Moderating Role of TSHR and PTPN22 Gene Polymorphisms in Effects of Excessive Fluoride on Thyroid: a School-Based Cross-Sectional Study.

We aimed to investigate the relationship between the effects excessive of fluoride on thyroid health in children and the moderating role of thyroid stimulating hormone receptor (TSHR) or protein tyrosine phosphatase nonreceptor-22 (PTPN22) gene polymorphisms. Four hundred thirteen children (141 with dental fluorosis and 198 boys) were enrolled from both historical endemic and non-endemic areas of fluorosis in Tianjin, China. The fluoride exposure levels, thyroid health indicators, and TSHR (rs2268458) and PTPN22 (rs3765598) polymorphisms were examined. Multiple logistic models were applied to

TSH and thyroxine in cord blood of subjects from an endemic area.

Thyroxine (T4) and TSH were assayed in the triplet: maternal venous blood at delivery (M), cord blood (C) and total capillary dry spot blood of the corresponding children 4 - 5 days postpartum (Nb). Fifty-four triplets in all (spontaneous deliveries, full term infants from the Cîmpulung endemic goiter area) were thus determined by sampling out the native families. The controls were 10 triplets from Bucharest, a non-endemic goiter area. A group of 31 pregnant women from the endemic area who have received iodinated oil in the 3rd - 4th month of pregnancy for prophylactic purposes are brought int

The Marine-Lenhart syndrome revisited.

The coexistence of thyroid autonomy (Plummer's disease) and Graves' disease has been termed "Marine-Lenhart syndrome". During the last years, several papers have been published on the development of Graves' disease shortly after radioiodine therapy of Plummer's disease (autonomy). Especially in patients with elevated thyroid antibodies, the incidence of this event is significantly higher after radioiodine therapy of autonomy. A review of the literature dating back to 1911 is discussed in this paper. The original paper published by Marine and Lenhart comes to the conclusion that the two disease

[Must the recurrent nerve in thyroid gland resection always be exposed? A prospective randomized study].

It was the purpose of this study to establish whether it is necessary to identify the recurrent laryngeal nerve intraoperatively when resecting the endemic nodular goiter. We prospectively formed two groups from 800 subtotal unilateral resections ("nerves at risk"). In group I (382 subtotal resections, 48.4%) the nerve was not identified intraoperatively. In group II (413 subtotal resections, 51.6%) the recurrent laryngeal nerve was routinely identified in all cases. The operative technique was standardized, giving special attention to the "anterior lamella". For all patients, preoperative and

[Surgery of the goiter].

Surgery of the goiter has been greatly influenced by Theodor Kocher all over the world. Dedicated to his understanding of goiterogenesis he is considered the father of the prevention and elimination of the wide spread iodine deficiency goiter disease in Switzerland by introducing the iodinesation of salt. Therefore Switzerland is the only country in Europe, which is no longer an iodine deficiency region but remains an endemic goiter region. The traditional conservative Kocher type of surgical resection of the multinodular goiter showed to harbour the problem of a high recurrence rate. The anal

[Iodine-induced hypothyroidism (author's transl)].

Excessive administration of iodine over a long period may cause hypothyroidism in suitably disposed individuals and provoke the formation of a goiter. The underlying reason for this is the pathological lack of adaptation of the thyroid gland to high doses of iodine with persisting inhibition of hormone production through excess of intracellular iodine (Wolff-Chaikoff effect). Asthmatic patients--as in the case described - are usually affected, when they have taken iodine-containing expectorants over a long time. Provocation of an iodine-induced hypothyroidism by the recently required iodine pr

Clinical picture of endemic cretinism in central Apennines (Montefeltro).

Endemic cretinism is still present in an endemic goiter area of the central Apennines (Montefeltro) (goiter prevalence 55%; mean urinary iodine level 39 micrograms/g creatinine). Clinical and biochemical features of patients with myxedematous, neurologic, and mixed cretinism were studied. Also, in this area, as in most other, neurologic cretinism is more prevalent than myxedematous and mixed forms. The hormonal profiles of the three types of cretinism were clearly different. Nevertheless, all myxedematous cretins had some neurologic disorders (hyperreflexia, increased muscle tone, disorder of

The disorders induced by iodine deficiency.

