Impact of COVID-19 pandemic on reusable device precleaning training for front-line staff.

High-level disinfection and sterilization are complex processes, requiring initial and ongoing training of frontline staff.1 A key component of appropriate disinfection and sterilization is point-of-use precleaning performed by front-line staff. Our facility implemented an annual hospital-wide education and competency program for staff that perform precleaning of reusable medical devices.

Automated endoscope reprocessors.

The ASGE Technology Committee provides reviews of existing, new, or emerging endoscopic technologies that have an impact on the practice of GI endoscopy. Evidence-based methodology is used, with a MEDLINE literature search to identify pertinent clinical studies on the topic and a MAUDE (U.S. Food and Drug Administration Center for Devices and Radiological Health) database search to identify the reported complications of a given technology. Both are supplemented by accessing the "related articles" feature of PubMed and by scrutinizing pertinent references cited by the identified studies. Contro

The Aquacath hydrophilic coated single-use urinary catheter.

Single-use hydrophilic catheters are now being used widely throughout this country, Europe and America, for intermittent self-catheterization or carer-assisted intermittent catheterization, with considerable success. They are also reported to provide greater comfort for users. The latest catheter to become available on prescription is the Aquacath coated urinary catheter, produced by Seton Continence Care. This hydrophilic catheter performed very well in a trial comparing coating stability after wetting of various coated catheters (Jones, 1997). The teaching of intermittent self-catheterizatio

Interference of Residual 99m Tc-MIBI Activity in Syringe Needle on 99m Tc-DMSA Scan Interpretation; Medical Error.

A 4-month-old infant with a small right kidney underwent a 99m Tc-DMSA scan showing unjustifiable findings including global cortical loss in the left kidney, increased liver and bowel uptake. Retrospective review revealed reuse of a sterile needle contaminated with residual 99m Tc-MIBI, compromising scan quality and falsely elevating right kidney function. A repeat scan showed normal left kidney uptake and significantly reduced right kidney function. This case highlights the importance of employing safe techniques with disposable equipment, thorough workflow review, and transparent error repor

Humidifiers for oxygen therapy: what risk for reusable and disposable devices?

INTRODUCTION: Nosocomial pneumonia accounts for the vast majority of healthcare-associated infections (HAI). Although numerous medical devices have been discussed as potential vehicles for microorganisms, very little is known about the role played by oxygen humidifiers as potential sources of nosocomial pathogens. The purpose of this research was to evaluate the safety of the reuse of humidifiers by analysing the rate of microbial contamination in reusable and disposable oxygen humidifiers used during therapy, and then discuss their potential role in the transmission of respiratory pathogens.

Promotion of Surgical Masks Antimicrobial Activity by Disinfection and Impregnation with Disinfectant Silver Nanoparticles.

BACKGROUND: The COVID-19 pandemic is requesting highly effective protective personnel equipment, mainly for healthcare professionals. However, the current demand has exceeded the supply chain and, consequently, shortage of essential medical materials, such as surgical masks. Due to these alarming limitations, it is crucial to develop effective means of disinfection, reusing, and thereby applying antimicrobial shielding protection to the clinical supplies. PURPOSE: Therefore, in this work, we developed a novel, economical, and straightforward approach to promote antimicrobial activity to surgi

Evaluating the efficacy of reprocessing contaminated single use devices for re-use in intensive care unit in Ethiopia.

BACKGROUND: Single use devices (SUDs) are designed for single use. However, in low-income countries, they are often reprocessed and reused. Because these devices are intended for single use, no standardized protocol for reprocessing these devices currently exists. We developed a protocol for reprocessing SUDs in low-income settings and assessed the efficacy of this protocol in eliminating bacterial burden from a subset of used respiratory SUDs. METHODS: Reprocessing of these devices was conducted in accordance with the National IPC reference manual with minor modifications. The surfaces of us

The presence of fecal test soil protects bacteria during cleaning/disinfection.

Reusable medical devices (RMDs) must be reprocessed between uses to render them safe for each use and each patient. Cleaning used devices removes organic and inorganic soil making them either safe for reuse or ready for disinfection/sterilization depending on the device. Although cleaning is an important step in a RMD's life cycle, it is not always a priority during device design. In addition, when performing cleaning validation, it is recommended that the manufacturer takes into consideration, what the most appropriate or worst case conditions are in terms of type of soil or the presence of b

"It Eats My Heart": Identifying Knowledge Gaps in Injection Drug-Related Endocarditis Among Hospitalized Patients.

BACKGROUND: Infective endocarditis (IE) rates have risen sharply, spurred by injection drug use (IDU). Public health interventions to prevent infectious complications of IDU have primarily focused on communicable diseases, such as HIV and viral hepatitis. We assessed patients' knowledge of IE in relation to IDU practice. METHODS: From 2021 to 2022, we conducted in-person semi-structured interviews (SSIs) with 16 adults hospitalized with IDU-related IE at an academic medical center in North Carolina. SSIs explored participants' knowledge and experience of IE and IDU practices. The SSIs were au

Mask Reuse in the COVID-19 Pandemic: Creating an Inexpensive and Scalable Ultraviolet System for Filtering Facepiece Respirator Decontamination.

