Quantifying clinical severity of physics errors in high-dose rate prostate brachytherapy using simulations.

PURPOSE: To quantitatively evaluate through automated simulations the clinical significance of potential high-dose rate (HDR) prostate brachytherapy (HDRPB) physics errors selected from our internal failure-modes and effect analysis (FMEA). METHODS AND MATERIALS: A list of failure modes was compiled and scored independently by 8 brachytherapy physicists on a one-to-ten scale for severity (S), occurrence (O), and detectability (D), with risk priority number (RPN) = SxOxD. Variability of RPNs across observers (standard deviation/average) was calculated. Six idealized HDRPB plans were generated,

Trend analysis of the dosimetric impact of anatomical changes during proton therapy for maxillary sinus carcinoma.

PURPOSE: Anatomical changes, such as shrinkage and aeration, can affect dose distribution in proton therapy (PT) for maxillary sinus carcinoma (MSC). These changes can affect the dose to the target and organs at risk (OARs); however, when these changes occur during PT is unclear. This study aimed to investigate the dosimetric impact of anatomical changes during PT. MATERIALS AND METHODS: Fifteen patients with MSC were enrolled in this study. Initial PT plans were generated based on initial computed tomography (CT) images. Several repeat CT images were obtained to confirm anatomical changes du

Clinical evaluation of a real-time inverse planning for Gamma Knife radiosurgery by convex optimization: a prospective comparative trial in a series of vestibular schwannoma patients.

BACKGROUND: Gamma Knife radiosurgery (GKRS) inverse dose planning is currently far from competing effectively with the quality of dose planning developed by experienced experts. A new inverse planning (IP) method based on « efficient convex optimization algorithms » is proposed, providing high-quality dose plans in real time. MATERIALS AND METHODS: Eighty-six patients treated by GKRS for vestibular schwannomas (VS) were recruited. The treatment plans created by the first author, who has 27 years of experience and has developed and delivered more than 15,000 dose plans, served as reference. A

On the evaluation of edgeless diode detectors for patient-specific QA in high-dose stereotactic radiosurgery.

PURPOSE: In this work, the potential of an innovative "edgeless" silicon diode was evaluated as a response to the still unmet need of a reliable tool for plan dosimetry verification of very high dose, non-coplanar, patient-specific radiosurgery treatments. In order to prove the effectiveness of the proposed technology, we focused on radiosurgical treatments for functional disease like tremor or pain. METHODS: The edgeless diodes response has been validated with respect to clinical practice standard detectors by reproducing the reference dosimetry data adopted for the Treatment Planning System

Dose simulations of an early 20th century kilovoltage pneumonia radiotherapy technique performed with a modern fluoroscope.

To simulate an early 20th century viral pneumonia radiotherapy treatment using modern fluoroscopy and evaluated it according to current dose guidelines. Monte Carlo was used to assess the dose distribution on an anthropomorphic phantom. Critical organs were: skin, breasts, esophagus, ribs, vertebrae, heart, thymus, and spinal cord. A 100 kVp beam with 3 mm Al HVL, 25 × 25 cm2 posterior-anterior (PA) field and 50 cm source-to-surface distance were simulated. Simulations had a resolution of 0.4 × 0.4 × 0.06 cm3 and a 6% uncertainty. Hundred percent dose was normalized to the skin surface and res

Patient-Centric Head and Neck Cancer Radiation Therapy: Role of Advanced Imaging.

The traditional 'one-size-fits-all' approach to H&N cancer therapy is archaic. Advanced imaging can identify radioresistant areas by using biomarkers that detect tumor hypoxia, hypercellularity etc. Highly conformal radiotherapy can target resistant areas with precision. The critical information that can be gleaned about tumor biology from these advanced imaging modalities facilitates individualized radiotherapy. The tumor imaging world is pushing its boundaries. Molecular imaging can now detect protein expression and genotypic variations across tumors that can be exploited for tailoring treat

Predictive Models to Determine Clinically Relevant Deviations in Delivered Dose for Head and Neck Cancer.

PURPOSE: This study aimed to improve the understanding of deviations between planned and accumulated doses and to establish metrics to predict clinically significant dosimetric deviations midway through treatment to evaluate the potential need to re-plan during fractionated radiation therapy (RT). METHODS AND MATERIALS: A total of 100 patients with head and neck cancer were retrospectively evaluated. Contours were mapped from the planning computed tomography (CT) scan to each fraction cone beam CT via deformable image registration. The dose was calculated on each cone beam CT and evaluated ba

Radiotherapy for Hodgkin Lymphoma: Reducing Toxicity While Maintaining Efficacy.

Radiation therapy (RT) for Hodgkin lymphoma has changed dramatically over the past couple of decades, from the very large extended-field RT with prophylactic treatment of all the major lymph node regions to the very limited involved-site RT with treatment only of the initially macroscopically involved lymphoma volume in the combined modality setting. Technological developments in imaging, treatment planning, and treatment machines have enabled very significant reductions in radiation doses to normal organs without jeopardizing the coverage of the lymphoma. Hence, the risks of long-term adverse

Importance of deformable image registration and biological dose summation in planning of radiotherapy retreatments.

The purpose of this study is to evaluate the effect of nonrigid and fractionation-corrected dose summation on total doses in radiotherapy and to demonstrate the benefits of such dose summation on clinical decision-making for planning of retreatments. Dose summation of organs at risk (OARs) was investigated for 3 clinical cases with need of retreatment to the same site: head and neck, brain, and mediastinum. Three different summation methods over old and new radiotherapy treatment plans are presented and compared: (1) rigid raw sum with rigid registration of the planning images and direct dose

Paddle-based rotating-shield brachytherapy.

