OBJECTIVE: Posterior endoscopic cervical foraminotomy (PECF) is a minimally invasive surgical technique for treating cervical radiculopathy. Traditionally, PECF is performed under general anesthesia in the prone position, but concerns over anesthesia-related complications have led to the exploration of local anesthesia in the lateral decubitus position as an alternative. This study aims to compare the clinical outcomes, safety, and efficacy of PECF performed under local anesthesia in the lateral decubitus position versus general anesthesia in the prone position.
METHODS: We conducted a retros

BACKGROUND: Posterior cervical foraminotomy for cervical spondylotic radiculopathy requires precise neural decompression while maintaining facet joint integrity. Ideally, the achievement of appropriate nerve root decompression should be confirmed using intraoperative computed tomography or other imaging modalities, although this is not generally done because of the need to avoid radiation exposure and/or because the procedure is very widely performed, even at facilities without expensive equipment. Magnetic resonance imaging (MRI)-ultrasound (US) fusion imaging has the potential to provide a s

INTRODUCTION: Anterior Cervical Discectomy and Fusion (ACDF) has been the gold-standard surgical treatment for cervical radiculopathy. However, advancements in the motion-preserving alternatives Cervical Disc Replacement (CDR) and Posterior Cervical Foraminotomy (PCF) necessitate a contemporary comparison of their effectiveness and safety profiles. To address this, we performed a national database study to compare postoperative complication rates between CDR and PCF for cervical radiculopathy patients.
METHODS: Patients who underwent a one-level CDR or PCF from 2017 to 2022 and diagnosis of c

Lumbar foraminal stenosis can be surgically treated by foraminal decompression or facet joint resection and fusion (transforaminal lumbar interbody fusion, TLIF). While conventional foraminal decompression poses a risk of segmental instability, the endoscopic approach (extended endoscopic lumbar foraminotomy, EELF) resects only the ventral part of the facet joint with a horizontal surgical trajectory. A prospective observational study was performed to analyze the cost-effectiveness of EELF versus TLIF. Patients with dominant unilateral radicular pain from lumbar foraminal stenosis at or above

CASE: This case report describes a 46-year-old woman undergoing right-sided L5 to S1 decompression who received liposomal bupivacaine (LB) for postoperative analgesia and developed unintentional epidural anesthesia with symptoms mimicking cauda equina syndrome. The patient's symptoms resolved 72 hours postoperatively, approximately the length that LB typically lasts. At the 16-month follow-up, the patient demonstrated complete neurological function with no lower extremity strength or sensation deficits.
CONCLUSIONS: Tracking of LB into the epidural space after lumbar surgery may cause transie

OBJECTIVE: Minimally invasive spine surgery (MISS) techniques have similar long-term outcomes compared to open surgery for patients undergoing 1- or 2-level discectomy, foraminotomy, or laminectomy. However, the rate of cerebrospinal fluid (CSF) leaks with both techniques has not been well established in the literature. This study sought to compare the rate and clinical impact of CSF leak in open lumbar foraminotomy, discectomy, or laminectomy with comparable MISS approaches.
METHODS: A total of 863 patients undergoing 1- or 2-level discectomy, foraminotomy, or laminectomy by either MISS an o

Foraminal stenosis causing nerve root compression and radiculopathy is a relatively common pathology in the lumbar spine. The treatment of choice, when surgery is indicated, is foraminal decompression at the affected level, usually through a conventional midline open foraminotomy. Minimally invasive lateral foraminotomy with partial lateral facetectomy is a potentially effective surgical alternative when such a surgery is warranted. The evaluation of the efficacy and benefits of this approach for treatment of radiculopathy; an assessment of facet integrity; and, a detailed description of the p
