A considerable diagnostic value of endoscopy for the detection of recurrent ulcers and other postoperative syndromes was shown on the basis of the experience with endoscopies in 21 patients after gastroenterostomies , 281 patients after gastric resection and 96 patients after vagotomies performed for the ulcer disease of the stomach and duodenum. The methods of examining and the endoscopic picture after different operations are found to be of particular character. To study the state of the afferent loop after Bilroth -II resection of the stomach it is expedient to associate endoscopy with rent

Cancer-related gastrectomy can cause post-gastrectomy syndrome, which includes weight loss, dumping syndrome, and reflux esophagitis and negatively affects the quality of life. Comprehensive and individualized patient management is required; however, there is a limit to Western medicine's ability to treat these symptoms. Kampo, a traditional Japanese medicine, adapts treatments to each individual's symptoms and constitution. We treated a 68-year-old male patient with post-gastrectomy syndrome using senpukukataishasekito, a Kampo medicine. He was diagnosed with Stage II-A gastric cancer at age

Obesity in women of reproductive age is a major issue. It is associated with reduced fertility and an increased risk of obstetric and perinatal complications. Bariatric surgery is the most effective treatment for severe obesity, leading to substantial weight reduction, improvement of metabolic disorders, and enhanced fertility. Consequently, an increasing number of women are becoming pregnant after undergoing bariatric surgery. The aim of this paper is to review current recommendations and research data regarding the care of women after bariatric surgery in the periconceptional and perinatal p

BACKGROUND: To evaluate the clinical significance of regulatory dual-channel digestive tract reconstruction with a pylorus-preserving approach in advanced gastric cancer through a multicenter study.
METHODS: A total of 137 gastric cancer patients who underwent radical gastrectomy at three hospitals in Mianyang, China, between March 2018 and March 2023 were included. Patients were divided into a study group (n=68) undergoing regulatory dual-channel reconstruction with pylorus preservation and a control group (n=69) receiving total gastrectomy with Roux-en-Y reconstruction. Operative time, intr

Bariatric surgery (BS) has emerged as an efficient approach for addressing obesity, offering long-term benefits encompassing substantial weight loss and improving metabolic disorders. Many women of childbearing age opt for BS to enhance their health and well-being. The weight loss achieved through these procedures can positively impact pregnancy outcomes, but it's crucial to consider potential drawbacks. Micronutrient deficiencies, such as anemia resulting from iron or vitamin B12 deficiency, are a legitimate concern. Making the decision to have a BS is a complex process with many possible obs

Pylorus-preserving resections of the stomach were fulfilled in 118 patients with type I ulcers. The detected drop of the acid production in the stomach with ulcers of such localization was considered to be an indication to the removal of not more than 1/2 of the stomach. A necessary condition of pyloropreserving resection is the leaving of the pylorus zone not more than 2-3 cm. Two patients died (one from peritonitis and one from pulmonary insufficiency), 5 patients had disturbed evacuatory function of the stomach related with anastomosis. Long-term results were followed-up in 113 patients. Al

Esophagoduodenostomy and loop esophagojejunostomy with or without enteroenterostomy may not be the optimal reconstructive procedures after total gastrectomy because of esophagitis and dumping. More effective esophagointestinal anastomoses have been described [1,3,4,8,9,12]. We have had good success using a Roux-en-Y esophagojejunostomy with and without a jejunal pouch after total gastrectomy for the Zollinger-Ellison syndrome and agree that a jejunal pouch associated with Roux-en-Y esophagojejunostomy as described by Hunt and Lawrence is an excellent reconstructive procedure [1,13]. OBJECTIVE: To demonstrate the feasibility and safety of pylorus-preserving gastrectomy (PPG) accompanied by complete suprapyloric and infrapyloric lymph node dissection.
DESIGN: Retrospective review.
SETTING: A university hospital in Japan.
PATIENTS: Fifteen patients underwent PPG, and 28 patients underwent conventional distal gastrectomy (CDG) with Billroth I anastomosis. All patients had early gastric cancer, with either limited invasion in the mucosal layer or invasion into the submucosal layer.
INTERVENTIONS: In the PPG procedure, the distal part of the stomach was resected while retai

