Pulmonary hypertension (PH) is a vascular disease characterized by remodeling of the pulmonary arteries and right heart failure. Chronic obstructive pulmonary disease (COPD) patients often have PH, which can worsen symptoms and raise morbidity and mortality. There are several reasons for increased pulmonary vascular resistance, pulmonary vascular remodeling, and ultimately the development of PH in COPD. These factors include genetics, inflammation caused by chemicals breathed, and changes in the alveoli seen in COPD and its physiology. Genes involved in mRNA conversion, subcellular localizatio

Examination and sampling of the pulmonary airways can be of great benefit in patients with respiratory disease. Transtracheal and endotracheal washes are minimally invasive procedures that allow for blind sampling of the larger airways for cytologic and culture analysis. Flexible tracheobronchoscopy allows for direct visualization of the trachea, bronchi, and oropharynx. Bronchoalveolar lavage helps to obtain samples from the small airways and alveoli during bronchoscopy. The combined use of these techniques can help clinicians diagnose challenging respiratory cases. The adult respiratory distress syndrome (ARDS) developed in 2 patients with blastomycosis. In one, ARDS apparently followed the rupture of an infected subcarinal lymph node into a bronchus. Extensive involvement of alveoli with Blastomyces dermatitidis was noted at autopsy in both patients. Four additional cases of blastomycosis with definite or presumed ARDS are reviewed. The clinical course of these 6 patients was characterized by a prolonged prodrome (median, 36 days), consistent with pulmonary blastomycosis, and a rapid demise (median, 5 days) associated with ARDS.

Diseases of the small airspaces represent an increasingly important health problem. Asthma is primarily a disease of airway dysfunction, while chronic obstructive pulmonary disease (COPD) is associated with abnormalities in both the small airways and the alveoli. Conventional diffusion magnetic resonance imaging (MRI) of hyperpolarized noble gases, because of the short T(2)* of the gas, is only capable of monitoring diffusion over short times and hence only short distances. Diffusion imaging is therefore only sensitive to changes in small structures of the lung (primarily the alveoli), and wil

Twenty cases of primary varicella pneumonia, 16 in adults and four in children, were studied. Two adults and four children died, two of the latter with complicating bacterial infections. In two patients the primary cause of death was severe alveolar-capillary block. Staphylococcal septicemia, midbrain hemorrhage and meningoencephalitis were primarily responsible for death in other patients.Radiologically, the lungs showed diffuse, poorly marginated, nodular lesions, often peribronchial in location, more easily defined in the thinner peripheral lung fields, with an alveolar acinar pattern, tend

Bedside lung ultrasound may be an effective method for the assessment of lung recruitment in newborns with extracorporeal membrane oxygenation (ECMO). We report a case of a neonate who had severe hypoxemia with persistent pulmonary hypertension and massive pneumothorax due to meconium aspiration syndrome and was treated with ECMO. Positive pressure mechanical ventilation resulted in persistent massive air leakage from the disrupted pulmonary tissue. When ECMO was initiated, a "total lung rest" ventilation strategy was used to facilitate healing of the lung rupture and absorption of the pneumot

Fine particulate matter is defined as the particulate matter with an aerodynamic diameter of < 2.5 μm, i.e., PM2.5. Its surface absorbs large amounts of toxic and hazardous substances, which can deposit in pulmonary alveoli through respiration and reach other organs through pulmonary ventilation. Many studies have confirmed that PM2.5 is closely associated with pulmonary and cardiovascular diseases. Nonalcoholic fatty liver disease(NAFLD)has similar risk factors as these diseases, as well as obesity, hyperlipidemia, and type 2 diabetes, and it is considered a part of metabolic syndrome. In thi

OBJECTIVE: To describe radiological appearances of systemic air emboli versus intravascular air from putrefaction.
MATERIALS AND METHODS: The hospital trauma database was searched for patients who underwent computed tomography (CT) autopsy. The studies were reviewed and evaluated for intravascular gas. The appearances and location of intravascular air were characterised.
RESULTS: Four cases of intravascular gas were identified out of 15 cases of CT autopsy performed from March 2004 to December 2006. In three cases, intravascular air was predominantly in the arterial system, coupled with seve

