The outcome of patients with pleural effusion of indeterminate cause at thoracotomy.

In 51 patients, the cause for pleural effusion remained indeterminate immediately after thoracotomy. Thirty-one (60.8%) had no recurrence of the effusion, and no cause became apparent during a follow-up period of from 1 1/2 to 15 years. Two patients (3.9%) died relatively soon after thoracotomy, but death was not clearly related to the pleural effusion. In 18 patients (35.3%), the cause of the effusion became apparent from 12 days to 6 years after thoracotomy. In 13 of these 18 patients, malignancy (6 patients with lymphoma, 4 with malignant pleural mesothelioma, and 3 with other malignancy) w

Diaphragm plication using a subcostal approach as an alternative to a thoracotomy.

Diaphragm paralysis is a rare complication in pediatric heart surgery (0.28%-5.6%, depending on the procedure). When unilateral paralysis happens in adults, it is rarely symptomatic. However, it can cause respiratory distress syndrome when it happens in newborn or young children. The clinical diagnosis comes with a failure to wean from ventilation. The chest X-ray shows the ascension of the concerned hemidiaphragm. The goal of the surgical treatment is to pull down the paralyzed diaphragm and to increase the ventilating capacity. The classical surgical approach is a posterior-lateral thoracoto

Damage Control Thoracotomy: A Systematic Review of Techniques and Outcomes.

BACKGROUND: Damage control surgery is the practice of delaying definitive management of traumatic injuries by controlling hemorrhage in the operating room and restoring normal physiology in the intensive care unit prior to definitive therapy. Presently, damage control or "abbreviated" laparotomy is used extensively for abdominal trauma in an unstable patient. The application of a damage control approach in thoracic trauma is less established and there is a paucity of literature supporting or refuting this practice. We aimed to systematically review the current data on damage control thoracotom

Detorsion of the Pulmonary Torsion: A Rare Post-thoracotomy Complication.

Pulmonary torsion is a rare but serious condition. Without prompt treatment it progresses to lobar ischaemia, pulmonary infarction and finally fatal gangrene. We present a case of this rare complication in a 61-year-old woman following thoracic operation without involving any lung resection. Careful post-operative clinical observation, chest X-ray and CT scans are crucial for precise diagnosis of lobar torsion. The bedside radiograph provided initial evidence of torsion. Computed tomography scans revealed the presumptive diagnosis of right upper lobe torsion. On exploration, a 70° rotation of

Clamshell thoracotomy: a unique approach to a massive intrathoracic schwannoma.

Schwannomas (neurilemmomas) are benign tumors arising from the Schwann cells of the neural sheath. They are typically well-encapsulated lesions and in the chest they are most commonly seen within the posterior mediastinum, often originating along the intercostal nerves. Several operative approaches have previously been described for the resection of these tumors, including thoracoscopic techniques and posterolateral thoracotomy. We report a case of a massive schwannoma (27 cm, maximum diameter), unresectable by described approaches, which was successfully excised using a clamshell thoracotomy.

Local and systemic impacts of pleural oxygen exposure in thoracotomy.

The pleural cavity is normally in a state of negative pressure and low oxygen tension. It is exposed to the atmosphere during thoracic surgery. However, no reports of pathophysiological investigation of the effects of pleural oxygen exposure involved in thoracotomy are available. In this study, the effects of pleural oxygen exposure on systemic and pleural inflammation were investigated. Male Wistar rats (9 weeks old) were placed on mechanical ventilation and underwent thoracotomy with lipopolysaccharide (LPS) administration, which simulates latent inflammatory condition. The pleural cavity wa

The efficacy of intrathecal morphine in post-thoracotomy pain management.

This study compared the analgesic efficacy of intrathecal (IT) morphine plus IV patient-controlled analgesia (PCA) morphine with IV PCA morphine alone in 33 patients undergoing thoracotomy randomized to two groups: the IT morphine group (n=17) received 10 microg/kg morphine 1 h before the end of surgery, while the control group (n=16) did not. All patients had access to an IV PCA pump post-operatively that delivered 2 mg morphine boluses. Post-operative pain and sedation scores, respiratory and haemodynamic parameters, and morphine demand and delivery were assessed up to 48 h. Post-operative p

Use of levobupivacaine for the treatment of postoperative pain after thoracotomies.

Continuous thoracic epidural analgesia with an opiod-local anaesthetic mixture is the most appropriate strategy to control postoperative pain in thoracic surgery. Levobupivacaine, the pure S(-) enantiomer of racemic bupivacaine, has less cardiotoxic and neurotoxic potential but similar anaesthetic properties of its native agent. There are no studies in thoracic surgery that had established the minimal efficient concentration of this anaesthetic when used with an epidural opioid. The advantages of administering opioids in addition to local anaesthetics in the epidural space are the possibility

Combined thoracotomy and laminectomy for spinal cavernomas with intrathoracic growth.

