Studies of anaesthesia in relation to hypertension. VII: Adrenergic responses to laryngoscopy.

Arterial pressure and plasma catecholamine concentrations were measured in 16 normotensive and 10 hypertensive patients undergoing elective vascular surgery. Following induction of anaesthesia, both arterial pressure and plasma noradrenaline concentrations decreased in both groups. Following laryngoscopy, there was a moderate increase in arterial pressure in both normotensive and hypertensive patients. In normotensive patients, laryngoscopy was associated with a moderate increase in plasma noradrenaline concentration. There was no change in adrenaline concentration. By contrast, there was a ma

Supervisor instruction during neonatal intubation using video laryngoscopy: an observational study.

BACKGROUND: Supervisors of intubation mostly rely on feedback from operators to guide intubation attempts using direct laryngoscopy (DL). In contrast, when using video laryngoscopy (VL), supervisors share the operators' view of the airway during intubation attempts which may allow for better guidance. We wished to analyse the instructions given by supervisors during neonatal VL intubation and assess whether they prompted operators to perform the desired actions. METHODS: We reviewed video recordings of neonatal intubations that combined the airway views captured with VL, synchronised with ext

Intersegmenter Variability in High-Speed Laryngoscopy-Based Glottal Area Waveform Measures.

OBJECTIVES/HYPOTHESIS: High-speed videoendoscopy (HSV) has potential to objectively quantify vibratory vocal fold characteristics during phonation. Glottal Analysis Tools (GAT) version 2018, developed in Erlangen, Germany, is software for determining various glottal area waveform (GAW) quantities. Before having GAT analyze HSV videos, segmenters have to define glottis manually across videos in a semiautomatic segmentation protocol. Such interventions are hypothesized to induce variability of subsequent GAW measure computation across segmenters and may attenuate GAT measures' reliability to a c

Awake Flexible Fiberoptic Laryngoscopy to diagnose glossoptosis in Robin Sequence patients.

BACKGROUND: Robin Sequence (RS) is usually defined as the combination of micrognathia, glossoptosis and upper airway obstruction. No objective criteria to diagnose RS exist. To compare management strategy results, a single RS definition using objective criteria is needed. The most frequently used primary diagnostic tool for glossoptosis is awake Flexible Fiberoptic Laryngoscopy (aFFL). OBJECTIVES: To determine the reliability of the aFFL videos as an independent diagnostic tool itself, rather than on the complete evaluation of a patient. DESIGN, SETTING, PARTICIPANTS: All RS individuals from

Direct Versus Video Laryngoscopy for Intubating Adult Patients with Gastrointestinal Bleeding.

INTRODUCTION: Video laryngoscopy (VL) has been advocated for several aspects of emergency airway management; however, there are still concerns over its use in select patient populations such as those with large volume hematemesis secondary to gastrointestinal (GI) bleeds. Given the relatively infrequent nature of this disease process, we sought to compare intubation outcomes between VL and traditional direct laryngoscopy (DL) in patients intubated with GI bleeding, using the third iteration of the National Emergency Airway Registry (NEARIII). METHODS: We performed a retrospective analysis of

Unilateral vocal fold paralysis: can laryngoscopy predict recovery? A prospective study.

OBJECTIVE: To determine the prognostic value of laryngoscopy in predicting the recovery of unilateral vocal fold paralysis. METHOD: A prospective study was carried out of all patients with unilateral vocal fold paralysis without a progressive lesion or arytenoid dislocation. RESULTS: Among the 66 candidates, 15 recovered. Patients with interarytenoid paralysis (p < 0.001) or posterolateral tilt of the arytenoid (p = 0.028) had less chance of recovery. Among 51 patients who did not recover, 25.49 per cent regained phonatory function by compensatory movement of the normal side; the rest requir

Preoperative laryngoscopy in thyroid surgery: Do patients' subjective voice complaints matter?

