Active middle ear implants in patients undergoing subtotal petrosectomy: new application for the Vibrant Soundbridge device and its implication for lateral cranium base surgery.

OBJECTIVE: The functional outcome of ossiculoplasties in chronic ear and lateral cranium base surgery depends on the presence of a ventilated middle ear space and is guided by the existence or absence of ossicular remnants. In patients with poorly ventilated middle ears, after multiple previous operations, missing stapes suprastructure, or after partial temporal bone resection for tumor removal, restoration of conductive hearing is not possible. The direct placement of a vibrating floating mass transducer (FMT) onto the round window membrane with obliteration of the surgical cavity is a new op

Effect of leukotriene inhibitor on cochlear blood flow in salicylate ototoxicity.

Our previous studies showed that salicylate ototoxicity is associated with decreased levels of vasodilating prostaglandins (PGs) and increased vasoconstricting leukotrienes (LTs) in the perilymph and reduced cochlear blood flow (CoBF). The purpose of this study was to test the hypothesis that leukotriene inhibitor prevents salicylate ototoxicity by preventing abnormal elevation of LT levels in the inner ear, thus averting a decrease in CoBF resulting from abnormal levels of arachidonic acid metabolites in the inner ear. Ototoxicity was induced in chinchillas by either local round window membra

Performance of the round window soft coupler for the backward stimulation of the cochlea in a temporal bone model.

The round window vibroplasty is a feasible option for the treatment of conductive, sensorineural and mixed hearing loss. Although clinical data suggest a satisfying clinical outcome with various coupling methods, the most efficient coupling technique of the floating mass transducer to the round window is still a matter of debate. For this, a soft silicone-made coupler has been developed recently that aims to ease and optimize the stimulation of the round window membrane of this middle ear implant. We performed a temporal bone study evaluating the performance of the soft coupler compared to the

[Prediction of facial nerve fronting with temporal bone high resolution computed tomograph].

Objective:To report a new CT measurement to predict the exposure of round window niche during facial nerve recess surgery. Methods:Forty CT scans were measured by conventional facial-round window line and limiting facial-round window line method, and compared with the exposure of round window niche after the opening of the facial recess. Results:There was no significant difference in the sensitivity and specificity of the two CT measurement methods to predict the exposure degree of the round window niche, and the Kappa consistency test of the two methods also showed a strong correlation. Mechanical Effects of Cochlear Implants on Residual Hearing Loss: A Finite Element Analysis.

The effects of cochlear implants on residual hearing loss is investigated through a finite element model of human auditory periphery consisting of the cochlea and middle ear. The simulation results show that a round window stiffness is the dominant factor in residual hearing loss. The increased round window stiffness to five times caused over 4 dB residual hearing loss at low frequencies below 500 Hz. Without considering round window ossification, inserting a cochlear implant can show at most 4 dB difference of residual hearing loss in magnitude from the no-implant case although the cochlear i

Traumatic rupture of the eardrum with round window fistula.

Traumatic perilymphatic round window fistula may manifest itself with minimal cochleovestibular symptoms. Its association with tympanic membrane perforation secondary to ear washing has previously never been published. The rapid complete recuperation of cochleovestibular function after a conservative obliteration of the round window niche with fat and a myringoplasty, leads us to suggest early surgical intervention when there is evidence or high suspicion of perilymphatic fistula with a traumatic membrane perforation.

Noise exposure of the inner ear during robotic drilling.

INTRODUCTION: Preserving the cochlear structures and thus hearing preservation, has become a prominent topic of discussion in cochlear implant (CI) surgery. Various approaches and soft surgical techniques have been described when approaching the inner ear. Robot-assisted cochlear implant surgery (RACIS) reaches the round window in a minimally invasive manner by following a trajectory of minimal trauma. This involves the drilling of a keyhole trajectory to the round window, through the facial recess, with no need for a complete mastoidectomy. It involves less drilling, less drilling time and le

Middle-ear and inner-ear contribution to bone conduction in chinchilla: The development of Carhart's notch.

While the cochlea is considered the primary site of the auditory response to bone conduction (BC) stimulation, the paths by which vibratory energy applied to the skull (or other structures) reaches the inner ear are a matter of continued investigation. We present acoustical measurements of sound in the inner ear that separate out the components of BC stimulation that excite the inner ear via ossicular motion (compression of the walls of the ear canal or ossicular inertia) from the components that act directly on the cochlea (cochlear compression or inertia, and extra-cochlear 'third-window' pa

[Lesions of round window membrane].

