Changes in Blood Profile from Steady State in Patients with Sickle Cell Anemia Admitted for Vaso-occlusive Crisis and Acute Chest Syndrome.

Close to half of all patients with sickle cell disease (SCD) will have at least one episode of acute chest syndrome (ACS) during their lifetime. Multiple cells and molecules involved with the inflammatory cascade play a role in the development of ACS. We found that patients with SCD who developed ACS as a complication of a vaso-occlusive crisis (VOC) had a significant increase in leukocytes and decrease in platelets from their steady state when compared with a separate admission for VOC without ACS development. No significant change from steady state hemoglobin or reticulocyte count was noted

Patients with sickle cell disease and asthma have a higher risk for acute chest syndrome: A systematic review and meta-analysis of observational studies.

Sickle cell disease (SCD) is a hereditary hematologic disorder characterized by abnormal hemoglobin polymerization, leading to vaso-occlusion, hemolysis and multi-organ complications. Acute chest syndrome (ACS) is a severe pulmonary complication and a leading cause of morbidity and mortality in patients with SCD. Asthma, a prevalent comorbidity in SCD, has been implicated in worsening disease outcomes, including increased ACS risk. This systematic review and meta-analysis aimed to evaluate the association between asthma and ACS in patients with SCD by synthesizing data from observational studi

Unique Changes in the Incidence of Acute Chest Syndrome in Children With Sickle Cell Disease Unravel the Role of Respiratory Pathogens: A Time Series Analysis.

BACKGROUND: Acute chest syndrome (ACS) is a life-threatening complication of sickle cell disease (SCD). Although respiratory pathogens are frequently detected in children with ACS, their respective role in triggering the disease is still unclear. We hypothesized that the incidence of ACS followed the unprecedented population-level changes in respiratory pathogen dynamics after COVID-19-related nonpharmaceutical interventions (NPIs). RESEARCH QUESTION: What is the respective role of respiratory pathogens in ACS epidemiology? STUDY DESIGN AND METHODS: This study was an interrupted time series

Impact of VEGFA -583C > T polymorphism on serum VEGF levels and the susceptibility to acute chest syndrome in pediatric patients with sickle cell disease.

We investigated the association of VEGFA -583C > T on VEGF serum levels and acute chest syndrome (ACS) in 351 pediatric patients with sickle cell disease (SCD), of whom 90 had ACS, and 261 were ACS-free controls. Significant differences in -583C > T minor allele and genotype frequencies were seen between ACS cases and controls, evidenced by enrichment of -583T/T genotypes in patients with ACS, which were linked with reduction in VEGF serum levels. VEGFA -583C > T and reduced VEGF serum levels may influence ACS risk in patients with SCD, which will aid in identifying patients with SCD who are a

Safety of purified poloxamer 188 in sickle cell disease: phase I study of a non-ionic surfactant in the management of acute chest syndrome.

Acute chest syndrome (ACS) is the most common cause of death in patients with sickle cell anemia. Its management is primarily palliative. We performed a Phase I evaluation of purified poloxamer 188 (a non-ionic surfactant) in the management of ACS. Forty-three patients with sickle cell disease and ACS were treated with doses as high as 2960 mg/day by continuous intravenous (IV) infusion. The maximum tolerated dose has not been identified. No evidence of renal toxicity or other limiting adverse events were found. One adult patient died due to sepsis and adult respiratory distress syndrome, whic

Integrated Use of Conventional Chest Radiography Cannot Rule Out Acute Aortic Syndromes in Emergency Department Patients at Low Clinical Probability.

OBJECTIVES: Guidelines recommend chest radiography (CR) in the workup of suspected acute aortic syndromes (AASs) if the pretest clinical probability is low. However, the diagnostic impact of CR integration for the rule-in and rule-out of AASs is unknown. METHODS: We performed a secondary analysis of the ADvISED multicenter study. Emergency department outpatients were eligible if an AAS was clinically suspected. Clinical probability was defined with the aortic dissection detection risk score (ADD-RS). CR was evaluated blindly by a radiologist, who judged on mediastinum enlargement (ME) and oth

Improved identification of acute coronary syndromes with delta cardiac serum marker measurements during the emergency department evaluation of chest pain patients.

Current findings from the American College of Emergency Physicians (ACEP) are that no serum marker reliably identifies or excludes acute myocardial infarction (AMI) within 6 h of symptom onset. The ACEP recommends repeat serum marker testing 6-10 h after symptom onset for CK-MB mass and subform, and 8-12 h after symptom onset for cardiac troponin I and T before making an exclusionary diagnosis of non-AMI chest pain. A new approach for identifying myocardial necrosis is to rely on time changes in the serum marker value over an abbreviated time interval (slope or delta values) as opposed to the

The Great Cardiac Masquerade: Distinguishing Acute Coronary Syndrome From Myocarditis and Spontaneous Coronary Artery Dissection in a Middle-Aged Woman With Chest Pain.

Spontaneous coronary artery dissection (SCAD) is a rare, nonatherosclerotic cause of acute coronary syndrome (ACS), resulting from the formation of a false lumen and propagation of an intramural hematoma within the coronary artery wall. This process can lead to luminal narrowing, myocardial ischemia, and anginal symptoms. Because SCAD often presents with clinical and electrocardiographic features similar to obstructive ACS, as well as other close mimics such as myocarditis, misdiagnosis is common. Given the differences in management strategies, this can lead to suboptimal or even harmful outco

CHA2 DS2 -VASc score and clinical outcomes of patients with chest pain discharged from internal medicine wards following acute coronary syndrome rule-out.

