RET mutations in exons 13 and 14 of FMTC patients.

RET is a receptor tyrosine kinase gene which is responsible for three different inherited cancer syndromes namely multiple endocrine neoplasia type 2A (MEN 2A), type 2B (MEN 2B) and familial medullary thyroid carcinoma (FMTC) as well as for Hirschsprung disease (HSCR), a congenital disorder affecting the intestinal motility. Germ-line mutations in the RET exons 10 and 11 were demonstrated in the majority of the MEN 2A and FMTC patients. On the other hand, one codon of RET exon 16 is preferentially changed in MEN 2B patients. Recently, a germ-line mutation in the exon 13 was described in one FM

Chapter 6: Syndromic primary hyperparathyroidism.

Syndromic primary hyperparathyroidism has several features in common: younger age at diagnosis when compared with sporadic primary hyperparathyroidism, often synchronous or metachronous multi-glandular involvement, higher possibility of recurrence, association with other endocrine or extra-endocrine disorders, and suggestive family background with autosomal dominant inheritance. Hyperparathyroidism in multiple endocrine neoplasia type 1 is the most common syndromic hyperparathyroidism. It is often asymptomatic in adolescents and young adults, but may be responsible for recurrent lithiasis and/

Changing concepts in the management of hereditary and sporadic medullary thyroid carcinoma.

Recent linkage of the gene for multiple endocrine neoplasia type 2A and 2B to the centromeric region of chromosome 10 has provided new insight into the causes of medullary thyroid carcinoma and has provided tools to diagnose gene carriers status for this syndrome with greater than 90% certainty. This review focuses on how these advances influence the clinical management of both sporadic and hereditary medullary thyroid carcinoma and discusses how tests based on the genetic linkage studies will aid in the early diagnosis and treatment of this syndrome. In addition, the authors have focused on s

RET oncogene mutations in 75 cases of familial medullary thyroid carcinoma in Japan.

The familial form of medullary thyroid carcinoma (MTC) is caused by mutations of the RET protooncogene. We registered 60 multiple endocrine neoplasia (MEN) 2A patients, 12 familial non-MEN medullary carcinoma (FMTC) patients, and three MEN2B patients with a confirmed RET germline mutation. All 60 MEN2A patients had RET mutations in a cysteine-rich domain. Seven of the FMTC patients had a mutation in cysteine-rich domain, and the other five had a mutation in codon 768, which encodes a tyrosine-kinase domain. Two of the MEN2B patients had a mutation in codon 918, and one patient had a double mut

The phenotypes associated with ret mutations in the multiple endocrine neoplasia type 2 syndrome.

Different specific mutations in the ret tyrosine kinase give rise to different clinical types of the inherited cancer syndrome multiple endocrine neoplasia type 2 (MEN 2). The explanation for these genotype-phenotype correlations is not yet certain. Several lines of evidence suggest that they result either from different levels of RET activation induced by different mutations or, in one class of mutation, possibly from altered substrate specificity of the RET tyrosine kinase. ret is expressed during development in a lineage of neuroectodermal cells that gives rise to the thyroid C cells and th

Clinical presentation of MEN 2A in index vs. non-index patients.

PURPOSE: Differences in syndromic manifestations of multiple endocrine neoplasia 2 A (MEN2A) between index and non-index patients are ill-defined. METHODS: Cross-sectional analysis of 602 REarranged during Transfection (RET) carriers (156 index and 446 non-index patients) who underwent thyroidectomy, adrenalectomy, and/or parathyroidectomy between 1985 and 2022, stratified by mutational risk. RESULTS: Index patients were 5.8-13.9 years older at thyroidectomy than non-index patients, at which point they had developed 10.6-14.4 mm larger medullary thyroid cancers. Correlations between index st

Primary hyperparathyroidism in a patient with primary aldosteronism.

BACKGROUND: Primary hyperparathyroidism is one of the most common causes of hypercalcemia. Inherited forms of primary hyperparathyroidism like Multiple Endocrine Neoplasia Type 1, Multiple Endocrine Neoplasia Type 2a, Hyperparathyroidism-Jaw Tumor Syndrome or isolated familial tumors are not common for our population. RESULTS: We present a case of primary hyperparathyroidism in a 38-year-old Turkish man with hyperaldosteronism (Conn's syndrome). CONCLUSION: Genetic studies could not reveal any mutation. We could not identify any inherit form of the diseases. We wanted the first-line relative

Wilms tumor associated with hirschsprung disease.

Hirschsprung disease (HD) has been associated with multiple renal malformations. We describe a child with HD who also had Wilms tumor (WT). Because the RET protooncogene, the major susceptibility gene for HD, also plays a role in normal kidney development and in the multiple endocrine neoplasia type 2 cancer syndrome, we analyzed our patient's constitutional DNA for mutations in RET commonly found in multiple endocrine neoplasia type 2A. Although this limited evaluation failed to show abnormalities, we speculate that comprehensive analysis of the RET gene may identify mutations to account for

Multicentric B-cell lymphoma, phaeochromocytoma and amyloid-rich thyroid carcinoma in a common warthog (Phacochoerus africanus): a case of multiple endocrine neoplasia type 2?

