[Are conduction studies in the median nerve obligatory for the diagnosis of the carpal tunnel syndrome: a review].

Most the of doctors, including surgeons, who deal with diagnosing and treatment of carpal tunnel syndrome believe that median nerve conduction studies are obligatory for accurate diagnosis of the syndrome, providing various arguments to support this opinion. Based on available, current literature, a critical analysis of such an attitude was conducted. There is no definitive scientific evidence supporting a greater diagnostic importance of abnormal conduction in median nerve, than classical symptoms and signs of carpal tunnel syndrome. There is no enough evidence that electrophysiological studi

[Relationship between adverse neural tension and nerve conduction studies in patients with symptoms of the carpal tunnel syndrome].

The purpose of this study was to evaluate, through a series of cases, the relationship between the adverse neural tension of median nerve (ANTm) and the electrophysiological involvement in 38 patients with symptoms of the carpal tunnel syndrome (CTS), submitted to nerve conduction studies (NCS). The main measures had been ANTm (in degrees) obtained through the test of neural tension provocation (TNTP) and parameters of the NCS, divided into three groups: normal, without severe electrophysiological alteration and with severe electrophysiological alteration. Significant correlations were found b

Recurrent laryngeal motor nerve conduction studies in a rat model: Establishing an objective measure for investigating laryngeal innervation.

INTRODUCTION/AIMS: Disease or injury can cause neuromuscular changes to the larynx that can affect voice, breathing, and swallowing. Motor nerve conduction studies have had limited use in the study of laryngeal neurophysiology, despite their importance in other anatomic sites. The aim of this study was to explore the feasibility of performing recurrent laryngeal motor nerve conduction studies (rlMNCS) in a rat model. METHODS: rlMNCS were performed in 15 rats under anesthesia. A bipolar stimulating electrode was placed on the recurrent laryngeal nerve (RLN) 5 mm below the cricoid cartilage. Vi

Effectiveness of ultrasonography and nerve conduction studies in the diagnosing of carpal tunnel syndrome: clinical trial on accuracy.

BACKGROUND: The aim of this study was to evaluate the effectiveness of two diagnostic tests routinely used for diagnosing carpal tunnel syndrome (CTS)-ultrasonography (US) and nerve conduction studies (NCS)-by comparing their accuracy based on surgical results, with the remission of paresthesia as the reference standard. METHODS: We enrolled 115 patients, all of the female gender with a high probability of a clinical diagnosis of CTS. All patients underwent US and NCS for a diagnosis and subsequent surgical treatment. As a primary outcome, the accuracy of the US and NCS diagnoses was measured

Diagnostic Significance of Ultrasonographic Measurements and Median-Ulnar Ratio in Carpal Tunnel Syndrome: Correlation with Nerve Conduction Studies.

BACKGROUND AND PURPOSE: We determined the reliability of ultrasonography (US) measurements for diagnosing carpal tunnel syndrome (CTS) and their correlation with symptom duration and electrophysiology findings. We determined whether the ratio of the median-to-ulnar cross-sectional areas (CSAs) can support CTS diagnoses. METHODS: The pisiform CSA (CSA(pisiform)), swelling ratio (SR), palmar bowing, and CSA(pisiform)/ulnar CSA (CSA(ulnar)) measurements made in two subgroups of CTS patients (having sensory affection alone or having both sensory and motor affection) were compared with controls. Use of nerve conduction studies and the pressure-specified sensory device in the diagnosis of carpal tunnel syndrome.

Sixty-nine patients with signs of carpal tunnel syndrome (CTS) underwent nerve conduction studies (NCS) and testing with the Pressure-Specified Sensory Device (PSSD). A total of 102 tests were performed (28 bilateral). Twenty patients underwent a carpal tunnel release and were retested after 4 to 6 months. The Symptom Severity Score (SSS) was calculated before and after surgery. A control group of 20 hands in 10 asymptomatic volunteers underwent identical testing. The NCS sensitivity was 87% with a specificity of 90% whereas the PSSD sensitivity was 81% with a specificity of 65%. The combined

Utility of nerve conduction studies for carpal tunnel syndrome by family medicine, primary care, and internal medicine physicians.

INTRODUCTION: Nerve conduction studies (NCS) are increasingly being performed at the point-of-service by family medicine, primary care, and internal medicine (FM/PCP/IM) physicians. Carpal tunnel syndrome (CTS) is a common neuropathy often diagnosed with the aid of NCS. METHODS: A retrospective analysis of a point-of-service NCS data registry was conducted; 1190 patients who underwent NCS by 613 FM/PCP/IM physician practices, for evaluation of CTS were analyzed. Utility measures included demographic and electrophysiological characteristics of study population, adherence to evidence-based test

Current practice in the use of nerve conduction studies in carpal tunnel syndrome by surgeons in the netherlands.