Pulmonary alveolar proteinosis (PAP) is a rare lung disorder in which surfactant-derived lipoproteins accumulate excessively within pulmonary alveoli, causing severe respiratory distress. The importance of granulocyte/macrophage colony-stimulating factor (GM-CSF) in the pathogenesis of PAP has been confirmed in humans and mice, wherein GM-CSF signaling is required for pulmonary alveolar macrophage catabolism of surfactant. PAP is caused by disruption of GM-CSF signaling in these cells, and is usually caused by neutralizing autoantibodies to GM-CSF or is secondary to other underlying diseases.

The retention of occupational and environmental particles in the lung is a primary determinant of biological effects. In the distal respiratory tract, particle clearance includes phagocytosis by alveolar macrophages (AMs), migration to the terminal bronchiole, and transport of AMs and particles by the mucociliary escalator. With increasing particle exposure, a focal collection of particle-laden macrophages results at the respiratory bronchiole (RB) which is that site in the clearance pathway demanding the greatest traverse by these cells after a commencement from the alveoli. With the greatest

Diffuse alveolar filling disease (DAFD) is characterized by diffuse lesions in both lungs caused by the abnormal filling of bilateral alveolar spaces (and sometimes the terminal bronchioles) with substances such as water, blood, lipids, inflammatory cells, pathogens, or tumor cells. Clinically, it is characterized mainly by dyspnea and hypoxemia, while imaging examinations reveal diffuse ground-glass opacities or consolidation shadows in both lungs. In the early stage, lung volume generally remains unchanged, but pulmonary interstitial fibrosis may occur in the advanced stage. Typical diseases

The mammalian lung completes its last step of development, alveologenesis, to generate sufficient surface area for gas exchange. In this process, multiple cell types that include alveolar epithelial cells, endothelial cells, and fibroblasts undergo coordinated cell proliferation, cell migration and/or contraction, cell shape changes, and cell-cell and cell-matrix interactions to produce the gas exchange unit: the alveolus. Full functioning of alveoli also involves immune cells and the lymphatic and autonomic nervous system. With the advent of lineage tracing, conditional gene inactivation, tra

INTRODUCTION: Fat embolism syndrome is a severe complication of long bone fractures, corresponding to the obstruction of small vessels by microdroplets of fat, originating from medulla ossium. Pulmonary involvement, present in 90% cases, makes the severity of the disease.
CASE REPORT: We report the case of a 22-year-old man who presented, two days after industrial accident causing an opened tibial fracture, acute dyspnea with hemoptysis. Angio-CT-scan didn't show any proximal vascular obstruction, but parenchymal sections showed diffuse, bilateral and multifocal hyperdensities predominating a

Emphysema is an almost constant finding in the lungs of chronic obstructive pulmonary disease patients. Several types of emphysema are recognized by pathologists, but only the centrilobular (CLE) and panlobular (PLE) emphysemas are found in association with smoking. In this review, the morphological and functional differences between CLE and PLE are described, and it is suggested that they could arise as different abnormalities as a result of the same insult, cigarette smoke. In CLE: 1) the destruction of the lung is uneven and originates around the airways; 2) the membranous bronchioles are t

Hislop, A., Sanderson, M., and Reid, L. (1973).Thorax, 28, 435-441. Unilateral congenital dysplasia of lung associated with vascular anomalies. In three cases of unilateral lung dysplasia the structural changes have been analysed quantitatively to throw light on the nature of the anomaly and stage of its appearance. In one case the pulmonary artery was hypoplastic and the systemic supply normal but increased, suggesting onset in late intrauterine life or early childhood: in the other two cases the blood supply was only systemic, from both normal and abnormal arteries, suggesting onset in the e