Dumbbell shaped epidural cavernomas are uncommon lesions. Of these, the extraforaminal extension is usually limited, and the lesions are removed surgically by an extended laminectomy. We present two cases of dumbbell shaped epidural cavernomas with a considerable intrathoracic growth. They were resected through a combined thoracotomy and laminectomy. Only in the case in which a wide approach to the posterior mediastinum was performed it was possible to completely remove the lesion. Since it is possible to anticipate this histological diagnosis by the magnetic resonance imaging characteristics,

Transmediastinal approach to exploring the lung contralateral to the thoracotomy site.

OBJECTIVE: The approach to contralateral lung through the mediastinum is assumed useful in managing oddly distributed bilateral lung tumors. SUBJECTS AND METHODS: To remove a tumor located in the contralateral lung, a transmediastinal approach from the thoracotomy site to the contralateral lung was used in 6 patients having oddly distributed bilateral lung tumors, 1 of which was located in the contralateral lung close to the anterior or posterior mediastinum. RESULTS: All cases were treated successfully. One patient required an additional small incision on the contralateral anterior chest wa

Prolonged survival following multiple thoracotomies for metastatic hepatoblastoma.

A 14-month-old boy presented with hepatoblastoma, which was completely excised. He had pulmonary metastases, diagnosed 1 year later, treated with chemotherapy followed by resection at age 2 1/2 years. At the age of 3 1/2 years a further metastasis was shown on CT scan and this was resected. Two further metastses were resected at ages 4 1/2 and 5 1/2 years. At age 9 1/2 he had a further isolated pulmonary metastasis resected and is again disease-free more than 30 months from the last thoracotomy. Prolonged survival following metastatic hepatoblastoma can be achieved with repeated thoracotomies,

[Right thoracotomy to approach mitral or tricuspid valve in reoperation].

Four patients underwent mitral valve re-replacement, and required tricuspid annuloplasty in two, through a right thoracotomy. There was no major postoperative morbidity and recovery was full and uneventful. The indications for selection of this approach were isolated mitral valve disease or combined tricuspid and mitral valve disease, severe adhesion between heart and sternum on CT scan, intact aortic valve and no severe reduction of respiratory function. We recommend right thoracotomy to approach atrioventricular valves in selected situations in patients for reoperations.

Left thoracotomy approach for resection of carcinoma of the oesophagus and cardia.

Oesophagogastrectomy is the best available treatment for patients with carcinoma of the oesophagus or cardia. It provides better longevity than other types of therapy and remains the standard against which combined modality treatment should be compared. Our experience with two hundred and twenty-four patients who underwent surgical exploration performed transdiaphragmatically by way of a left thoracotomy formed the basis of this report. Of these 224 patients, 201 (89.7%) underwent resection, 15 were bypassed and the remaining 5 patients were intubated. Of those resected, 79 patients required a

[Place of thoracotomy in thoracic complications of hydatid cysts of the liver].

The authors present 16 cases of hydatid cyst of the liver with rupture into the thorax, operated by thoracotomy. In 15 cases, the cyst ruptured into the right hemithorax and in one case, it ruptured into the left hemithorax. We adopted the classification proposed by our master S. Mestiri, which completes that proposed by Dévé. The diagnosis is essentially based on thoraco-abdominal ultrasonography, which visualises the hydatid cyst, reveals intrathoracic collections, demonstrates the diaphragmatic tear and studies the biliary tract looking for obstruction or dilatation. The diagnosis was estab

Intraoperative assessment of nodal staging at thoracotomy for carcinoma of the bronchus.

In the staging of operable carcinoma of the bronchus, accurate assessment of mediastinal nodal status is essential in order to determine the feasibility and desirability of resection. In the UK, frozen section analysis is often logistically difficult, and many surgeons rely upon naked-eye assessment. To evaluate the accuracy of this approach, we have reviewed 100 thoracotomies performed for cancer in this unit and compared the naked-eye assessment of nodal staging with the ultimate histological findings. A total of 287 lymph nodes were examined. There were 14 false positives and 10 false negat

The results of esophagogastrectomy without thoracotomy for adenocarcinoma of the esophagogastric junction.

Between 1980 and 1988, 98 patients with adenocarcinoma of the esophagogastric junction were seen at the University of Western Ontario. Eighty-two patients underwent resection of the celiac lymph nodes, lesser curve and cardia of the stomach, and thoracic esophagus through abdominal and neck incisions avoiding thoracotomy. The esophagus was replaced by a stomach tube in 80 patients or by a colon tube in two patients. Two of 82 patients died while hospitalized. Early postoperative morbidity included anastomotic leaks that closed spontaneously (13), transient hoarseness (10), myocardial infarctio
