Autograft aortic arch extension and sleeve resection for bronchial compression after interrupted aortic arch repair.

Successful correction of bronchial compression and severe bronchomalacia complicating repair of interrupted aortic arch was achieved using transverse aortic arch extension with a pulmonary artery autograft and left bronchial sleeve resection. This procedure increased space within the aortic arch and eliminated bronchial narrowing with excellent results.

An adult patient with congenital pulmonary airway malformation and an esophageal cyst.

Combined occurrence of both congenital pulmonary airway malformation (CPAM) and esophageal cyst is rare and its diagnosis requires a carefully pathologic examination. Differential diagnoses include malignant neoplasms of pulmonary origin, pulmonary inflammatory pseudotumor, bronchogenic cysts and other congenital lesions of esophagus and lung. This paper presented a rare case report of a 31-year-old adult male with combined type III CPAM and an esophageal cyst. The patient was misdiagnosed and treated for pneumonia and tuberculosis before being admitted to our hospital. To eliminate symptoms a

[Atrial fibrillation: healing by focal high frequency catheter ablation?].

Atrial fibrillation is the most common sustained arrhythmia causing substantial morbidity and probably increasing the risk of death. Most commonly, it is divided into a paroxysmal form, when--by definition--episodes end spontaneously, or a persistent one that lasts and requires a medical or electrical intervention for its termination. It might be called permanent, when no further attempts seem to be indicated for its elimination. Until recently, therapeutic strategies aimed at preventing cardiac embolism and at restoring and maintaining sinus rhythm by antiarrhythmic drugs. Long-term efficacy

Aspergillary bronchopneumonia: an unusual cause of atelectasis and asphyxia in a leukemic patient.

A 22-year-old man in his first relapse of T-acute lymphoblastic leukemia developed fever and a pulmonary infiltrate after 23 days of granulocytopenia. Although having been under amphotericin B for 10 days, productive purulent cough ensued, with right lobe atelectasis and acute ventilatory failure that resolved after the elimination of a thick gelatinous bronchial plug. Sputum cultures yielded Candida Albicans and Staphylococcus epidermidis, and microscopic examination of the sputum plug disclosed Aspergillus hyphae. The patient died 9 days after, of a disseminated Aspergillus infection, confir

Effect of hemorrhagic shock on intrapulmonary right-to-left shunt (QS/QT) and dead space (VD/VT).

The effect of hemorrhagic shock on intrapulmonary right-to-left shunt and dead space ratio was investigated in spontaneously breathing dogs under barbiturate anesthesia. Intrapulmonary right-to-left shunt was reduced sufficiently to offset the marked increase in arteriovenous oxygen difference and PaO2 did not fall. Dead space ratio was increased markedly; therefore, in shock, sufficient CO2-elimination depends on severe hyperventilation. According to this empirical findings, a decrease in PaO2 in shock must not be interpreted as a consequence of the shock itself, but it implies an additional

Focal ablation for atrial tachycardia from the double-exit of the Marshall bundle inducing atrial fibrillation.

Atrial fibrillation (AF) from the ligament/vein of Marshall (LOM/VOM) has previously been described. We report the case of a 23-year-old woman with an antiarrhythmic drug-resistant AF induced by two distinct atrial tachycardias (ATs). Focal ablation of these ATs from the double-exit of the Marshall bundle using a three-dimensional map eliminated AF triggering, even though pulmonary vein electrical isolation is the cornerstone for paroxysmal AF. Such mechanisms are important as triggering factors to plan ablation for paroxysmal AF. Focal ablation for triggering and inducing AF, originating from

Human rhinovirus 16 induces an ICAM-1-PKR-ATF2 axis to modulate macrophage functions.

