Tail-Cuff Technique and Its Influence on Central Blood Pressure in the Mouse.

BACKGROUND: Reliable measurement of blood pressure in conscious mice is essential in cardiovascular research. Telemetry, the "gold-standard" technique, is invasive and expensive and therefore tail-cuff, a noninvasive alternative, is widely used. However, tail-cuff requires handling and restraint during measurement, which may cause stress affecting blood pressure and undermining reliability of the results. METHODS AND RESULTS: C57Bl/6J mice were implanted with radio-telemetry probes to investigate the effects of the steps of the tail-cuff technique on central blood pressure, heart rate, and te

Restraint-Based ECG and Arterial Pressure Assessment Do Not Reliably Detect Drug Induced QTc Prolongation and Hypotension: Evidence From Case Studies.

Telemetry-based methods are recommended as best practice for monitoring changes in cardiovascular (CV) function in conscious nonrodent species during the development of new chemical entities. Such methods allow for unrestrained data collection in animals over extended time periods with high sensitivity to detect small effects. The CV profile of new drugs is also assessed in repeat-dose toxicology studies, routinely utilizing non-invasive blood pressure, heart rate, and ECG measurement methods that require physical or chemical restraint. These methods are limited to a short "snapshot" data coll

Can a low-cost exercise monitor provide useful heart rate monitoring for use in low-resource emergency departments?

OBJECTIVE: Our objective was to study the clinical monitoring capabilities of a low-cost fitness wristband while measuring patient satisfaction with a mobility permitting device in the emergency department. METHODS: Patients enrolled were on continuous three-lead telemetry monitoring in a high acuity zone of the emergency department. Patients were given a fitness band to wear while simultaneously monitored with standard three-lead monitor. A brief survey was conducted upon study end, and data was compared between wristband and three-lead telemetry. Median heart rate (HR) values were calculate

ECG Artifact Masquerading as Ventricular Tachycardia During Dofetilide Initiation.

BACKGROUND: Motion artifacts can cause interference over the heart's native electrical activity, leading to confusion regarding the underlying rhythm. CASE SUMMARY: A 67-year-old-man presenting for dofetilide initiation had telemetry concerning for ventricular tachycardia, resulting in a pause in dofetilide loading. Upon further review of the telemetry strips surrounding the event, it was determined that the abnormal rhythm was secondary to motion artifact, and medication was resumed without complications. DISCUSSION: Motion artifact or electrical disturbances on electrocardiogram or telemet

False Asystole Alarms Post-Temporary Pacemaker Placement Due to Pseudo-fusion.

An infant with congenital heart block and hemodynamically significant bradycardia underwent therapeutic temporary pacing wires placement. Post-operatively, frequent "asystole" alarms were observed on telemetry causing distress to both the family and the nursing staff. Investigation of these alarms showed that pacemaker malfunction led to monitor pseudo-malfunction. The alarms were alleviated with mindful setting of the pacemaker and telemetry monitor parameters. This case highlights the challenges of pacemaker placement and monitoring of very small infants in the intensive care setting. Awaren

Do stable non-ST-segment elevation acute coronary syndromes require admission to coronary care units?

BACKGROUND: Clinical practice guidelines recommend admitting patients with stable non-ST-segment elevation acute coronary syndrome (NSTE ACS) to telemetry units, yet up to two-thirds of patients are admitted to higher-acuity critical care units (CCUs). The outcomes of patients with stable NSTE ACS initially admitted to a CCU vs a cardiology ward with telemetry have not been described. METHODS: We used population-based data of 7,869 patients hospitalized with NSTE ACS admitted to hospitals in Alberta, Canada, between April 1, 2007, and March 31, 2013. We compared outcomes among patients initia

[Telemetric transmission system for in vivo measurement of the stress load of an internal spinal fixator].

