Pre-hospital cooling in community-acquired heat stroke (CAHS): evidence, challenges, and strategies.

BACKGROUND: Heat stroke (HS) is a critical medical emergency characterized by severe hyperthermia and central nervous system dysfunction, occurring predominantly in conditions of high ambient temperatures or through physical exertion. With rising global temperatures, the incidence and severity of HS are expected to increase, presenting substantial public health challenges. OBJECTIVES: This review synthesizes current evidence on cooling methods for HS, addresses challenges in managing community-acquired heat stroke (CAHS), and proposes strategies to enhance pre-hospital and emergency departmen

Sex differences in rural prehospital ST-segment elevation myocardial infarction care.

BACKGROUND: In rural settings, women with ST-elevation myocardial infarction (STEMI) are less likely to receive timely reperfusion than men. We explore factors that may impact time to reperfusion by sex for patients with STEMI. METHODS: We conducted a cohort study of adults with STEMI activations from 2016 to 2020 using regional North Carolina STEMI registry data, which included eight rural emergency medical services (EMS) agencies and three percutaneous coronary intervention (PCI) centres. The primary outcome was EMS first medical contact to PCI in ≤90 min. By sex, we evaluated prehospital t

Crash Telemetry-Based Injury Severity Prediction is Equivalent to or Out-Performs Field Protocols in Triage of Planar Vehicle Collisions.

INTRODUCTION: With the increasing availability of vehicle telemetry technology, there is great potential for Advanced Automatic Collision Notification (AACN) systems to improve trauma outcomes by detecting patients at-risk for severe injury and facilitating early transport to trauma centers. METHODS: National Automotive Sampling System Crashworthiness Data System (NASS-CDS) data from 1999-2013 were used to construct a logistic regression model (injury severity prediction [ISP] model) predicting the probability that one or more occupants in planar, non-rollover motor vehicle collisions (MVCs)

Short-term exposure to ambient particulate matter and emergency ambulance dispatch for acute illness in Japan.

Short-term exposure to air pollution may be linked to negative health outcomes that require an emergency medical response. However, few studies have been undertaken on this phenomenon to date. The aim of this study therefore was to examine the association between short-term exposure to ambient suspended particulate matter (SPM) and emergency ambulance dispatches (EADs) for acute illness in Japan. Daily EAD data, daily mean SPM and meteorological data were obtained for four prefectures in the Kanto region of Japan for the period from 2007 to 2011. The area-specific association between daily EAD

Prehospital point-of-care ultrasound in ruptured abdominal aortic aneurysms-a retrospective cohort study.

BACKGROUND: Patients with ruptured abdominal aortic aneurysm (rAAA) require immediate vascular treatment to survive. The use of prehospital point-of-care ultrasound (POCUS) may support clinical assessment, correct diagnosis, appropriate triage and reduce system delay. The aim was to study the process of care and outcome in patients receiving prehospital POCUS versus patients not receiving prehospital POCUS in patients with rAAA, ruptured iliac aneurysm or impending aortic rupture. METHODS: We performed a retrospective cohort study in patients diagnosed with rAAA in the Central Denmark Region

9-1-1 Call Data Modeling to Assess the Influence of a Mass Gathering on Host Community Metrics for Syndromic Surveillance.

INTRODUCTION: Multi-day planned mass gatherings are prevalent in various forms of sports competitions, concerts, professional, political, and religious conferences. The largest, the World Cup competition, will be hosted by large cities of the United States, Canada, and Mexico in June and July, 2026. These can present risks to the health of spectators, participants, organizers, and host communities, which if discovered faster, facilitates more effective responses in preventing morbidity/mortality. This study demonstrates visually modeled 9-1-1 medical data that uniquely contributes to existing

Unequal access to timely care: Geographic variation in prehospital time among injured patients at risk of hemorrhagic shock.

BACKGROUND: Hemorrhage remains the leading cause of preventable traumatic deaths, with many fatalities occurring before hospital arrival. Although geographic differences in prehospital time (PHT) are recognized, contemporary national estimates and their implications for resuscitation readiness are not well defined. This study aimed to characterize geographic variation in PHT among trauma patients at risk of hemorrhagic shock to inform strategies for earlier intervention. STUDY DESIGN AND METHODS: We analyzed 2020-2023 data from the National Emergency Medical Services (EMS) Information System

Health care system resilience - Evaluating the effect of the COVID-19 pandemic on emergency medical service demand in Germany: A case study from the city of Jena.

BACKGROUND: The COVID-19 pandemic has impacted health systems globally, including emergency medical services (EMS). This study evaluates the absorptive and adaptive resilience of EMS in Jena, Germany, by analysing how the system managed significant pandemic-induced shocks to demand and operational workflows. METHODS: We conducted an observational, retrospective analysis of EMS missions in Jena from January 2018 to December 2021. Data on mission volumes, patient demographics, health issues, and dispatch and treatment intervals were analysed. Lockdown periods were compared to corresponding pre-

[Systems saving lives: a systems-based approach to cardiac arrest management].

