Cervical enamel projection related to furcation involvement.

Reviewing the literature the authors show the importance of cervical enamel projections (CEP) in the involvement of molar furcation. The dento-gingival relationship of CEP structure is peculiar for not having connective attachment but only long junctional epithelium. In numerous studies, most mandibular and maxillary molars presented CEP, mainly of little extension, with an incidence ranging from 8.8 to 87.4%. The studies showed a relationship between cervical enamel projection and the presence of inflammatory periodontal disease, and furcation involvement of molars. These data suggest that a

Induction of cementogenesis by recombinant human osteogenic protein-1 (hop-1/bmp-7) in the baboon (Papio ursinus).

Recombinant human osteogenic protein-1 (hOP-1), a member of the bone morphogenetic protein family, was examined for its efficacy in periodontal regeneration. Twelve furcation defects, surgically prepared in the first and second mandibular molars, were treated with bovine insoluble collagenous matrix in conjunction with 0.0 (control), 100 and 500 mu g of recombinant hOP-1 per g of matrix. After 60 days of healing, histological and histometric analyses on serial, undemineralized sections cut at 7 mu m showed substantial cementogenesis on the exposed dentine of furcations treated with both doses

Clinical comparison of cellulose and expanded polytetrafluoroethylene membranes in the treatment of class II furcations in mandibular molars with 6-month re-entry.

The present study compared two barrier membranes, ePTFE and cellulose, used to treat Class II furcations in mandibular molars. Fifteen patients with no history of systemic diseases and presenting matched pair defects were selected. The following soft tissue measurements were taken at baseline (after the hygienic phase), and 6 months after surgery: gingival recession, probing depth, clinical attachment level, and width of keratinized tissue. At the time of membrane placement, and 6 months later (re-entry), the following hard tissue parameters were recorded: cemento-enamel junction (CEJ) to alve

Added benefit of L-PRF to autogenous bone grafts in the treatment of degree II furcation involvement in mandibular molars.

BACKGROUND: Tissue regeneration within the periodontally involved furcation area is one of the most challenging aspects of periodontal surgery. The aim of this study was to evaluate the additional benefit of leukocyte and platelet-rich fibrin (L-PRF) to autogenous bone grafts (ABGs) in the treatment of mandibular molar degree II furcation involvement, comparing the clinical outcomes with those from open flap debridement (OFD)+ABG and OFD alone treatments. METHODS: Fifty-four patients, exhibiting one buccal or lingual mandibular molar furcation defect, were randomly assigned to three treatment

Craniofacial and dental characteristics of three Japanese individuals with genetically diagnosed SATB2-associated syndrome.

Craniofacial defects are one of the most frequent phenotypes in syndromic diseases. More than 30% of syndromic diseases are associated with craniofacial defects, which are important for the precise diagnosis of systemic diseases. Special AT-rich sequence-binding protein 2 (SATB2)-associated syndrome (SAS) is a rare syndromic disease associated with a wide variety of phenotypes, including intellectual disability and craniofacial defects. Among them, dental anomalies are the most frequently observed phenotype and thus becomes an important diagnostic criterion for SAS. In this report, we demonstr

Periodontal regeneration around natural teeth.

1. Evidence is conclusive (Table 2) that periodontal regeneration in humans is possible following the use of bone grafts, guided tissue regeneration procedures, both without and in combination with bone grafts, and root demineralization procedures. 2. Clinically guided tissue regeneration procedures have demonstrated significant positive clinical change beyond that achieved with debridement alone in treating mandibular and maxillary (buccal only) Class II furcations. Similar data exist for intraosseous defects. Evidence suggests that the use of bone grafts or GTR procedures produce equal clini

Treatment of infrabony periodontal defects with esterified hyaluronic acid: clinical report of 19 consecutive lesions.

This work sought to investigate the clinical efficacy of esterified hyaluronic acid for treating deep periodontal defects. Nineteen defects, 18 infrabony and one mandibular molar furcation, were treated. Defects with probing pocket depths (PPDs) of at least 6 mm were consecutively included in the study. The PPD, gingival recession, and clinical attachment level (CAL) were evaluated at each defect before treatment and 1 year after surgery. A full-thickness flap was raised and the roots were accurately planed; hyaluronic acid in the form of fibers was then packed into the defect to completely fi

A Novel Evidence-Based Periodontal Prognosis Model.

