Confidence intervals and point estimates for treatment effects in adaptive enrichment designs.

Adaptive enrichment designs allow subgroup selection of the patient population within a confirmatory trial via an interim analysis. However, this design complicates treatment effect estimation and uncertainty quantification. This paper introduces a p-value inversion method using various sample space orderings to construct confidence intervals either unconditionally or conditional on the subgroup selected for a general class of two-stage two-group designs. In addition, the p-value functions can be used to derive median-unbiased estimators and conditional moment estimators. Through simulation it

Sex differences in sleep and alcohol consumption outcomes following a digital insomnia intervention.

BACKGROUND: Poor sleep is a well-established risk factor for heavy drinking, and evidence suggests that sleep could serve as a potential treatment target for reducing alcohol consumption. The relationship between poor sleep and problematic drinking appears to be stronger among females, but no studies to date have assessed sex differences in alcohol consumption following insomnia treatment. Here, we combine the samples from two clinical trials to investigate sex differences in the effects of a digital cognitive behavioral therapy for insomnia (Sleep Healthy Using the Internet; SHUTi) on sleep a

Tranexamic acid after aneurysmal subarachnoid hemorrhage is associated with worse outcome in males: post hoc analysis of the ULTRA trial.

BACKGROUND: The ULTRA trial demonstrated no improvement in clinical outcome at 6 months in patients with subarachnoid hemorrhage (SAH) when treated with ultra-early and short-term tranexamic acid (TXA). In this post hoc analysis, we evaluated the effect of TXA in females and males on clinical outcome after aneurysmal SAH (aSAH). METHODS: The ULTRA trial was a prospective, randomized, controlled, open-label trial, conducted between 2013 and 2020. Patients with spontaneous SAH were assigned to ultra-early, short-term TXA in addition to usual care, or usual care only. In this study, we only incl

Comparative response to pharmacological management of asthma in overweight/obese children versus their counterparts: a systematic review and call for data.

BACKGROUND: Obese children with asthma experience higher morbidity than their counterparts; whether different treatments should be considered remains to be clarified. OBJECTIVE: To explore the potential group differences in the treatment response to any asthma medications between overweight/obese and normal/underweight children. METHODS: We systematically reviewed case-control, cohort studies and randomised controlled trials (RCTs) examining treatment effects by weight status, identified in four bibliographic databases from inception until January 2025. A narrative synthesis was presented be

Overall and Sex-Specific Predictors of Secukinumab Effectiveness in European Patients With Psoriatic Arthritis: Results From the EuroSpA Collaboration.

OBJECTIVE: Identifying patient characteristics that predict treatment response in psoriatic arthritis (PsA) may help optimize treat-to-target strategies. We aimed to explore overall and sex-specific predictors of secukinumab (SEC) effectiveness in European patients with PsA treated in routine care. METHODS: We analyzed data from 14 registries in the European Spondyloarthritis (EuroSpA) Research Collaboration. Patients aged ≥ 18 years at diagnosis, initiating SEC treatment from January 2015 to January 2021, were included. Multiple imputation was used for missing covariates at treatment start (

Irritable bowel syndrome subtype predicts treatment response and defines distinct mechanistic phenotypes in OAB-IBS comorbidity: a prospective observational study.

PURPOSE: To investigate the heterogeneity of treatment response in patients with concomitant Overactive Bladder (OAB) syndrome and Irritable Bowel Syndrome (IBS), including diarrhea-predominant (IBS-D), constipation-predominant (IBS-C), and mixed-type (IBS-M), by stratifying outcomes by both treatment strategy and IBS subtype. We hypothesized that IBS subtype would predict therapeutic response and define distinct mechanistic phenotypes. METHODS: In this prospective, observational cohort study (N = 144, screened from 259), patients were allocated to one of four non-randomized cohorts: OAB-Targ

Prediction of Treatment Benefit With Internet-Based Cognitive Behavioral Therapy for Depression.