This paper reviews present knowledge on the etiology, pathophysiology, complications, prevention, and therapy of the disorders induced by iodine deficiency. The recommended dietary allowances of iodine are 100 micrograms/day for adults and adolescents, 60-100 micrograms/day for children aged 1 to 10 years, and 35-40 micrograms/day in infants aged less than 1 year. When the physiological requirements of iodine are not met in a given population, a series of functional and developmental abnormalities occur including thyroid function abnormalities and, when iodine deficiency is severe, endemic goi

Spectrum and prevalence of nodular thyroid diseases detected by ultrasonography in the Western Black Sea region of Turkey.

OBJECTIVE: The aim of the study was to investigate thyroid diseases and the prevalence of goiter by ultrasonography (US) in a moderately iodine deficient area. MATERIAL-METHODS: The MELEN Study is a prospective cohort study on the prevalence of thyroid diseases in Turkish adults. A total of 2233 subjects with a mean age of 50 (age range 18 to 92) were submitted to study. Thyroid US was performed and interpreted by the same experienced physician. Goiter prevalence was defined according to Gutekunst's criteria. RESULTS: The most common thyroid disease was multinodular goiter (MNG) (42%), follo

Adverse effects of iodides on thyroid function.

The administration of pharmacologic quantities of iodine such as iodides for the treatment of pulmonary disease, organic iodine present in medications and x-ray contrast dyes, and the ingestion of iodine-rich natural foods, may result in goiter, hypothyroidism, or hyperthyroidism, especially in patients with underlying thyroid disease. Medications containing iodide may induce hypothroidism in euthyroid patients with Hashimoto's thyroiditis, 131I or surgically treated Graves' disease, or following hemithyroidectomy for nodules; and they may induce hyperthyroidism in patients with endemic iodine

[Is iodination of cooking salt still necessary? Current studies on iodine supply in Switzerland].

Salt with 3.75 mg iodide per kg was introduced in Switzerland stepwise in the individual cantons between 1922 and 1952. The iodide content was raised to 7.5 mg in 1962 and to 15 mg per kg in 1980. 92% of retail salt and 78% of all salt for human consumption (including salt used in industrial food processing) was iodized in 1989. Under this measure, prevalence of grade 1b or larger goiter dropped continuously to a present value of 1.3% in school-children and 0.3% in male army recruits. Endemic cretinism has disappeared completely. Urinary iodine has reached the desired range of 150 +/- 77 (SD)

[Neck tumors: struma].

Goiters are most commonly caused by increased, usually nodular growth of thyroid tissue ('simple' goiter), stimulation of thyroid growth by autoantibodies (Grave's disease) or thyroiditis. Rarer causes of goiter are malignant thyroid tumors and hereditary disturbances of thyroid hormone synthesis goiter: due to iodine deficiency should be found no longer in Switzerland because of the adequate iodine supplementation in this country. Differentiating the various types of goiters is usually possible on the basis of clinical signs and a few laboratory tests (e.g. measurement of thyroid antibodies).

Historical aspects of iodine deficiency control.

In 1895, iodine was characterized as an essential element of thyroid tissue by Baumann. The efficacy of iodine to prevent goiter was demonstrated by Marine in Northern USA in 1916-1920. Severe endemic goiter and cretinism had been almost entirely eliminated from continental Western Europe and Northern America before the 1930's; however large populations elsewhere and even some places in Western Europe (Sicily) were still affected up to the 2000's. Public health consequences of iodine deficiency are not limited to endemic goiter and cretinism. Iodine deficiency disorders include also increased

A technique for thyroidectomy.

For several decades following the turn of the century, most thyroid surgery was necessary because of endemic goiter and Grave's disease. A lack of safe anesthesia and an inability to control hyperthyroidism preoperatively necessitated a rapid operating technique. This often meant blind resection of the bulbous anterior and lateral portions of the gland, resulting in considerable hemorrhage and injury to the recurrent laryngeal nerve in a significant number of cases. With the subsequent introduction of iodine into food and the use of radioactive iodine and the antithyroid drugs, surgery for the

Endemic goitre in Sarawak, Malaysia: I. Somatic growth and aetiology.

A comparative epidemiological and anthropometric survey was conducted among Ibans, the largest indigenous ethnic group in Sarawak, in three regions where the endemicity of goitre exhibited marked differences , to assess the effect of endemic goitre on somatic growth. In the Ai river region the prevalence of goitre was 99.5%; 35% having grade 3 goitres, 55% grade 2 goitres and 9.5% grade 1 goitres. At Rubu the prevalence of endemic goitre was 74%; 3% having grade 3 goitre, 16% grade 2 goitre and 55% grade 1 goitre. In the Bajong region relatively few people were detected with goitre and most of