UNLABELLED: Human rhinovirus (HRV) infections are the leading cause of disease exacerbations in individuals with chronic pulmonary diseases, primarily due to impaired macrophage functions, resulting in defective bacterial elimination. We previously demonstrated that HRV16 impairs macrophages' functions in an ARL5b-dependent manner. In permissive cells, ARL5b acted as an HRV16 restriction factor and was repressed. Here, we delve into the dual regulation of ARL5b by HRV16 in both cell types. We analyzed the effect of HRV16 on primary human macrophages using neutralizing antibodies, specific inhi

Chimeric autoantibody receptor T cells for targeted depletion of myeloperoxidase-specific anti-neutrophil cytoplasmic antibody-producing B cells.

Anti-neutrophil cytoplasmic antibody (ANCA) vasculitides are autoimmune disorders driven by autoantibodies against neutrophil granule proteins, myeloperoxidase (MPO) and proteinase 3, which cause necrotizing small-vessel inflammation with prominent lung and kidney involvement, resulting in pulmonary hemorrhage and glomerulonephritis. Current remission-induction regimens rely on broad immunosuppression and global B cell depletion, which are effective but associated with substantial infection risk and relapse that often necessitates repetitive, long-term treatment. Here, we develop MPO chimeric

Eliminating Algorithmic Racial Bias in Clinical Decision Support Algorithms: Use Cases from the Veterans Health Administration.

The Veterans Health Administration uses equity- and evidence-based principles to examine, correct, and eliminate use of potentially biased clinical equations and predictive models. We discuss the processes, successes, challenges, and next steps in four examples. We detail elimination of the race modifier for estimated kidney function and discuss steps to achieve more equitable pulmonary function testing measurement. We detail the use of equity lenses in two predictive clinical modeling tools: Stratification Tool for Opioid Risk Mitigation (STORM) and Care Assessment Need (CAN) predictive model

RGS5 as a Biomarker of Pericytes, Involvement in Vascular Remodeling and Pulmonary Arterial Hypertension.

INTRODUCTION: Pulmonary arterial hypertension (PAH) is a life-threatening disease characterized by a sustained rise in mean pulmonary artery pressure. Pulmonary vascular remodeling serves an important role in PAH. Identifying a key driver gene to regulate vascular remodeling of the pulmonary microvasculature is critical for PAH management. METHODS: Differentially expressed genes were identified using the Gene Expression Omnibus (GEO) GSE117261, GSE48149, GSE113439, GSE53408 and GSE16947 datasets. A co-expression network was constructed using weighted gene co-expression network analysis. Novel

Initial and late results after catheter intervention for neonatal critical pulmonary valve stenosis and atresia with intact ventricular septum: a technique in continual evolution.

Critical pulmonary valve stenosis or atresia with intact ventricular septum is a rare congenital cardiac defect that can be technically difficult to alleviate in the catheterization laboratory. Over the past 10 years, several techniques and modifications with variable results have been advocated to facilitate the valvuloplasty procedure. This report describes a single operator's experience using various techniques in 28 neonates with critical pulmonary stenosis or atresia who were considered candidates for transcatheter intervention. The first two patients underwent a gradational balloon valvu

Norwood procedure with non-valved right ventricle to pulmonary artery shunt improves ventricular energetics despite the presence of diastolic regurgitation: a theoretical analysis.

When the Norwood procedure is conducted for the hypoplastic left heart syndrome using a non-valved right ventricle (RV) to pulmonary artery (PA) shunt, diastolic regurgitation from PA to RV may have an adverse effect on postoperative hemodynamics. In this study, we examined the impact of the diastolic regurgitation on ventricular energetics by computational analysis using a combination of a time-varying elastance chamber model and a modified three-element Windkessel vascular model. This study revealed that use of the valved or non-valved RV-PA shunt eliminated pulmonary over-circulation which

Laser ablation of the pulmonary veins by using a fiberoptic balloon catheter: implications for treatment of paroxysmal atrial fibrillation.

