Determining a Consensus-Based Measure for Critical Illness and Injury for Children Cared for By Emergency Medical Services.

OBJECTIVES: Accurate triage of patients from the field or community EDs to higher levels of care is essential for emergency, disaster, and surge response. We sought to define a hospital-based measure of critical illness and/or injury to support the development of emergency medical services-based models for this outcome. METHODS: We convened a 22-member Delphi panel of clinicians with subspecialities including pediatrics, pediatric emergency medicine, pediatric surgery, trauma, prehospital medicine, and emergency medical services. We considered key diagnostic, intervention, and clinical outcom

Management of trauma in remote and austere settings: civilian implications of military-derived prolonged field care principles - A narrative review.

Prolonged Field Care (PFC) refers to the delivery of advanced trauma care in austere environments when evacuation to definitive treatment is significantly delayed. Although originally developed within military medicine, its principles are increasingly relevant to civilian trauma systems operating in remote, rural, wilderness, disaster, and resource-limited settings. This narrative review provides an updated overview of PFC concepts and their potential applicability to civilian trauma care, focusing on key domains such as airway management, haemorrhage control, crush syndrome, analgesia and sed

Duckweed Proteins in Neuro-Nutrition: Mechanistic Insights and Translational Relevance for Military and Extreme Environments.

Modern military performance increasingly depends on cognitive resilience and neurological health, making nutrition a strategic determinant of combat readiness. This review integrates nutritional neuroscience and military medicine to explore duckweed as a next-generation neuro-nutritional resource. Duckweed species possess protein content (25-45% dry weight) and a complete essential almost amino acid (EAA) profile that may contribute to pathways involved in neurotransmitter synthesis, neuroplasticity, and stress adaptation. Beyond amino acids, duckweed delivers neuroprotective compounds such as

Expert consensus on the evaluation and diagnosis of combat injuries of the Chinese People's Liberation Army.

The accurate assessment and diagnosis of combat injuries are the basis for triage and treatment of combat casualties. A consensus on the assessment and diagnosis of combat injuries was made and discussed at the second annual meeting of the Professional Committee on Disaster Medicine of the Chinese People's Liberation Army (PLA). In this consensus agreement, the massive hemorrhage, airway, respiration, circulation and hypothermia (MARCH) algorithm, which is a simple triage and rapid treatment and field triage score, was recommended to assess combat casualties during the first-aid stage, whereas

The role of the dentist at crime scenes.

The medical response to a mass casualty further complicates the hectic environment that follows a terrorist event. In addition to providing treatment, medical professionals may discover items or persons of interest to the pending investigation and should be aware of how to handle these situations appropriately. Examples of case law are provided to illustrate how practitioners' actions could help or hinder prosecution. The traditional forensic role of dental professionals is identifying victims through dental records. In this article, the dental professional is considered a member of a disaster

The medicolegal and forensic aspects of fires.

Fires, their consequences and their investigations, continue to provide forensic scientists, especially those involved in medicolegal investigation (the medical examiner or forensic pathologist), with constant work and variations in problems. The recent history of mass disasters involving high-rise buildings, transport accidents, and arson-related accidents in nightclubs and prisons has emphasized the necessity for corrective and preventive means to ensure safety to the occupants of any of these areas. Problems presented by fires include the determination of the cause of the fire, the identifi

Preliminary DNA identification for the tsunami victims in Thailand.

The 2004 Southeast Asia Tsunami killed nearly 5,400 people in Southern Thailand, including foreign tourists and local residents. To recover DNA evidence as much as possible from the seriously decomposed bodies, we explored procedures of sample preparation from both bone and tooth samples as well as both mitochondrial and nuclear markers. Despite having failed to recover enough DNA for nuclear marker typing, we succeeded in obtaining fully informative results for mitochondrial markers (HV1 and HV2) from 258 tooth samples with a success rate of 51% (258/507). Using an organic DNA extraction meth

Requirements of radioprotectors for military and emergency needs.

Uncontrolled exposure to radiation from nuclear weapons, space, or accidents presents challenges unlike those met in radiotherapy. For example, the time and quality of exposure are uncertain. Thus it is necessary to develop radioprotectors that can be given chronically (e.g. as vitamin combinations), or shortly before exposure (e.g. as WR-2721 and related compounds), or after exposure (e.g. as immunomodulators) to cover all possible situations of use. Also, for military purposes, performance capability, as well as survival from acute effects, must be maintained. In contrast to clinical and exp

Leptospirosis-clinical review and updates on therapeutics.