IMPORTANCE: Treatment benefit prediction models may help identify which patients will benefit most from internet-based cognitive behavioral therapy (iCBT) for depression; it is unclear whether such models can support useful treatment allocation across programs. OBJECTIVE: To develop a model predicting individual treatment benefit from iCBT for depression across programs and to externally validate it in independent samples. DESIGN, SETTING, AND PARTICIPANTS: This prognostic study used individual participant data from 5 randomized clinical trials of 3 iCBT programs conducted between 2012 and 2

Faster and greater antidepressant response to intravenous ketamine in bipolar compared with unipolar treatment-resistant depression: Diagnostic and sex-related findings from a naturalistic study.

BACKGROUND: Intravenous ketamine has emerged as a rapid-acting antidepressant intervention that has shown promising effects in patients with treatment-resistant depression, although evidence remains heterogeneous and less well established across diagnostic subtypes. Nevertheless, real-world data comparing diagnostic subtypes and sex-related differences remain limited. This observational study examined the antidepressant efficacy, tolerability, and dissociative effects of intravenous ketamine in patients with treatment-resistant unipolar depression (TRD) and treatment-resistant bipolar depressi

Estimating Optimal Site-Specific Individualized Treatment Rules With Limited Data Sharing.

Estimation of optimal individualized treatment rules (ITRs) tailored to patient characteristics is a crucial component of precision medicine. When such problems arise in multicenter randomized trials with known treatment assignment probabilities, the optimal ITR may vary across sites because of site-level heterogeneity, and directly pooling data across centers can lead to biased ITR estimation. Meanwhile, privacy constraints often prevent investigators from accessing individual-level data from all centers. We therefore propose Heterogeneous Distributed Learning (HD-learning) to estimate optima

Brain stimulation and VEGF concentrations: meta-analysis, meta-regression, and hierarchical cluster analysis of 200 correlations.

BACKGROUND: Studies on VEGF responses to brain stimulation (BS) are inconsistent. METHODS: We systematically reviewed and meta-analyzed these data. RESULTS: Overall meta-analyses for the three main models (time, group, time×group) showed no significant effects (p>0.05). Given the high heterogeneity across interventions and populations, we conducted extensive exploratory subgroup analyses to generate hypotheses. These revealed several within-subgroup effects, but they were based on limited data constraining their interpretability, with several single studies and others on 2-4 observations. Fo

Heterogeneity of Response to Early Start Denver Model: Identifying Developmental Trajectories and Predictors of Cognitive Outcomes.

Autism is a highly heterogenous neurodevelopmental condition that can significantly compromise the quality of life. For children showing atypical trajectories of development in their early years, early and intensive intervention has shown promise in enhancing the development of language, cognition, and social skills. Yet, marked variability in intervention response remains insufficiently understood. Characterizing this heterogeneity is essential for informing intervention strategies tailored to each child's developmental profile. The aim of the present work was to identify subgroups based on i

Development of a baseline battery to optimize depression treatment assignment in primary care: Balancing breadth and brevity.

BACKGROUND: Psychotherapy and antidepressant medications are effective treatments for depression on average, yet individual response varies widely. Predicting which treatment is most effective for a given patient requires a comprehensive yet feasible assessment of baseline characteristics. This paper describes the development of a baseline battery to guide treatment allocation between behavioral activation and fluoxetine for adults with depression in primary care in India. METHODS: We used a six-step, multimethod approach to develop the battery. We updated a prior review to identify construct

Identification of vital sign trajectory phenotypes and treatment response heterogeneity in critically ill patients with ischemic stroke: A multicenter study with external validation.

Critically ill patients with ischemic stroke exhibit heterogeneous hemodynamic patterns, yet previous trajectory-based studies have focused on single vital sign parameters. We conducted a multicenter retrospective cohort study to identify vital sign trajectory phenotypes, evaluate their prognostic value compared with traditional severity scores, and explore treatment response heterogeneity. Using group-based multi-trajectory modeling of six vital signs (systolic blood pressure, diastolic blood pressure, mean arterial pressure, heart rate, respiratory rate, and oxygen saturation) during the fir

Sex-based effectiveness in buprenorphine/naloxone and methadone treatment for opioid use disorder: Findings from the OPTIMA trial.

