Detection of fragmentation while dusting during retrograde intrarenal laser lithotripsy: a novel computer vision and perception pipeline.

Dusting during ureteroscopy with laser lithotripsy is popular because it minimizes the need for basketing of fragments. However, one of the challenges with dusting is adjusting power settings to be efficient and to limit the inadvertent generation of fragments. Detection of fragment production in real time using computer vision could help facilitate energy adjustments to optimize dusting. With IRB approval, we recorded eight consecutive ureteroscopy with laser lithotripsy procedures performed for intrarenal stones with a single-use flexible ureteroscope. We used a Dornier Thulio laser to dust

Health-related quality of life after thulium fiber laser lithotripsy: a prospective study according to stone localization.

To evaluate postoperative changes in health-related quality of life (HRQoL) after endoscopic stone surgery and to determine whether recovery patterns differ according to stone localization. In this prospective observational study, 50 patients undergoing endourological stone surgery were stratified according to stone localization (renal, ureteral, and bladder). HRQoL was assessed using the SF-36 questionnaire preoperatively, on postoperative day 1, and at 1, 3, and 6 months. Stone-free status was evaluated at postoperative month 1 using KUB radiography and/or non-contrast CT when clinically ind

Intrarenal Fluid Temperature During Laser Lithotripsy: A Mini Review by the European Association of Urology Endourology Section.

Laser lithotripsy involves thermal risks due to heat generated in the surrounding fluid. The concept of thermal dose, which accounts for both temperature and exposure duration, provides a more nuanced understanding of tissue tolerance than fixed temperature thresholds, considering both the magnitude of the temperature increase and the length of exposure to each specific temperature. The concept of a thermal safety distance defines the spatial extent of potential damage, supporting safe power limits of 10 W in the ureter and 20 W in the kidney. Importantly, the type of laser (holmium:YAG, thuli

[Results of the first multicenter study assessing the efficiency of a tissue sensor during thulium fiber laser lithotripsy].

INTRODUCTION: Currently, the main surgical approach for urinary stone removal is minimally invasive endoscopic procedures. Various laser systems compatible with most modern instruments are used for stone disintegration. Due to high efficacy and minimal fragment retropulsion during fragmentation, the domestic thulium fiber laser has become a serious alternative to the conventional holmium laser. At the same time, the safety profile of thulium fiber lithotripsy remains debatable because of the potential risk of irrigation fluid heating and damage to surrounding tissues during fragmentation.