Neoplastic diseases are poorly documented in Suidae species. This report describes a 15-year-old female common warthog (Phacochoerus africanus) with multicentric lymphoma, phaeochromocytoma and C-cell carcinoma. The animal had multiple episodes of weakness and epistaxis in the months before death, until it was euthanized for acute lethargy and recumbency. Post-mortem examination revealed haemoabdomen originating from enlarged lymph nodes at the mesenteric root. The warthog had generalized lymphadenopathy, consistent with B-cell lymphoma based on CD20 immunohistochemistry. The adrenal glands we

Pheochromocytoma recurrence in hereditary disease: does a cortical-sparing technique increase recurrence rate?

BACKGROUND: Posterior retroperitoneoscopic adrenalectomy is an appealing approach for patients with hereditary pheochromocytoma and lends well to cortex preservation. We sought to examine pheochromocytoma recurrence in patients with hereditary pheochromocytoma in the era of posterior retroperitoneoscopic adrenalectomy and evaluate the predictors of recurrence. METHODS: Patients with hereditary pheochromocytoma who underwent adrenalectomy for pheochromocytoma between 1995 and 2020 with biochemical cure and follow-up >1 year were identified. Recurrence was defined as plasma metanephrines above

Sporadic Medullary Thyroid Carcinoma: Towards a Precision Medicine.

Medullary thyroid carcinoma (MTC) is a neuroendocrine malignant tumor originating from parafollicular C-cells producing calcitonin. Most of cases (75%) are sporadic while the remaining (25%) are hereditary. In these latter cases medullary thyroid carcinoma can be associated (multiple endocrine neoplasia type IIA and IIB) or not (familial medullary thyroid carcinoma), with other endocrine diseases such as pheochromocytoma and/or hyperparathyroidism. RET gene point mutation is the main molecular alteration involved in MTC tumorigenesis, both in sporadic and in hereditary cases. Total thyroidecto

Non-mammalian models of multiple endocrine neoplasia type 2.

Twenty-five years ago, RET was identified as the primary driver of multiple endocrine neoplasia type 2 (MEN2) syndrome. MEN2 is characterized by several transformation events including pheochromocytoma, parathyroid adenoma and, especially penetrant, medullary thyroid carcinoma (MTC). Overall, MTC is a rare but aggressive type of thyroid cancer for which no effective treatment currently exists. Surgery, radiation, radioisotope treatment and chemotherapeutics have all shown limited success, and none of these approaches have proven durable in advanced disease. Non-mammalian models that incorporat

Pregnancy complicated by multiple endocrine neoplasia type IIA (Sipple's syndrome).

A patient with preexisting multiple endocrine neoplasia type IIA had normal 24-hour urinary metanephrine and vanillylmandelic acid excretions before and during pregnancy. After a benign prenatal course, the patient had a term spontaneous vaginal delivery. Multiple endocrine neoplasia type IIA antedating pregnancy may be associated with a normal obstetric outcome in the absence of a phenochromocytoma.

Novel sequence variants of the genes associated with the multiple endocrine neoplasia syndromes 1 and 2. analysis by an "in silico approach.".

MEN syndromes (1 and 2) are hereditary tumor syndromes inherited as autosomal dominant traits. The genes that harbor the mutations responsible for the development of these syndromes have been cloned in recent years. In the present study we applied an "in silico" approach to find previously undescribed sequence variants of the RET protooncogene and the MEN1 gene. Sequence comparisons were performed at the National Center for Biotechnology Information Database by the search tool blastn. We found several sequence alterations in both coding and non-coding sequences. The majority of polymorphisms d

The 2017 Endocrine Society annual meeting included several communications and debates on the conservative adrenal surgery in bilateral hereditary pheochromocytomas (BHP), bilateral adrenal macronodular hyperplasia (BAMH) and primary hyperaldosteronism (PHA). The general principle is to preserve a part of the adrenal cortex to prevent the occurrence of a definitive adrenal insufficiency. In BHP, cortical sparing surgery allows more than 50% of patients to maintain normal corticotropic function at 10 years with a low recurrence rate (~ 10%). Since the adrenal medulla cannot be removed entirely,

Molecular mechanisms of development of multiple endocrine neoplasia 2 by RET mutations.

Biological and biochemical effects of multiple endocrine neoplasia type 2A (MEN-2A), type 2B (MEN-2B) and familial medullary thyroid carcinoma (FMTC) mutations on Ret function were investigated by transfection of NIH 3T3 cells. All mutations examined conferred transforming activity on Ret at variable levels. Cysteine mutations detected in MEN-2A and FMTC induced disulphide-linked homodimers of Ret on the cell surface, leading to activation of its intrinsic tyrosine kinase. Of these cysteine-mutated proteins, Ret with a codon 634 mutation had the highest transforming activity. The activity of R

RET mutations in multiple endocrine neoplasia type 2 and Hirschsprung disease.

Mutations in the RET proto-oncogene have been found in a large proportion of families affected by the multiple endocrine neoplasia type 2 syndrome and in familial and sporadic Hirschsprung disease. This review discusses the role of the RET receptor tyrosine kinase in the etiology of these dominantly inherited disorders of neural crest development. In addition, the role of genetic screening is considered in the identification and clinical management of individuals at risk for these diseases.
