Subjective sensory symptoms associated with axonal and demyelinating nerve injuries after mandibular sagittal split osteotomy.

PURPOSE: The effect of the type of nerve injury on subjective sensory disturbances and recovery has not been addressed in orthognathic surgery. Using neurophysiologic monitoring during 19 bilateral sagittal split osteotomy operations, we were able to classify intraoperative inferior alveolar nerve injuries as either axonal or demyelinating. This study aimed to analyze the quality and extent of the subjective sensations experienced by the patients after these 2 injury types at different time points up to 12 months. MATERIALS AND METHODS: Of the 36 injured nerves, 21 showed signs of demyelinati

[Nerve injury during mandibular third molar surgery. The importance of preoperative diagnosis and surgical skill].

A 26-year-old woman was referred by a dentist to an oral and maxillofacial surgeon after an unsuccessful attempt to remove a mandibular third molar. A panoramic radiograph showed the remaining root of the third molar and the patient suffered from insensitivity of the lower lip. The root of the third molar was removed surgically. The sensitivity of the lip recovered completely within three months. Injury of the inferior alveolar nerve and the lingual nerve is a serious complication after mandibular third molar removal. The inferior alveolar nerve is at risk for injury if the third molar root is

Evaluation, prevention, and treatment of inferior alveolar nerve injury in bilateral sagittal split mandibular osteotomy.

BACKGROUND: Bilateral sagittal split osteotomy (BSSO) is a widely adopted surgical procedure for correcting mandibular deformities, yet neurosensory disturbance (NSD) of the inferior alveolar nerve (IAN) remains a significant postoperative complication. This complication adversely impacts patients' quality of life due to persistent sensory abnormalities in the lower lip and chin region. MAIN BODY: This narrative review summarizes anatomical risks and prevention/management strategies. Cone-beam CT (CBCT) may clarify canal anatomy and support risk stratification. Nerve-sparing osteotomy modific

Peripheral nerve blocks for acute trigeminal neuralgia involving maxillary and mandibular branches: a case report.

Trigeminal neuralgia (TN) is neuropathic pain that affects the trigeminal nerve branches. Facial pain experienced by patients with TN is typically intense and excruciating. The second and third branches (maxillary and mandibular) are commonly affected. This case report focuses on the potential treatment options for acute TN attacks involving these branches. The proposed approach involves extra-oral peripheral blocks using local anesthetics. Pain levels were measured using a visual numeric scale (VNS) with potential side effects and other relevant documented information. The patients showed res

Artificial intelligence in positioning between mandibular third molar and inferior alveolar nerve on panoramic radiography.

Determining the exact positional relationship between mandibular third molar (M3) and inferior alveolar nerve (IAN) is important for surgical extractions. Panoramic radiography is the most common dental imaging test. The purposes of this study were to develop an artificial intelligence (AI) model to determine two positional relationships (true contact and bucco-lingual position) between M3 and IAN when they were overlapped in panoramic radiographs and compare its performance with that of oral and maxillofacial surgery (OMFS) specialists. A total of 571 panoramic images of M3 from 394 patients

Preoperative imaging findings as predictors of postoperative inferior alveolar nerve injury following mandibular cyst surgery.

The purpose of the present study was to predict inferior alveolar nerve (IAN) exposure and associated neurosensory deficits after mandibular cyst surgery based on imaging findings. The study includes 193 sites in 184 consecutive patients who underwent enucleation of cystic lesions around a mandibular third molar (MM3) and extraction of the associated MM3. Absence/presence of white lines on panoramic radiographs (PR) and absence/presence of cortication surrounding the mandibular canal on computed tomography (CT) are evaluated as predictor variables. Outcome variables are operative IAN exposure

Effect of Low-intensity Pulsed Ultrasound on the Mandibular Remodeling Following Inferior Alveolar Nerve Transection.

Objective To investigate the effects of low-intensity pulsed ultrasound (LIPUS) on the mandibular remodeling following inferior alveolar nerve transection (IANX) and to optimize the parameters of LIPUS in the treatment of nerve injury. Methods IANX was performed in male Sprague-Dawley rats. Four weeks after IANX,the effect of daily LIPUS (from day 1) on the transected inferior alveolar nerve was examined in terms of sensitivity to mechanical stimulation. Moreover, histopathologic changes of mandibles were analyzed by micro-CT,HE staining,Masson trichrome staining, and immunohistochemical stainin

Meta-analysis of the Incidence of Lingual Nerve Deficits After Mandibular Bilateral Sagittal Split Osteotomy.

