Clinical implications of a damaged cochlea: pure tone thresholds vs information-carrying capacity.

The pure tone audiogram is an accurate measure of auditory threshold as a function of stimulus frequency. However, it does not provide the complete picture in patients with sensorineural hearing loss (SNHL) because it is not a direct measure of damage to the cochlear epithelium or of the associated limits on information-carrying capacity that restrict word recognition. Diagnostic use of the audiogram leads to the error of viewing SNHL as "dB of hearing loss," which may seem reversible by gain. Cochlear disorders, on the other hand, often give rise to abnormal thresholds because regions are dam

[A case of transient auditory agnosia and schizophrenia].

We report a case of transient functional auditory agnosia and schizophrenia and discuss their relationship. A 30-year-old woman with schizophrenia reporting bilateral hearing loss was found in history taking to be able to hear but could neither understand speech nor discriminate among environmental sounds. Audiometry clarified normal but low speech discrimination. Otoacoustic emission and auditory brainstem response were normal. Magnetic resonance imaging (MRI) elsewhere evidenced no abnormal findings. We assumed that taking care of her grandparents who had been discharged from the hospital ha

Remote versus in-clinic cochlear implant programming during rehabilitation: clinical outcomes and patient perspectives.

PURPOSE: Increasing cochlear implant (CI) demand due to broader candidacy and an ageing population challenges in-clinic rehabilitation. Remote programming offers a promising alternative to improve accessibility. This study evaluates its non-inferiority and optimal timing during rehabilitation. MATERIALS AND METHODS: Forty-three postlingually deafened adults were divided into three groups: standard in-clinic fitting (n = 21), remote fitting during the second session (n = 12), and remote fitting during the third session (n = 10). Auditory outcomes at the fourth session were assessed using speec

Tinnitus and its management in patients with vestibular schwannoma.

Tinnitus is the second most common presenting symptom of vestibular schwannoma (VS) after hearing loss. There is conflicting evidence regarding the impact of tinnitus and its contribution to the overall quality of life in VS, and there are many theories regarding the pathogenesis of tinnitus in VS, including cochlear, neural, and central mechanisms. Assessment should include speech audiometry in addition to pure-tone audiometry, as VS patients often have worse speech discrimination than would be expected from their pure-tone thresholds. There is no objective measure of tinnitus. Questionnaires

Hearing loss after "refractory" secretory otitis media.

CONCLUSIONS: The results indicate that SOM of extreme duration, and maybe also the treatment of SOM, are risk factors for developing permanent hearing loss, both conductive and sensorineural. OBJECTIVE: Fluctuating or persisting hearing loss of varying degrees is known to accompany secretory otitis media (SOM). The aim of this study was to detect possible hearing sequelae in young adults who had suffered from "refractory" SOM during childhood. MATERIAL AND MEDTHODS: A total of 33 subjects (age 16-25 years) with previous SOM that had persisted for a mean of 11 years (range 6-19 years) were ret

Surgical Impact of Coupling an Active Middle Ear Implant to Short Incus Process.

INTRODUCTION: Since 1996 the active middle-ear implant Vibrant Soundbridge (VSB) is used to treat mild-to-severe sensorineural hearing losses. The former standard surgical approach for incus vibroplasty included a mastoidectomy and a posterior tympanotomy, crimping the Floating-Mass Transducer (FMT) to the long incus process. The introduction of the short process (SP) coupler allows the attachment of the FMT to the incus body making the procedure of a posterior tympanotomy unnecessary. The aim of this study was to evaluate the duration of surgery (surgical case length) and to compare the audio
