Antimicrobial Resistance Profile of Enterococcal Isolates From Clinical Specimens at a Tertiary Care Hospital in Western Maharashtra, India.

Background Enterococcus, once benign intestinal flora, has transformed into formidable nosocomial pathogens as a result of the accelerated emergence of antibiotic resistance represents a major global health challenge, particularly within hospital settings. Enterococcus has grown more prevalent in nosocomial infections, such as urinary tract infections (UTIs), surgical site infections (SSIs) and bacteremia. The potential emergence of vancomycin-resistant Enterococcus (VRE) strains further complicates treatment choices for multi-drug resistant (MDR) infections. This study evaluated the magnitude

Antibiotic resistance profiles of seven genomospecies of Corynebacterium jeikeium analyzed by whole genome sequencing.

Corynebacterium jeikeium is an opportunistic human pathogen with high genomic diversity. In the past, multi-drug resistance has been considered a hallmark of C. jeikeium. However, some strains of C. jeikeium are fully susceptible and thus, their reclassification would avoid the empirical therapy with glycopeptides. One hundred eighty-six clinical isolates and four reference isolates identified as C. jeikeium between 1994-1999 and 2012-2019, respectively, were analyzed by matrix-assisted laser desorption/ionization time of flight mass spectrometry (MALDI-TOF MS) and 16S ribosomal RNA (16S rRNA)

Changing dynamics of bloodstream infections due to methicillin-resistant Staphylococcus aureus and vancomycin-resistant Enterococcus faecium in Germany, 2017-2023: a continued burden of disease approach.

BACKGROUND: Antimicrobial resistance is a global threat to public health, with methicillin-resistant Staphylococcus aureus (MRSA) and vancomycin-resistant Enterococcus faecium (VREfm) being major contributors. Despite their clinical impact, comprehensive assessments of changes of the burden of bloodstream infections in terms of Disability-Adjusted Life Years (DALYs) and attributable deaths over time are lacking, particularly in Germany. METHODS: We used data from the Antimicrobial Resistance Surveillance system, which covered about 30% of German hospitals. Bloodstream infections were defined

Frequency, characteristics and impact of multiple consecutive nosocomial infections in patients with decompensated liver cirrhosis and ascites.

BACKGROUND: Nosocomial infections are a particular threat for patients with liver cirrhosis. It is not uncommon that individuals develop even several consecutive infections during a single hospital stay. We aimed to investigate the impact and characteristics of multiple, consecutive nosocomial infections. METHODS: A total of 514 consecutive patients with liver cirrhosis and ascites were included and followed up for 28 days for nosocomial infection, death or liver transplantation (LTx). Laboratory values were assessed at the time of hospitalization as well as at the onset of each new infectiou

Implementation and impact of an audit and feedback antimicrobial stewardship intervention in the orthopaedics department of a tertiary-care hospital: a controlled interrupted time series study.

A prospective audit and feedback antimicrobial stewardship intervention conducted in the Orthopaedics Department of a university hospital in Portugal was evaluated by comparing an interrupted time series in the intervention group with a non-intervention (control) group. Monthly antibiotic use (except cefazolin) was measured as the World Health Organization's Anatomical Therapeutic Chemical defined daily doses (ATC-DDD) from January 2012 to September 2016, excluding the 6-month phase of intervention implementation starting on 1 January 2015. Compared with the control group, the intervention gro

An outbreak of vanA vancomycin-resistant Enterococcus faecium in a hospital with endemic vanB VRE.

BACKGROUND: In Australia, vanB vancomycin-resistant Enterococcus faecium (VREfm) has been endemic for over 20 years, but vanA VREfm isolates have rarely been reported. METHODS: This outbreak report describes an outbreak of vanA VREfm in the intensive care unit (ICU) and cardiothoracic surgery (CTS) wards of a Melbourne hospital in 2015-2016. After the cluster was initially identified in the ICU ward, an active screening programme was implemented. VRE isolates were typed using in silico multi-locus sequence typing. In addition, to screening, enhanced environmental cleaning, chlorhexidine gluco

Incidence and prevalence of antimicrobial resistance in outpatients with cancer: a multicentre, retrospective, cohort study.

BACKGROUND: Infections are the second leading cause of death in patients with cancer and are often caused by resistant bacteria. However, the frequency of antimicrobial resistance (AMR) in outpatients with cancer is not well understood. We aimed to compare the frequency of AMR bacterial pathogens in outpatients with and without cancer. METHODS: This retrospective cohort study evaluated antimicrobial susceptibility of bacteria isolated from adults (aged ≥18 years) with and without cancer seeking care in 198 outpatient health-care settings in the USA. Data were collected using the BD Insights R

Enhanced terminal room disinfection and acquisition and infection caused by multidrug-resistant organisms and Clostridium difficile (the Benefits of Enhanced Terminal Room Disinfection study): a cluster-randomised, multicentre, crossover study.

BACKGROUND: Patients admitted to hospital can acquire multidrug-resistant organisms and Clostridium difficile from inadequately disinfected environmental surfaces. We determined the effect of three enhanced strategies for terminal room disinfection (disinfection of a room between occupying patients) on acquisition and infection due to meticillin-resistant Staphylococcus aureus, vancomycin-resistant enterococci, C difficile, and multidrug-resistant Acinetobacter. METHODS: We did a pragmatic, cluster-randomised, crossover trial at nine hospitals in the southeastern USA. Rooms from which a patie

Rapid gastrointestinal loss of Clostridial Clusters IV and XIVa in the ICU associates with an expansion of gut pathogens.