PURPOSE: The authors present a novel paddle-based rotating-shield brachytherapy (P-RSBT) method, whose radiation-attenuating shields are formed with a multileaf collimator (MLC), consisting of retractable paddles, to achieve intensity modulation in high-dose-rate brachytherapy. METHODS: Five cervical cancer patients using an intrauterine tandem applicator were considered to assess the potential benefit of the P-RSBT method. The P-RSBT source used was a 50 kV electronic brachytherapy source (Xoft Axxent™). The paddles can be retracted independently to form multiple emission windows around the

Analysis of fractionation correction methodologies for multiple phase treatment plans in radiation therapy.

PURPOSE: Radiation therapy is often delivered by multiple sequential treatment plans. For an accurate radiobiological evaluation of the overall treatment, fractionation corrections to each dose distribution must be applied before summing the three-dimensional dose matrix of each plan since the simpler approach of performing the fractionation correction to the total dose-volume histograms, obtained by the arithmetical sum of the different plans, becomes inaccurate for more heterogeneous dose patterns. In this study, the differences between these two fractionation correction methods, named here

Reirradiation Collaborative Group (ReCOG) consensus on standards for dose evaluation and reporting in patients with multiple courses of radiation therapy: an AAPM/ACRO/ASTRO/CARO/COMP/CADRA/CPQR/ESTRO/NRG-endorsed consensus statement.

As cancer survivors live longer, technologies improve, and reirradiation (reRT) becomes more common, standardised methods for the assessment and reporting of cumulative radiation doses are needed to allow treatment optimisation and integration with other medical specialties managing these complex patients. This consensus statement, developed by an international collaboration of radiation oncologists, physicists, and other experts in the Reirradiation Collaborative Group, proposes a framework for consistent evaluation, documentation, reporting, and clinical decision making in reRT. An optimized knowledge-based planning method for craniospinal irradiation integrated with auto-contouring.

BACKGROUND: Craniospinal irradiation (CSI) is a complex treatment requiring precise delineation of extended plan target volumes (PTV) and multiple organs at risk (OARs). The process traditionally demands substantial time and expertise from physicians and medical physicists. While artificial intelligence (AI) has shown promise in auto-contouring and auto-planning in other tumors, its application to CSI remains limited due to challenges in model generalizability and outlier sensitivity. METHODS: We developed an automated workflow integrating deep learning-based auto-contouring and machine learn

Dose optimization with fully flexible vertex positioning for LATTICE radiotherapy.

BACKGROUND: Lattice radiotherapy (LATTICE) is a technique of spatially fractionated radiation therapy (SFRT) that delivers high radiation doses to specific regions (vertices) within a large tumor, forming a spatially modulated "lattice" pattern, while surrounding areas receive lower doses to minimize damage to healthy tissues. Although the original conception of LATTICE did not prescribe any rigorous symmetry, and early clinical implementations relied on manual vertex placement tailored to tumor shape and anatomical constraints, more recent automated approaches have introduced regular patterns

Minibeam-pLATTICE: A novel proton LATTICE modality using minibeams.

BACKGROUND: LATTICE, a form of spatially fractionated radiation therapy (SFRT) that delivers high-dose peaks and low-dose valleys within the target volume, has been clinically utilized for treating bulky tumors. However, its application to small-to-medium-sized target volumes remains challenging due to beam size limitations. PURPOSE: To address the challenge of applying LATTICE radiation therapy to small-to-medium-sized targets, this work proposes a novel proton LATTICE (pLATTICE) modality using minibeams, namely minibeam-pLATTICE, that can extend the LATTICE approach for small-to-medium targ

Nibbling toward optimal sphere packing for lattice spatially fractionated radiotherapy of large and bulky unresectable tumors.

BACKGROUND: The Spatially Fractionated Radiotherapy (SFRT) treatment technique is characterized by its highly heterogeneous dose distribution, with an alternating pattern of high- and low-dose areas inside large tumors. For lattice SFRT, a technique that utilizes high-dose spheres, a common obstacle is maximizing the number of high-dose spheres within the tumor while maintaining spacing rules to preserve the low-dose regions. PURPOSE: We propose a novel algorithm that uses the Rödl nibble methodology to optimize the number of lattice vertices placement for SFRT planning. By increasing the num

BrachyPlan: A fine-grained efficient dose-guided inverse planning strategy for low-dose-rate brachytherapy.

Brachytherapy delivers highly conformal doses for malignancies ranging from pancreatic to head-and-neck cancers, yet today's treatment-planning systems still depend on extensive manual manipulation and dose engines of uncertain accuracy. We introduce BrachyPlan, an end-to-end pre-operative framework that automates low-dose-rate (LDR) brachytherapy planning while preserving clinical precision. The system combines: 1) a real-time Monte Carlo dose engine that produces high-fidelity, heterogeneity-aware dose maps, and 2) a dose-guided inverse-planning algorithm that optimally places seeds to meet

Efficient operator-splitting minimax algorithm for robust optimization.

BACKGROUND: The treatment uncertainties such as patient positioning can significantly affect the accuracy of proton radiation therapy (RT). Robust optimization can account for these uncertainties during treatment planning, for which the minimax approach optimizes the worst-case plan quality. PURPOSE: This work will develop an efficient minimax robust optimization algorithm for improving plan quality and computational efficiency. METHODS: The proposed method reformulates the minimax problem so that it can be conveniently solved by the first-order operator-splitting algorithm (OS). That is, th
