Diagnostic approach to primary retroperitoneal pathologies: what the radiologist needs to know.

Retroperitoneal soft tissue lesions represent a wide range of disease processes with overlapping imaging findings. Familiarity with the CT and MR characteristics of these conditions is important to guide clinical evaluation. We review the tissue types, characteristic clinical, demographic, and imaging features of retroperitoneal tumors and tumor-like non-neoplastic conditions with CT and MR correlation, including anatomic and imaging clues, and provide a diagnostic approach to aide the radiologist in making a specific diagnosis.

Conditions of the spine.

Sequelae of congenital, developmental, infectious, degenerative, and neoplastic processes present themselves throughout adolescence, a time of rapid growth and spinal development. The author outlines the elements of an accurate diagnostic physical examination, the signs and symptoms of various spinal deformities, and their diagnosis and treatment. The potential causes of back pain in children and adolescents are explored. The author cautions against dismissing complaints of back pain in the skeletally immature population as mere "growing pains."

Immunologic abnormalities in a patient with multiple neoplasms.

Pernicious anemia, antithyroid antibodies, IgG(K) M component, depressed production of other immunoglobulins and impaired function of peripheral blood lymphocytes developed in a patient with adenocarcinoma of the colon, transitional-cell carcinoma of the tongue and clear-cell parathyroid adenoma. This case demonstrates that immunologic abnormalities and multiple malignant neoplasms may coexist in one patient. It lends support to the hypothesis that neoplastic processes and autoimmunity may both be related to impaired immune surveillance.

Enlargement of the spinal cord: inflammation or neoplasm?

Intramedullary spinal tumours are uncommon lesions that can cause significant difficulties in the differential diagnosis between inflammatory diseases such as multiple sclerosis and acute disseminated encephalomyelitis, and vascular malformations or neoplasms. We report five cases in which the history and the clinical symptoms suggested an inflammatory process of the spinal cord but the MRI characteristics were those of neoplastic lesions. Both non-neoplastic and neoplastic intramedullary lesion may have very similar symptoms, and even CSF abnormalities, but in every one of our cases, a more d

Challenges in the diagnostic of bilharziasis: a case report.

INTRODUCTION AND IMPORTANCE: Bilharziasis is characterized by a localized mass of fibrous, inflammatory tissue that usually contains numerous eggs and often involves the serosa and mesentery. It often raises the issue of differential diagnosis with neoplastic processes. We report the case of a 12-year-old child with a rectal bilharziasis. CASE PRESENTATION: We report the case of a 12-year-old child and black ethnicity with no medical history, who was referred by his parents for treatment of rectal discharge lasting 5 months. Physical examination revealed an irregular rectal mass, which was co

Neoplastic and proliferative disorders of the perinephric space.

The perinephric space is a well-marginated central compartment of the retroperitoneum, located between the anterior and posterior pararenal spaces. Various neoplastic and proliferative disorders can affect the perinephric space, and there is a wide array of imaging findings. Although many perinephric lesions may extend directly from the kidney and adrenal gland, other lesions occur in the perinephric space due to haematogenous spread, as part of a systemic disease, or by extension from an adjacent retroperitoneal compartment. Imaging plays a pivotal role in the diagnosis of perinephric disease

Paraneoplastic acral vascular syndrome in a patient with non-small cell lung adenocarcinoma with KRAS G12A oncogenic mutation.

Paraneoplastic acral vascular syndrome (PAVS) is a rare condition characterised by signs of digital ischaemia, including Raynaud's phenomenon, acrocyanosis and digital gangrene, in the context of an underlying neoplastic process. Prompt recognition of this syndrome, as well as accurate identification and classification of the associated neoplastic condition, are crucial. This report describes the case of a woman diagnosed with PAVS secondary to stage IV non-small cell lung adenocarcinoma harbouring a Kirsten Rat Sarcoma viral homolog oncogene (KRAS) G12A mutation (p.Gly12Ala).

Migration of a bladder diverticulum stone into the rectum revealing an advanced rectal process.

Rectovesical fistulas are rare and typically result from inflammatory or neoplastic conditions in the small pelvis. They occur within a pelvic region that has been altered by local inflammatory or neoplastic processes. This explains the challenges in detecting these fistulas using conventional imaging techniques, despite the effectiveness of CT and MRI. Colonic diverticulosis is the primary cause of colovesical fistulas. To date, there have been no reported cases of a rectovesical fistula associated with a bladder diverticulum in the literature. We present a case of a patient who presented wit

Multidrug resistance proteins in rheumatoid arthritis, role in disease-modifying antirheumatic drug efficacy and inflammatory processes: an overview.

Drug resistance is generally accepted as an important cause of treatment failure for patients with neoplastic or infectious diseases. Molecular mechanisms underlying drug resistance include the action of drug efflux pumps belonging to the super-family of ATP binding cassette (ABC) proteins, which mediate the cellular extrusion of a large variety of therapeutic drugs, a phenotype that is referred to as multidrug resistance (MDR). Unlike neoplastic and infectious diseases, chronic inflammatory diseases have received little attention. The potential role of ABC transporters in determining the effi

Giant-cell tumor in soft parts in a patient with osseous Paget's disease: diagnosis by fine-needle aspiration.

In this report, the cytological features and differential diagnosis of a case of giant-cell tumor (GCT) in soft tissues in a patient with osseous Paget's disease (PD) are described, with histological confirmation. Characteristic cytological findings include a diffuse cellular population of neoplastic cells composed of two types: one group of polygonal or round mononuclear small cells, exhibiting a thin-rim, dense cytoplasm and single hyperchromatic nuclei; and the second group of multinucleated, osteoclast-type giant cells with dozens of round nuclei, showing occasional micronucleoli. Diffuse disease of the liver: radiologic-pathologic correlation.

