Renal artery stenosis is a prevalent condition that has been implicated in the pathogenesis of renovascular hypertension, chronic kidney disease, and cardiac disturbance syndromes. The most common causes of renal artery stenosis are atherosclerosis and fibromuscular dysplasia. Endovascular revascularization of renal artery stenosis is controversial, but may have significant benefits for treatment of renovascular symptoms in carefully selected patients. In this review, we evaluate the literature and discuss indications for endovascular revascularization of renal artery stenosis. Technical aspec

OBJECTIVE: Atherosclerotic stenosis or obstruction of the innominate artery is rare. Traditional surgical management is a technically demanding intervention with acceptable, but not negligible, rates of morbidity and mortality. Endovascular approaches to supraaortic lesions have been successful and are now the preferred treatment for stenoses of the brachiocephalic vessels. The use of cerebral protection devices in subclavian and innominate interventions is less established.
CLINICAL PRESENTATION: A 58-year-old woman had Takayasu giant cell arteritis with a history of a left middle cerebral a

To protect against a distal embolism in the vertebral artery, subclavian artery stenting can be achieved by positioning a filter device or balloon in the ipsilateral vertebral artery. However, treatment with these devices is not easy, and it is also an inaccurate method for cerebral protection. We developed a balloon-tipped occlusion catheter (Optimo(®) occlusion catheter) without an introducer sheath, which allowed us to perform the minimally invasive endovascular therapy (EVT). Herein, we report two EVT cases for subclavian artery disease treated with an Optimo(®) occlusion catheter via the

Mechanical circulatory support devices, such as the Impella catheter (Abiomed, Danvers, Massachusetts), continue to become more commonplace in patients undergoing high-risk percutaneous coronary intervention (PCI) or those in cardiogenic shock. Thrombus in the left ventricle is a contraindication to Impella placement. Here, we present a patient with an anterior ST elevation myocardial infarction who underwent primary PCI with subsequent development of cardiogenic shock followed by Impella placement, who then later developed an LV thrombus. The Impella was removed after placement of a Sentinel

OBJECTIVES: This study sought to evaluate the safety and feasibility of percutaneous retrieval of left atrial appendage closure (LAAC) devices with an endoscopic grasping tool.
BACKGROUND: Transcatheter LAAC is a mechanical stroke prevention strategy in patients with nonvalvular atrial fibrillation (AF) who are poor candidates for long-term oral anticoagulation. However, these LAAC devices can be inadvertently released into an unfavorable location, the device might migrate to a different (unfavorable) position within the left atrial appendage (LAA) or may embolize from the heart into the aort

BACKGROUND: Each of the embolic protection devices used in carotid artery stenting (CAS) has advantages and disadvantages. The prospective, multicenter, single-arm EMPiRE Clinical Study investigated a proximally placed device (GORE Flow Reversal System) that provides distal neuroprotection during CAS by reversing blood flow in the internal carotid artery, thereby directing emboli away from the brain.
METHODS: The study evaluated 30-day outcomes in 245 pivotal high-surgical-risk patients (mean age, 70 years; 32% symptomatic; 16% ≥80-years old) with carotid stenosis who underwent CAS using the

Thrombus on implants is dangerous, necessitating anticoagulant coatings for most of blood-contacting devices with various substances. So far, there is no universal strategy of the pertinent surface modification for different substrates, which significantly hinders the efficient research and development of blood-contacting medical devices. Herein, we propose a modular strategy to construct anticoagulant coatings on substrates including polymers, metals and nonmetallic inorganics. Inspired by the principle of primary antibody and secondary antibody in biological immunoassay, we develop a univers

OBJECTIVES: We performed microscopic examination of the debris collected by a distal protection device and investigated the usefulness of grayscale and integrated backscatter intravascular ultrasound (IB-IVUS) for the prediction of distal embolization during percutaneous coronary intervention (PCI) in cases of unstable angina.
BACKGROUND: The prediction of distal embolization during PCI has not been studied in depth because assessment of distal embolization is difficult.
METHODS: We prospectively studied 39 consecutive patients with unstable angina who underwent PCI with a filter distal prot

Carotid artery stenting (CAS) is considered to be an acceptable alternative to carotid endarterectomy (CEA), particularly in patients at increased risk for CEA. The safety of CAS has improved significantly over the past decade. Adjunctive use of embolic protection devices (EPDs), is thought to be partially responsible for the improved outcomes. Among these, proximal EPDs are based on the surgical principle of stopping or reversing flow by clamping the carotid arteries, much in the same way carotid endarterectomy accomplishes neuroprotection. This is achieved through external and common carotid