In 15 cases, a rupture of the round window membrane could be found via exploratory tympanotomy. The main sign in these cases was an inner ear deafness; vertigo and tinnitus were less frequent. These findings correspond well with reports in the literature. Since there is no specific clinical sign of a ruptured membrane, tympanotomy is necessary in all suspicious cases. Chances of restoration or maintenance of hearing are high in cases of early repair of the fistula, but prospects diminish as the time interval between injury and repair increases. Vertigo and tinnitus disappear even by a delayed

Conditions for highly efficient and reproducible round-window stimulation in humans.

Round-window stimulation is a new clinical approach for the application of active middle-ear implants. To investigate factors influencing the efficiency of round-window stimulation, experiments in 6 human temporal bones were performed with different actuator geometries and coupling conditions. The experiments show that the amplitude ratio between stapes and round-window actuator vibration is most efficient when using a 1.0-mm diameter rod with a 30° inclined tip geometry and an attached silicone pad. In this case, the amplitude ratio is 0.34 for frequencies up to 1.5 kHz and 0.27 for frequenci

Finite element analysis of round-window stimulation of the cochlea in patients with stapedial otosclerosis.

An active actuator coupled to the round window (RW) can transmit mechanical vibrations into the cochlea and has become a therapeutic option of hearing rehabilitation for patients with stapedial otosclerosis. A finite-element model of the human ear that includes sound transmission effects of the vestibular and cochlear aqueducts of the inner ear is adopted in this study for investigating the cochlear response to RW stimulation under stapes fixation. There are two effects due to otosclerosis of the stapes: the fixation of the stapedial annular ligament (SAL) and the increase of the stapes mass.

Intratympanic Iodine Contrast Injection Diffuses Across the Round Window Membrane Allowing for Perilymphatic CT Volume Acquisition Imaging.

HYPOTHESIS: Whether the round window membrane (RWM) is permeable to iodine-based contrast agents (IBCA) is unknown; therefore, our goal was to determine if IBCAs could diffuse through the RWM using CT volume acquisition imaging. INTRODUCTION: Imaging of hydrops in the living human ear has attracted recent interest. Intratympanic (IT) injection has shown gadolinium's ability to diffuse through the RWM, enhancing the perilymphatic space. METHODS: Four unfixed human cadaver temporal bones underwent intratympanic IBCA injection using three sequentially studied methods. The first method was direc

Design study of a miniaturized displacement transducer (MDT) for an active middle ear implant system.

People suffering from moderate to severe hearing loss can be treated with active middle ear implants. A new approach in this field is to implant an electromechanical transducer onto the round window membrane in order to improve coupling and be able to treat patients with middle-ear problems. In this paper the design study for a miniaturized displacement transducer (MDT) for the round window is presented. Based on a requirement analysis, the basic principle and analytical modeling of the actuator is shown. A parameter variation study results in an optimized actuator configuration that is able t

Stapedius reflexes to electrical stimulation in the rabbit for the assessment of hearing.

The possibility of using electrically induced stapedius reflexes as a means of objective hearing evaluation was investigated in the rabbit as an animal model. The contralateral stapedius reflex to acoustic and electrical stimulation was measured in anesthetized rabbits. For electrical stimulation, the middle ear was opened surgically and stimulation was applied by a monopolar electrode placed at or into the round window. Contraction of the middle ear muscle was monitored by digital sampling and averaging of the impedance changes in the contralateral ear. Acoustically evoked reflexes were recor

Round window closure affects cochlear responses to suprathreshold stimuli.

OBJECTIVES/HYPOTHESIS: The round window acts as a vent for releasing inner ear pressure and facilitating basilar membrane vibration. Loss of this venting function affects cochlear function, which leads to hearing impairment. In an effort to identify functional changes that might be used in clinical diagnosis of round window atresia, the current investigation was designed to examine how the cochlea responds to suprathreshold stimuli following round window closure. STUDY DESIGN: Prospective, controlled, animal study. METHODS: A rat model of round window occlusion (RWO) was established. With th

Minimizing hearing loss: A novel surgical approach for round window exposure in miniature pigs.

BACKGROUND: A delicate operation for exposing the round window without affecting the hearing function is critical for auditory research. Despite the different surgical approaches proposed for miniature pigs, hearing protection during operation is still challenging. The efficient solution to this problem is significant for accurate auditory research. NEW METHOD: Eight healthy Diannan miniature pigs, aged three months and of both sexes, with normal hearing, were utilized. Three pigs were employed for anatomical studies. Based on the findings from these anatomical investigations, we propose the
