Repurposing pharmaceutical excipients as an antiviral agent against SARS-CoV-2.

The limited time indorsed to face the COVID-19 emergency and large number of deaths across the globe, poses an unrelenting challenge to find apt therapeutic approaches. However, lead candidate selection to phase III trials of new chemical entity is a time-consuming procedure, and not feasible in pandemic, such as the one we are facing. Drug repositioning, an exploration of existing drug for new therapeutic use, could be an effective alternative as it allows fast-track estimation in phase II-III trials, or even forthright compassionate use. Although, drugs repurposed for COVID-19 pandemic are c

Compassionate Use of the PASCAL Transcatheter Valve Repair System for Severe Tricuspid Regurgitation: A Multicenter, Observational, First-in-Human Experience.

OBJECTIVES: The purpose of this observational first-in-human experience was to investigate the feasibility and safety of the PASCAL transcatheter valve repair system and its impact on short-term clinical outcomes in patients with severe tricuspid regurgitation (TR). BACKGROUND: Transcatheter repair of severe TR is a promising treatment option for patients at prohibitive surgical risk. Large leaflet coaptation gaps and tethering represent common features that challenge the application of transcatheter repair techniques. METHODS: Twenty-eight patients with severe TR were treated with the PASCA

Defibrotide for the Treatment of Hepatic Veno-Occlusive Disease: Final Results From the International Compassionate-Use Program.

Hepatic veno-occlusive disease, also called sinusoidal obstruction syndrome (VOD/SOS), is an unpredictable and potentially fatal complication of hematopoietic cell transplantation (HCT) or nontransplantation-associated chemotherapy/radiotherapy. In cases of severe hepatic VOD/SOS, typically defined by associated multiorgan failure (MOF, also known as multiorgan dysfunction), mortality exceeds 80%. Preclinical and early clinical data have provided a rationale for defibrotide treatment in hepatic VOD/SOS. Based on this evidence and in recognition of the dismal prognosis for these patients, defib

[Shift of focus in the financing of Hungarian drugs. Reimbursement for orphan drugs for treating rare diseases: financing of enzyme replacement therapy in Hungary].

Focusing on the benefits of patients with rare disease the authors analysed the aspects of orphan medicines financed in the frame of the Hungarian social insurance system in 2012 in order to make the consumption more rational, transparent and predictable. Most of the orphan drugs were financed in the frame of compassionate use by the reimbursement system. Consequently, a great deal of crucial problems occurred in relation to the unconventional subsidized method, especially in the case of the highest cost enzyme replacement therapies. On the base of the findings, proposals of the authors are pr

Expanding the scope of PI3K-δ inhibition: Leniolisib treatment in PRKCD deficiency.

Leniolisib, a selective phosphoinositide 3-kinase δ (PI3Kδ) inhibitor, is approved in several countries for the treatment of activated PI3Kδ syndrome (APDS) in patients 12 years of age and older. We report the first successful compassionate use of leniolisib in another inborn error of immunity, protein kinase C δ deficiency. In addition to infectious complications, the 14-year-old patient experienced lymphoproliferation in the form of splenomegaly, lymphadenopathy, and thymic hyperplasia; trilineage cytopenia; multiple forms of autoimmunity; and interstitial lung disease. Decision to initiate

Paediatric screening in Italy as a gateway to secondary prevention in type 1 diabetes.

This article explores Italy's pioneering national paediatric screening initiative for type 1 diabetes (T1D), the first of its kind to be mandated by the Italian law for the general population aged 1-17 years. This initiative is designed to facilitate early detection and secondary prevention of T1D and coeliac disease (CD), aiming to identify children in presymptomatic stages of T1D, regardless of family history. Emphasis is placed on autoantibody screening for T1D and CD to refine risk prediction and enhance secondary prevention efforts. Furthermore, the anti-CD3 + T cell monoclonal antibody t

Case report: Successful treatment with contezolid in a patient with tuberculous meningitis who was intolerant to linezolid.

Tuberculous meningitis (TBM) is the most common form of central nervous system tuberculosis (TB) and the most severe form of extrapulmonary TB. It often presents with non-specific symptoms initially and has a high mortality and disability rate. With good central nervous system penetration, linezolid is recommended for treating drug-resistant, severe, or refractory tuberculous meningitis in China. Despite the benefits of linezolid on TBM treatment, the adverse effects of long-term therapy, such as myelosuppression, peripheral neuritis, and optic neuritis, are notable and can be severe and even

Sofosbuvir inhibits yellow fever virus in vitro and in patients with acute liver failure.

INTRODUCTION AND OBJECTIVES: Direct antiviral agents (DAAs) are very efficient in inhibiting hepatitis C virus and might be used to treat infections caused by other flaviviruses whose worldwide detection has recently increased. The aim of this study was to verify the efficacy of DAAs in inhibiting yellow fever virus (YFV) by using drug repositioning (a methodology applied in the pharmaceutical industry to identify new uses for approved drugs). MATERIALS AND METHODS: Three DAAs were evaluated: daclatasvir, sofosbuvir and ledipasvir or their combinations. For in vitro assays, the drugs were dil

ABOUT PATIENTS, "INVENTORS", JOURNALISTS, SCIENTISTS AND IRBs (TO SAY NOTHING OF THE INSTITUTIONS): CCSVI AND MS.

In this article, the Author analyzes her own experience as a member of the IRB that approved a trial to determine the efficacy of a disobstruction procedure of extracranial veins by means of angioplasty in patients with multiple sclerosis (MS). The so-called "liberation therapy" was proposed by an Italian vascular surgeon, who theorized a condition called "chronic cerebrospinal venous insufficiency" (CCSVI) as playing a role in the pathogenesis of MS. This approval, given after an animated discussion amongst IRB members, lacked any solid scientific evidence of a causal relationship between CCS

Bacteriophages in the treatment of cutaneous infections and skin disorders: therapeutic advances and future directions.

SUMMARYSkin and soft tissue infections represent a major clinical challenge. This is particularly true given the rise in antimicrobial resistance, the increasing prevalence of chronic wounds, and the growing number of immunocompromised patients. Conventional antibiotic therapies are frequently compromised by multidrug-resistant pathogens, biofilm formation, and disruption of the skin microbiome, underscoring the urgent need for alternative or adjunctive antibacterial strategies. Bacteriophages, viruses that specifically infect and lyse bacteria, have re-emerged as promising therapeutic agents

Precision bacteriophage therapy for multidrug-resistant recurrent urinary tract infections: From biological rationale to clinical translation in Taiwan.

Recurrent urinary tract infections (rUTIs), driven by multidrug-resistant (MDR) uropathogens, intracellular reservoirs, and biofilms, progressively circumvent conventional antibiotic therapy. Bacteriophage (phage) therapy has re-emerged as a precision medicine strategy offering narrow host specificity, self-amplification at the infection site, and intrinsic antibiofilm activity via depolymerases. This minireview evaluates the biological rationale and translational evidence for phage therapy in MDR-rUTIs, emphasizing the clinical distinction between standardized trials for acute uncomplicated i

Exploring the next "phage" of treating non-tuberculous mycobacterial infections.

Non-tuberculous mycobacteria (NTM) can present a serious clinical threat due to their resistance to conventional antibiotics and their ability to cause persistent infections. As antimicrobial resistance (AMR) represents one of the most significant global health threats, mycobacteriophages (MPs)-viruses that specifically target mycobacterial species-are emerging as a promising therapeutic addition or alternative. This review explores the biological characteristics, therapeutic mechanisms, and clinical potential of MPs, with a primary focus on their use against NTM infections. Based on data from