INTRODUCTION: Opioid use disorder (OUD) is a public health challenge globally. Few studies examine sex differences in opioid agonist therapy outcomes, especially in contexts of high potency opioid consumption. Clarifying these could inform personalized treatment strategies. METHODS: We conducted exploratory sex-stratified analyses using data from a 24-week, multicentric, pragmatic, open-label randomized controlled trial. Participants with OUD (n male=179, n female=93) were randomized to flexible take-home dosing of buprenorphine/naloxone (n male=95, n female=43) or standard methadone (n male=

Dynamic gene expression signatures predict psychotherapy response in PTSD.

Static blood-based biomarkers provide limited insight into psychiatric illness and recovery, largely because they capture only a single, unperturbed snapshot of inherently dynamic biological systems. In this study, we applied a dynamic, longitudinal dual-state approach to characterize peripheral blood mononuclear cell (PBMC) transcriptional trajectories associated with psychotherapy outcomes in post-traumatic stress disorder (PTSD). PBMC transcriptional profiles were generated for veterans with PTSD (n = 46, 21, and 17, respectively) and controls (n = 26, 26, and 23) at pre-treatment, post-tre

Effectiveness and premature termination of inpatient dialectical behavior therapy for men and women with borderline personality disorder: A comparative study.

Fewer men than women with borderline personality disorder (BPD) may be found in clinical and research settings, and so the majority of dialectical behavior therapy (DBT) treatment studies have focused on women. Because of this, DBT has not been sufficiently studied for treating BPD in men. The aim of this study was to examine the influence of gender on symptom trajectory differences and on the frequency and reasons for premature termination of treatment during a 3-month certified inpatient DBT program. The treatment-seeking sample included n = 1,991 women and n = 295 men with BPD who received

Predictors of Response to Neoadjuvant Therapy in Human Epidermal Growth Factor Receptor 2 (HER2)-Positive Breast Cancer and Pathological Evaluation of HER2 Heterogeneity.

Human epidermal growth factor receptor 2 (HER2) gene amplification and heterogeneity influence anti-HER2 therapy efficacy, but quantitative assessment of HER2 expression heterogeneity remains scarce. This study evaluates HER2 protein heterogeneity and its effect on neoadjuvant therapy (NAT) outcomes in HER2-positive breast cancer. In total, 295 patients with HER2-positive breast cancer treated with neoadjuvant anti-HER2 therapy were included. HER2 expression was quantified by the percentage of HER2-stained cells across intensities, and heterogeneity was assessed using quadratic entropy. The ov

Treatment effect modifiers of virtual reality-based versus standard cognitive behavioral therapy for paranoia in schizophrenia spectrum disorders: an exploratory moderator analysis of clinical and demographic characteristics in the FaceYourFears trial.

BACKGROUND: This exploratory study examined baseline characteristics modifying treatment effects on paranoia in individuals diagnosed with schizophrenia spectrum disorders following a 10-session virtual reality-based cognitive behavioral therapy for psychosis (VR-CBTp) or standard CBTp. METHODS: All participants in the FaceYourFears trial were included (n=254; CBTp n=128; VR-CBTp n=126). General linear and logistic regression models examined baseline variables associated with end-of-treatment paranoia. In covariate analyses, regression coefficients quantified associations across treatments. I

Identifying treatment-responsive patient subgroups in a neutral clinical trial of Intensive blood pressure reduction in acute intracerebral hemorrhage: A post hoc explainable machine learning analysis.

The Antihypertensive Treatment of Acute Cerebral Hemorrhage (ATACH-2) trial reported no overall benefit from intensive blood pressure (BP) reduction in intracerebral hemorrhage (ICH), potentially masking benefit in select patient subgroups. We evaluated whether explainable machine learning could identify treatment-arm subgroups in whom BP reduction is associated with improved outcomes. Using the ATACH-2 dataset, we trained an XGBoost model on two-thirds of the control arm (n = 326) to predict 3-month poor outcome (modified Rankin Scale >3). The model was then applied to treatment-arm patients
