Retrospective cohort study of statin prescribing for primary prevention among people living with HIV.

OBJECTIVE: To compare statin prescribing rates between intermediate-risk people living with human immunodeficiency virus (HIV; PLWH) and intermediate-risk patients without a diagnosis of HIV for primary prevention of atherosclerotic cardiovascular disease (ASCVD). METHODS: Retrospective cohort study . Electronic health record data were used to identify a cohort of PLWH aged 40-75 years with a calculated 10-year ASCVD risk between 7.5%-19.9% as determined by the Pooled Cohort Equation (PCE). A matched cohort of primary prevention non-HIV patients was identified. The primary outcome was the pro

Concurrent Prescribing of Opioids and Sedative-Hypnotic Drugs for Long-Term Use in Australian General Practice: A Cross-Sectional Analysis Using MedicineInsight.

The number of unintentional deaths involving opioid and/or benzodiazepine use continues to increase in Australia. This study examined patterns of concurrent prescribing of opioids and benzodiazepines/Z-drugs (BZDs) for long-term use in Australian general practice. A cross-sectional analysis was undertaken using MedicineInsight, a national database of de-identified general practice electronic health records. We estimated the proportion of patients (per 1000, ‰) in 2017 receiving concurrent prescriptions for opioid and BZD medications for long-term use (≥ 3 prescriptions within 90 days). Poisson

Missed opportunities in medical therapy for patients with heart failure in an electronically-identified cohort.

BACKGROUND: National registries reveal significant gaps in medical therapy for patients with heart failure and reduced ejection fraction (HFrEF), but may not accurately (or fully) characterize the population eligible for therapy. OBJECTIVE: We developed an automated, electronic health record-based algorithm to identify HFrEF patients eligible for evidence-based therapy, and extracted treatment data to assess gaps in therapy in a large, diverse health system. METHODS: In this cross-sectional study of all NYU Langone Health outpatients with EF ≤ 40% on echocardiogram and an outpatient visit fr

Pediatric Medication Prescribing Across Urgent Care Visits: An Epidemiologic View from a Primary Care Setting in the Kingdom of Saudi Arabia.

Background: Urgent care clinics (UCCs) embedded within primary healthcare settings play a vital role in managing acute, non-life-threatening conditions in children. However, limited data exist on medication prescribing patterns in such settings in the Kingdom of Saudi Arabia (KSA), particularly regarding antibiotic use. This study aimed to describe the epidemiology of pediatric urgent care visits and identify factors associated with prescribing within a model primary healthcare (PHC) center. Methods: A retrospective chart review was conducted for all urgent care visits made by pediatric patien

Steroid prescribing in primary care increases prior to Hodgkin lymphoma diagnosis: A UK nationwide case-control study.

BACKGROUND: Steroid use is associated with increased risk of Hodgkin lymphoma (HL). However, allergic symptoms commonly treated with steroids are also presenting features of HL in some patients, thereby introducing protopathic bias in estimates of aetiological associations. It is therefore important to examine steroid prescribing patterns pre-diagnosis to understand timing of associations and when healthcare use increases before cancer diagnosis to inform future epidemiological study design. METHODS: We analysed steroid prescribing in 1232 HL patients and 7392 matched controls using primary c

Do clinicians follow heuristics in prescribing antidepressants?

BACKGROUND: While clinicians commonly learn heuristics to guide antidepressant treatment selection, surveys suggest real-world prescribing practices vary widely. We aimed to determine the extent to which antidepressant prescriptions were consistent with commonly-advocated heuristics for treatment selection. METHODS: This retrospective longitudinal cohort study examined electronic health records from psychiatry and non-psychiatry practice networks affiliated with two large academic medical centers between March 2008 and December 2017. Patients included 45,955 individuals with a major depressiv

Changes in opioid prescribing during the COVID-19 pandemic in England: an interrupted time-series analysis in the OpenSAFELY-TTP cohort.

BACKGROUND: The COVID-19 pandemic disrupted health-care delivery, including difficulty accessing in-person care, which could have increased the need for strong pharmacological pain relief. Due to the risks associated with overprescribing of opioids, especially to vulnerable populations, we aimed to quantify changes to measures during the COVID-19 pandemic, overall, and by key subgroups. METHODS: For this interrupted time-series analysis study conducted in England, with National Health Service England approval, we used routine clinical data from more than 20 million general practice adult pati

Prescription of antibiotics by general practitioners for patients with a diagnosis of SARS- CoV-2 infection: Analysis of an electronic French health record.

OBJECTIVE: To define the factors associated with overprescription of antibiotics by general practitioners (GPs) for patients diagnosed with COVID-19 during the first wave of the pandemic. METHODS: Anonymised electronic prescribing records of 1370 GPs were analysed. Diagnosis and prescriptions were retrieved. The initiation rate by GP for 2020 was compared with 2017-2019. Prescribing habits of GPs who initiated antibiotics for > 10% of COVID-19 patients were compared with those who did not. Regional differences in prescribing habits of GPs who had consulted at least one COVID-19 patient were a

Antimicrobial stewardship in critical care.

Antimicrobial resistance (AMR) remains one of the most significant threats to global health, largely driven by inappropriate antibiotic use. This review outlines the current principles and practices of antimicrobial stewardship (AMS) in the context of critical care. We address the global burden of AMR, the development of resistance and the implementation of AMS strategies from multidisciplinary, diagnostic and pharmacokinetic perspectives. The use of rapid diagnostics, biomarkers such as procalcitonin, and the integration of electronic health records are discussed as enablers of targeted thera

Tylenol or acetaminophen: a recurrent fixed drug eruption perpetuated through the use of inconsistent drug terminology.

An 87-year-old man with a history of osteoarthritis presented with worsening knee pain. He was prescribed acetaminophen with codeine. A few days later, he developed a rash on his right buttock and proximal thigh, similar to a rash he experienced in the past when he took over-the-counter (OTC) acetamenophen and an unknown lozenge to treat a presumed viral illness. A fixed drug eruption (FDE) was diagnosed and the patient was asked to avoid Tylenol and other OTC lozenges. Tylenol was entered as an allergy in the electronic medical records. However, since Tylenol, not acetaminophen was listed in