BACKGROUND: Cerebral embolism remains a concern during transcatheter aortic valve replacement (TAVR). Cerebral embolic protection (CEP) devices have been developed to mitigate this risk, but their clinical benefit remains unclear.
METHODS: We conducted a systematic review and meta-analysis of randomized controlled trials comparing TAVR with and without CEP devices. Primary endpoint was overall stroke; secondary endpoints included disabling stroke, all-cause mortality, life-treating bleeding, vascular complications related to CEP access and acute kidney injury.
RESULTS: Eight trials comprisin

OBJECTIVE: Secondary interventions for renal artery restenosis (RAS) after renal artery stenting are common, despite limited data about their effectiveness. This study was designed to evaluate the outcomes of endovascular treatment of recurrent RAS.
METHODS: We conducted a retrospective review of patients who underwent renal artery stenting between 2001 and 2011 at Dartmouth-Hitchcock Medical Center. Patients who required secondary interventions were compared with control patients who underwent only primary interventions for RAS. Multivariate regression models were used to identify factors as

Carotid artery lesions are frequently composed of friable, thrombotic, ulcerated and/or hemorrhagic materials which can embolize during surgical or endovascular interventions. The use of embolic protection devices (EPD) during carotid angioplasty and stenting (CAS) has been proven to be associated with a reduction of the embolic load. Many studies indicate that the clinical results of CAS are comparable with the best surgical series, when EPD are routinely applied. The proximal EPD work by interrupting or reversing the blood flow in the common carotid artery/internal carotid artery (CCA/ICA).

INTRODUCTION: Cerebrovascular events are one of the most serious consequences after transcatheter aortic valve replacement (TAVR). More than half of the cerebrovascular events following TAVR are due to procedure-related emboli. Embolic protection devices (EPDs) have the potential to decrease cerebrovascular events during TAVR procedure. However, randomized controlled trials (RCTs) have not conclusively determined if EPDs are effective, likely because of small number of patients enrolled. However, meta-analyses and propensity-matched analyses from large registries have shown efficacy and sugges

BACKGROUND: The carotid stent design may influence the risk of embolization during carotid artery stenting. The aim of the study was to assess this risk by comparing the quantity of embolized material captured by filters during carotid artery stenting, using different stent designs.
METHODS: We conducted a single-center retrospective study of patients undergoing carotid artery stenting for asymptomatic carotid stenosis >70% (2010-2022) in a tertiary academic hospital (Padua University Hospital, Italy). Carotid stents were classified according to their design as open-cell (OCS), closed-cell (C

One of the most feared complications of transcatheter aortic valve implantation (TAVI) is stroke, with increased mortality and disability observed in patients suffering a stroke after TAVI. There has been no significant decline in stroke rates seen over the last 5 years; attention has therefore been given to strategies for cerebral embolic protection. With the emergence of new randomized trial data, we sought to perform an updated systematic review and meta-analysis to examine the effect of cerebral embolic protection during TAVI both on clinical outcomes and on neuroimaging parameters. We per

Atherectomy devices are often utilized to treat heavily calcified peripheral arterial lesions; however, these procedures carry a risk of distal embolization. A 57-year-old patient on hemodialysis underwent JETSTREAM Atherectomy for a heavily calcified superficial femoral artery lesion. Despite successful debulking and drug-coated balloon angioplasty, distal embolization occurred, leading to irreversible toe ischemia and subsequent amputation. Micro-computed tomography and histology of the amputated specimen revealed embolized nodular calcification and cholesterol crystals, confirming plaque-de

BACKGROUND: The benefits of adjunctive mechanical devices to prevent distal embolization in patients with acute myocardial infarction (AMI) are still a matter of debate. The aim of this meta-analysis was to combine data from all randomized trials conducted with adjunctive mechanical devices to prevent distal embolization in AMI.
METHODS: The literature was scanned by formal searches of electronic databases (MEDLINE and Central) from January 1990 to October 2006, scientific session abstracts (from January 1990 to October 2006), and oral presentation and/or expert slide presentations (from Janu

The purpose of this study was to examine the possible beneficial effect of the PercuSurge GuardWire cerebral protection device based on balloon occlusion of the distal internal carotid artery and debris aspiration for patients undergoing carotid artery stenting (CAS). A total of 268 CAS procedures were attempted under cerebral protection using the PercuSurge GuardWire system in 242 patients (194 men; mean age, 71.2 +/- 9.4 years; range, 40-91). The lesions were mainly atherosclerotic; 64% were symptomatic. Technical success was 99.3%. All lesions were stented except three postangioplasty reste