For stabilizing fractures of the spine, a number of implantable devices are available. However, not much is known about the loads acting on these devices. In order to measure the forces and moments within the implant, the Dick internal fixation device was modified and fitted out with a hermetically sealed inductively powered telemetry unit. An integrated 8-channel telemetry chip was specially developed to measure the signals of six strain gauge sensors, the temperature of the implant and the supply voltage. Since the internal fixation devices are always implanted in pairs, two telemetry units

In Vitro Assessment of 3T MRI Effects on Non-3T-Compatible Cardiac Implantable Electronic Devices.

BACKGROUND: The safety of cardiac implantable electronic devices (CIEDs) during 3-Tesla (3T) MRI remains uncertain, particularly for devices not designed for 3T use. We experimentally evaluated 3T MRI effects on 3T-compatible and non-3T-compatible CIEDs. METHODS: Fifty-eight CIEDs (37 pacemakers, 21 ICDs; 3T MRI-conditional n = 31, 1.5-T MRI-conditional n = 14, and non-MRI-conditional n = 13) were tested in a 3T scanner. Each device was immersed in saline with connected leads and programmed to asynchronous or demand pacing with MRI mode ON or OFF. Device interrogation before and after scannin

Pacing spikes without visible QRS complexes: Failure to capture?

A man in his 60s with advanced infra-Hisian conduction disease underwent dual-chamber pacemaker implantation with left bundle branch area pacing. Postimplant telemetry demonstrated intermittent pacing spikes apparently not followed by QRS complexes, raising concern for failure to capture or lead dislodgment. However, the patient remained asymptomatic and device interrogation showed preserved sensing, stable lead impedance, and normal capture thresholds. Careful inspection of the telemetry tracing revealed preserved T waves and subtle low-amplitude ventricular depolarizations despite the appare

Ambulatory electrocardiographic monitoring between artifacts and misinterpretation, management errors of commission and errors of omission.

BACKGROUND: The aim of the study is to contrast the role of conventional ambulatory electrocardiographic monitoring (AEM) artifacts with a less emphasized problem with potentially more serious implications, that is, the failure to recognize, and therefore misinterpret, a genuine arrhythmia episode in the AEM recording. METHODS: The study material included 500 Holter recordings and 500 recordings from the cardiac telemetry unit. RESULTS: Electrocardiographic (ECG) artifacts were more common in telemetry recordings (5.6%) compared to Holter recordings (4%) for a total of 4.8%. There were 35 ex

Ventricular Tachycardia in a Pediatric Patient with High-Risk Thrombotic Thrombocytopenia Purpura.

An 8-year-old previously healthy male was diagnosed with thrombotic thrombocytopenic purpura (TTP) and increased serum cardiac troponin I. Telemetry recorded non-sustained ventricular tachycardia, without ST-segment changes or other abnormalities on serial electrocardiogram. This case illustrates that cardiac monitoring by telemetry should be considered in high-risk TTP with elevated cardiac troponin.

Predictors of Atrial Fibrillation in Patients With Cryptogenic Stroke.

BACKGROUND AND PURPOSE: Many patients diagnosed with cryptogenic stroke or transient ischemic attack are subsequently found to have atrial fibrillation (AF) on outpatient cardiac telemetry monitoring. Identification of predictive factors for the detection of AF could assist with patient selection to increase the yield of telemetry and hasten initiation of appropriate secondary stroke prevention. METHODS: This was a retrospective cross-sectional study of patients diagnosed with cryptogenic stroke at a comprehensive stroke center and referred for at least 21 days of prolonged outpatient telemet

Dataset of EEG power integral, spontaneous recurrent seizure and behavioral responses following combination drug therapy in soman-exposed rats.

This article investigated the efficacy of the combination of antiepileptic drug therapy in protecting against soman-induced seizure severity, epileptogenesis and performance deficits. Adult male rats with implanted telemetry transmitters for continuous recording of electroencephalographic (EEG) activity were exposed to soman and treated with atropine sulfate and the oxime HI-6 one minute after soman exposure and with midazolam, ketamine and/or valproic acid 40 min after seizure onset. Rats exposed to soman and treated with medical countermeasures were evaluated for survival, seizure severity,

Increased ventricular ectopy precedes Torsades de Pointes in patients with prolonged QT.

