Oral lesions in a typical case of pemphigus vulgaris were investigated electron microscopically. The region of the basement membrane shows only minor changes. The basal cells have normal hemidesmosomes whereas the desmosomes disappear. Acantholytic cells sometimes reveal remnants of desmosomes. The tonofilaments show perinuclear centralization. The findings seem to indicate that the immunopathological process affects the cellular walls and thus separates existing desmosomal contacts and at the same time inhibits new desmosome formation.

Pemphigus vulgaris (PV) is an autoimmune bullous skin disease caused primarily by autoantibodies (PV-IgG) against the desmosomal adhesion proteins desmoglein (Dsg)1 and Dsg3. PV patient lesions are characterized by flaccid blisters and ultrastructurally by defined hallmarks including a reduction in desmosome number and size, formation of split desmosomes, as well as uncoupling of keratin filaments from desmosomes. The pathophysiology underlying the disease is known to involve several intracellular signaling pathways downstream of PV-IgG binding. Here, we summarize our studies in which we used

Large cytoplasmic aggregates (Mallory bodies) containing randomly arranged 10 to 20 nm thick filaments are characteristic structures of hepatocytes in certain liver diseases, especially alcoholic hepatitis of man and griseofulvin intoxication of mouse. Biochemical and immunological studies have shown that such Mallory bodies contain structural proteins related to the cytokeratins present in epithelial cells of various kinds, normal hepatocytes included. We have studied the mode of reorganization of the hepatocyte cytoskeleton during involution of Mallory bodies experimentally induced in mice b

Abnormal desmosomes were found in the endometrial epithelial cells of rats treated orally with gossypol acetic acid at a dose of 60 mg/kg/day for 30 days. Desmosomes in the endometrium of rats treated at 40 mg/kg/day were similar to those of controls. Abnormalities included an increase in desmosome number with prominent filamentous interconnections, asymmetry, disorganization of tonofilaments, missing tonofilaments, missing desmosomal plaques that did not appear to line up with those of the adjacent cell. It is thought that the antifertility actions of gossypol may be due to a disturbance of d

Pathogenic mutations have now been described in ten different desmosomal proteins: plakophilin 1 (PKP1) and 2 (PKP2); desmoplakin; plakoglobin; desmoglein 1, 2, and 4; desmocollin 2, and 3 corneodesmosin. Nevertheless, the first report of an inherited desmosomal gene disorder, published in 1997, involved loss-of-function mutations on both alleles of PKP1, the PKP1 gene. Loss of PKP1 expression in human skin leads to skin erosions and crusting, notably with perioral fissuring as well as palmoplantar hyperkeratosis with painful cracking of the skin. Other more variable features include abnormali

The importance of desmosomal cell adhesion to human health is evidenced by the autoimmune disease pemphigus vulgaris (PV), in which autoantibodies against the extracellular domain of the desmosomal cadherin desmoglein 3 cause potentially fatal blistering of the skin and mucous membranes. Tucker et al. describe how enhanced expression of a desmosomal cytoplasmic plaque protein, plakophilin-1, protects keratinocytes from PV IgG-induced loss of cell adhesion by inducing calcium-independent hyperadhesive desmosomes. This study beautifully demonstrates that desmosomal adhesion can be modulated by t

Pemphigus is a life-threatening autoimmune blistering disease of skin and mucous membranes that has two major subtypes based on clinical and histological features, pemphigus vulgaris (PV) and pemphigus foliaceus (PF). Autoantibodies against the PV antigen (desmoglein 3) and the PF antigen (desmoglein 1) are involved in the pathogenesis of blister formation. In the present study, the location of epitopes recognized by autoantibodies of patients with PV and PF was studied by postembedding immunogold electron microscopy. PV and PF autoantibodies were observed bound predominantly to the intercellu

Desmoglein 3 (Dsg3) is a desmosomal cadherin mediating cell adhesion within desmosomes and is the antigen of the autoimmune blistering skin disease pemphigus vulgaris. Therefore, understanding of the complex desmosome turnover process is of high biomedical relevance. Recently, super resolution microscopy was used to characterize desmosome composition and turnover. However, studies were limited because adhesion measurements on living cells were not possible in parallel. Before desmosomal cadherins are incorporated into nascent desmosomes, they are not bound to intermediate filaments but were su

BACKGROUND: ABO blood group antigens (ABH antigens) are carbohydrate chains glycosylated on epithelial and red blood cells. Recent findings suggest reduced ABH expression in psoriasis and atopic dermatitis, a chronic inflammatory skin disease with retained scale. H antigen, a precursor for A and B antigens, is synthesized by fucosyltransferase 1 (FUT1). Desmosomes, critical for skin integrity, are known to require N-glycosylation for stability. We investigate the impact of H antigens, a specific type of glycosylation, on desmosomes in keratinocytes.
METHOD: Primary human keratinocytes were tr

Inflammatory cardiomyopathy (ICM), resulting from persistent myocardial inflammation, is associated with life-threatening ventricular arrhythmias (VA). Recent studies suggested overlap between ICM-associated VA and desmosomal diseases affecting the intercalated discs (ID), which are critical for electromechanical coupling. Genetic variants in desmosomal genes cause arrhythmogenic cardiomyopathies and genodermatoses, supporting a potential link between desmosomal dysfunction and arrhythmic risk. This single-center pilot study explored cutaneous abnormalities in patients with ICM complicated by

