehabilitation of a patient who had been treated with a hemimaxillectomy, reconstruction with a latissimus dorsi vascularized free flap, and radiotherapy for carcinoma of the sinus some years previously. Limited jaw opening, difficult access through the flap to the bony site, and the very small amount of bone available in which to anchor the implant inspired the development and use of a new "reverse zygomatic" implant. For this treatment, site preparation and implant insertion were accomplished using an extraoral approach. The implant was used along with two othe