As the current COVID-19 pandemic illustrates, not all hospitals and other patient care facilities are equipped with enough personal protective equipment to meet the demand in a crisis. Health care workers around the world use filtering facepiece respirators to protect themselves and their patients, yet during this global pandemic they are forced to reuse what are intended to be single-use masks. This poses a significant risk to these health care workers along with the people they are trying to protect. Ultraviolet germicidal irradiation (UVGI) has been validated previously as a method to effec

Gamma radiation sterilization of N95 respirators leads to decreased respirator performance.

In response to personal protective equipment (PPE) shortages in the United States due to the Coronavirus Disease 2019, two models of N95 respirators were evaluated for reuse after gamma radiation sterilization. Gamma sterilization is attractive for PPE reuse because it can sterilize large quantities of material through hermetically sealed packaging, providing safety and logistic benefits. The Gamma Irradiation Facility at Sandia National Laboratories was used to irradiate N95 filtering facepiece respirators to a sterilization dose of 25 kGy(tissue). Aerosol particle filtration performance test

Conserving water in and applying solar power to haemodialysis: 'green dialysis' through wiser resource utilization.

Natural resources are under worldwide pressure, water and sustainable energy being the paramount issues. Haemodialysis, a water-voracious and energy-hungry healthcare procedure, thoughtlessly wastes water and leaves a heavy carbon footprint. In our service, 100 000 L/week of previously discarded reverse osmosis reject water--water which satisfies all World Health Organisation criteria for potable (drinking) water--no longer drains to waste but is captured for reuse. Reject water from the hospital-based dialysis unit provides autoclave steam for instrument sterilization, ward toilet flushing, j

Prospective Randomized Trial of Single-Use vs Reusable Cystoscope for Ureteral Stent Removal.

Background: Reusable cystoscopes are associated with risks of cross-contamination, need for chemical reprocessing, mechanical breakdown, maintenance costs, and labor for cleaning. A sterile, single-use cystoscope may reduce or eliminate the source of these infections while also decreasing exposure to reprocessing chemical agents. In this multicenter, randomized trial, we sought to compare single use with reusable endoscopes for ureteral stent removal. Methods: A three-center, randomized, dual-arm postmarket clinical trial was performed to compare the single-use cystoscope (Ambu A/S) with stand

Dry heat sterilization as a method to recycle N95 respirator masks: The importance of fit.

In times of crisis, including the current COVID-19 pandemic, the supply chain of filtering facepiece respirators, such as N95 respirators, are disrupted. To combat shortages of N95 respirators, many institutions were forced to decontaminate and reuse respirators. While several reports have evaluated the impact on filtration as a measurement of preservation of respirator function after decontamination, the equally important fact of maintaining proper fit to the users' face has been understudied. In the current study, we demonstrate the complete inactivation of SARS-CoV-2 and preservation of fit

The importance of sampling technique and rinse water for assessing flexible gastrointestinal endoscope reprocessing: A 3-year study covering 59 centers.

BACKGROUND: Various guidelines recommend several sampling techniques to verify endoscope reprocessing, but a comparative study of the efficiency for recovering microorganisms was rare. Our goal was to compare different sampling techniques for the postreprocessing endoscope to assess residual bacterial contamination and analysis of the critical factors affecting the endoscope reprocessing failure. METHODS: From 2016 to 2018, 3 techniques, the conventional flushing sampling method, flush-brush-flush sampling method (FBFSM), and pump-assisted sampling method (PASM), were compared covering all 59

Defining the Costs of Reusable Flexible Ureteroscope Reprocessing Using Time-Driven Activity-Based Costing.

PURPOSE: Careful decontamination and sterilization of reusable flexible ureteroscopes used in ureterorenoscopy cases prevent the spread of infectious pathogens to patients and technicians. However, inefficient reprocessing and unavailability of ureteroscopes sent out for repair can contribute to expensive operating room (OR) delays. Time-driven activity-based costing (TDABC) was applied to describe the time and costs involved in reprocessing. MATERIALS AND METHODS: Direct observation and timing were performed for all steps in reprocessing of reusable flexible ureteroscopes following operative

Choosing reusable operating theatre devices at an Australian health service avoids hundreds of tonnes of waste and could save millions of dollars per annum.

Healthcare services rely on large amounts of disposable medical devices, even though similar reusable devices exist. This retrospective audit of operating theatre and endoscopy procurement data across two hospitals aimed to examine the number and mass of simple single-use devices and cost (purchasing and disposal) of such devices in operating theatres where a reusable device alternative exists. This was assessed at one health service across two hospitals with a total of 19 operating theatres and five endoscopy procedure rooms. The main outcome was the number of single-use devices procured and