Plakoglobin (Pg) and desmoplakin (DP) are adapter proteins within the desmosome, providing a mechanical link between desmosomal cadherins as transmembrane adhesion molecules and the intermediate filament cytoskeleton. As in the severe skin blistering disease pemphigus, autoantibodies against desmosomal adhesion molecules induce loss of keratinocyte cohesion at least in part via p38 mitogen-activated protein kinase (p38MAPK) activation and depletion of desmosomal components, we evaluated the roles of Pg and DP in the p38MAPK-dependent loss of cell adhesion. Silencing of either Pg or DP reduced

Epidermal cell adhesion depends on the intercellular interactions of transmembrane cadherin glycoproteins, which form the basis of adherens junctions and desmosomes. Pemphigus is a blistering disease of the skin and mucous membranes characterized by autoantibodies against the cell surface desmosomal cadherins, desmoglein (Dsg) 3 and Dsg1. An unanswered question in pemphigus pathophysiology is the mechanism of acantholysis, or loss of keratinocyte cell adhesion. One longstanding theory for pemphigus pathogenesis is the concept of steric hindrance, in which pathogenic pemphigus autoantibodies ca

In pemphigus, autoantibodies against the desmosomal cadherins desmoglein (DSG) DSG1 and DSG3 cause intraepidermal blistering. Recently, we found that increasing cAMP with the phosphodiesterase-4 inhibitor apremilast stabilizes keratinocyte cohesion in pemphigus. This effect is paralleled by phosphorylation of the desmosomal plaque protein plakoglobin (PG) at serine 665 (S665). Here, we investigated the relevance of PG phosphorylation at S665 for stabilization of keratinocyte cohesion and further characterized the underlying mechanisms. Ultrastructural analysis of a recently established PG-S665

Epithelial tissue integrity is maintained through specialized intercellular junctions known to coordinate homeostatic processes. In this context, outside-in signaling and mechanotransduction through desmosomal cadherins, the building blocks of desmosomes and main stress bearers in epithelial tissue, are only starting to emerge. To better understand the dual function of desmosomal cadherins in structural integrity and cellular signaling, we here performed a systematic, unbiased review on pathogenic signaling effectors identified in models and patients with pemphigus vulgaris (PV). PV is an auto

Pemphigus vulgaris is a life-threatening blistering skin disease caused by autoantibodies destabilizing desmosomal adhesion. Current therapies focus on suppression of autoantibody formation and thus treatments directly stabilizing keratinocyte adhesion would fulfill an unmet medical need. We here demonstrate that apremilast, a phosphodiesterase 4 inhibitor used in psoriasis, prevents skin blistering in pemphigus vulgaris. Apremilast abrogates pemphigus autoantibody-induced loss of keratinocyte cohesion in ex-vivo human epidermis, cultured keratinocytes in vitro and in vivo in mice. In parallel

Desmosomes are multi-protein cell-cell adhesion structures supporting cell stability and mechanical stress resilience of tissues, best described in skin and heart. The kidney is exposed to various mechanical stimuli and stress, yet little is known about kidney desmosomes. In healthy kidneys, we found desmosomal proteins located at the apical-junctional complex in tubular epithelial cells. In four different animal models and patient biopsies with various kidney diseases, desmosomal components were significantly upregulated and partly miss-localized outside of the apical-junctional complexes alo

The demonstration of circulating autoantibodies directed against the constitutive desmosomal plaque proteins desmoplakin (dp) I and II in mucocutaneous lesions in a subset of patients with erythema multiforme major, suggests that humoral immune mechanisms may play a role in the pathogenesis of this severe skin disease. In this study we identified a specific peptide sequence--YSYSYS--representing an antigenic binding site for the human autoantibodies. This epitope is localized at the extreme carboxy terminal domain of dp thought to be responsible for the assembly of keratin filaments with desmo

Cell-cell contacts are crucial for intercellular cohesion and formation of endothelial and epithelial barriers. Desmosomes are the adhesive contacts providing mechanical strength to epithelial intercellular adhesion and therefore are most abundant in tissues subjected to high mechanical stress such as the epidermis and heart muscle. Desmogleins (Dsg) besides intercellular adhesion serve as signalling hubs regulating cell behaviour. In desmosomal diseases such as the autoimmune blistering skin disease pemphigus or arrhythmic cardiomyopathy (AC), which is caused by mutations of desmosomal compon

BACKGROUND: In pemphigus circulating IgG is present with the desmosomal cadherins desmoglein (Dsg) 1 and 3. In the epidermis of patients, this IgG deposits in a pattern that is often partly granular and does not reflect the normal Dsg distribution.
OBJECTIVE: To understand why the IgG deposits in a granular pattern in the skin of patients with pemphigus.
PATIENTS/METHODS: We analysed the distribution of IgG and desmosomal adhesion molecules in skin biopsies of 18 patients with pemphigus vulgaris (PV) and 10 with pemphigus foliaceus (PF) by double staining immunofluorescence. The effect of Ig

The structure of the septum in bigeminal pregnancy was examined via the electron microscope after normal pregnancy. The cellular junctions were desmosomes and "gap junctions" in the amnionic epithelium, whereas in the chorionic epithelium "gap junctions" and "tight junctions" were found occasionally besides desmosomes. The basement membrane was multilayered and discontinuous both in the amnion and in the chorion. Since, physiologically, the desmosomes and the gap junctions cannot be regarded as a diffusion barrier against proteins, and since the rare tight junctions are without any significanc