INTRODUCTION: Bilateral sagittal split osteotomy is a relatively common maxillofacial surgical procedure. The most commonly reported complication is neurosensory disturbances (NSD). The inferior alveolar nerve is by far the most commonly affected but sporadic reports of NSD of the lingual nerve also exist. The aim of this meta-analysis is to estimate the incidence of lingual nerve NSD following bilateral sagittal split osteotomy. MATERIALS AND METHODS: A systematic search of the English literature was conducted in February 2015 including several databases and using numerous MeSH terms. Articl

Lingual nerve deficit following mandibular third molar removal: review of the literature and medicolegal considerations.

The removal of mandibular third molars is probably the most frequently performed procedure in oral and maxillofacial surgery, and it is the most common surgical procedure associated with lingual nerve deficit. Lingual sensory impairment remains a clinical problem in oral and maxillofacial surgery and has serious medical and legal implications. In fact, damage to the lingual nerve is a common cause of litigation in dentistry. The purpose of this article was to review the literature about lingual nerve deficit following mandibular third molar removal and discuss the associated medicolegal aspect

Marginal mandibular nerve injury during neck dissection and its impact on patient perception of appearance.

BACKGROUND: Neck dissection to remove cervical lymph nodes is common practice in head and neck cancer management. The marginal mandibular nerve may be injured during neck dissection, particularly of level 1. The rate of injury to this nerve is underreported in the literature and its impact on patients is not well defined. METHODS: An observational study was undertaken on patients who had undergone neck dissection over a 5-year period. The patients were examined for weakness and given a questionnaire related their perception of their appearance and their function. RESULTS: Sixty-six patients

Evaluation of clinically relevant landmarks of the marginal mandibular branch of the facial nerve: A three-dimensional study with application to avoiding facial nerve palsy.

Injury to the marginal mandibular branch of the facial nerve (MMN) during surgery often results in poor functional and cosmetic outcomes. A line two finger breadths or 2 cm inferior to the border of the mandible is commonly used in planning neck incisions to avoid injury to the MMN. The purpose was to compare the two finger breadth/2 cm landmarks in predicting MMN course, and their accuracy/reliability. Thirty-one cadaveric specimens were scanned to obtain 3D surface topography (FARO® scanner). Four independent raters pinned the inferior border of the mandible and a two finger breadth line and

Comparative Evaluation of Classical Inferior Alveolar Nerve Block and Gow-gates Nerve Block for Surgical Removal of Mandibular Third Molar: A Prospective Study.

BACKGROUND: The most commonly used nerve block procedure to anesthetize the mandibular arch is the classical inferior alveolar nerve block (IANB). In 1973, Gow-Gates developed a new procedure known as the Gow Gates nerve block, to achieve anesthesia in the same area with fewer complications. METHODOLOGY: The study comprised 80 patients who reported for the surgical removal of impacted third molar. The patients were randomly assigned into two groups- Group I received Gow-gates nerve block and Group II were administered classical IANB. Positive aspiration, meantime for the onset of anesthesia,

Isolated Marginal Mandibular Nerve Injury Due to Prone Positioning During Cervical Spine Surgery: A Case Report.

Injury to the marginal mandibular nerve (MMN) as an isolated complication of posterior cervical spine surgery performed in the prone position is exceedingly rare, with only a handful of documented instances in indexed surgical literature. We present the case of a 60-year-old gentleman who developed unilateral lower facial deviation immediately following a seven-hour posterior cervical decompression and multilevel instrumented stabilization procedure. The deficit was confined entirely to the distribution of the MMN, with no involvement of other facial nerve branches or adjacent peripheral nerve

Impacted mandibular first molar with root encircling the inferior alveolar nerve and an odontogenic cyst formation.

BACKGROUND: Dentigerous cysts are among the most common benign lesions encountered in oral and maxillofacial surgery. While these cysts typically develop in association with impacted mandibular third molars, their occurrence in relation to mandibular first molars represents an exceptionally rare clinical entity. This study aims to systematically describe the surgical management, standardized postoperative care protocols, and long-term follow-up strategies for this rare presentation.