Commensal gastrointestinal bacteria resist the expansion of pathogens and are lost during critical illness, facilitating pathogen colonization and infection. We performed a prospective, ICU-based study to determine risk factors for loss of gut colonization resistance during the initial period of critical illness. Rectal swabs were taken from adult ICU patients within 4 hours of admission and 72 hours later, and analyzed using 16S rRNA gene sequencing and selective culture for vancomycin-resistant Enterococcus (VRE). Microbiome data was visualized using principal coordinate analyses (PCoA) and

Optimized Surveillance Testing for Toxigenic Clostridioides difficile to Scale Prevention Programs Across Clinical Settings.

Clostridioides difficile is the leading cause of healthcare acquired infections (HAIs). Healthcare systems lack scalable strategies to identify patients colonized with toxigenic strains to inform prevention programs. We developed a scalable program using rectal swabs, collected for vancomycin resistance Enterococci surveillance, with ex vivo amplification of C. difficile in Cdiff Banana Broth (BB). Optimized confirmatory testing of BB-positive tubes used the rapid C. difficile QuickCheck assay with Cepheid's Xpert® C. difficile/Epi (Xpert) to confirm toxin gene carriage for QuickCheck results

Virulome and genome-wide association study of vancomycin-resistant Enterococcus faecium from bloodstream infections versus colonization isolates.

INTRODUCTION: Vancomycin-resistant Enterococcus faecium (VREfm) are multidrug-resistant pathogens that cause severe nosocomial infections. The knowledge of virulence profiles of VREfm for the development of invasive infections is still limited. The aim of this study was to analyse gene profiles and genetic variants of VREfm and their association with the development of VREfm bloodstream infections (VRE-BSI). METHODS: One-hundred twenty randomly selected VRE-BSI isolates from a state-wide surveillance study in the German state of North Rhine-Westphalia were included in the analysis and matched

Novel 1,3,5-Triazinyl Aminobenzenesulfonamides Incorporating Aminoalcohol, Aminochalcone and Aminostilbene Structural Motifs as Potent Anti-VRE Agents, and Carbonic Anhydrases I, II, VII, IX, and XII Inhibitors.

A series of 1,3,5-triazinyl aminobenzenesulfonamides substituted by aminoalcohol, aminostilbene, and aminochalcone structural motifs was synthesized as potential human carbonic anhydrase (hCA) inhibitors. The compounds were evaluated on their inhibition of tumor-associated hCA IX and hCA XII, hCA VII isoenzyme present in the brain, and physiologically important hCA I and hCA II. While the test compounds had only a negligible effect on physiologically important isoenzymes, many of the studied compounds significantly affected the hCA IX isoenzyme. Several compounds showed activity against hCA XI

Evaluation of Microbial Efficacy of Silver-Containing Wound Dressings: Standardized Approach for Addressing Emerging Microbial Threats.

INTRODUCTION: Effective wound management is crucial, especially during outbreaks of new microorganisms or antibiotic-resistant strains. Developing new wound dressings can be impractical due to time and cost constraints, and redesigning existing products poses significant challenges. Therefore, evaluating the effectiveness of current wound dressings is essential. This study aims to assess the antibacterial properties of existing wound dressings without altering their design. Its objective is to present a systematic, design-agnostic approach for quantifying antimicrobial activity against a range

Inhibition effect of flavophospholipol on conjugative transfer of the extended-spectrum β-lactamase and vanA genes.

Flavophospholipol (FPL) is an antimicrobial feed additive that has been approved for use in livestock animals and has the potential to decrease horizontal dissemination of antimicrobial resistance genes. Since previous studies showed that FPL has an inhibitory effect on plasmid transfer, in vitro experiments have proven the efficacy of FPL in reducing the conjugative transfer of plasmids encoding the extended-spectrum β-lactamase (ESBL) and vanA genes. These are among the most important antimicrobial resistance loci known. ESBL-producing Escherichia coli and vancomycin-resistant Enterococcus f

Fecal colonization and infection with Pseudomonas aeruginosa in recipients of allogeneic hematopoietic stem cell transplantation.

BACKGROUND: Pseudomonas aeruginosa, especially multidrug-resistant (MDR) isolates, is an important pathogen in allogeneic hematopoietic stem cell transplant (HCT) recipients. The ability to identify patients at risk for these infections and administer appropriate empiric therapy, particularly during episodes of neutropenia, may improve outcomes and also direct infection control and antimicrobial stewardship efforts. Many transplant centers obtain routine surveillance stool cultures (SSCs) from HCT recipients to test for colonization with vancomycin-resistant enterococci, and extended-spectrum

Evaluation of the risks of shedding Salmonellae and other potential pathogens by therapy dogs fed raw diets in Ontario and Alberta.

Dogs that participate in animal-assisted interventions (AAIs), often called 'therapy dogs', commonly interact with humans whose immune systems are not functioning optimally. The advisability of feeding raw meat (including poultry) to these animals remains a highly contentious issue, in spite of increasing evidence that raw meat is frequently contaminated with Salmonella. We set out to determine if consuming raw meat influences the risk of therapy dogs shedding Salmonella and other pathogens. Two hundred healthy therapy dogs from Ontario and Alberta were enrolled. Between May 2005 and November