Leptospirosis is a globally distributed zoonotic bacterial disease caused by Leptospira spp. Mortality and morbidity burdens are greatest in low-to-middle-income tropical countries, where frequent natural disasters and poor infrastructure drive outbreaks that disproportionately affect marginalised communities. Leptospirosis is a common but overlooked cause of acute undifferentiated fever. This neglect is augmented by limited diagnostics, insufficient treatment evidence, and the absence of a coordinated global health policy. Diagnosis requires clinical vigilance and timely testing, although acc

Bioterrorism: An analysis of biological agents used in terrorist events.

BACKGROUND: The Covid19 pandemic has reignited debates and discussions around healthcare systems' biosecurity vulnerabilities and cast a spotlight on the potential weaponization of biological agents. Terrorist and violent extremist groups have already attempted to incite the intentional spread of Covid19 and to use it as an improvised form of a biological weapon. This study aims to provide an epidemiological description of all terrorism-related attacks using biological agents sustained between 1970 and 2019. METHODS: Data collection was performed using a retrospective database search through

Social problems in disaster victim identification following the 2006 Pangandaran tsunami.

Disasters, both natural and man-made, are inevitable. Interpol and the ICRC have produced Disaster Victim Identification (DVI) Guidelines to assist examiners in the identification of disaster victims. Nevertheless, problems remain in DVI practice, as was evident following the 2006 tsunami in Pangandaran, a popular tourist destination in West Java, Indonesia. The problems included the desire of families to bury their relatives quickly, the decomposed condition of the dead bodies, the inadequate security which hampered the identification process, the shortage of space to conduct identification,

[Expert consensus on emergency open wound irrigation].

Wound irrigation is one of the most effective means to prevent open wound infection after injury. Wound irrigation can reduce the risk of wound infection and promote wound healing by removing the cell fragments, surface bacteria, dressings residues, residual foreign body and wound exudates loosely attached to the wound. The effect of wound irrigation mainly depends on the pressure of irrigation, the direction of fluid flow, the type of liquid, temperature and volume/amount of irrigation fluid. In response to the phenomenon that emergency open wounds are not irrigated in a standardized manner i

Mercury and Movement Disorders: The Toxic Legacy Continues.

Mercury (Hg) exists in the environment as inorganic (metallic Hg vapor, mercurous and mercuric salts) or organic (bonded to a structure containing carbon atoms) forms. Neurotoxic effect of Hg is known for years. While the organic form (methylmercury (meHg)) led to the Minamata incidence in Japan and "wonder-wheat" disaster in Iraq, the "mad hatters" and "Danbury shakes" were related to the inorganic elemental form (Hg vapor). Human exposure to toxic Hg continues in the modern world to a large extent by artisanal gold mining, biomass combustion, chloralkali production, and indigenous medicine u

Neurocovid-19: A clinical neuroscience-based approach to reduce SARS-CoV-2 related mental health sequelae.

Coronavirus Disease 2019 (COVID-19), caused by SARS-CoV-2, is a disaster due to not only its psychosocial impact but it also to its direct effects on the brain. The latest evidence suggests it has neuroinvasive mechanisms, in addition to neurological manifestations, and as seen in past pandemics, long-term sequelae are expected. Specific and well-structured interventions are necessary, and that's why it's important to ensure a continuity between primary care, emergency medicine, and psychiatry. Evidence shows that 2003 SARS (Severe Acute Respiratory Syndrome) survivors developed persistent psy

Ebola Outbreak in Democratic Republic of Congo: Lessons Learned, or Ignored?

Infectious diseases became an increasing public health threat as humans transitioned from nomadic hunter-gatherer societies to stable, agrarian communities. It is accurate to say the international community was not optimally prepared for the 2014-2015 Ebola virus disease (EVD) outbreak in West Africa, which eventually spread and caused secondary cases in the United States. From that experience, much was learned about the management of an EVD outbreak, from prevention and treatment, to the need for a "whole of society" response. However, it is clear from the evidence that much still needs to be

The Danish Disaster Victim Identification effort in the Thai tsunami: organisation and results.

Following the December 2004 tsunami in Thailand experts from many countries, including Denmark, went to Thailand to help with identification work. The Interpol system for Disaster Victim Identification (DVI) was employed for the identification of the many casualties. This paper describes the work of the Danish teams in Thailand from the 30th December 2004 until the 6th June 2005. The investigation covers all Danes reported missing directly after the tsunami in Thailand on the 26th December 2004 and who were later found deceased, or, in one case, never recovered. The AM and PM forms were review