BACKGROUND AND OBJECTIVE: Focal sources of paroxysmal atrial fibrillation may be treatable by electrical isolation of the pulmonary veins from the left atrium. A new fiberoptic balloon catheter was tested as an alternative to radiofrequency catheter ablation for creation of circumferential thermal lesions at the pulmonary vein orifice. STUDY DESIGN/MATERIALS AND METHODS: In vitro and in vivo experiments were conducted in canine hearts to demonstrate efficacy and optimize ablation dosimetry. Continuous-wave, 1.06-microm, Nd:YAG laser radiation was delivered radially through diffusing optical f

Emerging therapies and new directions in the treatment of pulmonary arterial hypertension.

Pulmonary arterial hypertension (PAH) is a severe and progressive disease with limited survival prospects under currently available therapies. Since the 2022 edition of the European Society of Cardiology and European Respiratory Society guidelines on pulmonary hypertension, substantial clinical evidence has emerged, supporting a new treatment algorithm for PAH as presented at the 7th World Symposium on Pulmonary Hypertension 2024 and the following proceeding papers. Key updates include the introduction of sotatercept as a second-line therapy leading to a revised definition of maximal medical t

[Mild pulmonary valve stenosis: the possible spontaneous cure in the natural history of the defect].

OBJECTIVE: To determine the long-term natural history of patients with mild pulmonary valve stenosis. METHODS: Throughout a 24-year evolutive period (1980 to 2004), 83 patients with pulmonary valve stenosis were evaluated, with 29 of them presenting a mild form of the defect. Of these 29, 13 patients had a long-term follow up. The mean age at the first and last follow-up visits was 34 months (1 month to 15 years) and 10.5 years (3 to 24 years), respectively. In addition to evolutive elements, those regarding clinical, electrocardiographic and echocardiographic aspects were also assessed. RES

Pulmonary Rehabilitation in COPD: A Reappraisal (2008-2012).

Chronic Obstructive Pulmonary Disease (COPD) is a complex pathological condition associated with an important reduction in physical activity and psychological problems that contribute to the patient's disability and poor health-related quality of life. Pulmonary rehabilitation is aimed to eliminate or at least attenuate these difficulties, mainly by promoting muscular reconditioning. The scope of this paper has been the analysis of the literature on pulmonary rehabilitation in COPD patients has appeared in the last five years, focusing on the principal outcomes obtained. The results demonstrat

Repurposing rosiglitazone, a PPAR-γ agonist and oral antidiabetic, as an inhaled formulation, for the treatment of PAH.

Peroxisome-proliferator-activated-receptor-gamma (PPAR-γ) is implicated, in some capacity, in the pathogenesis of pulmonary arterial hypertension (PAH). Rosiglitazone, an oral antidiabetic and PPAR-γ agonist, has the potential to dilate pulmonary arteries and to attenuate arterial remodeling in PAH. Here, we sought to test the hypothesis that rosiglitazone can be repurposed as inhaled formulation for the treatment of PAH. We have tested this conjecture by preparing and optimizing poly(lactic-co-glycolic) acid (PLGA) based particles of rosiglitazone, assessing the drug particles for pulmonary a

Diagnosis of pulmonary leiomyosarcoma extending into the main bronchus using repeated transbronchial cryobiopsy.

The diagnosis of pulmonary leiomyosarcoma using bronchoscopy is difficult, and surgical resection is often performed for definitive diagnosis and curative therapy. We report a case of pulmonary leiomyosarcoma, successfully diagnosed using repeated transbronchial cryobiopsy (TBCB). A 69-year-old-woman was found to have an oval mass in the left hilar region extending into the left main bronchus on computed tomography (CT). All transbronchial biopsy specimens were necrotic, but repeated TBCB removed the necrotic tissue from the tumour and finally led to the diagnosis of pulmonary leiomyosarcoma.

Preterm birth and respiratory disease in later life.

Chronic respiratory diseases are a common complication of preterm birth, particularly among very immature infants or those suffering from bronchopulmonary dysplasia. Major progress in the treatment of preterm newborns has changed the pattern of late respiratory complications. The major respiratory problem in infancy and early childhood is respiratory exacerbations caused by infections (particularly viral ones), which need hospitalization. The symptoms become mild in school-age children; however, a group of children still present with chronic airway obstruction defined by recurrent episodes of