The current practice in and the opinion about the treatment of carpal tunnel syndrome by surgeons in the Netherlands was evaluated in respect of the extent to which electrodiagnostic studies are used or needed to confirm the diagnosis. Questionnaires were sent to all Dutch surgeons who operate on patients with carpal tunnel syndrome. The response rate was 47% (324 out of 686). The majority of neurosurgeons and orthopaedic surgeons seldom operate without electrodiagnostic confirmation in line with the Dutch consensus guideline on this subject. In contrast, plastic surgeons operate more often on

Correlating nerve conduction studies and clinical outcome measures on carpal tunnel syndrome: lessons from a randomized controlled trial.

The reported relationships between nerve conduction studies (NCS) and outcome measures in carpal tunnel syndrome (CTS) are weak to moderate. However, selection of patients may have confounded nonrandomized studies. NCS have potentially great value in selecting patients for a specific treatment and in objectively assessing the efficacy of treatments in CTS, especially if they correlate significantly with clinical outcome measures. To investigate the relationship between clinical outcome measures for the severity of complaints and NCS in patients treated for CTS, data were obtained from a multic

The role of forearm mixed nerve conduction study in the evaluation of proximal conduction slowing in carpal tunnel syndrome.

OBJECTIVE: A decrease of forearm median motor conduction velocity (CV) is a common electrophysiological finding in carpal tunnel syndrome (CTS), ascribed to two possible mechanisms: either conduction block or slowing of the fastest myelinating fibers in the carpal tunnel, or retrograde axonal atrophy (RAA) with retrograde conduction slowing (RCS). We hope to utilize both direct and derived forearm median mixed nerve conduction studies to clarify the mechanism of the decrease of forearm median motor CV in CTS. METHODS: Seventy-five CTS patients and 75 age-matched control subjects received conv

Carpal tunnel syndrome in systemic sclerosis: prevalence, clinical correlates, and diagnostic performance of ultrasound compared with nerve conduction studies.

BACKGROUND: Carpal tunnel syndrome (CTS) is a frequent yet under-recognized complication in systemic sclerosis (SSc), where hand symptoms may overlap with vasculopathy and musculoskeletal involvement. Consequently, diagnosis often requires nerve conduction studies (NCS), while the role of ultrasound remains incompletely defined. We aimed to determine the prevalence of CTS in SSc using NCS as the reference standard, identify associated clinical and disease-related factors, and assess the diagnostic performance of ultrasound. METHODS: We conducted a cross-sectional study including consecutive SS

Clinic-based nerve conduction studies reduce time to surgery and are cost effective: a comparison with formal electrophysiological testing.

INTRODUCTION: Carpal tunnel syndrome is the most common compression neuropathy affecting the upper limb. Clinical diagnosis is not always clear and electrophysiological testing can be indicated when considering a patient for decompression surgery. The downside of electrophysiological testing is cost and increased time to surgery. Newer methods of performing nerve conduction studies in clinic have become available. MATERIALS AND METHODS: We investigated the use of a clinic-based, handheld, non-invasive electrophysiological device (NC-stat®) in 71 patients with suspected carpal tunnel syndrome

Reproducibility of normal facial motor nerve conduction studies and their relevance in the electrophysiological assessment of peripheral facial paralysis.

To determine the intra-examiner intertrial reproducibility of normal facial motor nerve conduction studies (FNCS) and their relevance in electrophysiological assessments of peripheral facial paralysis, 52 patients with acute unilateral Bell's palsy were examined on two separate occasions 1 months apart. Three electroneurographic methods were assessed. On the unaffected side of the face, FNCS are reliable when performed by a single examiner over time. Nevertheless, compound muscle action potential (CMAP) baseline-to-peak and peak-to-peak amplitude showed a rather high intertrial variability. Re

Radial motor nerve conduction study in posterior interosseous nerve syndrome and multifocal motor neuropathy.

BACKGROUND: Finger extension weakness could be a presentation of either posterior interosseous nerve (PIN) syndrome or multifocal motor neuropathy (MMN). However, there is a delay in the diagnosis of MMN in cases with a selective radial weakness, as they are frequently misdiagnosed as PIN. OBJECTIVE: To analyze which variables in nerve conduction studies could aid in the early diagnosis of MMN. METHODS: We reviewed charts of patients with diagnoses of MMN or PIN syndrome, from 2014 to 2022, in a single Brazilian reference center. Electrophysiological parameters included in the analysis were