Out-of-hospital cardiac arrest remains a major public health challenge worldwide, with survival largely determined by the timeliness, quality, and organization of care. The 2025 Systems saving lives guidelines of the European Resuscitation Council (ERC) emphasize that meaningful improvements in outcomes depend not on isolated interventions but on the coordinated functioning of the entire system of care. This paper, representing the position of the Hungarian Resuscitation Society, summarizes the key elements of the guideline. The recommendations focus on the concepts of the chain of survival an

Use of a nationwide call center for Ebola response and monitoring during a 3-day house-to-house campaign - Sierra Leone, September 2014.

During May 23, 2014-January 10, 2015, Sierra Leone reported 7,777 confirmed cases of Ebola virus disease (Ebola). In response to the epidemic, on August 5, Sierra Leone's Emergency Operations Center established a toll-free, nationwide Ebola call center. The purpose of the call center is to encourage public reporting of possible Ebola cases and deaths to public health officials and to provide health education about Ebola to callers. This information also functions as an "alert" system for public health officials and supports surveillance efforts for the response. National call center dispatcher

Out-of-Hospital Cardiac Arrest: Public-Access Defibrillation and System Approaches to Minimize Avoidable Delay.

Out-of-hospital cardiac arrest (OHCA) remains a leading cause of sudden death worldwide, with wide variation in reported incidence and outcomes driven by heterogeneity in registries, emergency medical services (EMS) organization, and case definitions. Despite substantial advances in resuscitation systems, survival after EMS-treated OHCA generally remains below 10%, and outcomes are critically time dependent. Delays in emergency call activation, bystander cardiopulmonary resuscitation (CPR), and-most importantly-early defibrillation are associated with a rapid decline in return of spontaneous c

Ebola virus disease control in Poland - are we ready for fight?

Despite very low risk assessment, Polish authorities should be prepared for imported EVD cases and be ready to protect public against the spread of Ebolavirus (EBOV). There is the consistent system of infectious diseases surveillance and control in entire country, regulated by law. In Poland the Public Sanitary Inspection has jurisdiction over infectious diseases surveillance. A reporting system functions at three levels. The entire spectrum of viral hemorrhagic fevers is included in the list of 59th notifiable diseases and causative organisms. EVD is regarded as sever, often fatal communicabl

Ask the experts: a practitioner-centred approach to resilience in emergency medical services.

Background: Resilience in high-stress occupations is commonly framed as a trait, an outcome, or the absence of psychopathology. None of these framings captures what practitioners in emergency medical services actually do, and interventions built on trait-based models have produced weak or inconsistent effects. Objective: This study argues that frontline resilience in emergency medical services is better understood as a domain-specific expert skill, and that work mode - a deliberately entered state, akin to an athlete's game face, in which the demands of the task rather than the emotional force

Policy Spotlight Effects on Critical Time-Sensitive Diseases: Nationwide Retrospective Cohort Study on Taiwan's Hospital Emergency Capability Categorization Policy.

BACKGROUND: Taiwan's categorization of hospital emergency capability (CHEC) policy is designed to regionalize and dispatch critical patients. The policy was designed in 2009 to improve the quality of emergency care for critical time-sensitive diseases (CTSDs). The CHEC policy primarily uses time-based quality surveillance indicators. OBJECTIVE: We aimed to investigate the impact of Taiwan's CHEC policy on CTSDs. METHODS: Using Taiwan's 2005 Longitudinal Health Insurance Database, this nationwide retrospective cohort study examined the CHEC policy's impact from 2005 to 2011. Propensity score

Modelling eCPR eligibility after out-of-hospital cardiac arrest: a 10-year retrospective temporospatial analysis from a mixed urban-rural region in Germany.

BACKGROUND: Survival with good neurological function after out-of-hospital cardiac arrest (OHCA) remains low. Extracorporeal cardiopulmonary resuscitation (eCPR) may benefit selected patients, but it is unclear how many patients in a mixed urban-rural European region would be eligible, whether regional case volumes justify dedicated eCPR programmes, and whether an in-hospital strategy after intra-arrest transport can meet a 60-minute low-flow target. METHODS: We conducted a retrospective cohort study of OHCA cases in the city and county of Göttingen, Germany (January 2015-February 2025). eCPR

Linkage of Emergency Medical Services and Hospital Data: A Necessary Precursor to Improve Understanding of Outcomes of Prehospital Care.

Objective: Linking emergency medical services (EMS) data to hospital outcomes is important for quality assurance and research initiatives. However, non-linkage due to missing or incomplete patient information may increase the risk of bias and distort findings. The purpose of this study was to explore if an optimization strategy, in addition to an existing linkage process, improved the linkage rate and reduced selection and information bias. Methods: 4,150 transported patients in a metropolitan EMS system in Alberta, Canada from 2016/17 were linked to two Emergency Department (ED) databases by
