Implementation of a billable transitional care model for stroke patients: the COMPASS study.

BACKGROUND: The COMprehensive Post-Acute Stroke Services (COMPASS) pragmatic trial compared the effectiveness of comprehensive transitional care (COMPASS-TC) versus usual care among stroke and transient ischemic attack (TIA) patients discharged home from North Carolina hospitals. We evaluated implementation of COMPASS-TC in 20 hospitals randomized to the intervention using the RE-AIM framework. METHODS: We evaluated hospital-level Adoption of COMPASS-TC; patient Reach (meeting transitional care management requirements of timely telephone and face-to-face follow-up); Implementation using hospi

Acute and Subacute Cough.

Cough is one of the most common reasons for health care visits. This article seeks to illustrate the classification, appropriate workup, and management of acute and subacute cough. It highlights etiologies such as bronchitis, pneumonia, and exacerbation of heart failure and asthma. There is an emphasis placed on potentially life-threatening pathologies associated with cough and how to appropriate triage cough severity in an emergency setting. The pathophysiology of cough and how it may lead to severe barotraumatic complications such as pneumothorax and Mallory Weiss tears is discussed.

Impact of Surgical Airways on Location of Death, Healthcare Utilization, and Costs Among Patients With Head and Neck Cancer: A Population-Based Study.

BACKGROUND: Patients with surgical airways experience barriers to high quality end-of-life (EOL) care. Recognizing differences in healthcare utilization and location of death among patients with surgical airways may inform improvements in EOL care delivery. This study aimed to compare location of death and selected aspects of healthcare in the last 90 days of life between head and neck cancer (HNC) patients with and without a surgical airway. METHODS: Retrospective decedent cohort study of health administrative data from the Institute of Clinical Evaluative Sciences in Ontario, Canada of indi

Tibial subacute osteomyelitis with intraosseous abscess: an unusual complication of intraosseous infusion.

Intravenous (IV) access is a critical step in patient care, especially in the emergency and/or trauma setting. Recently, intraosseous (IO) infusion has re-emerged as a recommended alternative to central venous access in both the pediatric and the adult patient. We present the case of an older adult male patient several months after emergency tibial IO infusion, now with left shin pain, and the MRI and culture findings diagnostic of subacute osteomyelitis with IO abscess, an unusual complication of IO infusion.

Cardiac structural and molecular alterations in rodent models of temporal lobe epilepsy.

OBJECTIVE: Cardiac structural and molecular changes are prevalent in people with chronic epilepsy, possibly contributing to an increased risk of premature mortality. However, understanding of the underlying pathophysiological mechanisms is limited. Here, we investigated the subacute and chronic changes in cardiac structure and ion channel/exchanger expression in different rodent models of temporal lobe epilepsy (TLE). METHODS: Two models of TLE were used: the kainic acid-induced post-status epilepticus (KASE) model in Wistar rats and the electrical self-sustained status epilepticus (SSSE) mod

Home-Based Primary Care: Adaptability Criteria for the Bedroom Layout and the Furnitures/Technological Equipments.

Within the past decade, advances in medical technology, the desires and complex care needs of an ageing population, and innovative care delivery models have initiated a shift from providing care in hospitals to outpatient settings. And more recently, the acceleration and amplification of these factors is pushing healthcare options even further from the traditional inpatient and outpatient settings towards acute and subacute care in the home. This has led the medical community to look toward providing more tools and methods of care that patients can access safely right from home and the designe

Spermatic cord knot: a clinical finding in patients with spermatic cord torsion.

Pertinent history taking and careful examination often taper the differentials of the acute scrotum; congruently the ability to diagnose acute spermatic cord torsion (SCT) when radiological adjuncts are not available is highly imperative. This observational study serves to present a series of 46 cases of spermatic cord torsion whereby we hypothesize the identification of a clinical knot on scrotal examination as an important clinical aid in making a decision to surgical exploration in patients with acute and subacute SCT, especially in centers where imaging resources are unavailable.

Chronic meningitis: still a diagnostic challenge.

Chronic meningitis is characterized by a progressive, subacute onset of lepomeningeal disease and persisting cerebrospinal fluid (CSF) abnormalities such as elevated protein level and pleocytosis for at least one month. The array of aetiologies is very wide comprising non-infectious (carcinomatous, systemic disease, toxic) and infectious (classic and opportunistic pathogens). The evaluation encompasses a careful history, complete physical examination and laboratory tests. The specific diagnosis may remain a challenge in some cases. Algorithms for the differential diagnosis are provided for bot

A clinical and epidemiological study of lupus erythematosus at a tertiary referral dermatology clinic in Korea.

Dermatological examination is critical for the evaluation of lupus erythematosus. However, little is known about the epidemiology and clinical characteristics of the lupus erythematosus patients that visit dermatology clinics with the chief complaint of skin lesions, especially among Asian populations. We performed this study to determine the epidemiology of cutaneous lupus erythematosus for three subtypes: acute cutaneous lupus erythematosus, subacute cutaneous lupus erythematosus, and chronic cutaneous lupus erythematosus, and for lupus erythematosus non-specific skin disease. Also, we sough

[Solitary nontuberculous mycobacterial infection of a cervical lymph node caused by Bartonella henselae (cat scratch disease)].

Cat scratch disease (CSD) is a common cause of subacute regional lymphadenopathy. Cats are the principal reservoir of Bartonella henselae, the etiologic agent in most cases of CSD. Because the histopathological findings are typical but not specific, a wide range of other agents must be considered in differential diagnosis. We report on a patient with a solitary swelling of the neck which remained over months with initial features suggestive of unspecific lymphadenitis, but who, after careful history taking and further serological testing, had cat scratch disease.

The Complex Role of the Complement C3a Receptor (C3aR) in Cerebral Injury and Recovery Following Ischemic Stroke.

The Complement C3a Receptor (C3aR) plays a multifaceted role along the varying temporal phases of brain injury following cerebral ischemia. C3aR is a G-protein-coupled receptor (GPCR) that binds to its ligand, C3a an anaphylatoxin generated during activation of the complement cascade. During ischemia, complement is activated as part of the initial inflammatory response, with C3aRs playing a time-dependent role in both brain injury and repair mechanisms. In the acute phase (minutes to hours post-ischemia), C3aR activation promotes the recruitment of immune cells and the release of chemokines an

Passive mobilisation of the shoulder in subacute stroke patients with persistent arm paresis: A randomised multiple treatment trial.

BACKGROUND: Performing a careful but effective mobilisation of the hemiplegic shoulder is essential for optimal muscle activation and to preserve the passive range of motion (PROM) needed to perform functional tasks. Studies concerning passive mobilisation of the post-stroke shoulder are scarce. OBJECTIVES: A randomised multiple treatment trial was conducted to compare the effects of different mobilisation techniques on shoulder PROM. METHOD: Eleven participants with upper limb paresis in the subacute phase after stroke underwent three different mobilisation techniques (3 × 4 weeks):(1) comb