Cross-sectional imaging is playing an increasing role in diagnosis of diffuse liver diseases because it clarifies, in many cases, the overlap in clinical and laboratory manifestations often present in diffuse hepatic processes and thus may eliminate the need for a biopsy. Advances in cross-sectional imaging, particularly in magnetic resonance (MR) imaging, enable further characterization of hepatic parenchymal and architectural changes, allowing closer correlation with underlying pathologic changes. Advanced imaging techniques can be used to characterize a variety of metabolic, vascular, toxic

Paraganglioma of the vulva.

A case of vulvar paraganglioma is reported. Following presentation with vulvar pain, a 1-cm nodule was excised from the labium minus of a 58-year-old woman. Histologically, the tumor was composed of nests of round eosinophilic cells with moderately pleomorphic nuclei, beneath an intact squamous epithelium. Ultrastructural studies indicated two cell types within the neoplasm: chief cells with numerous small neurosecretory granules and peripheral slender sustentacular cells. A reticulin stain confirmed the "zellballen" nature of the neoplasm, and the neoplastic cells showed moderate argyrophilia

Lectin histochemistry of human bone marrow: investigation of trephine biopsy specimens in normal and reactive states and neoplastic disorders.

The lectin binding pattern of bone marrow cells in normal and reactive states and in various neoplastic disorders was investigated using trephine biopsy specimens taken from the iliac crest. The tissue samples were routinely processed (fixed in formalin and embedded in paraffin wax) and subjected to mild decalcification with EDTA. The following results were obtained. (1) More than half of the 23 fluoresceinated lectins used reacted with normal blood cells and/or their neoplastic derivatives. Inhibition tests with the appropriate sugars confirmed the specificity of binding for the majority, but

Basement membrane components in normal, dysplastic, neoplastic laryngeal tissue and metastatic lymph nodes.

The immunohistochemical localization of the basement membrane (BM) components laminin, type IV collagen and fibronectin was analyzed in normal, dysplastic and neoplastic laryngeal specimens. The distribution of these macromolecules was also investigated in metastatic lymph nodes. A regular and continuous staining for laminin and type IV collagen was present in normal and mild dysplastic epithelium (LIN I); interruptions and reduplications were more evident in severe dysplasia (LIN III), together with an increased positivity for fibronectin in the subepithelial connective tissue. Lytic Mastoid Lesion in a Patient with Otalgia.

The differential diagnosis for an isolated lytic mastoid lesion is broad, encompassing various conditions requiring careful consideration. These include granulomatous disorders such as Langerhans cell histiocytosis and sarcoidosis, neoplastic processes like multiple myeloma, leukemia, lymphoma, and metastases, primary bone diseases such as Paget's disease, fibrous dysplasia, and osteitis fibrosa cystica, as well as infectious causes like osteomyelitis. In this report, we present a patient with otalgia and an isolated lytic mastoid lesion.

Conjunctival lymphangiectasia in a pediatric patient with neurofibromatosis type 1.

Conjunctival lymphangiectasia, a rare condition characterized by dilated lymphatic channels in the bulbar conjunctiva, generally develops as a consequence of a local lymphatic scarring or distal mechanical outflow obstruction following surgery, radiotherapy, neoplastic disease, or other disease processes. The actual cause often remains unknown. We report a case of a unilateral conjunctival lymphangiectasia in a 7-year-old girl with neurofibromatosis type 1 (NF-1) and discuss a possible association between the two entities.

Dual-Energy CT in Musculoskeletal Imaging: What Is the Role Beyond Gout?

OBJECTIVE. Dual-energy CT (DECT) involves the acquisition of CT attenuation data at two different energy levels. In musculoskeletal imaging, gout detection is the most validated clinical indication for DECT. However, there is emerging evidence for several other clinical musculoskeletal applications, including metal artifact reduction and bone marrow imaging for traumatic, neoplastic, and inflammatory processes. CONCLUSION. In this article, we review the current role of DECT in musculoskeletal imaging, primarily focusing on nongout entities.

Clinical and pathological characterization of a feline intestinal large granular lymphocyte lymphoma (LGLL).

A five-year-old neutered male domestic cat presented with a three-month history of vomiting, hyporexia, progressive weight loss, and abdominal pain. Ultrasound revealed a significant increase in the thickness of the ileum, suggestive of a neoplastic process. Enterectomy and enteroanastomosis was performed, and the sample was sent for histopathological examination. Grossly, the intestine was tortuous and was irregular. In the antimesenteric portion, a 1.4 cm thick, annular, homogenous, white and soft mass was present slightly reducing luminal diameter. Histologically, the neoplastic cells were

How Reciprocal Interactions Between the Tumor Microenvironment and Ion Transport Proteins Drive Cancer Progression.

Solid tumors comprise two major components: the cancer cells and the tumor stroma. The stroma is a mixture of cellular and acellular components including fibroblasts, mesenchymal and cancer stem cells, endothelial cells, immune cells, extracellular matrix, and tumor interstitial fluid. The insufficient tumor perfusion and the highly proliferative state and dysregulated metabolism of the cancer cells collectively create a physicochemical microenvironment characterized by altered nutrient concentrations and varying degrees of hypoxia and acidosis. Furthermore, both cancer and stromal cells secre

Basic principles of cancer genetics.

All cancer can be described as genetic, that is, due to altered DNA. Many of these mutations will be accumulated during the normal division of cells. However, some people may inherit abnormal genes, which predispose those individuals to high risk of certain malignancies. These individuals can sometimes be identified as having a family history of affected individuals, some of which might have early age of onset or multiple malignancies. Specific genes have been identified as being associated with certain of these malignancies. The hereditary cancers include (but are not limited to) ovary, breas
