Trauma patients with tibia/fibula fractures are associated with an increased risk of torso, severe head, and severe spine injuries compared to patients with femur fractures.

BACKGROUND: Although previous studies have demonstrated an association between lower extremity fractures and concomitant torso (thorax and abdomen) injuries in trauma patients, they do not compare different types of fractures. Therefore, we investigated the risk of associated torso injuries between femur and tibia/fibula fractures, hypothesizing that trauma patients with femur fractures are at higher risk of torso injuries compared to patients with tibia/fibula fractures. METHODS: The Trauma Quality Improvement Program database (2010-2016) was queried for patients presenting with either femur

Pediatric Orthopaedic Surgeon Use of Removable Splints for Common Pediatric Distal Radius and Fibula Fractures: A 10-year Update.

BACKGROUND: Management of pediatric fractures continually evolves, with a documented shift toward removable devices, such as splints, braces, or boots, for stable injuries. However, there is a need to evaluate current practice patterns. METHODS: A self-administered, 22-question online survey was open to all practicing orthopaedic surgeon members of Pediatric Orthopaedic Surgeons of North America (POSNA) between June and September 2025. Questions were consistent with a previous 2015 survey. Demographic information, immobilization practices, comfort with removable devices, complications, and ba

Three internal fixation methods for Danis-Weber-B distal fibular fractures: A biomechanical comparison in an osteoporotic fibula model.

A common agreement for the surgical treatment of osteoporotic ankle fractures has not been defined yet although locking plates are preferred for fractures with poor bone quality. This study aims to evaluate the mechanical stability of locked and conventional plates on osteoporotic Danis-Weber-B-fibula fracture models. Fractured custom-made osteoporotic fibulae were treated with neutralization plate plus lag screw, locking plate plus lag screw, or a standalone locking plate. Load until failure was applied mimicking single-leg stance. Stiffness, failureload, and interfragmentary movements were i

Outcome of Ilizarov External Fixator for the Treatment of Gap Non-uniting Mid Shaft Tibia-fibula Fractures: Our Experience.

Many patients come with open fracture tibia-fibula initially managed by surgical toileting and the application of indigenous uniaxial external fixator in our country. Many of them lead to non-uniting fracture or sometimes signs of union absent within 4 months from the time of initial fracture and become infected also. This quasi experimental study included 40 skeletally matured patients was conducted from 05 February 2014 to 05 February 2018 in the department of Orthopedics, Mymensingh Medical College Hospital, Mymensigh, Bangladesh. The purpose of this study was to evaluate the efficacy of Il

Lengthening z-osteotomy of the fibula to correct persistent talar shift following open reduction internal fixation of ankle fractures.

BACKGROUND: In cases where ankle fracture union has been compromised by persistent syndesmotic diastasis following open reduction internal fixation, both external rotation and shortening of the fibula have been identified as prominent features. OBJECTIVE: This study reports a technique that uses a z-osteotomy to achieve both lengthening and internal rotation of the fibula to correct persistent talar shift following ankle fracture fixation. METHODS: Four patients with persistent talar shift following open reduction internal fixation for an ankle fracture received z-osteotomy of the fibula to

The proximal fibula should be examined in all patients with ankle injury: a case series of missed maisonneuve fractures.

BACKGROUND: The Maisonneuve fracture is a spiral fracture of the proximal third of the fibula. It occurs from violent twisting of the ankle that characteristically causes ligament damage and severe instability. Most patients complain of significant ankle pain but very little pain over the fracture. The clinical and radiographic examination is usually directed to the ankle region; and the proximal fibula is often ignored. OBJECTIVE: The authors intend to show the ease of missing the proximal fibular fracture when the clinical examination is directed to the ankle region. They discuss the import

Management of post-operative delayed proximal tibia and fibula fracture with Ayurvedic medicines and Panchakarma: A case report.

Some of the fractures, associated with road traffic accidents (RTAs), pose significant challenges in recovery and quality of life. In this case study, we document the treatment of a 42-year-old male patient with tibia and fibula fractures, sustained during an RTA. Initially managed through open reduction and external fixation, the patient's recovery stagnated for about 3 months, prompting the integration of Ayurvedic therapies, including herbal formulations and Panchakarma. After starting Ayurvedic treatments, including oral Lakshadi Guggul, Mashadi Yoga, Asthi-Majja Yoga, and Panchakarma proc

Distal Third Minimally Displaced Stress Fracture of Fibula in a Military Recruit: A Case Report and Review of Literature.

BACKGROUND: Stress fractures frequently occur in athletes and military personnel due to their rigorous training and are typically treated conservatively with immobilization and rehabilitation. However, a displaced distal third stress fracture of the fibula that requires surgical intervention is a rare occurrence. CASE PRESENTATION: A 20-year-old male military recruit under training for 1 month presented with a week-long history of pain and swelling in his right ankle and difficulty in walking. A plain radiograph revealed a transverse fracture of the fibula with a minimal displacement, consist

Successful Plate Fixation with Long Intramedullary Fibula Bone Graft for Periprosthetic Femur Fracture: A Case Report.