BACKGROUND: Chest-pain patients deemed safe for discharge from internal medicine wards might still be at risk for adverse outcomes. HYPOTHESIS: CHA2 DS2 -VASc score improves risk stratification of low-risk chest-pain patients discharged after acute coronary syndrome (ACS) rule-out. METHODS: We accessed medical records of patients who were admitted to internal medicine wards at a single medical center during 2010-2016 and discharged following an ACS rule-out. Patients were classified according to CHA2 DS2 -VASc score: 0-1 (low), 2-3 (intermediate), >3 (high). Primary endpoint was occurrence o

High-risk plaque detected on coronary CT angiography predicts acute coronary syndromes independent of significant stenosis in acute chest pain: results from the ROMICAT-II trial.

BACKGROUND: It is not known whether high-risk plaque, as detected by coronary computed tomography angiography (CTA), permits improved early diagnosis of acute coronary syndromes (ACS) independently to the presence of significant coronary artery disease (CAD) in patients with acute chest pain. OBJECTIVES: The primary aim of this study was to determine whether high-risk plaque features, as detected by CTA in the emergency department (ED), may improve diagnostic certainty of ACS independently and incrementally to the presence of significant CAD and clinical risk assessment in patients with acute

Chest pain in the emergency room: value of the new HEAR score identifying patients with low risk of acute coronary syndrome without troponin testing.

BACKGROUND: Chest pain is a frequent reason for Emergency Department (ED) visits, although only a minority of patients ultimately present an Acute Coronary Syndrome (ACS). Current diagnostic strategies rely heavily on troponin testing, but a new clinical tool requiring no blood sampling, the HEAR score, has recently been proposed. This study aimed to evaluate the performance of the HEAR score for identifying low-risk patients presenting with chest pain, using 45-day Major Adverse Cardiovascular Events (MACE) as the primary outcome. METHODS: All patients presenting with chest pain to the ED of

Changes in circular RNA expression in acute chest syndrome and vaso-occlusive crisis in sickle cell disease: analysis of a public RNA-seq cohort.

BACKGROUND: Sickle cell disease (SCD) is a monogenic hemoglobinopathy characterized by recurrent vaso-occlusive crises (VOCs) that affect any organ and progress to multi-organ failure. Acute chest syndrome (ACS) is a major complication and leading cause of death. This study explored whole-blood circRNA signatures as candidate molecular markers for VOC and ACS and examined their functional relevance through miRNA mapping and pathway enrichment. RESEARCH DESIGN AND METHODS: A publicly available total RNA-seq dataset (GSE139912) was analyzed for circRNA expression at baseline (n = 12), VOC (n =

Pulmonary ultrasound and pulse oximetry versus chest radiography and arterial blood gas analysis for the diagnosis of acute respiratory distress syndrome: a pilot study.

INTRODUCTION: In low-resource settings it is not always possible to acquire the information required to diagnose acute respiratory distress syndrome (ARDS). Ultrasound and pulse oximetry, however, may be available in these settings. This study was designed to test whether pulmonary ultrasound and pulse oximetry could be used in place of traditional radiographic and oxygenation evaluation for ARDS. METHODS: This study was a prospective, single-center study in the ICU of Harborview Medical Center, a referral hospital in Seattle, Washington, USA. Bedside pulmonary ultrasound was performed on ICU

Accuracy of the chest radiograph to identify bilateral pulmonary infiltrates consistent with the diagnosis of acute respiratory distress syndrome using computed tomography as reference standard.

PURPOSE: The purpose of the study is to evaluate the diagnostic accuracy of the anteroposterior chest radiograph to detect pulmonary abnormalities consistent with acute respiratory distress syndrome (ARDS). MATERIALS AND METHODS: Ninety patients who met criteria for ARDS regardless of the radiographic one and had near simultaneous chest radiograph and computed tomography (CT) performed were identified. These radiologic studies were reviewed blindly and independently by 2 radiologists for the presence or absence of bilateral pulmonary abnormalities consistent with ARDS using defined radiologic

Fatal overdose due to prescription fentanyl patches in a patient with sickle cell/beta-thalassemia and acute chest syndrome: A case report and review of the literature.

Introduced into clinical practice in the 1960s, the analgesic fentanyl is 100 times more potent than morphine. Various methods of administration exist including the transdermal Duragesic patch system, widely used in chronic pain and palliative care settings. Numerous, often imaginative methods of abuse of fentanyl patches have been reported; the majority of fatal fentanyl overdose cases resulting from deliberate abuse or suicide. We describe the accidental overdose of a young black male with sickle cell/beta-thalassemia who had been using the Duragesic system for almost 2 years. At autopsy the

Dose-Response Effects of Anterior Chest Wall Loading on Lung Mechanics, Regional Ventilation, and Hemodynamics in Acute Respiratory Distress Syndrome: A Prospective Physiologic Study.

BACKGROUND: In acute respiratory distress syndrome (ARDS), global airway pressures do not distinguish lung from chest wall mechanics, limiting bedside identification of regional overdistension. Anterior chest wall loading (ACWL) has been proposed to transiently increase nondependent pleural pressure and unmask regional overdistension, but its dose-dependent effects remain uncertain. The authors hypothesized that low loads reduce transpulmonary driving pressure (ΔP) whereas higher loads promote derecruitment and hemodynamic compromise. METHODS: In this prospective, exploratory study, 30 patien
