Effect of time to first ambulation on recurrence after PELD.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: To evaluate the effect of time to first ambulation on recurrence after percutaneous endoscopic lumbar discectomy (PELD). METHODS: From July 2017 to August 2018, 90 patients with lumbar intervertebral disc herniation underwent PELD surgery. According to the initial walking time, i.e., the time until the patient could walk after the operation, the operations were divided into three groups: early stage, middle stage, and late stage. The follow-up period was 3 months, and complete follow-up data were obtained. The visual analog scale (VAS) and

Vasoseal hemostasis following coronary interventions with abciximab.

Femoral arteriotomy management using a collagen vascular hemostasis device (VasoSeal, Datascope Corporation, Montvale, New Jersey) was studied in 50 consecutive patients following interventional coronary procedures performed with abciximab (ReoPro, Eli Lilly and Company, Indianapolis, Indiana). Low-dose weight-adjusted or no heparin was employed. The first 25 patients were permitted to sit up after 6 hours with ambulation the following day. The second 25 patients were allowed to sit up after 1 hour and ambulate after 6 hours. Despite early activity and ambulation, there were no hemorrhagic com

Timeframes for "early" mobilisation after abdominal and cardiothoracic surgery: evidence- and consensus-based suggestions of definitions.

BACKGROUND AND AIM: There is currently no consensus on how to define “early” mobilisation following surgery. The aim of this study was to develop definitions of early mobilisation after cardiothoracic and abdominal surgery. METHODS: We conducted a study with multiple methods designs, including a survey, clinical observations and consensus discussions. A national survey investigated Swedish healthcare professionals’ opinions on the typical timeframe for first mobilisation for eight defined categories of surgery, and timeframes for “early” mobilisation. The following consensus discussions were b

Mobilising the patient: With emphasis on innovative technologies.

Treatment of trauma patients and fractures has changed dramatically throughout the years. From conservative methods to nowadays various kinds of screws, pins, plates and nails for optimal fixation of fractures. This lead to changes in post-operative management as well, from bedrest to (partial) weight bearing. Some patients however have very limited to no ability to mobilise, such as critical ill patients on the Intensive Care Unit, amputees or spinal cord injured patients. Due to innovations such as hydrotherapy, osseointegrated prosthesis and exoskeletons, even these people can mobilise. Tha

Brazilian Guidelines for Early Mobilization in Intensive Care Unit.

Immobility can cause several complications, including skeletal muscle atrophy and weakness, that influence the recovery of critically ill patients. This effect can be mitigated by early mobilization. Six key questions guided this research: Is early mobilization safe? Which patients are candidates for early mobilization? What are the contraindications? What is the appropriate dose, and how should it be defined? What results are obtained? What are the prognostic indicators for the use of early mobilization? The objective of this guideline was to produce a document that would provide evidence-bas

An Approach to Transmetatarsal Amputation to Encourage Immediate Weightbearing in Diabetic Patients.

Transmetatarsal amputation remains the standard treatment for the unsalvageable diabetic forefoot; however, this operation is often complicated by wound dehiscence, ulceration, and the need for additional surgery and tendon balancing. The technique described in the present report provides an uncomplicated suturing method for closure of a standard transmetatarsal amputation. A drill hole is created through the first, second, and fourth metatarsals, which facilitates added stability to the plantar flap of the residual metatarsals. The patients are encouraged to begin protected weightbearing as e

Mobilization of intensive care cardiac surgery patients on mechanical circulatory support.

Historically, intensive care cardiac surgery patients remained on bed rest for several days postoperatively to prevent complications and promote rest and healing. Over time, the cardiac surgery discipline has acknowledged the benefits of early mobility, including reduced risk of venous thromboembolism and pulmonary emboli, improved pulmonary toilet, prevention of pneumonia, decreased length of stay, reduced deconditioning, and need for rehabilitation, among others advantages. These benefits have changed clinical practice, with emphasis on early extubation, progressive mobility, and reduced len

Understanding critically ill patients hemodynamic response to mobilization: using the evidence to make it safe and feasible.

In today's critical care environment, we face a difficult but essential task. We must provide comprehensive, compassionate, complex, technological care without causing harm to our patients. To foster a patient-safe environment, we must examine care practices and processes to reduce the chance of error. Successful early mobilization of critically ill patients can reduce several complications including atelectasis and ventilator-associated pneumonia and shorten ventilator time along with cognitive and functional limitations that linger 1 to 5 years after discharge from the intensive care unit. Recurrent posttraumatic posterior dislocation of the hip. A case report and review of the literature.

We present the case of a 53-year-old male patient who presented with recurrent posttraumatic posterior hip dislocations, Epstein-Thompson type 1. No specific abnormality or pre-existing pathology was found. After closed reduction and rapid mobilization the patient was discharged from our institution. Although he had sustained six dislocations of his right hip over a period of 17 years, he once again recovered unlimited function without any signs of complications. In this specific non-compliant patient, early mobilization following each one of his successive dislocation episodes did not result

Early ankle mobilization, Part II: A one-year follow-up of acute, lateral ankle sprains (a randomized clinical trial).

A 1-year follow-up of military members who received either early ankle mobilization or cast immobilization following a moderate or severe lateral ankle sprain was conducted to determine the long-term effects between these two forms of conservative treatment. Functional job- and sports-related disabilities together with subjective complaints were assessed. A significantly large proportion (44%) of all subjects were symptomatic at follow-up. Those receiving early ankle mobilization had slightly more residual subjective complaints compared with those receiving cast immobilization. There was, howe

Extensor tendon injuries. Presentation of a series of ninety-nine cases.

A series of 99 cases of injuries of the extensor system, followed in the 3 Traumatology and Orthopedics Ward (Dijon, France) is presented. Results were analyzed according to Verdan's classification. In most cases, a Jenning "barb-wire" type repair, introduced in France by Y. Allieu, was made. Early mobilization and skin tension problems are emphasized; two cases of degloving on the dorsum of the hand are presented with details. As well, a special place concerning the results of repair in the proximal interphalangeal joint must be reserved, because of the very frequent association of articular

Positive effect of the combination of multilevel contracture release and glucocorticoid treatment in Duchenne muscular dystrophy.

PURPOSE: In the 1980s the first results of an early multilevel contracture release (MLCR) in patients suffering from progressive Duchenne muscular dystrophy (DMD) showed a positive effect on ambulation. Despite the demonstrated positive effects of prolongation of walking this treatment is not part of current guidelines. The aim of our study was to evaluate the effect of MLCR as well as its combination with glucocorticoid (GC) treatment on ambulation. METHODS: Data of all boys (n = 86) with DMD treated in our outpatient department were analyzed regarding the treatment and loss of independent a