Background and objectives: Treatment of a displaced or comminuted periprosthetic distal femur fracture is challenging, especially in patients with osteoporosis. In this case report, we shared our successful surgical experience of using a long intramedullary fibula bone graft in a plate fixation surgery for a periprosthetic distal femur fracture in an extremely elderly patient with osteoporosis. Case report: A 95-year-old woman with severe osteoporosis (bone mineral density level: -3.0) presented with right knee pain and deformity after a fall, and a right periprosthetic distal femur fracture w

Fibula pro-tibia vs standard locking plate fixation in an ankle fracture saw bone model.

BACKGROUND: Locking plate fixation in osteoporotic ankle fractures may fail due to cut-out or metalwork failure. Fibula pro-tibia fixation was a technique prior to the advent of locking plates that was used to enhance stability in ankle fractures by achieving tri or tetra-cortical fixation. With locking plates, the strength of this fixation construct can be further enhanced. There is lack of evidence currently on the merits of tibia-pro-fibula augmented locking plate fixation of unstable ankle fractures. AIM: To assess if there is increased strength to failure, in an ankle fracture saw bone m

Tibia Stress Fracture and Subsequent Nonunion Associated With High Morbidity Following Vascularized Fibula Graft Harvest.

There is little in the literature about tibial stress fracture after partial fibulectomy. As fibula strut grafting and vascularized fibula flap grafting becomes more common in the field of reconstructive surgery, the incidence of this complication will likely rise. The authors present the case of a 54-year-old woman with tibial stress fracture and subsequent nonunion following vascularized fibula graft harvest. The aim of this case report is to present a unique tibial stress fracture and nonunion following vascularized fibula flap for mandibular reconstruction and a discussion of the orthopedi

Bicortical screw fixation of distal fibula fractures with a lateral plate: an anatomic and biomechanical study of a new technique.

One of the potential drawbacks of lateral plating of distal fibula fractures is less than satisfactory fixation of unicortical screws commonly placed in the distal fragment to avoid implant penetration of the ankle joint. This study examines the anatomy of the distal fibula, proposes new techniques for bicortical screw fixation and radiographic evaluation of screw placement, and compares pullout strength of unicortical versus bicortical screws in this area. Sixteen pairs of human cadaver feet were used in this study. It was found that a large percentage of the surface area of the distal fibula

Late stress fracture of a well-incorporated autologous fibula strut graft in the cervical spine: a case report.

Four-level corpectomy for cervical spondylotic myelopathy presents special challenges to successful outcomes. At our institution, where the senior author has performed over 300 multilevel corpectomies with autologous fibula reconstruction, only 11 four-level procedures have been performed. It is important to consider the length of time required for complete revascularization of these long strut grafts as well as unusual postural or gait biomechanics the patient may have. Patients with altered gait or trunk stability who require anterior surgery for cervical spondylotic myelopathy may be best s

Iatrogenic Fibula Head Avulsion Fracture in Fixed Valgus Arthritic Knee During Total Knee Arthroplasty.

INTRODUCTION: Total knee replacement in valgus knees is more challenging than in varus knees, which needs meticulous handling and decision-making from the surgical approach to the rehabilitation protocol. In this case, we report an iatrogenic fibula head fracture during total knee replacement for a valgus knee with no concomitant common peroneal nerve palsy. This is the first report of this kind in the literature. CASE REPORT: We report a case of a 67-year-old male with fixed valgus and flexion deformity of the right knee, who underwent total knee arthroplasty by lateral para-patellar approac

A retrospective assessment of the clinical efficacy of different internal fixation methods in the treatment of distal fibula fractures in the elderly.

An ankle fracture is a fracture of the distal tibia or fibula that forms the ankle joint, usually associated with ligament and soft tissue injury, and is a common type of lower limb fracture and one of the most common types of fracture in the elderly. Although ankle fractures are one of the most common injuries seen by orthopedic trauma surgeons, there is no uniform protocol for the diagnosis and treatment of ankle fractures in the elderly, and there are many controversial indications for surgery. The aim of this study is to assess the clinical efficacy of different internal fixation methods i

Incidence and risk factors for iatrogenic distal tibia/fibula fracture during Ponseti technique of clubfoot treatment.

The incidence and risk factors of distal tibia/fibula fracture, an uncommon complication during Ponseti manipulation, are unknown. We evaluated 222 virgin clubfeet of patients aged less than 3 years for fractures of distal tibia/fibula. Incidence rate was found to be 1.3% per year. Associated risk factors were identified as neglected, syndromic clubfeet, feet requiring greater than 10 casts, post-tenotomy dorsiflexion less than 10° and casting by physician with less than 3 years of casting experience, with odds ratios of 14, 28, 4.9, 3.7 and 3.4, respectively. Most of these fractures healed wi

Comparison of intramedullary nail and plating in treatment of diaphyseal tibial fractures with intact fibulae: A randomized controlled trial.

BACKGROUND: Tibial fracture without concomitant fibular fracture is an injury that has long attracted notice for the fact that it sometimes heals cleanly, other times causes various problems when the bone does not heal, or misaligns. In this randomized clinical trial, we assessed two treatment modalities plating and intramedullary nailing for treatment of closed, noncomminuted tibial fractures with intact fibulae. MATERIALS AND METHODS: During the three year period, 1470 patients with leg fractures were treated and out of which, 114 were eligible to enter the study. Of the eligible patients,