OBJECTIVE: The aim of the study was to assess the impact of pouch reconstruction on perioperative outcomes, postprandial symptoms, nutritional and anthropometric parameters, and overall quality of life after total gastrectomy for gastric malignancy.
SUMMARY BACKGROUND DATA: The short-term perioperative risks and longer term functional value of creating a small-bowel reservoir after total gastrectomy are contended.
METHODS: A search for randomized and non-randomized studies comparing reconstruction with or without a pouch was conducted. Treatment effects were computed using pairwise random-ef

The method of operative treatment of high ulcers of the posterior gastric wall has a number of advantages and is technically less difficult than other available methods. The use of the method includes preservation of blood supply of the gastric stump and pyloric sphincter, resection of the posterior gastric wall with ulcer, creation of the gastric tube from the anterior wall and putting a direct anastomosis with the preservation of the pyloric sphincter which allows avoidance of incompetence of the anastomosis sutures and such late disturbances as dumping syndrome, gastric stump gastritis and

Background/Objectives: Although Billroth I (BI) and Billroth II (BII) are standard reconstructions after distal gastrectomy, evidence on their longitudinal quality-of-life (QoL) trajectories remains inconclusive. This study compared postoperative QoL patterns between BI and BII groups using a validated gastric cancer-specific instrument. Methods: We analyzed 234 patients (BI n = 51, BII n = 183) who underwent laparoscopic distal gastrectomy. QoL was assessed using the KOQUSS-40 questionnaire at 1, 3, 6, 9, and 12 months postoperatively. To address selection bias, inverse probability of treatme

Anatomic and physiological changes introduced by gastric surgery result in postgastrectomy syndromes in approximately 20% of patients. Most of these disorders are caused by operation-induced abnormalities in the motor functions of the stomach, including disturbances in the gastric reservoir function, the mechanical-digestive function, and the transporting function. Division of the vagal innervation to the stomach and ablation or bypass of the pylorus are the most significant factors contributing to postgastrectomy syndromes. Either rapid or slow emptying may result, depending on the relative i

An analysis of the experience at the University of California Hospital, San Francisco, with the diagnosis and treatment of fifty-nine postgastrectomy syndromes shows that stomal obstruction (sixteen patients), the most common syndrome, was best treated by total reconstruction rather than stomal revision. Disappointment with the results of other procedures for the dumping syndrome (50 per cent improvement) has convinced us of the need to adopt the use of reversed jejunal interposition for surgical treatment of this condition. Because a precise etiologic diagnosis of bilious vomiting is often el

Gastric resection or vagotomy plus drainage are not easy alternatives for the treatment of duodenal ulcer. More complications are met after resection, more relapses after vagotomy, whereas the postoperative sequelae are the same for both operations. Vagotomy of the parietal cells is designed to prevent all such sequelae by leaving the innervation of the antrum intact so that drainage can be omitted. Experience has so far shown that very few complications occur, whereas severe and slight dumping and diarrhoea are virtually eliminated. Gastric emptying times are slightly affected (a little longe

Gastric emptying of liquid and solid meals was studied before and after operation in 22 male patients who were admitted to a prospective randomized trial of truncal vagotomy and antrectomy and proximal gastric vagotomy in the treatment of duodenal ulceration. After operation the emptying of both solid and liquid meals was biphasic, with a passive early phase and a later controlled active phase. A significant increase in early liquid emptying was produced by both operations, and after truncal vagotomy and antrectomy also with the solid meal. Active emptying of the liquid meals was unaffected by

Congenital diaphragmatic hernia (CDH) is a rare condition, with many affected patients remaining asymptomatic, while others may present with non-specific respiratory manifestations. The coexistence of a CDH and a giant hiatal hernia is particularly uncommon. We report the case of a pediatric patient presenting with both a giant hiatal hernia and a CDH, accompanied by distinctive facial features. The initial presentation included poor oral intake and abnormal chest sounds, as observed by the patient's mother. A diagnostic evaluation using chest radiography and computed tomography confirmed the