BACKGROUND: Although routine preoperative laryngoscopy has been standard practice for many thyroid surgeons, there is recent literature that supports selective laryngoscopy. We hypothesize that patients' preoperative voice complaints do not correlate well with abnormalities seen on preoperative laryngoscopy. METHODS: A retrospective chart review of a 3-year, single-surgeon experience was performed. Records of patients undergoing thyroid surgery were reviewed for patient voice complaints, prior neck surgery, surgeon-documented voice quality, and results of laryngoscopy. RESULTS: Of 464 patien

Flexible fiberoptic laryngoscopy in children: effect of sitting versus supine position.

Currently no standard exists as to the exact technique of flexible fiberoptic laryngoscopy (FFL) for children. Our objective was to determine the effects of examination in the sitting versus supine position on upper airway findings during FFL in children using a standardized technique and a grading system. In this prospective study, each child acted as his or her own control. Thirty children underwent FFL at a tertiary care children's hospital (mean age 4.5 years). FFL was performed during spontaneous ventilation in both positions, using a standardized sedation technique administered by a sing

Laryngoscopy in bilateral vocal fold immobility: can you make a diagnosis?

OBJECTIVES: This study explores whether videoendoscopic findings and patient history help make the diagnosis in bilateral vocal fold immobility (BVFI). METHODS: Medical records from 1995 to 2003 were searched to identify patients with posterior glottic stenosis (PGS) and bilateral vocal fold paralysis (BVFP) who also had videoendoscopic examinations. Videoendoscopic examination findings that could help differentiate PGS from BVFP were identified a priori. A weighted scoring index, based on the adjusted odds ratios of significant examination findings on multiple logistic regression, was derive

Comparing the posture and comfort of anaesthesiologists during laryngoscopy and tracheal intubation in the head-elevated laryngoscopy position in supine position and with a 25° backup: A randomised clinical crossover trial.

BACKGROUND AND AIMS: The head-elevated laryngoscopy position (HELP) and a 25° backup have been proposed to enhance glottic visualisation, yet concerns about ergonomic discomfort hinder their widespread adoption. This study compares the comfort and posture adopted by anaesthesiologists while performing laryngoscopy and tracheal intubation with patients in HELP while in a supine position or with 25° backup. METHODS: The study included 48 patients aged 18-60 years with normal airways and 12 experienced anaesthesiologists. Patients were randomised into two groups using permuted block randomisatio

Comparison of Complications of Injection Laryngoplasty via Direct Versus Flexible Laryngoscopy.

OBJECTIVES: Injection laryngoplasty (IL) is commonly performed for glottic insufficiency and has historically been performed under general anesthesia via direct laryngoscopy (DL), with an increasing number of procedures being performed in the office setting via flexible laryngoscopy (FL). Existing literature regarding complications of IL primarily addresses immediate peri-procedural complications and adverse reactions to the injection material. This is the first study utilizing a large multi-institutional database comparing complications of IL performed via DL versus FL. STUDY DESIGN: Retrosp

Vocal Cord Monitoring by Flexible Fiberoptic Laryngoscopy During Thyroid Radiofrequency Ablation.

Introduction: Thermal injury to recurrent laryngeal nerve (RLN) during radiofrequency ablation (RFA) can produce temporary or permanent vocal cord paralysis.1 Hydrodissection with cold 5% glucose of "danger triangle" protects RLN during RFA.2 When RFA is performed under local anesthesia, RLN function is monitored by patients producing vocal sounds.3 Large lesions requiring longer RFAs warrant general sedation where voice cannot be assessed, therefore, an additional technique for RLN protection is advisable. Observation of passive symmetrical vocal cord movements during breathing by laryngeal u

Orthogonal Region Selection Network for Laryngeal Closure Detection in Laryngoscopy Videos.

Vocal folds (VFs) play a critical role in breathing, swallowing, and speech production. VF dysfunctions caused by various medical conditions can significantly reduce patients' quality of life and lead to life-threatening conditions such as aspiration pneumonia, caused by food and/or liquid "invasion" into the windpipe. Laryngeal endoscopy is routinely used in clinical practice to inspect the larynx and to assess the VF function. Unfortunately, the resulting videos are only visually inspected, leading to loss of valuable information that can be used for early diagnosis and disease or treatment
