Perinatal Health of Women with Intellectual and Developmental Disabilities and Comorbid Mental Illness.

OBJECTIVE: Women with intellectual and developmental disabilities (IDD) have high rates of adverse perinatal outcomes. However, the perinatal health of women with co-occurring IDD and mental illness (dual diagnosis) is largely unknown. Our objectives were to 1) describe a cohort of women with dual diagnosis in terms of their social and health characteristics and 2) compare their risks for adverse maternal and neonatal outcomes to those of women with IDD only. METHOD: We conducted a population-based study using linked Ontario (Canada) health and social services administrative data to identify

Taiwanese parents' perceptions of their very low-birth-weight infant with developmental disabilities.

This study explores the perceptions and experiences of Taiwanese parents in coping with the unfolding evidence of a disability, their response to the official diagnosis, and their views about their child's developmental disability. This descriptive qualitative study is a partial analysis of data from a larger study including 19 Taiwanese parents of very low-birth-weight infants with developmental disability at 6 and 12 months of corrected age. Four themes were generated: uncertainty and worry about developmental progress, search for meaning and supernatural will, desire for normality and attit

The capacity to give informed consent in a homeless population with developmental disabilities.

This study examined homeless persons with developmental disabilities and their ability to understand the informed consent process for research using a capacity-to-consent screener. Bivariate associations with the inability to pass the screener on the first attempt were noted with three factors: an eighth grade or less in education, chronic homelessness, and mental retardation diagnosis. With multiple regression, the only outcome associated with inability to pass the screener on the first attempt was an eighth grade or less education. This study indicates the need for consideration of developme

Bridging the equity gap: health promotion for adults with intellectual and developmental disabilities.

Health is influenced by political, economic, social, cultural, environmental, behavioral and biological conditions--either positively or negatively. Health promotion aims to make these factors more favorable through health advocacy. Advocating for physical, mental, and social health requires that individuals with I/DD have opportunities to identify and realize their aspirations, develop the capacity to satisfy their needs, and possess the ability to adapt and/or cope with the environment. Because health is both an individual and a social responsibility, effective health promotion strategies mu

Psychopharmacological treatments in persons with dual diagnosis of psychiatric disorders and developmental disabilities.

People with developmental disabilities are at considerable risk for the development of comorbid psychiatric conditions. Psychopharmacological treatments may have a crucial role in a multidisciplinary and multimodal approach to the management of psychopathology in this population. Psychiatric illnesses that are particularly amenable include mood disorders, anxiety disorders, schizophrenia, and attention deficit hyperactivity disorders (ADHDs) and antidepressants, mood stabilisers, anxiolytics, antipsychotics, and stimulants should be considered, respectively. ADHD may also respond to alpha(2)-a

A review of choice research with people with severe and profound developmental disabilities.

During the last 15-20 years, a significant amount of research has focused on the issue of choice among people affected by severe-profound developmental disabilities. Studies have been directed at (a) assessing the ability of those people to choose between different options and express preferences that could be used for reinforcement or occupational purposes, (b) building choice opportunities within those people's daily situations, and (c) evaluating the possible effects of choice making on those people's performance and behavior. This paper reviews the aforementioned studies and comments on th

The American Academy of Developmental Medicine and Dentistry: eliminating health disparities for individuals with mental retardation and other developmental disabilities.

Recent reports by Special Olympics International and the U.S. Surgeon General have revealed significant disparities and unmet health needs encountered by persons with mental retardation and other developmental disabilities (MR/DD). Factors contributing to these disparities include deinstitutionalization, increased survival of individuals with MR/DD, lack of appropriately trained providers, and inadequate financing of dental services. To address these problems, a group of academically oriented dentists and physicians formed the American Academy of Developmental Medicine and Dentistry (AADMD). T

Examining Differences in Hospitalization Among Adults With and Without Intellectual and Developmental Disabilities.

The primary aim of this study was to compare the common reasons for hospitalization among adults with and without intellectual and developmental disabilities (IDD) in the United States. We examined hospitalizations for adults with and without IDD using the 2017 to 2019 National Inpatient Sample. Adults with IDD were matched to a sample without IDD, stratified by age (18-49 vs. ≥50), and compared for reasons for hospital admission and outcome differences. Adults with IDD, especially those 18 to 49, were frequently hospitalized for schizophrenia, while septicemia was the primary cause for those

Factors influencing engagement in smoking cessation for adults with Intellectual and Developmental Disabilities.

INTRODUCTION: People with Intellectual and Developmental Disabilities (IDD) may experience tobacco use patterns that place them at elevated risk for nicotine dependence. However, there are few available interventions tailored for people with IDD, suggesting people with IDD may rely on approaches developed for people without disabilities. The purpose of this study was to identify factors influencing engagement in tobacco treatment among adults with IDD. METHODS: Semi-structured interviews with key stakeholders (n = 39) were conducted, including adults with IDD who currently smoked cigarettes (

Pain assessment in children and young people with developmental disabilities: a scoping review.

Paediatric pain has been identified in the literature as being undertreated and often misdiagnosed. This article details a scoping review that aimed to identify the facilitators and barriers to using pain assessment tools for children and young people with developmental disabilities. Two themes were identified as barriers to pain assessment: inexperience of healthcare professionals; and a lack of education for healthcare professionals. Two themes were identified as facilitators of pain assessment: parental assistance and knowledge; and education and training for healthcare professionals. The s

When Development Falls out of Sync: A Developmental Timing Framework for Learning Disabilities.

Learning disabilities are traditionally conceptualized as domain-specific disorders arising from deficits in reading, attention, or numerical processing. Although these approaches have substantially advanced understanding of dyslexia, attention-deficit/hyperactivity disorder (ADHD), and dyscalculia, they do not fully explain the marked heterogeneity, developmental variability, frequent comorbidity, or changing learning trajectories observed across children. This conceptual review proposes a developmental timing framework in which learning disabilities emerge, in many cases, from maturational a

Economic evaluation of caregiver interventions for children with developmental disabilities: A scoping review.

Globally, families with children with developmental disabilities (DDs) experience challenges, including social isolation, stigma, and poverty, especially in low-income settings in Africa. Most children with DDs in Africa remain unidentified and receive no formal support. Caregiver interventions focusing on education and training for the carers/parents have been shown to be adaptable and low intensity in implementation, although the economic evidence is limited. This review aimed to describe the evidence and methodological aspects of economic evaluations for caregiver interventions for DDs. The

Safety and efficacy of sedation for pediatric patients with cancer and developmental disabilities.

BACKGROUND: Children with developmental disabilities (DDs) tend to have more fear of loud noises and needles than those without. However, how DDs affect sedation during painful procedures in pediatric patients with cancer is unclear. This study aimed to evaluate the sedative doses and adverse events (AEs) associated with sedation using a combination of midazolam and ketamine for bone marrow aspiration and intrathecal therapy. METHODS: The AEs evaluated included hypoxia, vomiting, tremors, excitement, disturbances, apnea, and bradycardia. The study included seven participants with autism spect