OBJECTIVE: Torsades de Pointes (TdP) is a potentially lethal ventricular tachydysrhythmia. Prolonged heartrate corrected QT interval (QTc) predicts TdP; however, with poor specificity. We performed this study to identify other predictors of TdP among patients with prolonged QTc. METHODS: We performed a retrospective case control study with 2:1 matching at an urban academic hospital. We searched our hospital electrocardiogram (ECG) database for tracings with heartrate ≤ 60, QTc ≥ 500, and QRS < 120, followed by a natural language search for electronic records with "Torsades," "polymorphic VT,"

Low-dose angiotensin II enhances pressor responses without causing sustained hypertension.

Subpressor doses of angiotensin II (SP-Ang II) cause a slow increase in blood pressure in rats as assessed by tail cuff plethysmography (TCP), reflecting either sustained hypertension or an exaggerated pressor response to diverse stimuli. We examined whether subpressor doses of Ang II enhance blood pressure responses to simple stress (handling of trained rats for TCP). We implanted telemetry in Sprague-Dawley rats. After 10 days of recovery and TCP training, we implanted osmotic minipumps with either SP-Ang II (50 ng/kg per minute) or vehicle, and then measured systolic blood pressure continuo

Effect of telemetric monitoring in the first 30 days of continuous positive airway pressure adaptation for obstructive sleep apnoea syndrome - a controlled pilot study.

Obstructive sleep apnoea syndrome (OSAS) is characterised by repetitive collapse of the upper airway during sleep. Continuous positive airway pressure (CPAP) applied via a mask is the standard treatment for OSAS. CPAP adherence is crucial in therapy to prevent the deleterious consequences of OSAS. We hypothesised that a combination of supervision by telemetry together with targeted telephone support in the first month of CPAP would increase CPAP adherence and treatment success. A total of 113 OSAS patients followed by telemetry-triggered interventions used the device for 5.3 h/night on 28/30 n

Primary pneumonic plague in the African Green monkey as a model for treatment efficacy evaluation.

BACKGROUND: Primary pneumonic plague is rare among humans, but treatment efficacy may be tested in appropriate animal models under the FDA 'Animal Rule'. METHODS: Ten African Green monkeys (AGMs) inhaled 44-255 LD(50) doses of aerosolized Yersinia pestis strain CO92. Continuous telemetry, arterial blood gases, chest radiography, blood culture, and clinical pathology monitored disease progression. RESULTS: Onset of fever, >39°C detected by continuous telemetry, 52-80 hours post-exposure was the first sign of systemic disease and provides a distinct signal for treatment initiation. Secondary e

SafeR and escape junctional rhythm: A singular trigger for pacemaker-mediated tachycardia.

A 70-year-old man, who previously received a dual chamber pacemaker for paroxysmal AV block (Sorin Symphony DR 2550), was noted on telemetry to have multiple episodes of rapid ventricular pacing at approximately 120bpm. Evaluation of the telemetry strips revealed that all of the rapid ventricular pacing episodes were initiated by brief runs of escape junctional rhythm. Programmed bradycardia parameters were AAI SafeR with lower rate limit of 50bpm.

Effects of telemetric epicardial leads and ventricular catheters on arrhythmia incidence in cynomolgus monkeys.

INTRODUCTION: Utilization of implantable bio-telemetry devices represents a common approach to contemporary cardiovascular safety assessment. Depending on the specific needs of the study design, and corresponding surgical methodologies employed, application of telemetry devices may have more or less liability to interact with ongoing physiology. The potential for intrathoracic procedures (epicardial/intracardiac ECG lead arrangements, left ventricular catheterization) to influence baseline cardiovascular function, and particularly arrhythmia status is currently an important topic of considerat