OBJECTIVE: Patients with periodontal disease and the dental professionals responsible for their care want to know which teeth are expected to respond favorably to periodontal treatment and which teeth are likely to be lost in the short and long term. A number of different periodontal prognosis systems have been previously proposed but do not consider important patient-level factors, such as smoking and diabetic control, in the calculation of the expected outcome and often use subjective measures that introduce potential inaccuracies. The aim of this report is to translate the best available ev

Cementogenesis and the induction of periodontal tissue regeneration by the osteogenic proteins of the transforming growth factor-beta superfamily.

The antiquity and severity of periodontal diseases are demonstrated by the hard evidence of alveolar bone loss in gnathic remains of the Pliocene/Pleistocene deposits of the Bloubank Valley at Sterkfontein, Swartkrans and Kromdrai in South Africa. Extant Homo has characterized and cloned a superfamily of proteins which include the bone morphogenetic proteins that regulate tooth morphogenesis at different stages of development as temporally and spatially connected events. The induction of cementogenesis, periodontal ligament and alveolar bone regeneration are regulated by the co-ordinated expre

Enhancing furcation involvement classification on panoramic radiographs with vision transformers.

BACKGROUND: The severity of furcation involvement (FI) directly affected tooth prognosis and influenced treatment approaches. However, assessing, diagnosing, and treating molars with FI was complicated by anatomical and morphological variations. Cone-beam computed tomography (CBCT) enhanced diagnostic accuracy for detecting FI and measuring furcation defects. Despite its advantages, the high cost and radiation dose associated with CBCT equipment limited its widespread use. The aim of this study was to evaluate the performance of the Vision Transformer (ViT) in comparison with several commonly

Micro-CT characterisation of the furcation area in human primary molars.

PURPOSE: Non-instrumentation endodontic treatment precludes that an antibiotic paste, placed on the furcation area of primary molars, will be able to heal furcation lesions and treat an infected root canal. Thus, the aim of the study was to characterise the furcation area of primary molars using micro-CT. METHODS: Primary molars, extracted due to pathological conditions, were scanned, and after reconstruction, dentine thickness at the furcation area was measured, resorption gaps and accessory canals were quantified, and mean grey values at the furcation were compared with two other areas of r

Anatomic characteristics of the furcation and root surfaces of molar teeth and their significance in the clinical management of marginal periodontitis.

Treatment of periodontitis which involves the furcation area of molar teeth has always been a challenge. Sixty extracted human molar teeth were used for an anatomic study to clarify the possible morphologic factors which may affect the diagnosis, prognosis, and clinical management of periodontally involved teeth. Teeth were embedded in methylmethacrylate and cut in 70 microm cross sections from the cementoenamel junction to the apex. Cervical enamel projection, root and root trunk lengths, root proximities as well as depths of root concavities coronally, apically, and at the furcation roof wer

Microsurgical treatment of deep maxillary Class II trifurcation defects combining root resection and guided tissue regeneration: a case series.

Furcation defects pose a significant challenge in periodontal therapy. While regenerative approaches are generally preferred, resective procedures may remain essential depending on defect morphology and root configuration. This case series aims to demonstrate the clinical benefits of combining resective and regenerative techniques using periodontal microsurgery to treat maxillary molars with deep trifurcation defects. Three patients with extensive Class II trifurcation defects in maxillary molars, unresponsive to prior non-surgical periodontal therapy, were managed using a dental operating mic

Orthodontic treatment of periodontal defects. A systematic review.

Several studies have been published focusing on the possibility to treat patients affected by periodontal defects by means of orthodontic treatment. The aim of this systematic review is to evaluate the efficacy of the orthodontic treatment applied to the therapy of infraosseous defects, gingival recessions, and furcation lesions. To establish an appropriate search strategy, the PICO assessment worksheet was used. Only Randomized Controlled Trials and Systematic Reviews were selected for this review. The electronic search (from January 1966 to January 2008) and the hand search (from January 198

Guided tissue regeneration update.

The ultimate goal of periodontal therapy is to restore periodontal tissues lost through disease or trauma. The most predictable way to accomplish this goal is by guided tissue regeneration (GTR). The principle of GTR is to give preference to certain cells to repopulate the wound area to form a new attachment apparatus. Clinically this is accomplished by placing a barrier over the defect thereby excluding gingival tissues from the wound during early healing. The first generation of GTR barriers were non-resorbable which implies that they have to be removed in a second surgical procedure. Resorb
