[The role of the inner ear biological barrier in targeted drug delivery for inner ear diseases].

The endolymphatic barrier, composed of tight junctions and ion transporters, regulates the ionic gradient between endolymph and perilymph and plays a pivotal role in maintaining inner ear function. Disruption of this biological barrier can result in ionic imbalance, causing both auditory and vestibular dysfunction. Medication needs to enter the perilymph instead of the endolymph by passing through the perilymphatic barrier to reach the hair cells. Targeted drug delivery has emerged as a promising therapeutic strategy for inner ear diseases, including intratympanic injection, intracochlear infu

Cutting edge of inner ear MRI.

CONCLUSION: Recent advances in clinical MR imagers, such as the 3-Tesla, multi-channel phased-array coil and novel pulse sequences, allow the evaluation of subtle alterations in the inner ear fluid environments and breakdown of the blood-labyrinthine barrier. Intratympanic injection of Gd-DTPA allows the imaging detection of endolymphatic hydrops in patients. OBJECTIVES: To describe the current status of inner ear MRI and future directions for imaging. MATERIALS AND METHODS: Based on our experiences and literature research, a brief review of the history and recent developments of inner ear M

Bortezomib-Induced Sensorineural Hearing Loss May Be Reversible with Intratympanic Dexamethasone.

Background: Bortezomib, a proteasome inhibitor used in multiple myeloma (MM), is associated with several adverse effects, most notably peripheral neuropathy. Ototoxicity, however, remains a rare and underrecognized complication. Case presentation: We report the case of a 74-year-old man with MM who developed sudden unilateral sensorineural hearing loss following subcutaneous bortezomib administration. Audiometry confirmed severe right-sided hearing loss. MRI of the internal auditory canal was normal. Given the absence of other ototoxic agents, bortezomib was identified as the likely causative

Middle Ear Histopathology Following Magnetic Delivery to the Cochlea of Prednisolone-loaded Iron Oxide Nanoparticles in Rats.

Delivery of therapy to the cochlea is a challenge and limits the efficacy of therapies meant to treat hearing loss, reverse tinnitus, and protect hearing from chemotherapy regimens. Magnetic injection is a technique that uses magnetic fields to inject nanoparticles from the middle ear into the cochlea, where they can then elute therapy to treat hearing disorders. To evaluate the safety of this treatment in the middle ear, 30 rats were subdivided into 6 groups and treated by single or multiple intratympanic injections of saline, prednisolone, nanoparticles, or nanoparticles loaded with predniso
